Study of the Density of Smoke and Harmful Gases of Adhesive Indirect Moxibustion (접착식 간접구의 연기 밀도 및 유해가스 발생 확인 연구)

  • Objectives: We measured the density of smoke and harmful gases emitted from burning adhesive indirect moxa. Through the test we aimed to find out if there was an excessive amount of smoke emitted and if it included harmful gases. Methods: 9 types of adhesive indirect moxa were chosen. The buffer layers which do not burn during treatment were removed and 10g of each moxa were made into powder and put into a holder. A smoke density chamber (Smoke Density Chamber FTT. U.K) and Fourier transform infrared spectroscopy (FT-IR I 4001. MIDAC U.S.A.) were used to measure the density of smoke and harmful gases emitted from burning moxa by ISO 5659-2 test. Results: The result of measuring maximum smoke density showed that the regular indirect adhesive moxa (A-F) emitted high density smoke of 172.1-291.4Ds. The smokeless moxas, Seoam moxas, emitted the least amount of 3.4-5.5Ds. Concentrations of 7 typical harmful gases (CO, HCl, HCN, HBr, HF, SO2, NOx) were measured and all of the moxas emitted CO due to incomplete combustion. 4 types of moxa emitted NOx and all smokless moxas emitted NOx. HBr, HCN, HCl, HF, SO2 were not found in any of the moxas. Conclusions: The amount of harmful gases emitted from burning moxa was much lower than short-term exposure standards of chemical and physical factors (Ministry of Labor 2010-44). Further experiments measuring gases from moxa combustion should be done in larger environments similar to normal medical clinics.

  • Objective : The purpose of this study is to investigate the mechanism and effect of moxibustion with monkshood cake, slice & black pepper cake. objectively, to be used as the quantitative data through the measurement of temperatqre, and to grasp the thermodynamic characteristics of moxibustion with monkshood cake, slice & black pepper cake. Methods : We have selected of the moxibustion with monkshood cake, slice & black pepper cake. indirect moxibustion. We make a comparative study of the thermodynamic characteristics of moxibustion with monkshood cake, slice & black pepper cake. We examined combustion times, temperatures, temperature gradients in each period during a combustion of moxa. Results & Conclusions : 1. We can design the moxibustion with monkshood cake that it has thermodynamic characteristics of 173sec effective combustion time, $44^{\circ}C$ maximum temperature, $0.22^{\circ}C/sec$ ascending maximum temperature, if we use 3mm thickness or 3mm and below of monkshood cake and the moxa cone is formed the conical shape that the base diameter was 8mm, the height was 10mm, the density was $600mg/cm^3$. 2. We can design the moxibustion with monkshood cake that it has thermodynamic characteristics of 205~271sec effective combustion period time, $44.6{\sim}46.1^{\circ}C$ maximum temperature, $0.18{\sim}0.24^{\circ}C/sec$ ascending maximum temperature, if we use 3mm thickness of monkshood cake and the moxa cone is formed the conical shape that the base diameter was 8mm, the height was 10mm, the density was $480{\sim}720mg/cm^3$.

Credibility of a Newly Developed Sham Moxibustion (피부 열전도 온도에 근거를 둔 거짓 뜸 개발 및 평가 연구)

  • Background : To demonstrate the efficacy of moxibustion therapy, randomized controled trials are required. But, clinical trials of moxibustion had limitations due to the absence of a sham moxibustion model for an appropriate placebo. Objectives : To develop a new sham moxibustion model based on the thermal characteristics of commercial indirect moxibustion, especially temperature, and to evaluate whether it could be applied in clinical trials. Methods : By applying heat insulation, we created a sham moxibustion device that was indistinguishable from a real one with the naked eye. It also stimulated heat but had inert remedial value. A clinical trial was performed on subjects to test double blinding. The subjects were randomly assigned into two groups, a treatment group and a sham group. Acupoint Zusanli($ST_{36}$) was used in each group for 3 times. A sham acupuncture credibility questionnaire was modified into a moxibustion credibility questionnaire and was filled out after treatment. Results : No major difference was detected in the subjects' baseline data. Most subjects and practitioners could not distinguish the sham moxibustion device from the real one. But, subjects who had experience of moxibustion therapy more likely to distinguish the sham moxibustion device from the real one than subjects who didn't have experience of moxibustion therapy. The treatment group showed a significant difference in the VAS(Visual Analog Scale) for intensity of sensation during treatment than that of the sham group. Conclusions : The sham moxibustion device in this study is proved sufficient and credible to be applied in investigations of the effect of moxibustion. But it is more appropriate for the people who don't have experience of moxibustion therapy.

