Correlation between Ki67 and Histological Grade in Breast Cancer Patients Treated with Preoperative Chemotherapy

  • Background and Aim: Breast cancer (BC) is a heterogeneous disease and cell proliferation markers may help to identify subtypes of clinical interest. We here analyzed the correlation between cell proliferation determined by Ki67 and HG in BC patients undergoing preoperative chemotherapy (PCT). Materials and Methods: We obtained clinical/pathological data from patients with invasive BC treated at our institution from 1999 until 2012. Expression of estrogen receptor (ER), progesterone receptor (PR), epidermal growth factor receptor type 2 (HER2) and Ki67 were determined by immuno-histochemistry (IHC). Clinicopathological subtypes were defined as: Luminal A, ER and/or PR positive, HER2 negative, HG 1 or 2; Luminal B, ER and/or PR positive, HER2 negative or positive and/or HG 3; triple negative (TN), ER, PR and HER2 negative independent of HG; HER2 positive, ER, PR negative and HER2 positive, independent of HG. By using Ki67, a value of 14% separated Luminal A and B tumors, independently of the histological grade. We analyzed correlations between Ki67 and HG, to define BC subtypes and their predictive value for response to PCT. Results: 1,560 BC patients were treated in the period, 147 receiving PCT (9.5%). Some 57 had sufficient clinicopathological information to be included in the study. Median age was 52 years (26-72), with 87.7% invasive ductal carcinomas (n=50). We performed IHC for Ki67 in 40 core biopsies and 50 surgical biopsies, 37 paired samples with Ki67 before and after chemotherapy being available. There was no significant correlation between Ki67 and HG (p=0.237), both categorizing patients into different subtypes. In most cases Ki67 decreased after PCT (65.8%). Only 3 patients had pathologic complete response (cPR). Conclusions: In our experience we did not find associations between Ki67 and HG. Determination of clinicopathological luminal subtypes differs by using Ki67 or HG.

Reliability and Validity of the Alcohol Use Disorders Identification Test - Consumption in Screening for Adults with Alcohol Use Disorders and Risky Drinking In Japan

  • Background: Alcohol is well established as a risk factor for cancer development in many organ sites. To assess the reliability and validity of the Alcohol Use Disorders Identification Test - Consumption (AUDIT-C) for detecting alcohol use disorders or risky drinking in Japanese adults the present study was conducted. Materials and Methods: A test-retest method was applied with a 2-week interval with 113 health care employees. The k coefficient, Cronbach's coefficient alpha, Spearman's correlation coefficient, and intraclass correlation coefficient (ICC) were determined and the validity of the AUDIT-C was analyzed using the data from a nationwide survey on adult alcohol use conducted in 2008 (n=4,123). Results: The reliability of the AUDIT-C score was high (${\kappa}$ coefficient=0.63, Cronbach's alpha=0.98, correlation coefficient=0.95, and ICC=0.95). According to the likelihood ratio and Youden index, appropriate cutoffs for the AUDIT-C were ${\geq}5points$ in men and ${\geq}4$ points in women. The sensitivity and specificity of these cutoffs for identifying ${\geq}8$ points on the AUDIT were 0.88 and 0.80, respectively, for men (positive likelihood ratio [LR+]=4.5) and 0.96 and 0.87, respectively, for women (LR+=7.7). The sensitivity and specificity of the cutoffs for identifying ${\geq}12$ points on the AUDIT were 0.90 and 0.84, respectively, for men (LR+=5.8) and 0.93 and 0.94, respectively, for women (LR+=15.8). The sensitivity and specificity of the cutoffs for identifying ${\geq}16$ points on the AUDIT were 0.93 and 0.80, respectively, for men (LR+=4.7) and 0.92 and 0.98, respectively, for women (LR+=55.6). With higher scores on the AUDIT, the specificity decreased and false-positives increased. The appropriate cutoffs for identifying risky drinking were the same for both genders. Conclusions: The reliability and validity of the AUDIT-C are high, indicating that it is useful for identifying alcohol use disorders or risky drinking among the general population in Japan, a group at high risk of cancer development.

