방사선사의 병원감염예방에 대한 인지도와 수행도에 관한 연구 (A Study on Radiologists' Awareness and Performance of Hospital Infection Prevention)

  • 본 연구는 방사선사의 병원감염관리에 대한 인지도와 수행도를 파악하여 병원감염관리 방안의 개선 및 교육을 위한 기초자료를 제공하기 위해 실시한 연구이다. 연구대상자의 병원감염관리에 대한 인지도와 수행도의 평균 평점은 150점 만점으로 인지도 141.05점과 수행도 138.15점으로 모든 영역에서 인지도가 높을수록 수행도가 높은 정상관계를 보였으나 인지도에 비해 수행도가 전체적으로 낮음을 확인 할 수 있었다. 인지도에 영향을 미치는 요인으로는 감염관리 교육의 필요성, 감염예방 교육여부, 환자의 질병상태 인지 여부 등으로 통계적으로 유의하게 나타났고, 수행도에 영향을 미치는 요인으로는 감염예방 교육여부, 환자의 질병상태 인지여부, 인지도 등으로 통계적으로 유의하게 나타났다.

Risk Factors of Clonorchis sinensis Human Infections in Endemic Areas, Haman-Gun, Republic of Korea: A Case-Control Study

  • Clonorchis sinensis is the most common fish-borne intestinal parasite in Korea. The aim of the present investigation was to survey the status of C. sinensis infection and analyze associated risk factors in residents of Haman-gun, Gyeongsangnam-do. A total of 5,114 residents from 10 administrative towns/villages voluntarily agreed to participate in the study, which comprised fecal examination, a questionnaire survey for risk factors, ultrasonography, and enzyme-linked immunosorbent assay for cancer biomarker detection in the blood. We detected C. sinensis eggs in 5.3% of the subjects. By region, Gunbuk-myeon had the highest number of residents with C. sinensis eggs. The infection rate and intensity were higher in male than in female residents. Based on the risk factor questionnaire, infection was highly associated with drinking, a history of C. sinensis infection, and the practice of eating of raw freshwater fish. Extension of the bile duct, infection intensity, and cancer biomarker detection significantly correlated with the presence of eggs in the study population. In conclusion, the development of feasible, long-term control policies and strategies for the elimination of C. sinensis in Korea is still required.

A contact investigation after exposure to a child with disseminated tuberculosis mimicking inflammatory bowel disease

  • Purpose: Tuberculosis (TB) is one of the most important diseases that cause significant mortality and morbidity in young children. Data on TB transmission from an infected child are limited. Herein, we report a case of disseminated TB in a child and conducted a contact investigation among exposed individuals. Methods: A 4-year-old child without Bacille Calmette-$Gu{\acute{e}}rin$ vaccination was diagnosed as having culture-proven disseminated TB. The child initially presented with symptoms of inflammatory bowel disease, and nosocomial and kindergarten exposures were reported. The exposed individuals to the index case were divided into 3 groups, namely household, nosocomial, or kindergarten contacts. Evaluation was performed following the Korean guidelines for TB. Kindergarten contacts were further divided into close or casual contacts. Chest radiography and tuberculin skin test or interferon-gamma-releasing assay were performed for the contacts. Results: We examined 327 individuals (3 household, 10 nosocomial, and 314 kindergarten contacts), of whom 18 (5.5%), the brother of the index patient, and 17 kindergarten children were diagnosed as having latent TB infection (LTBI). LTBI diagnosis was more frequent in the children who had close kindergarten contact with the index case (17.1% vs. 4.4%, P=0.007). None of the cases had active TB. Conclusion: This is the first reported case of TB transmission among young children from a pediatric patient with disseminated TB in Korea. TB should be emphasized as a possible cause of chronic diarrhea and failure to thrive in children. A national TB control policy has been actively applied to identify Korean children with LTBI.

치과의료기관 종사자의 감염예방 실천에 관한 연구 (A Study on the Practice for Infection Prevention of Dental Clinic Worker)

