Purpose: The purpose of this study is to report the results of culture test at the time of removal of metal devices used for management of ankle fractures and for analysis of contributing factors. Materials and Methods: We reviewed medical records of 132 patients with lower tibia and ankle fracture who had their metal devices removed during the period from January 2010 to February 2014. Patients with clinical signs of infection were excluded. Culture test was performed by taking the granulation tissue around the metal device at the time of removal. We divided the subjects into two groups, culture positive and negative. We then performed a retrospective review of each medical record of multiple factors that might contribute to the culture results, including laboratory results, medical history, material and size of metal device, indwelling period, and whether or not it was open injury. Results: Among 132 cases, six were culture positive. Enterococcus was detected in two cases and the others were Staphylococcus. No significant difference in medical history of patients and laboratory results, including C-reactive protein level, was observed between the culture positive and negative group. Culture positive rate was 5.4% in titanium and 3.9% in stainless steel. In terms of metal size, culture positive rate was 5.1% in small plates, 6.7% in large plates, and culture negative in intramedullary nails. The average indwelling period of metal device was 61.5 weeks in the culture positive group, and 68.6 weeks in the negative group. Nine cases were open fractures and all were in the culture negative group. Conclusion: Whether or not the culture result was positive, there were no meaningful contributing factors. Presence of bacterium on the metal device could not be screened by any laboratory results or other factors.
Background The extended latissimus dorsi flap is important for breast reconstruction. Unfortunately, donor site seroma is the most common complication of extended latissimus dorsi flap for breast reconstruction. Although using fibrin sealant in the donor site reduces the rate of seroma formation, donor site seroma remains a troublesome complication. The purpose of this study was to analyze the effectiveness of the combination of quilting sutures and fibrin sealant in the latissimus dorsi donor site for the prevention of seroma. Methods Forty-six patients who underwent breast reconstruction with extended latissimus flap were enrolled in the study. The patients received either fibrin sealant (group 1, n=25) or a combination of fibrin sealant and quilting sutures (group 2, n=21) in the extended latissimus dorsi donor site. Outcome measures were obtained from the incidence, volume of postoperative seroma, total drainage amount, indwelling period of drainage, and duration of hospital stay. Results The incidence of seroma was 76% in group 1 and 42.9% in group 2 (P=0.022). We also found significant reductions in seroma volume (P=0.043), total drainage amount (P=0.002), indwelling period of drainage (P=0.01), and frequency of aspiration (P=0.043). The quilting sutures did not affect the rate of drainage, tube reinsertion, or hospital stay. Conclusions The use of quilting sutures combined with fibrin sealant on the latissimus dorsi flap donor site is helpful for reducing the overall seroma volume, frequency of aspiration, and total drainage amount.
The aim of the present study was to clarify the effect of kisspeptin-10 on LH secretion in prepubertal ewes. In experiment 1, prepubertal ewes fitted with indwelling jugular catheters were randomly assigned to receive 0, 0.5, 1 or 2 mg of kisspeptin-10 dissolved in saline, and serial blood samples were collected at 15-min intervals for 180 min to analyze the response curves of LH after injection. In experiment 2, prepubertal ewes fitted with indwelling jugular catheters were injected with 0 or 1 mg of kisspeptin-10 dissolved in saline and the injection was repeated 3 times at 1 h interval and serial blood samples were collected at 15-min intervals for 210 min to analyze the response curves of LH after injection. The results showed that single intravenous administration of 0.5, 1 and 2 mg of kisspeptin-10 all could significantly increased LH secretion in prepubertal ewes, and the effect of 1 and 2 mg of kisspeptin-10 on LH secretion was higher than that of 0.5 mg group. The results also showed that repeated intravenous administration of kisspeptin-10 could effectively increase LH secretion and repeated administration did not influence the effect of kisspeptin-10 on LH secretion in prepubertal ewe. In conclusion, the present study indicated that single or repeated intravenous administration of kisspeptin-10 could effectively increase LH secretion in prepubertal ewes.