대용량 XML 문서의 키워드 검색을 위한 레이블링 기법 (A Labeling Methods for Keyword Search over Large XML Documents)

  • XML 문서가 점차 복잡해지면서 XML문서의 구조를 알 필요 없이 키워드로만 검색을 하는 키워드 검색 방식이 많이 사용되고 있다. XML문서 내에서 키워드 검색 방식을 사용하기 위해서는 문서 내의 모든 키워드에 레이블을 부여해야 하며, 구조적인 정보 또한 레이블 내에 충분히 표현해야한다. 하지만 기존 레이블링 방법들은 색인을 위한 단순정보만 레이블링 하거나, 증가하는 XML문서의 크기에 대응하기 어려운 형태로 구조적인 정보를 표현한다. 이는 XML문서가 커질수록 키워드검색성능이 떨어지거나, 공간 사용량이 기하급수적으로 증가하는 문제를 야기한다. 따라서 본 논문에서는 대용량 XML문서에 대한 키워드 검색 시 기존 레이블링 방식이 가지고 있던 문제점을 보완하는 새로운 레이블링 방식인 RPLS(Repetitive Prime Labeling Scheme)을 소개한다. 이 방법은 기존 소수 레이블방식을 개선하여 상위 레벨의 소수를 하위 레벨에서 반복 사용할 수 있도록 하여 레이블링을 위해 생성해야하는 소수의 수를 감소시키도록 한 것이다. 본 논문에서는 대용량 XML 문서의 키워드검색에 대한 RPLS 스킴의 효율성 검증을 위해 기존 레이블링 기법들과의 성능 비교 실험 결과도 제시한다.

Combined effects of dietary zinc at 3 years of age and obesity at 7 years of age on the serum uric acid levels of Korean children

  • BACKGROUND/OBJECTIVES: To assess the longitudinal associations of the antioxidant capacity of zinc and body mass index (BMI) with serum uric acid (SUA) in South Korean children. SUBJECTS/METHODS: Using follow-up data from the Ewha Birth and Growth Cohort, we included subjects who were seen at 3 and 7 years of age (n = 183; 90 boys, 93 girls). Daily zinc intake and BMI were assessed at 3 and 7 years of age. SUA measured at 7 years was used as the outcome variable. Using a general linear model, the effects of dietary zinc intake and BMI on SUA were assessed. We also assessed the combined effect of early dietary zinc intake and BMI on SUA in children. RESULTS: The dietary zinc intake at 3 years of age was negatively correlated (ρ = -0.18, P = 0.04), whereas the BMI at 7 years of age was positively correlated (r = 0.18, P = 0.01), with the SUA level at 7 years of age. The dietary zinc intake level at 3 years of age and the BMI level at 7 years of age were, together, significantly related to SUA in children at 7 years of age. SUA was lower in group 1 (normal-weight, high-zinc group) than in the other two groups (group 2: normal-weight, low-zinc and overweight, high-zinc group; and group 3: overweight, low-zinc group). Our results demonstrate the combined effect of zinc intake and BMI on SUA. The combined association remained significant in both the crude and adjusted models (P < 0.01). CONCLUSIONS: SUA was related to combined BMI and dietary zinc intake, and increased zinc intake and normal body weight had a beneficial effect on reducing SUA in children.

대용량 데이터베이스에서 다차원 인덱스를 사용한 효율적인 다단계 k-NN 검색 (Efficient Multi-Step k-NN Search Methods Using Multidimensional Indexes in Large Databases)

  • 본 논문에서는 다차원 인덱스 기반 다단계 k-NN 검색의 성능 향상 문제를 다룬다. 기존 다단계 k-NN 검색에서는 고차원 객체의 저차원 변환으로 인한 정보 손실로 k-NN 질의 결과 매우 큰 허용치(검색 범위)가 결정되어 범위 질의 결과로 많은 후보가 검색된다. 또한, 많은 후보는 후처리 과정에서 매우 많은 I/O 및 CPU 오버헤드를 발생시킨다. 본 논문에서는 이와 같은 고찰에 기반하여 범위 질의의 허용치를 줄여 후보 개수를 줄이고 이를 통해 성능을 향상시키는 방법을 제안한다. 먼저, k-NN 질의 결과로 결정된 허용치를 고차원 및 저차원 객체간 거리 비율로 강제 축소하여 범위 질의에 사용하는 허용치 축소 (근사적) 해결책을 제안한다. 다음으로, k-NN 질의 계수 k 대신 c k 를 사용하여 얻은 보다 타이트(tight)한 허용치로 범위 질의를 수행하는 계수 제어 (정확한) 해결책을 제안한다. 실제 객체 데이터를 사용하여 실험한 결과, 제안한 두 가지 해결책은 기존 다단계 k-NN 검색에 비해 후보 개수와 검색 시간 모두를 크게 향상시킨 것으로 나타났다.

