본 연구의 목적은 만성신부전환자의 혈액투석 후 삶의 재구성 경험을 이해하고, 그들의 생생한 경험의 의미를 밝히기 위함이며, 연구 방법은 질적연구 방법 중 Colaizzi의 현상학적 연구방법을 이용하였다. 연구대상은 만성신부전증을 진단받고 혈액투석 중인 8명의 환자를 대상으로 개별 심층면담을 통해 자료수집을 진행하였다. 연구결과 도출된 5개의 범주는 '예기치 못한 어려움의 시작', '혈액투석이 가져온 생존의 버거움', '재기의 원동력', '내일을 살기 위한 오늘의 선택과 집중', '정성껏 엮어가야만 하는 일상'으로 나타났다. 본 연구결과를 통해 의료인들은 만성신부전환자가 혈액투석을 하면서 경험하는 신체적, 정신적 고통과 변화된 삶의 방식에 적응하기 위하여 그들이 부단히 노력하는 방법을 심층적으로 이해하는데 도움이 될 것이다. 또한 향후 혈액투석 환자의 건강관리를 위한 효율적인 간호중재를 개발하는 데 기초자료가 될 수 있을 것이라 생각된다.
Objectives: This study was performed to investigate the effect of cold stress on heart rate variability and peripheral blood flow by Doppler ultrasound in healthy subjects. Methods: We classified the subjects by their Yin or Yang tendency using a Yin-Yang questionnaire. Cold stress was taken on the left hand of subjects for 10 seconds. Heart rate variability and peripheral blood flow were measured on the right radial artery before and after cold stress. Results: The Yang tendency group showed a significant decrease of heart rate after cold stress which was not seen in the Yin tendency group. The Yang tendency group showed a decrease of LF, total power, and increase of HF after cold stress, while the Yin tendency group showed the opposite. However, there were no statistically significant differences of heart rate variability analysis between the results of the two groups before and after cold stress. Recovery time of blood flow velocity by ultrasound Doppler was significantly different between the Yin and Yang tendency groups after cold stress. Conclusion: These results suggest that people with Yin or Yang tendency may show different responses in the autonomic nervous system. Further modified studies may include the responses to various manipulation techniques in acupuncture treatment and the individual responses according to pattern identification in traditional Korean medicine.
본 연구는 당뇨병 환자를 대상으로 인슐린 치료에 대한 문화적 관점과 경험을 탐색하기 위해 19명의 제1형과 2형 당뇨병 환자를 대상으로 포커스 그룹 인터뷰를 실시하였다. 인터뷰 내용을 기록하고 녹취한 후 내용분석을 통해 당뇨병 환자들이 경험하는 인슐린 치료에 대한 사전인식요인과 장애요인 및 촉진요인을 목록화하였으며 이들 3가지 목록에 포함되는 주요어를 도출하였다. 그 결과 인슐린에 대한 막연한 두려움이 사전인식요인의 주요어로 확인되었고 장애요인으로는 인슐린과 관련된 걱정(통증, 낮은 지식과 문해력, 자가 주사에 대한 낮은 자신감, 경제적 부담, 저혈당에 대한 두려움)과 의료진에 대한 양가감정, 의지하게 된 삶, 친구, 친척과 가족에 대한 감정, 불편감, 과거에 대한 후회, 부끄러움이 주요어로 도출되었다. 촉진요인으로는 깨달음과 신체적 회복 및 혈당조절에 대한 자신감이 확인되었다. 본 연구결과를 토대로 국내 당뇨병 환자의 인슐린 치료에 대한 경험과 지각에 대한 의료인의 이해가 필요하며 당뇨병 환자의 이러한 경험은 존중되어야 할 것으로 사료된다.
본 연구는 어릴 때 형성된 성격이 현실의 생활과 성장에 도움이 되지 않을 때, 이성과 감성을 통합하는 대화훈련을 통하여 자율적인 인간으로 재탄생하게 하는 프로그램을 만드는데 연구의 목적을 두었다. 연구방법은 현재 자신의 성격을 5가지 자아 상태로 분석하고, 그 결과를 수정 보완하는 방법을 찾는 것이다. 연구결과는 외적으로 나타난 자아 상태를 자아 상태 활성화기법으로 보완하는 방법을 알게 하고, 내적인 부분은 교류분석(TA)변화의 궁극적 목표인 삼위일체의 변화 즉, 자각성, 자발성, 친화성의 3가지 능력의 변화를 통해 자율성을 달성하게 하는 것이다. 이는 개인의 모든 자원을 사용하여 문제점을 해결하는 능력으로 해방시키거나, 회복을 실제 행해보이게 하는 것이다.
