A case of acute transverse myelitis following chickenpox (수두 감염 후 발생한 급성 횡단성 척수염 1예)

  • Acute transverse myelitis (ATM) in most patients is characterized by an abrupt onset of progressive weakness and sensory disturbance in the lower extremities with a preceding viral infection such as Epstein-Barr, herpes simplex, influenza, mumps and Varicella-zoster viruses (VZV). Although less frequent, some residual deficits including bladder dysfunction or weakness in the lower extremities may follow ATM, from which recovery usually begins within the first week of the onset of symptoms. In this report, we describe the case of a 9-year-old girl who experienced ATM following chickenpox and had bladder dysfunction as a sequela.

  • Ramsay Hunt syndrome is a viral associated disease with severe otalgia, vertigo, fever, herpetic eruptions on either side of the external auditory meatus and cavum concha, ipsilateral facial nerve palsy and cochleovestibular dysfunction. This syndrome may be the most common cause of unilateral facial paralysis and involvement of both the vestibular and cochlear branch of 8th cranial nerve. And loss of taste sensation may be developed in same involement site. This syndrome affects adults in most cases, and a samll number of children with herpes zoster oticus have been reported. And concomittantly CNS invlovement of this snydrome is very rare. We experienced a 7 years old aged patient of Ramsay Hunt syndrome who had evidence of aseptic meningitis, and this patient showed well reponses with Acyclovir and symptomatic therapy. So, we report this case with brief review of related literatures.

치료용 레이저를 이용한 대상포진 (Herpes Zoster)치료 19례에 대한 보고

  • 문헌에 의하면 대상포진의 원인은 바이러스에 대한 세포성 면역이 저하될 때, 또는 악성종양 등으로 면역억제제 치료를 받아서 2차적으로 면역이 저하될 때 체내에 잠재해 있던 바이러스가 재활되어 발병하는 것으로 추정하고 있다. 본 저자도 이에 대하여 의견을 같이하고 있는데 노령층의 $60\~70$대 여자환자수가 압도적으로 많았다는 점과, $20\~39$세 사이의 청${\cdot}$장년층에서는 단1명의 대상자가 없다는 점뿐 아니라, 본 대상자중 11세의 소녀는 7세때 뇌종양 수술을 2차례에 걸쳐 실시한 후 면역제제를 계속적으로 사용하는 있는 점 등은 문헌의 발병원인과 상당히 일치하였다. <전파 양식> 비말 감염 (droplet infection)으로 전파되며, 수두와 달리 전염성이 높지 않다. <잠복기> 잠복해 있던 바이러스가 언제 재활할지 알 수 없으므로 불명이다. 그러나 본조사에서는 대상포진에 대한 병력을 가진 사람이 없었고, 발생 원인을 본인 자신도 모르고 있었다. <합병증> Ramsay Hunt의 증후에 의하면 합병증은 외이도의 수포, 안면신경마비, 혀의 2/3부분의 미각상실, 간혹 청각 및 평행장애를 일으킨다고 한다. <치료> 대상포진의 대증요법으로 calamine lotion Burrow solution의 wet dressing 진통제 및 cytosine arabinoside나 adenosine arabinoside와 같은 항히스타인제, 최근에는 Acyclovir가 많이 사용되고 있는 것으로 알려졌다.

Infection and Pathogenesis Mechanisms of Marek's Disease Virus (마렉병 바이러스 감염과 병원성 발현 기전)

  • Like the other herpesviruses, the virion of MDV consists of an envelope, which surrounds an amorphous tegument. Within the tegument, and icosahedral capsid encloses a linear double-stranded DNA core. Although the genome structure of MDV indicates that it is an ${\alpha}-herpesvirus$ like herpes simplex and varicella-zoster viruses, biological properties indicate MDV is more akin to the ${\gamma}-herpesvirus$ group, which includes Epstein-Barr and Kaposi's sarcoma herpesviruses. These herpesviruses replicate lytically in lymphocytes, epithelial and fibroblastic cells, and persist in lymphoblastoid cells. MDV has a complex life cycle and uses two means of replication, productive and non-productive, to exist and propagate. The method of reproduction changes according to a defined pattern depending on changes in virus-cell interactions at different stages of the disease, and in different tissues. Productive (lytic) interactions involve active invasion and take-over of the host cell, resulting in the production of infectious progeny virions. However, some herpesviruses, including MDV, can also establish a non-productive (abortive) infection in certain cell types, resulting in production of cell-associated progeny virus. Non-productive interactions represent persistent infection, in which the viral genome is present but gene expression is limited, there is no structural or regulatory gene translation, no replication, no release of progeny virions and no cell death. Reactivation of the virus is rare, and usually the infectious virus can be re-isolated only after cultivation in vitro. MDV establishes latency in lymphoid cells, some of which are subsequently transformed. In this review article, recent knowledges of the pathogenesis mechanisms followed by MDV infection to sensitive cells and chickens are discussed precisely.