Objective: The brain computed tomography(CT) is an important tool throughout the clinical course of chronic subdural hematoma(CSDH). In CT findings, the density of CSDH is different in each case. We analyze management options and results according to the density of CSDH. Methods: Seventy one patients with CSDH, who had been managed in our institute from August 2001 to December 2003, were reviewed retrospectively. The authors divided the patients into six groups according to the density of hematoma; Group A-hypodense rather than the brain parenchyma, group B-isodense, group C-hyperdense, group D-mixed with hypodense and hyperdense, group E-mixed with isodense and hypodense and group F-mixed with isodense and hypersdense. In each group, the operation method, the duration of the indwelling catheter and prognosis were analyzed. Results: The patients who showed mixed density were 39 and, those who showed single density were 32. All the patients underwent burr hole drainage for the primary choice. There was only two cases that needed additional craniotomy. The average duration of indwelling catheter was $5.40{\pm}1.91$ days. Statistically the duration was not different in each group(p-value<0.05, $x^2$ test). Three cases recurred, one in group C, another in group D, and the last in group E. It had no statistical significance due to low incidence of recurrence. Conclusion: We conclude that burr hole drainage is an acceptable primary treatment option for CSDH even though the density of hematoma is different in every single case.
This study was performed in order to evaluate the risk factors for nosocomial urinary tract infection and the frequencies of organisms isolated, and to provide the epidemiologic and basic data of hospital acquired urinary tract infection in intensive care unit. A prospective analysis was performed with 1,235 urine samples following urinary bladder catheterization in 569 patients, who had no evidence of UTI at the time of catheter insertion, admitted to intensive care unit in Pusan P hospital between June 1997 and May 1998. To identify risk factors for UTI, clinical characteristics of infected patients were analyzed. We analyzed these data by percentage, chi-square and odd ratio. Obtained results were as follows: A total of 569 patients (male 341 and female 228) were an average age of 50.8 years and catheterization of 8.04 days. Incidence of UTI was 16.1% (199/1,235) and The risk factors of UTI were duration of catheterization over 7 days, no use of systemic antibiotics, summer and female, and During the first 7 days these risk factors were no use of systemic antibiotics, summer, place of first catheter insertion (ICU) and type of intensive care unit (NSICU). A total of 220 the isolated strains were Gram negative rod 83 (37.7%), yeast like fungi 74 (33.6%) and Gram positive cocci 63 (28.6%). The common organisms isolated were Enterococcus faecalis 23 (10.5%), Serratia marcescens 19 (8.6%), Pseudomonu spp.17 (7.7%), E. ooh 16 (7.3%), Staphylococcus epidemidis 11 (5.0%) mdklebsiellapneumoniae 8 (3.6%). Therefore, in these results 199 of 569 (35%) patients in ICU with indwelling urinary catheter developed UTI. The risk factors for UTI are prolonged duration of catheterization, no use of systemic antibiotics, summer, and female.