This is a nonequivalent control group pretest- posttest design in quasi experimental basis to assess how the thermal head and feet support influences on early stage weight gain in premature neonates. The clients were selected among the premature infants with the weight under 2000gms and with the gestational period under 37 weeks, admitted over 15 days in K university hospital, Seoul. The control group of 29 were selected from January 1998 to August 1998, who were without head and feet thermal support, and the experimental group of 30 were selected from September 1998 to May 1999, who were with the two thermal interventions. The results of the study follow, 1.The experimental group with thermal interventions showed more weight gain than the control group without thermal interven- tions, which was statistically significant between the two groups. 2.The physiological weight loss after birth showed less in the experimental group than in the control group. 3.The recovery of the birth weight after the physiologic weight loss showed no statistical difference between the experimental group and the control group. Consequently, the thermal head and feet supportive nursing intervention could be applied as a nursing intervention program to help the premature neonates' development. With the results above we should like to suggest the following: 1) A continuous application in the practice of our thermal supportive intervention for the premature neonates, a development of the content through evaluation, and a comparision of the results through a long time study. 2) A neccessity of deveopment of various study and cross comparision. 3) A neccessity of multi-angular study on the premature infants' characteristics influencing the thermal therapy and the study of the individual differences of the clients.
Korean medicated diet (KMD) is not a simple combination of food and Chinese drugs, but a special carefully constructed diet made from Sasang constitutional medicines, food and condiments under the theoretical guidance of diet preparation based on differentiation of symptoms and signs of traditional Sasang medicine (TSM). It combines the functional efficacy of medicine with the delicacy of food, and can be used to prevent and cure diseases, build up one's health and prolong one's life. Korean traditional medicated diet has a long history of development. Although influenced by Chinese medicine, Korean traditional medicine has been developed into a unique system of traditional medicine that has surpassed the continental medical practice, sublimating itself into a native medical practice suitable to Korean lifestyles and physical constitutions. In the 19th century, Lee Jema's Sasang medicine (medicine of four types of energy determining the physical constitution) was introduced. It is an integration of mind and body according to the individual's physical constitution that is categorized allowing a customized method of treatment ideal for each category-making the content of Korean traditional medicated diet even richer. The characteristics of Korean medicated diet are as follows: (1) Laying stress on the wole, selecting medicated diet on the basis of differential diagnosis. (2) Suitable for prevention and treatment, outstanding in effect. (3) Good in taste, convenient for taking. KMD refers to drink and food according to certain prescriptions, by processing and cooking that can be used either for prevention and cure of diseases, or for health care and recovery. The purpose of this review is to introduce TSM and KMD based on Sasang constitutional medicines.
A clinical trial was performed to assess clinical efficacy and/or reduction in relapses by gold-bead implantation into acupuncture points in dogs with canine chronic recurrent otitis externa (CCROE). Forty dog-patients randomly divided into two groups were diagnosed as suffering CCROE, having intact tympanic membranes and a history of recurrences. Treatments were: control group (CG), treated with commercially available antimicrobial otic droplets, dosed twice daily for 7 days; and experimental group (GBI- gold bead implants), treated as for CG plus the insertion of 13 gold-bead implants under light anesthesia. Overall per cent assessment of composite clinical progression and progression of individual clinical signs were recorded. Bilateral chronic external otitis was diagnosed in 60% of the cases and left or right otitis in 20% of the cases each. Logistic model for repeated measures analysis showed that GBI induced a better clinical recovery as far as lesion score of some clinical signs is concerned. The overall percent cures of each group showed statistically significant difference. A McNemar analysis revealed that higher number of relapses was observed in CG patients as compared to the GBI (P < 0.05). In particular during these days, lesion on the pinna showed in odd ratios analysis a 7:1 ratio (recurrence CG:GBI) and ear wax/pus (4:1) from D42 to D365. It is concluded that gold-bead implantation into acupuncture points improves resolution of some clinical signs and greatly reduce relapses in CCROE affected dogs after 1 year follow-up (98.75%).