흑미 호분층 추출물의 급여가 난소절제 흰쥐의 체내 지방, 혈중 지질 및 관련 호르몬 수준에 미치는 영향 (Supplementary Effects of Black Rice (Oryza sativa L.) Aleurone Layer Extract on Body Fat, Serum Lipid, and Serum Hormone Levels in Ovariectomized Rats)

  • 본 연구는 흑미(Oryza sativa L.) 호분층 추출물이 난소절제 흰쥐의 체내 지질 및 관련 호르몬 수준에 미치는 영향을 알아보고자 수행되었다. 흰쥐를 Sham군(S), 난소절제 control군(OVX), 난소절제 이소플라본 10 mg/kg 투여군(IF), 난소절제 흑미 30 및 90mg/kg 투여군(BRE-30, BRE-90)으로 나누어 12주 동안 고지방식이(45% calories from fat)를 급여하였을 때, 체중 증가량, 체지방무게 및 비만지수는 OVX군에서 증가하였으나, 이소플라본 및 BRE 투여에 의해 유의하게 감소되었다. 혈중 중성지방은 BRE-90군이 OVX군에 비해 유의적으로 낮았고, 혈청 adiponectin 수준은 BRE군에서 OVX군에 비하여 유의하게 증가하였으며, leptin 수준은 대조군인 OVX군에 비하여 유의적으로 감소하였다. 간 중 총지질과 중성지방 및 콜레스테롤은 유의적이지는 않았으나 BRE 투여군에서 OVX군보다 감소하는 경향을 보였다. 따라서, 흑미 호분층은 난소절제 흰쥐에서 지질 대사를 조절함으로써 비만 및 여성의 폐경기 관련 질병을 예방하는 식품이 될 수 있을 것으로 기대된다.

국내산 참나무속 수종 수피의 추출성분 (Chemical Constituents of Domestic Quercus spp. Barks)

  • 국내산 낙엽성 참나무류인 신갈나무, 상수리나무, 떡갈나무, 졸참나무, 갈참나무 및 굴참나무 수피의 추출성분의 구조를 규명하고 수종 상호간 성분의 특성 및 연관성 등을 조사하였다. 그 결과 신갈나무에서 화합물 2(ellagic acid, 0.03 g), 4 ((+)-catechin, 4.59 g), 6 (taxifolin, 3.35 g) 및 7 (glucodistylin, 20.52 g)을 상수리 나무에서는 화합물 1 (gallic acid, 0.18 g), 4 ((+)-catechin, 8.52 g), 5 ((+)-gallocatechin, 0.09 g), 6 (taxifolin, 0.54 g) 및 7 (glucodistylin, 3.28 g)을 떡갈나무에서는 화합물 1 (gallic acid, 0.38 g), 2 (ellagic acid, 0.11 g) 4 ((+)-catechin, 2.01 g), 5 ((+)-gallocatechin, 0.12 g) 및 7 (glucodistylin, 0.39 g)을 갈참나무에서는 2 (ellagic acid, 1.51 g), 4 (+)-catechin, 21.91 g) 및 7 (glucodistylin, 3.91 g)을 졸참나무에서는 2 (ellagic acid, 0.84 g), 4 ((+)-catechin, 0.82 g), 6 (taxifolin, 4.02 g) 및 7 (glucodistylin, 21.50 g)을 굴참나무에서는 1 (gallic acid, 0.24 g), 3 (caffeic acid, 0.05 g), 4 ((+)-catechin, 0.32 g) 및 7 (glucodistylin, 0.65 g)을 분리하여 구조를 규명하였다. 국내산 참나무속 6 수종의 수피에서는 화합물 4 ((+)-catechin)와 7 (glucodistylin) 이 공통적으로 분리되었으며 두 성분 중 함유량이 상대적으로 높은 glucodistylin은 참나무류 수피의 지표성분 으로 이용될 수 있을 것이다.

겨울철 부산항 주변해역의 수질과 표층퇴적물 환경특성 (Environmental Characteristics of the Seawater and Surface Sediment in the vicinity of Pusan Harbor Area in Winter)