  • Background : infection associated with otalgia, herpetic eruptions, facial palsy, trigeminal pain and cochleovestibular dysfunction such as hearing dishubance, tinnitus, vertigo and other symptoms, and is not frequently encountered in otolaryngologic field. And facial paralysis occured by this syndrome is rapid in onset, usually severe in degree, and poor in prognosis. Objectives : To evaluate the Oriental Medicine's treatment and prognosis of Herpes zoster oticus and to classify of peripheral Facial palsy on Oriental Medicine. Methods : Clinical observation and analysis about a case of Ramsay-Hunt syndrome III type was done, who visited Dongguk University Gangnam Hospital. This case is managed by integrated therapy of oriental and western medicine. In oriental medicine, to treate ear pain, heonggae-yeongoetang(荊芥蓮翹湯) was given and to treat herpetic eruption, 8 constitution acupuncture treatement was done. In western medicine, antiviral agent, acyclovir and steroid hormone, prednisolon were treated. Result : About 6 weeks treatment, this case which has worse prognosis than Bell's palsy, was completed cured without any complication. Conclusion : 1. Futher divided classification about non-infectious and infectious facial palsy include "Ramsay Hunt Syndrome" is needed. 2. In acute state, Jeonyohwadan's (纏腰火丹) treatement can be applied, but the difference of invasion part, primary symptom such as ear pain and herpetic eruption is needed another treatment. 3. Ear pain and herpetic eruption can be treated in part of poongyeol(風熱) and subyeol(濕熱). 4. In Acupuncture treatement, Jeonyohwadan's (纏腰火丹) treatement can be applied in acute stage and guanwasa's treatement can be applied in chronic stage. but futher research is needed. 5. This case, Ramsay Hunt Syndrome IlI type, was managed by integrated therapy such as heonggae-yeongoetang(荊芥蓮翹湯), 8 constitution acupuncture treatement, acyclovir and prednisolon and this treatement was good effect.

  • Ramsay Hunt Syndrome is caused by infection of herpes zoster and associated with otalgia, herpetic eruptions, facial palsy, trigerminal pain and cochleovestibular dysfunction such as deafness, tinnitus, vertigo and so on. A 60-year old female, who has tinnitus, peripheral facial palsy, deafness, vertigo, postauricular pain and herpetic eschar, had been admitted to Wonkwang University Oriental Jeonju Medicine Hospital. We treated this patient with Daehwangchijahwangbaeg-byung and Yongdamsagantang-gami, and the patients showed improvement in the symptoms. So we describe the process and contents about the way the patient was cured.

Short-term Results of Surgical Treatment in Esophageal Carcinoma (식도암의 외괴적 조기관찰 성적)

  • Twenty nine adult patients underwent surgical esohpagectomy and one, bypass procedure for documented carcinoma of esophagus and cadiac portion of stomach at Chonnam National University Hospital from Jan 1986 to April 1991. There were several kinds of esophagectomies including through transhiatal, left thoracotomy only, laparotomy and thoracotomy, and laparotomy and right thoracotomy and cervical incision. Twenty five and squamous cell carcinoma and 5, adenocarcinoma. The tumor locations were the upper third in 3, middle third in 12, lower third in 10 and cardiac portion of stomach in 5. After operation, 8[27%] patients were classified in Stage IIa, 6[20%] patients in Stage IIb, 15 patients[50%] in Stage III and one patient in Stage IV. Major postoperative complications included anastomotic narrowing in 3, limited suture line leak in 2, wound infection in 2, hoarseness in 2, pseudomembraneous enterocolitis in 1 and herpes zoster in 1. There was no death within 30 days of operation. Ten months survival was 100% for patients with Stage lIa, 67% for patients with Stage IIb, 50% for patients with Stage III. Furthermore, 20 months survival was 75% in IIIa, 33% in IIb, and 40% in III. But there were no significant differences in survivals among the stage. The actuarial survival is 58% at one year and 41% at two years, The periods of average survival is 589 days after operation.