A Combination of Herbal Medicine and Moxibustion Treatment is Effective for a Recalcitrant Cutaneous Warts in a Pediatric Patient: A Case Report

  • Although warts are a common skin condition/disease that recurs in childhood, treatment completion is difficult owing to the pain associated with conventional therapies. In this case, Korean herbal medicine, Guijakjihwang-Tang, and indirect moxibustion was used to treat a recalcitrant cutaneous wart without pain and recurrence. A 7-year-old boy presented with recurrent cutaneous warts on his right hand and received cryotherapy 3 times in the previous year. The wart recurred during the course of cryotherapy treatment, and the patient did not want to suffer any more pain from the previous treatment. Therefore, parents sought an efficient and painless therapy. The patient was treated for 2 months using Guijakjihwang-Tang combined with indirect moxibustion. The prominent lesion separated in the 2nd month of treatment and completely recovered without adverse events in the 3rd month. There was no recurrence over 12 months of follow-up. This report provides meaningful insights into the use of Korean medicine as a safe, painless and efficient treatment for warts in pediatric patients.

The Case Report about Six Cancer Patients Added Eriobotryae Folium Fomentation to Mitigate Pain (통증완화를 위해 비파엽 찜질을 병행한 암환자 6례에 대한 증례보고)

  • Objective: The purpose of this study is to see whether pain in cancer patients is decreased by Eriobotryae Folium Fomentation. Methods: We examined the change in pain among six cancer patients, prescribing Eriobotryae Folium Fomentation. As for evaluating methods, we used the case history of pain, the direct evaluation of pain, the indirect evaluation of pain. The direct evaluation of pain is subdivided by the language expression, the non-language expression, and Visual analog scale(VAS). The indirect evaluation of pain is subdivided by the condition of walking, mood and the condition of sleep. Results: After two weeks, any remarkable pain decrease is not observed in both of the direct evaluation and indirect evaluation.

Thermal Properties of Indirect Ceramic Moxibustion (온구기의 연소 특성에 관한 고찰)

  • 목적 : 뜸 연소 실험을 통해 간접구의 일종인 온구기의 연소 특성을 조사하였다. 방법 : 온구기를 유리판 위에 놓고 연소하여 50분간 온도 데이터를 수집하였다. 총 20개의 뜸봉을 연소하여 수집된 데이터를 분석하여 평균 온도, 최고 온도, 최고 온도 도달 시간, 최고 온도 지속 시간을 계산하였다. 또 혈류량을 증가시킨다고 보고된 온도($38^{\circ}C$)에 도달하는 시간, 그 이상의 온도로 지속되는 시간, C-fiber의 활성화를 통해 치료효과를 나타내는 것으로 보고된 온도($42-50^{\circ}C$)에 도달하는 시간과 그 범위 내의 지속시간을 계산하였다. 결과 : 평균 최고온도는 $50.4^{\circ}C$이고 평균 도달시간은 1,007초, 평균 지속시간은 27초였다. 평균최고 온도는 보고된 다른 간접구와 유사하였으나 평균 도달시간은 더 길었다. $38^{\circ}C$에 도달하는 평균시간은 410초이고 지속 시간은 1,813초였다. $42-50^{\circ}C$에 도달하는 평균시간은 521초이고 지속시간은 990초였다. 결론 : 온구기는 연소 특성이 보고된 다른 종류의 뜸보다 더 오랜 시간 동안 안정적인 열자극이 가능하여, 뜸치료의 임상 활용의 폭을 높일 수 있을 것으로 사료된다.

Literature Review on Research Methodology Assessing Properties of Moxibustion (뜸의 특성 연구 방법론에 대한 문헌고찰)

  • Objectives : The aim of this review was to summarize and assess researches reporting thermal properties of moxibustion. Methods and Materials : We searched one electronic database(PubMed) and two journals(The Journal of Korean Medicine and The Journal of Korean Acupuncture & Moxibustion Society) and for the articles published until Nov. 2012. The articles reporting thermal characteristic of moxibustion were selected and reviewed. 31 articles were included and assessed in this review. The type of moxibustion, the material beneath moxbustion, main outcome and the amount of information were reviewed. The amount of information was defined as 1 for one main outcome reported in each article. Results : Direct moxibustion were used in 15 articles and indirect moxibustion were used in 19 articles. 11 researches used anti-heating plate for the material beneath moxibustion, one article used steel ring, agar with anti-heating plate, six articles used skin and another six articles used heater. Two articles used animals and another two articles used metals. The mean value of the amount of information was $4.41{\pm}2.94$(SD). Conclusions : Research articles assessing properties of moxibustion have been consistently published since 1993, and employed various methodologies. The amount of information has been associated with four section analysis and is currently decreasing. Research assessing properties of moxibustion with clinical effectiveness is needed.

