온라인 커뮤니티를 이용한 미숙아 퇴원교육 프로그램의 효과 (Effects of a Discharge Education Program Using On-line Community for Parents of Premature Infants)

  • Purpose: This study was done to examine effects of a discharge education program using on-line community on parenting stress, baby care confidence for mothers of premature infants and health problem of premature infants. Methods: Using a non-equivalent control group pretest-posttest quasi-experimental design, 32 participants were assigned to an experimental (17) or a control group (15). Mothers in the experimental group were introduced to the discharge education program 2 weeks before the baby was discharged. They participated in the on-line community for 4 weeks after discharge. Parenting Stress Index and Baby Care Confidence Scale were utilized for data collection. Data were analyzed using descriptive statistics, $X^2$-test, Fisher's exact test, and Mann-Whitney U test with the SPSS program 16.0 version. Results: In the experimental group, mean scores for parenting stress were lower than for the control group (Z=-3.176, p=.001), while scores for confidence in baby care were higher than for the control group (Z=-3.195, p=.001). Conclusion: Findings indicate that discharge education programs using on-line community have the positive effect of decreasing parenting stress and increasing baby care confidence for mothers and decreasing health problems for infants. Therefore utilization of the internet and online community to educate parents of premature infants is recommended.

신생아의 피부표현 별 수분율 측정과 관련요인 분석 (Assessment of Hydration on the Stratum Corneum and the Influencing Factors in Neonates)

  • Purpose: The study was conducted to investigate the skin hydration level in various body sites and identify the influencing factors in neonates. Methods: An exploratory comparison study was designed to measure the stratum corneum hydration, using a National DM-R2 on the forehead, abdomen, buttocks, and the back of the hands and feet of 198 neonates including 92 premature infants. Results: The results showed 32.7%-36.5% of stratum corneum hydration for all sites. Premature infants revealed a higher hydration level on the peripheral sites (dorsal hand and feet) than those of the full-term infants, possibly resulting from therapeutic regimens including an incubator or radiant warmer. Infants in an incubator showed a higher hydration level than those in radiant warmers, suggesting more attention to fluid management for infants in the open environment. In addition, all stratum corneum hydration measurements except one, from the forehead, showed a positive correlation with postnatal age in full-term infants while showing no relation to any measurements in premature infants. Conclusion: The study demonstrated the very low skin hydration levels in hospitalized neonates, particularly in premature neonates with more susceptible skin hydration instability despite therapeutic interventions for fluid balance. More vigilant fluid management is imperative in neonates, particularly those in the open environment.

조산아의 추나 치료에 대한 문헌 고찰 (Chuna Manual Therapy for Premature Infants: A Literature Review)

  • Objectives This study aimed to explore literature evidence regarding Chuna manual therapy for premature infants and determine its applicability. Methods Eight electronic databases were searched, and randomized control trials (RCTs) applying Chuna manual therapy to premature infants were sought. The Cochrane risk of bias tool was employed to assess the methodological quality of each RCT. Results Two RCTs met the inclusion criteria and were subjected to analysis. Each study compared the usual care group with the pediatric Chuna group. The results indicated improvements in nutritional status, a reduction in hospitalization days, and enhancements in growth indicators. Conclusions Chuna manual therapy is worth applying to premature infants. However, evidence is limited due to a lack of well-designed RCTs. More qualified clinical trials are needed to obtain stronger evidence.

  • Objectives: This study compared the nutritional intakes of early and late preterm infants in a neonatal intensive care unit (NICU) and at home. The dietary problems and the need for community care services for premature infants were further investigated. Methods: This is a cross-sectional and descriptive study on 125 preterm infants and their parents (Early preterm n = 70, Late preterm n = 55). The data were collected by surveying the parents of preterm infants and from hospital medical records. Results: No significant differences were obtained between the early and late preterm infant groups when considering the proportion of feeding types in the NICU and at home. Early preterm infants were fed with a greater amount of additional calories at home and had more hours of tube feeding (P = 0.022). Most preterm infants had feeding problems. However, there was no significant difference between early and late preterm infants in the mental pain of parents, sleeping, feeding, and weaning problems at home. Many parents of preterm babies had no external support, and more than half the parents required community care to take care of their preterm babies. Conclusions: Regardless of the gestational age, most preterm infants have several problems with dietary intake. Our study indicates the need to establish community care services for preterm infants.

Predictive Factors for Severe Thrombocytopenia and Classification of Causes of Thrombocytopenia in Premature Infants