  • Ramsay-Hunt syndrome is a related but more serious disorder caused by herpes zoster viral infection of the geniculate ganglion. It is characterized by unilateral painful vesicular rash of the uvula, palate, auricle, ear canal, and postauricular area, but it can extend into the facial tissues as well. Paralysis of the facial nerve is often seen, and there can be disequilibrium and hearing problems also. We experienced continuous cervical epidural block (CCEB) with intermittent stellate ganglion block is effective in Ramsay-Hunt syndrome. CCEB should be considered to the treatment of choice in Ramsay-Hunt syndrome.

Study of Developmet of External Medicine Based on the Result of Prior Study and Folk Remedy Using Scolopendra subspinipes mutilans as a Single Medicine in Korea and China (한국과 중국에서의 오공을 단방으로 사용한 민간요법과 선행연구결과를 기반으로 한 외치 한약제제 개발에 대한 고찰)

  • Scolpendra subspinipes (centipede) is pungent in flavor, activates the channels to stop pain, and, removes toxins and resolves masses. So it has been used as a folk remedy. We researched folk remedies which used centipede as a single medicine and results of its prior clinical and experimental study, and figured out that centipedes are effective on ulcerative disease, herpes zoster, clavus, insect bite, inflammatory disease, furunculus, pain disease by external therapy, and oral administration is effective on anti-cancer such as hepatoma and uterine cancer, cerebrovascular disease, inhibition of atherosclerosis, sedation of central nervous system, antipyretic action, and anti-inflammation, and centipede Herb-Acupuncture is effective on inflammatory disease and pain disease. Centipede preparation materials for external therapy were sesame oil, perilla oil, vinegar, salt, and, burnt alum. Fumigation of centipede treated paronychia, pertussis, and hemorrhoids. For the external therapy, the way of centipede powder mixing with other materials has been used. It is needed that we use dusted powder to make skin absorb its ingredients more easily, and we study intensely minor ingredients to make centipede components absorbed more effectively and do pharmacological action. Also, fumigation, a kind of another external therapy, is considered to be effective treatment of upper respiratory infection like pertussis, so it needs an in-depth study on centipede fumigation.

The unseen spread: a case of disseminated tuberculosis with renal manifestation in a healthy adult

  • Disseminated tuberculosis (TB), resulting from the hematogenous spread of tubercle bacilli, typically affects immunocompromised individuals, such as those infected with the human immunodeficiency virus. However, risk factors in immunocompetent populations are not well understood. Here, we report a rare case of disseminated TB with CD4+ T-cell depletion in a previously healthy 35-year-old man. The patient presented with a 2-month history of intermittent gross hematuria, dysuria, loose stools, and weight loss. His medical history was unremarkable except for a herpes zoster infection 4 years prior to presentation. Laboratory tests revealed microscopic hematuria and pyuria; however, the urine culture was negative. Urine specimens tested positive for TB-polymerase chain reaction. Abdominal computed tomography revealed a focal filling defect in the left kidney, segmental wall thickening of the terminal ileum, and multiple enlarged lymph nodes with central necrosis. Chest computed tomography revealed active pulmonary TB. Colonoscopy confirmed intestinal TB in the terminal ileum and ileocecal valve, with positive TB-polymerase chain reaction results from sputum and ileal ulcer tissue. The patient was diagnosed with disseminated TB and was treated with standard anti-TB drugs. Although the human immunodeficiency virus test results were negative, the patient's CD4+ T-cell count was significantly low (278/μL). Follow-up tests after 1 month showed negative TB cultures; however, the patient's CD4+ T-cell depletion persisted, with counts remaining low after 1 year. This case highlights the rare occurrence of disseminated TB in immunocompetent individuals with CD4+ T-cell depletion and emphasizes the importance of CD4+ T-cell assessment in healthy patients presenting with disseminated TB.