The Review on the Domestic Studies of Moxibustion (최근 10년간 국내 뜸관련 연구에 대한 고찰)

  • Objectives : This study is to research the trend of the Moxibustion-related studies and to establish the hereafter direction for the study on Moxibustion. Methods : We reviewed the domestic articles published last ten years(2000~2009). Results : We have searched 45 articles in 4 journals of Korean Acupuncture & Moxibusion, Korean Oriental Medicine, Korean Meridian & Acupoint, Korean Oriental Internal Medicine. 1. There were the most research done 2003, 2008. Both of them have 8 articles. In other years there were 2 or more articles. 2. The pattern of study was as follow: the clinical studies were 23, studies of phisical Characteristics and new moxibustion method were 9, the philological study were 5, the reviewed studies were 3, the experimental studies were 2 and the others were 5. The rate of clinical studies was increased. 3. In the clinical studies, the case-control study and non case-control study were more than case study. The disease after stroke were the most predominant. The rates of direct moxibustion and indirect moxibusion were similar. 4. After beginning 2000s, there were more studies of thermodynamic Characteristics. In the mid 2000s, there were more studies of new moxibustion method appearing. Conclusions : We need more studies about various kinds of diseases, side effect and aftereffects. There should be convenient and useful methods of moxibustion.

Recent Research Trends in Moxibustion Treatment in Korea

  • A total of 1,235 studies were retrieved on June 23, 2019, from 3 databases. Selected 59 studies were evaluated by year of publication, study type, subject condition/disease, acupoint, standards for reporting interventions in clinical trials of moxibustion (STRICTOM), Cochrane risk of bias (RoB), and risk of bias assessment tool for non-randomized study (RoBANS). Most studies were conducted in 2011, after which the number of studies decreased. The most common study type was 25 case reports (CR), 16 uncontrolled clinical trials (UCT), 11 randomized controlled trials (RCT), and 7 controlled clinical trials (CCT). Moxibustion treatment was mainly used for musculoskeletal and circulatory diseases/conditions. A total of 83 acupoints were used, A-shi points being the most used. As for STRICTOM, an average of 7.4 items were satisfactory for UCT and CR without a control group, and an average of 9.4 items were satisfactory for RCT and CCT. RCT was assessed using the RoB, and many items were rated as uncertain. In this study, the need for RCT of moxibustion treatment in Korea was identified. The detailed description of study methods and results will provide evidence for the efficacy of moxibustion treatment in preventive and therapeutic aspects of Korean traditional medicine.

Experimental Study on the Characteristics of Combustion in the Indirect Moxibustion with Ginger (격강구(隔薑灸)의 연소(燃燒) 특성(特性)에 관한 실험적(實驗的) 연구(硏究))

  • Objective : The purpose of this study is to investigate the mechanism and effect of moxibustion with ginger objectively, to be used as the quantitative data through the measurement of temperature, and to grasp the thermodynamic characteristics of moxibustion with ginger. Methods : We have selected the indirect moxibustion with ginger among many indirect moxibustions. We produced a slice of ginger to a thickness of 3, 4, 5mm and the moxa cone having a diameter of 8mm, a height of 10mm for making a comparative study of characteristics of moxa cone according to change the density. We have made a comparative study of the thermodynamic characteristics of moxibustion with ginger with or without holes. We measured combustion times and calculated temperatures, temperature gradients in each period during a combustion of moxa. Results : 1. We found out it was not significantly influenced by the existence of the punched holes in a slice of ginger because the punched holes grow smaller immediately. 2. The duration of the preheating period became longer according to thickness of a slice of ginger and was not directly proportional to the density of moxa cone. The duration of the preheating period was extremely short when it burned a 100mg moxa cone. That was influenced by the density of moxa cone. 3. The duration of the heating period became longer according to thickness of a slice of ginger likewise the preheating period bacause the density of moxa cone had effected on the combustion characteristics. The duration of the heating period was extremely long when it burned a 100mg moxa cone. On the other hand the maximum temperature in the heating period was appeared that the combustion with a thin slice of ginger was highest and measured that the large density of moxa cone was higher. But the maximum temperature in the heating period was about $37.8^{\circ}C{\sim}44.2^{\circ}C$respectively lower in others. 4. The duration of the retaining period was some doubling shorter than that of the heating period that is concerned the shape of moxa cone. The temperature measured the close of a retaining period was $36.6^{\circ}C{\sim}41.8^{\circ}C$, that was considerably lower temperature. 5. The mean ascending temperature velocity and the mean descending temperature velocity were $0.042{\sim}0.073^{\circ}C/sec$, $-0.027{\sim}-0.064^{\circ}C/sec$ respectively. Then, the ascending temperature velocity was some faster than the ascending temperature velocity. Conclusions : The quantitative standard for obtaining the effective heating stimulation is that if the slice of ginger made a hole in it, we had to use the needle above 1.5mm diameter. The recommended size of a slice ginger is the 14mm diameter and the 2~3mm thickness. The moxa cone is formed the conical shape that the base diameter was 8mm, the height was 10mm, the density was $600mg/cm^2$.