The Purpose of this study is for clinical nurses to be aware of the significance of prevention against bacteriuria caused form foley catheterization, through probing variables related to the occurrence of bacteriuria which appears as the most frequent occurring infection, and try to lower bacteriuria by applying to nursing care at the clinical. For this study 46 patients with catheterization and the closed drainage system were sampled and investigated from among patients at Intensive Care Unit in Two hospitals affiliated to K University in Seoul. Those patients sampled had not shown bacteriuria before foley catheterization. The research design is to explore the effectiveness of prevention against bacteriuria in accordance with bladder irrigation and no bladder irrigation. Especially, the frequency of occurrence of bacteriuria examined so as to compare the effectiveness of bladder irrigation depending on the type of foley catheter between 2-lumen foley catheter and 3-lumen foley catheter. The results Were as follows. 1. The occurrence of bacteriuria in patient with bladder irrigation was 21.7%, while in patient without bladder irrigation 26.1%. 2. The occurrence of bacteriuria in patient without bladder irrigation according to duration of Indwelling catheter, was 4.3% after 48 h, 8.7% after 72 h and 21.7% after 96 h. 3. In case of 2-lumen foley catheter the occurrence of bacteriuria in patient with bladder irrigation was 0% after 72 h, and 4.35% after 96 h. In case of 3-lumen foley catheter, the occurrence of bacteriuria in patient with bladder irrigation was 13% after 48 h, 8.7% after 72 h and 4.3% after 96 h. 4. The occurrence of bacteriuria according to duration of catheterization was 8.7% after 48 h, 8.7% after 72 h and 15.2% after 96 h. In sex, female was 35% and male 15.4%. 5. The occurrence of bacteriuria according to mental state was 15% in clear state, while 29.6% in mental disorder. 6. In regard to a kind of microorganism induced bacteriuria, Gram negative bacteria was 63.7%, Gram positve bacteria 36.3%.
Purpose: The purpose of the non-equivalent control group pretest-posttest study was to test the effects of self-directed nursing practice using peer-tutoring, on the level of confidence, performance and learning satisfaction of nursing students in practicing core nursing skills. Methods: The data were collected from 80 junior nursing students at a nursing college in Daejeon before and after 3 weeks of intervention on practicing six core nursing skills, according to the highest order of priority. The subjects were divided into the peer-tutoring practice group (PTPG, n=40) and the lecturer-guided practice group (LGPG, n=40). The data were analyzed using mean, frequency, and t-test. Results: The PTPG scored statistically higher than the LGPG on the performance of 'indwelling urinary catheterization' and 'wearing protection equipment while entering quarantine room & disposing waste'. The PTPG scored statistically higher than the LGPG on the confidence of 'indwelling urinary catheterization' and 'inserting intravenous catheterization'. The PTPG scored statistically higher on learning satisfaction than LGPG. Conclusion: The results showed that self-directed nursing practice using peer-tutoring could be effective for nursing students in improving proficiency in core nursing skills and might be applied to core nursing skills training.
Objectives: The aim of this study is to describe the effects of treatment with traditional Korean medicine on a patient with voiding dysfunction caused by cerebral infarction. Methods: The patient was treated with herbal medicine, moxibustion, acupuncture, and electroacupuncture. The effects of these treatments were evaluated using the Korean version of the Modified Barthel Index and a post-void residual volume. Results: In this study, the Korean medicine Dangguishaoyao-san-hap-Jeoryeong-tang resulted in improvements in this patient with post-stroke urinary retention and successful removal of the indwelling catheter. Conclusion: The results suggest that traditional Korean medicine may be effective in ameliorating dysfunction caused by cerebral infarction.
Purpose: This study aimed to identify the colonization rate of carbapenem-resistant Enterobacteriaceae (CRE), the characteristics of CRE isolates, and risk factors for CRE colonization in patients transferred to the general wards of a small/medium-sized hospital. Methods: This retrospective study was conducted on patients who underwent CRE culture tests within 24 hours of admission among patients transferred to a small/medium-sized hospital. Forty-seven patients confirmed as positive for CRE were classified as belonging to the patient group. For the control group, 235 patients (five times the number of the patient group) were matched by sex, age, and diagnosis, and then selected at random. Data were analyzed using descriptive analysis and multiple logistic regression analysis. Results: The CRE colonization rate was 5% (47 out of 933 patients), and Klebsiella pneumoniae (68.0%) was the most common isolate of CRE. The positivity rate of carbapenemase-producing Enterobacteriaceae was 61.7%. The risk factors for CRE colonization included renal disease (odds ratio [OR]=4.93; 95% confidence interval [CI], 1.49-16.31), heart disease (OR=3.86; 95% CI, 1.35-11.01), indwelling urinary catheters (OR=4.43; 95% CI, 1.59-12.36), and cephalosporin antibiotic use (OR=8.57; 95% CI, 1.23-59.60). Conclusion: Having a comorbid renal or cardiac disease, an indwelling urinary catheter, or a history of exposure to cephalosporin antibiotics could be classified as risk factors for CRE colonization in patients transferred to small and medium-size hospitals. It is necessary to perform active infection control through proactive CRE culture testing of patients with risk factors.