  • 부산항 주변 해역의 이화학적 특성을 파악하기 위하여 1992년 12원에 해수 및 표층 퇴적물중 오염지표 항목들을 측정하였다. 염분과 영양염류, AOU와 영양염류의 상관관계로 보아 겨울철 부산항에서 영양염류의 농도는 주로 도시하수의 유입에 의해 좌우되며, 수주내의 유기물 분해와 같은 생화학적 과정에 의한 영양염 재생에 따른 영향도 다소 나타나고 있다. 부영양화 지수 (TSI)는 평균 33.6 (범위 0.5-258.1)이고, 북외항, 북내항, 남항 및 감천항에서 각각 평균 19, 50, 56 및 5로 남항에서 가장 높은 부영양화 지수를 보인다. 표층퇴적물중의 총강열감량, COD 및 총황화물의 평균 농도는 각각 $12.1\%$, 17.5mg/g.dry 및 1.18mg/g.dry 였으며 북내항에서 가장 높다 그리고, Ro(1975)의 자료에 비해 총강열감량과 COD는 북내항에서 2-3배정도 높은 농도를 보인다. 표층 퇴적물중 총유기탄소와 총유기 질소 및 총인의 평균 농도는 각각 24.9, 1.3 및 0.69mg/g.dry 였으며, 총유기탄소와 총인의 농도는 남항에서 가장 높은 농도를 보인 반면에, 총유기질소의 농도는 북내항에서 가장 높은 농도를 보였다. 특히 부산항 퇴적물중 TOC/TON 비값은 평균 22.5 (범위 10.2-60.2)로서 육상기원 유기물질의 퇴적이 우세하였다.

Validation of Age Estimation Methods Using Pulpal Volume Changes in Radiographs for Korean Adults

  • Purpose: It aims to verify the applicability of existing age estimation methods derived from data of foreign population groups to Korean population groups. Moreover it is to suggest a new way applicable to practical age estimation on the basis of newly calculated regression formulae from data of Korean population groups and develop a subsidiarily applicable method to the existing method. Methods: Ratio of pulp cavity to dental crown was calculated by measuring the height and width of dental crowns and pulp cavities at the cervical line from 4,034 first and second upper molars, first and second upper premolars, first and second lower molars and first and second lower premolars on both left and right sides of 400 patients who had been treated in Dental Hospital of Yonsei University College of Dentistry, and regression equations were derived from the values of the ratio. Results: The equation with correlation coefficients the highest among females was as follows: age=$107.96-75.684{\times}{\sharp}17$ TCHI-$53.741{\times}{\sharp}26$ TCVI-$40.664{\times}{\sharp}45$ TCVI-$56.307{\times}{\sharp}46$ TCVI. Randomized anohter Korean female subjects (n=20) are applicated to the new equation. Mean of error of estimate is 10.322 years, standard deviation is 12.852 years. Minimum of error of estimate is 1.018 years, maximum is 21.365 years Conclusions: The error range of age estimation was found to be slightly wider when the existing regression formulae of Drusini were applied to Korean population groups. Also age estimation in females using the ratio of pulp cavity to dental crown measured with the length and width of dental crowns and pulp cavities from maxillomandibular molars was observed to have the highest reliability in the research. However, we consider that advanced equations of regression are needed to apply to both molars and premolars of males and females in the future.

교통사고 후 발생한 경항통 환자에 대한 황련해독탕약침과 분리정제봉약침의 치료 효과 비교연구: 무작위 배정 (The Comparative Study on the Effect of HWANGRYUNHAEDOKTANG Pharmacopuncture and Essential Bee Venom Pharmacopuncture for Cervical Pain Caused by Traffic Accidents: A Randomized Controlled Trial)

  • Objectives The purpose of this study is to compare the effects of HWANGRYUNHAEDOKTANG Pharmacopuncture and Essential Bee Venom pharmacopuncture on cervical pain caused by traffic accident. Methods The clincal study was conducted to traffic accident patients who had admitted to Dae-Jeon University Cheonan Oriental Hospital from May 17, 2014 to September 2, 2014. The patients were simple randomization and divided into two groups. In one group, patients were treated with the oriental medicine and HWANGRYUNHAEDOKTANG Pharmacopuncture, While in the other group, patients were treated with the oriental medicine therapy and Essential Bee Venom pharmacopuncture therapy. visual analogue scale (VAS), pain threshold and neck disablity index (NDI) were used To estimate the efficacy of Pharmacopuncture treatment. Results 1) HWANGRYUNHAEDOKTANG Pharmacopuncture Group showed significant decrease of VAS and NDI after 3rd times treatment than before treatment (p<0.001). Essential Bee Venom Pharmacopuncture showed significant decrease of VAS, NDI and improve ment of pain threshold after 3rd times treatment than before treatment (p<0.01, p<0.001). 2) There was no significant difference between HWANGRYUNHAEDOKTANG Pharmacopuncture group and Essential Bee Venom Pharmacopuncture group in VAS, pain threshold and NDI. Conclusions We found out that HWANGRYUNHAEDOKTANG Pharmacopuncture and Essential Bee Venom Pharmacopuncture treatment are considered to be effective and useful in cervical pain caused by traffic accident.