Purpose: Cardiotoxicity is a serious late complication of breast cancer treatment. Individual treatment risk of specific drugs has been investigated. However, studies on the evaluation of the composite risk of chemotherapeutic agents are limited. Methods: We retrospectively analyzed the medical records of breast cancer patients who received adjuvant treatment and had available serial echocardiography results. Patients were assigned to subgroups based on chemotherapy containing anthracyclines (A), anthracyclines and taxanes (A+T), and radiotherapy (RT). The development of cardiac disease and serial ejection fraction (EF) were reviewed. EF decline up to 10% from baseline was considered grade 1 cardiotoxicity and EF decline >20% or absolute value <50% was considered grade 2 cardiotoxicity. The most recent medical records and echocardiography results over 1 year of chemotherapy completion were also reviewed. Late cardiotoxicity was defined as a lack of recovery of EF decline or aggravated EF decline from baseline. Results: In total, 123 patients were evaluated. A small reduction in EF was observed after chemotherapy in both chemotherapy groups. There were no significant differences between groups A and A+T in EF decline following chemotherapy. We could not find any differences in composite risk between the chemotherapy groups and the RT group during follow-up. Late cardiotoxicity was seen in 15.45% of patients. During follow-up, three patients were diagnosed with dilated cardiomyopathy. Conclusion: There was no significant composite risk elevation following adjuvant treatment of breast cancer. However, late cardiotoxicity was considerable and further research in this direction is necessary.
Salvia miltiorrhiza Radix is cultivated in Korea and China and is traditionally used to treat cardiovascular diseases. In this study, we developed and validated a quantitative analysis method for S. miltiorrhiza Radix using high-performance liquid chromatography (HPLC). Identification was performed using ultra performance liquid chromatography-tandem mass spectrometry. For quantitative analysis, we used seven marker compounds. Separation conditions for HPLC were optimized using an ODS column with gradient conditions of 1% formic acid in distilled water and 1% formic acid in acetonitrile, with a flow rate of 0.8 mL/min and a detection wavelength of 280 nm. This method showed good linearity ($R^2=0.9998$), precision (relative standard deviation ${\leq}3.3%$), accuracy (recovery of 94.16-102.89%), limit of detection ($7.53{\mu}g/mL$), and limit of quantification ($23.71{\mu}g/mL$). This approach successfully quantified marker compounds in S. miltiorrhiza Radix. The individual marker compounds were identified by comparing the molecular masses and retention times with does standard compounds. Marker compound contents of S. miltiorrhiza Radix were investigated with different cultivation regions. Seven marker compounds were detected and quantified in all samples. Among them, salvianolic acid B showed the highest contents and it ranged from 4.13 to 7.15%. The salvianolic acid B content (7.15%) of marker compound was the highest in Bonghwa, and the tanshinone IIA content (1.90%) was the highest in Pohang. The results of marker compounds and developed method were intended to provide a favorable reference for the study of S. miltiorrhiza Radix from different regions of Korea.
이 연구의 목적은 네팔 지진을 경험한 생존자의 피해경험을 이해하고자 하는 것이다. 연구 참여자는 네팔 카트만두 텐트촌에 거주하고 있는 아동 5명, 부모 5명과 관계자 3명 총 13명으로 개별면담과 시각자료(그림, 사진), 기록물을 활용하여 사례 내 분석과 사례 간 분석을 실시하였다. 그 결과 '평범한 일상, 소소한 즐거움'(지진 이전의 생활), '생사의 갈림길에서 살아남'(지진 당시 상황), '수수 방관하는 정부, 팔 걷어붙인 외국인들'(지진 이후의 수습과정), '곳곳에 남아 있는 그날의 흔적들'(현재의 생활), '다시 일어설 수 있기를 바람'(미래에 대한 소망)으로 범주화하여 제시하였다. 분석 결과를 토대로 자연재난을 예방하거나 대처하고 생존자들을 지원하기 위해 필요한 정책과 실천적 지침들을 제언하였다.