배경 : 단심실 교정을 시행함에 있어 수술 전 위험 인자가 많지 않다고 판단되는 경우에도 수술 후 장기간의 흉관 배액, 단백 소모성 장질환, 폐혈관 색전증, 사망 등의 불량한 결과를 얻을 수 있다. 이러한 측면에서, 단심실 교정에 대한 기존의 위험 인자 분석은 수술 결과를 예측함에 있어 미흡한 점이 있다고 할 수 있다. 저자 등은 폐혈관 유순도를 새로이 정의하고, 낮은 폐혈관 유순도가 수술 후 흉관 배액 기간을 길게 한다는 가설을 세워 이를 증명하고자 하였다. 대상 및 방법 : 2002년 1월부터 2005년 5월까지 심장 외 도관을 이용한 단심실 교정을 받은 총 96 명의 환자들의 기록을 후행적으로 분석하였다. 동 기간 중 기존의 단심실 교정을 심장 외 도관으로 교체한 경우는 연구 대상에서 제외하였다. 수술 후 늑막 삼출 기간의 위험 인자 분석에는 12가지 수술 전 위험 인자들을 지수화한 Fontan risk score (FRS) 및 기타 다양한 수술 전, 수술 중 위험 인자들을 포함시켰으며, 본 연구를 위하여 전기로 analogue를 폐순환에 적용하여 계산된 폐혈관 유순도 (pulmonary vascular compliance, PVC, $mm^2/mmHg/m^2)$를 위험인자로 추가하였다. 전기 회로 analogue에 의하면 PVC는 폐동맥 지수 (pulmonary artery index, $mm^2/m^2$)를 총폐저항 (total pulmonary resistance, Wood $Unit{\cdot}m^2$) 및 폐 혈류량 (pulmonary blood flow, $L/min/m^2$) 으로 나눈 값으로 정의되며, 이는 폐혈관의 크기와 저항, 폐 혈류량 등을 동시에 고려하는 변수라고 할 수 있다. 결과 변수인 흉관 거치 기간은 자연로그를 취해 정규 분포화하고 이를 log indwelling time (LIT)으로 정의하였으며, 분석 대상 위험 인자들과 LIT 의 관계에 대한 다중 선형 회귀분석을 시행하였다. 결과 : 조기 사망은 없었고 만기 사망은 4 명 (4.2%)이었으며, 단심실 교정시 fenestration이 추가된 경우는 1예 있었다(1 %). 수술 전 PVC, 흉관 거치 기간, LIT는 각각 ${6{\sim}94.8\;mm^2/mmHg/m^2}$ (중간값:24.8), $3{\sim}268$일 ( 간값 : 20 일 ), $1.1{\sim}5.6$ ( 평균: 2.9, 표준 편차: 0.8) 이었다. 단변 수 분석상 FRS, PVC, 체외 순환시간 (CPB) 및 술 후 12 시간째의 중심 정맥압 등이 LIT와 연관되었으나, 다변수 분석상 PVC (p=0.0018) 및 CPB (p=0.0024)만이 독립적으로 LIT를 예측하였다. 두 변수는 LIT 변이에 대하여 21.7%의 설명력이 있었으며, 두 변수를 이용한 회귀 분석식은 다음과 같았다. LIT=2.74-0.0158 PVC+0.00658 CPB. 결론: 새로이 정의된 폐혈관 유순도는 심장 외 도관을 이용한 단심실 교정 후의 흉관 거치 기간을 결정하는 중요한 예측 인자로서, 수술 전 위험 인자 분석에 유용하게 사용될 수 있다.