  • 본 연구는 치과위생사의 감염예방 실천도를 분석하고자 2014년 1월 15일부터 2월 25일까지 광주광역시와 전남지역의 치과병 의원에 근무하는 치과의료기관 종사자를 대상으로 편의표본추출법을 이용하여 선정하였으며 설문조사를 실시하여 다음과 같은 결론을 얻었다. 일반적 특성과 직업적 특성에 따른 감염예방 실천은 근무처가 대학병원 및 치과병원인 경우, 건강검진을 받은 경우 실천도가 높게 나타났으며 통계적으로 유의한 차이를 보였다(p<0.05). 감염사고실태에 따른 감염예방 실천은 진료 전 문진을 시행하는 경우, 감염성 질환자 진료 시 별도의 개인방호 장비를 착용하는 경우, 감염성 환자 진료 후 진료복을 매회 세탁하는 경우에 통계적으로 유의한 차이가 있었다(p<0.05). 감염관리에 대한 교육이 필요하다고 응답한 경우, 감염관리에 대한 이론 및 시술법에 대한 교육 경험이 있는 경우, 감염관리에 대한 교육시행 시 참여 의향이 있는 경우에 실천도가 높게 나타났으며 통계적으로 유의한 차이를 보였다(p<0.05). 감염관리에 대한 이론 및 시술법 교육경험과 교육 후 인식변화는 치과의원보다 치과병원이 더 높은 것으로 나타났으며 근무처에 따라 통계적으로 유의한 차이가 있었다(p<0.05). 감염예방 실천에 영향을 미친 요인은 감염관리 교육 필요여부, 감염성 환자 진료 후 진료복 세탁 여부, 근무처로 나타났다. 감염관리 교육이 필요하다고 생각하는 경우, 감염성환자 진료 후 진료복을 세탁하는 경우가 감염예방 실천을 잘하고 있는 것으로 나타났고, 치과의원이 치과병원보다 실천이 낮았으며 설명력은 15.8%였다. 이상의 결과로 볼 때 치과진료 감염예방을 위해 치과의료 기관 종사자들에게 지속적인 교육 프로그램이 제공되어 감염예방 실천으로 이어지도록 노력해야 할 것으로 생각된다.

Hepatitis C Viral Infection in Children: Updated Review

  • Hepatitis C virus (HCV) infection is a major medical challenge affecting around 200 million people worldwide. The main site of HCV replication is the hepatocytes of the liver. HCV is a positive enveloped RNA virus from the flaviviridae family. Six major HCV genotypes are implicated in the human infection. In developed countries the children are infected mainly through vertical transmission during deliveries, while in developing countries it is still due to horizontal transmission from adults. Minimal nonspecific and brief symptoms are initially found in approximately 15% of children. Acute and chronic HCV infection is diagnosed through the recognition of HCV RNA. The main objective for treatment of chronic HCV is to convert detected HCV viremia to below the detection limit. Children with chronic HCV infection are usually asymptomatic and rarely develop severe liver damage. Therefore, the benefits from current therapies, pegylated-Interferon plus ribavirin, must be weighed against their adverse effects. This combined treatment offers a 50-90% chance of clearing HCV infection according to several studies and on different HCV genotype. Recent direct acting antiviral (DAA) drugs which are well established for adults have not yet been approved for children and young adults below 18 years. The most important field for the prevention of HCV infection in children would be the prevention of perinatal and parenteral transmission. There are areas of focus for new lines of research in pediatric HCV-related disease that can be addressed in the near future.

의료기관 종사자의 잠복결핵감염 유병률과 위험요인 분석 (Analysis of Prevalence and Risk Factors for Latent Tuberculosis Infection among Healthcare Workers)

  • Purpose: The purpose of this study was to provide basic data on the infection prevention management program, which is one of the infectious disease control program by identifying the prevalence and risk factors of latent tuberculosis infection (LTBI) in healthcare workers. Methods: We surveyed a total of 3,046 LTBI test results, including those of 2,269 existing staff and 777 new employees. An interferon-gamma release assay (IGRA) for the diagnosis of LTBI was performed using QuantiFERON®-TB Gold In-Tube (QFT-IT). The risk factors of LTBI were analyzed using logistic regression analysis. Results: The overall prevalence of LTBI was 16.0% (487/3,046). The prevalence of LTBI in the existing staff was 17.9% (406/2,269) and the prevalence of LTBI in new employees was 10.4% (81/777). Multivariate logistic regression analysis revealed that the risk factors of latent tuberculosis infection among the existing staff were gender, age and work period wheres, the risk factor amongst the new employees depended on their age. Conclusion: The LTBI was not related to the type of occupation and work unit. Therefore, while establishing an infection control program for the prevention of tuberculosis infection at medical institurions, institutional heads and infection control experts should encompass a policy for all the employees.

Sources of Infection Among Confirmed Cases of COVID-19 in Jeju Province, Korea

  • Objectives: Jeju Province in Korea reported 627 coronavirus disease 2019 (COVID-19) cases between January 20, 2020, and March 31, 2021. This study analyzed the sources of infection among confirmed cases in Jeju Province, a self-governed island. Methods: The sources of infection were broadly categorized as follows: (1) infections from overseas (confirmed patients who reported travel overseas or contact with overseas travelers); (2) infections from outside Jeju Province (confirmed patients who had visited other provinces or had contact with individuals who had traveled to other provinces in Korea); and (3) unknown sources of infection (confirmed patients who were infected following contact with an infected person whose source of infection was unknown). The chi-square test was used to analyze the differences in the distributions of related variables for each source of infection. Results: Of the 627 confirmed cases, 38 (6.1%) were infections from overseas sources, 199 (31.7%) were from outside of Jeju Province, and 390 (62.2%) were from unknown sources. Jeju Province had no cases with an unknown source of infection during the first and second waves of the nationwide outbreak. Conclusions: Infections from overseas sources could be blocked from spreading to local communities in Jeju Province by conducting screening at the airport, along with the preemptive suspension of visa-free entry. In addition, considering the scale of the nationwide outbreak, measures must be established to delay outbreaks from unknown sources of infection caused by sources outside Jeju Province.