  • Purpose: This study investigated predictive factors for severe neonatal thrombocytopenia, which greatly increases the need for intensive care and is associated with a high mortality rate in premature infants. Factors adopted for prompt identification of at-risk newborns include blood test results and birth history. This study analyzed the relationship between the presence of severe neonatal thrombocytopenia and the mortality rate. The causes of thrombocytopenia in premature infants were also examined. Methods: This retrospective study evaluated 625 premature infants admitted to the neonatal intensive care unit (NICU) at Chung-Ang University Medical Center. The neonates were classified into 3 groups according to the severity of thrombocytopenia: mild ($100{\times}10^9/L{\leq}platelet<150{\times}10^9/L$), moderate ($50{\times}10^9/L{\leq}platelet<100{\times}10^9/L$), or severe (platelet<$50{\times}10^9/L$). Analysis of blood samples obtained at the onset of thrombocytopenia included platelet count, white blood cell (WBC) count, hemoglobin level, hematocrit level, absolute neutrophil count, and high-sensitivity C-reactive protein level. Results: Of the 625 premature infants admitted to our NICU, 214 were detected with thrombocytopenia. The mortality rate in thrombocytopenic neonates was 18.2% (39/214), whereas a mortality rate of only 1.0% was observed in non-thrombocytopenic neonates. The major causes of thrombocytopenia were perinatal insufficiency and sepsis in premature infants. Severe thrombocytopenia was noted more frequently in premature infants with higher WBC counts and in those with a younger gestational age. Conclusion: Platelet count, WBC count, and gestational age are reliable predictors for severe neonatal thrombocytopenia. The major causes of thrombocytopenia were perinatal insufficiency and sepsis in premature infants.

Effects of Dysphagia Treatment Applied to Infants with Pierre Robin Syndrome - Single Subject Research Design

  • Pierre Robin syndrome is characterized by micrognathia, glossoptosis, and cleft palate. Infants with Pierre Robin syndrome causes feeding difficulty, upper airway obstruction, and other symptoms. This study aims to examine the effects of applying dysphagia treatment to infants with Pierre Robin syndrome. The study participant was an infant who was born four weeks premature and referred for dysphagia treatment approximately 100 days after birth. At the initial assessment, the infant showed oral sensory sensitivity, a high level of facial and masticatory muscle tension, and a low stability of the chin and cheeks with almost no normal "sucking-swallowing-breathing" pattern. We set the baseline period and intervention period using the AB design. During the baseline period, non-nutritive sucking training using a rubber nipple was conducted without implementing an oral stimulation intervention. During the intervention period, non-nutritive sucking training and an oral stimulation intervention were performed. After the intervention period, the infant's daily oral intake and oral intake per time significantly increased compared to that during the baseline period. We observed that the oral intake time of the infant decreased during the intervention period compared to that in the baseline period, which indicated an improvement in control over the chin, tongue, and lip movements, a change in muscular tension, and stabilization of the "sucking-swallowing-breathing" pattern. We provided dysphagia treatment before breastfeeding, it was positive effects such as normal development of the infant, transition from tube feeding to bottle feeding, and enhancement of overall oral motor function.

One-Stage Repair of an Interrupted Aortic Arch with an Aortopulmonary Window in a Premature Neonate

  • Interrupted aortic arch with an aortopulmonary window is a rare congenital entity that is associated with high morbidity and mortality, especially in premature low-birth-weight infants, and the proper timing of surgical correction remains a matter of debate. We present the case of a premature infant weighing 1.6 kg who successfully underwent one stage surgical repair to treat interrupted aortic arch with an aortopulmonary window. The therapeutic management of this patient is described below, and a review of the literature is presented.

  • The study of prematurity stereotyping is important because the optimal development of premature infants may be threatened by the operation of the stereotape. The study was conducted in which mothers were asked to rate unfamiliar infants shown on videotapes. The infants were described as either full term or premature and as either male or female. Each mother was asked to rate the infant on 23,7-point adjective scales(S-KISS) and to answer several other questionnaires(LOT, PSI). In the study, infants labeled premature were rated more negatively than were infants labeled full term, but infants labeled male and female were rated similarly. The behavioral implications for child rearing as a result of this "prematurity stereotyping" were considered. Correlational analyses revealed significant relationships between prematurity stereotyping and parenting stress, optinism and parenting stress. Optimism effects parenting stress.

  • An urinoma(uriniferous pseudocyst, pararenal pseudocyst) denotes an encapsulated collection of urine in the perirenal or paraureteral space. It was usually reported in relation to trauma and acquired obstructive uropathy but rarely reported in renal infection including renal candidiasis. The mechanism is believed due to rupture of fornix through weakened portion of suppurated kidney and pyelosinus backflow by increased intrapelvic pressure in obstructive uropathy and fungus ball obstruction. We report a case of urinoma in a premature as the first case in Korea which developed as a complication of renal candidiasis.

  • Purpose : The purpose of this study was to investigate the effect of sensorimotor stimulation on the development of infant with low birth weight premature. Methods : Ten infants with low birth weight premature and ten normal infants participated in this study. We carried out test TIMP(Test of Infants Motor Performance) according pre intervention, post 3 weeks, PCA 40 week and normal infants. The intervention of sensorimotor stimulation applied to infants with low birth weight premature four times a week and fifteen minutes a once time. There was no intervention for normal infants. The collected dada were analyzed by ANOVA using by SPSS/PC 17.0 ver. program. Results : There was significant difference among three differential test period to improved developmental value in infants with low birth weight premature. According over time, numbers of observed items presented significant difference among test period, and elicited total score and total raw score was significant value(p=.00). There was no significant value that means sensorimotor stimulation affected on development of infants with low birth weight premature. Conclusions : In conclusion, sensorimotor stimulation had affects on the development of infants with low birth weight premature.