Awareness and Knowledge about Human Papillomavirus Infection and Vaccination among Women in UAE

  • Background: Cervical cancer is the second commonest female cancer worldwide. The 50-55 cases of cervical cancer are reported annually in the UAE. There is a scarcity of data from Middle Eastern region regarding knowledge and attitude of women towards HPV infection, cervical cancer prevention and HPV vaccine. The aim of our study was to assess the knowledge of women regarding HPV infection and vaccine in UAE. Materials and Methods: A cross-sectional survey of 640 women aged 18-50 years was conducted in Al-Ain district in UAE using convenience sampling. Women with previous diagnosis of cervical cancer, non-residents of UAE, younger than 18 or older than 50 years of age and those unable to speak Arabic or English were excluded from the study. Logistic regression analysis was performed to assess the association of HPV knowledge with independent factors like age, education etc. Results: Only 29% of our sampled women have ever heard of HPV infection. Only 15.3% women recognized it as STI. Only about 22% women have also heard of the HPV vaccine. Three quarter of the women in our study thought that cervical cancer can be prevented. About 28% recognized vaccine as a preventive measure against cervical cancer. Age (AOR 1.049, 95%CI 1.02-1.08) and husband's level of education were found to be significant (p value 0.015) after adjusting for women's age. Conclusions: The knowledge of HPV infection and vaccine is low in the UAE. Few women recognized HPV as sexually transmitted infection. Increasing age and husband's education are associated with better knowledge of HPV infection.

물리치료사의 병원감염에 대한 인식과 실천 (Awareness and Performance of Physical Therapists on Nosocomial Infection)

  • 본 연구의 목적은 병원 물리치료사들의 병원감염에 대한 인식도와 실천도를 조사하고 인식도와 실천도에 영향을 미치는 요인을 분석하여 물리치료실 내 병원감염 관리 프로그램 및 정책 수립을 위한 자료를 제공하고자 하는데 있다. 본 연구의 자료는 405명의 물리치료사로부터 수집하여 분석하였다. 본 연구에서는 일반적 특성, 병원감염에 대한 교육 상태 및 교육의 필요성 그리고 병원감염을 예방하기 위한 인식도와 실천도 등을 묻는 리커트 5점 척도의 자기 기입식 설문지를 사용하였다. 연구의 결과 물리치료사들의 병원감염 교육은 거의 이루어지지 않고 있었으며 이러한 교육 결여로 인해 물리치료사들의 병원감염 예방에 대한 인식도 및 실천도는 매우 낮은 실정이었으나 물리치료사들의 병원감염 프로그램의 필요성 인식과 병원감염 교육에 대한 요구는 매우 높은 수준이었다. 따라서 현행 물리치료학과 교육 중, 병원감염에 대한 교육 프로그램이 도입되어야 할 것으로 사료되며 이와 더불어 물리치료실을 운영하는 병원에서도 물리치료실의 병원감염 관리에 대한 교육 프로그램의 수립이 필요한 것으로 사료된다.

Knowledge of the General Community in Cordoba, Argentina, on Human Papilloma Virus Infection and its Prevention

  • Background: Most studies of human papilloma virus (HPV) are aimed at the natural history of the infection and its relation to cancer; however, there are few studies to assess knowledge of the general population. Our aim was analyze the degree of knowledge of Argentinians about HPV infection and its prevention. Materials and Methods: We conducted a voluntary, anonymous and non-binding survey with 27 multiple-choice items, in twelve private and public establishments, selected to include a broad population in terms of education, age and gender. The survey consisted of three sections: individual characteristics of the volunteer, HPV infection basic knowledge, its prevention and the virus relationship with other cancers. Results: One thousand two hundred ninety seven volunteers aged 18 to 80 participated. The total number of correct answers was 45.1%. The correct answers for relationship HPV and cervical cancer was 62.1%. Almost 55% did not know about types of HPV that the vaccines for protection. Statistical analysis showed that women, single people, workers, the better educated, those who have had a STDs or HPV and receiving information through medical or educational establishments had greater knowledge of the topic. Only 0.2% of participants answered all questions correctly. Conclusions: Knowledge plays an important role in health care and the deficiency found in our population could influence the success of the measures taken in the fight against cervical cancer. In this regard, we believe it would be appropriate, not only to emphasize early diagnosis and vaccine implementation, but also incorporate new communication strategies, facilitating reception of accurate and precise information by all strata of society.
