사람 U2OS 골육종 세포에서 Matrix Metalloproteinase의 발현에 Ibandronate가 미치는 영향 (Effects of Ibandronate on the Expression of Matrix Metalloproteinases in Human U2OS Osteosarcoma Cells)

  • 배경: 골육종은 소아 및 청소년기에 가장 흔하게 발생하는 악성 골종양 중 하나이다. 최근 수술적 치료와 항암 화학요법을 병행하여 생존률이 증가하였지만, 아직까지도 항암제는 항암제 내성 및 이차성 악성 종양의 발생 등 여러 문제점을 가지고 있다. 일부 종양은 matrix metalloproteinase(MMPs)의 발현이 증가되어 있고, MMP inhibitor에 대한 연구가 진행되고 있다. 반면 bisphosphonate(BPs) 제제는 골흡수를 억제하는 능력이 있으며, 파골세포와 관계된 골 병변에 광범위하게 사용되고 있다. 또 bisphosphonate 제제는 직접적인 항암효과를 가지고 있는 것으로 알려져 있다. 대상 및 방법: 골육종 세포주(U2OS)를 ibandronate(0, 0.1, 1, 10M)를 이용하여 48시간 동안 처치하였다. 세포의 생존능은 MTT assay를 이용하여, MMP-2와 MT1-MMP의 mRNA level은 reverse-transciption polymerase chain reaction을 이용하여 측정하였으며, MMP-2와 MT1-MMP 단백의 양은 Westernblot을, MMP-2의 활성은 Gelatin zymography를 이용하여 측정하였다. 또, ibandronate 처치 전후의 골육종 세포주의 침습성은 Matrigel invasion assay를 이용하여 측정하였다. 결과: 48시간 ibandronate 처치 후 U2OS 세포주의 침습력은 ibandronate에 대해 용량 의존적으로 감소하였다. 특히 10M 이내의 ibandronate는 세포독성이 나타나지 않았다. 젤라틴 융해능과 MMP-2 및 MT1-MMP의 단백 및 mRNA 정도역시 ibandronate 농도가 증가할수록 감소하였다. 결론: Ibandronate는 골육종 세포주의 MMP-2 및 MT1-MMP의 발현을 억제하였으며, 종양세포의 침습력을 감소시켰다. Bisphosphate의 종양세포 침습 억제 능력은 새로운 전이 억제제의 개발에 도움이 될 것으로 사료된다.

홍화 추출물이 치주인대세포, 조골세포 활성도에 미치는 영향 (The biologic effects of safflower(Carthamus tinctorius $Linn\acute{e}$) extract and Dipsasi Radix extract on periodontal ligament cells and osteoblastic cells)

  • Safflower(Carthamus tinctorius $Linn\acute{e}$ has been traditionally used for the treatment of blood stasis, and Dipsasi Radix has been used as a drug for fracture in Chinese medicine. The purpose of present study was to examine the biologic effects of safflower extract and Disasi radix extracts on the periodontal. ligament cells and osteoblastic cells and on the wound healing of rat calvarial defect. The ethanolic extract of safflower blossom, safflower seed and Dipsasi Radix(125, 250, and 500 ${\mu}g/ml$) were prepared as test group, and PDGF-BB(lOng/ml) and unsafonifiable fraction of Zea Mays L.(125, 250, and 500 ${\mu}g/ml$) were employed as positive control. The effects of each agents on the growth and survival, ALPase activity, expression of PDGF-BB receptor, chemotactic response of PDL cell and ATCC human osteosarcoma MG63 cells in vitro were examined. The tissue regenerative effect of each extracts was evaluated by histomorphometric measuring of newly formed bone on the 8mm defect in rat calvaria after oral administration of 3 different dosages groups : 0.02, 0.1 and 0.35g/kg, per day. It was also employed the same dosages of unsaponifiable fraction of Zea Mays L. as positive controls. Safflower blossom extract, safflower seed extract, and Dipsasi Radix extract stimulate the cellular activity of MG63 cells in concentration range of $125-500{\mu}g/ml$, and safflower bolssom extract and safflower seed extract stimulate also the cellular activity of periodontal ligament cells in concentration range of $250-500{\mu}g/ml$. In activity of ALPase, $250-500{\mu}g/ml$ of safflower blossom extracts showed significant stimulating effects on MG63 cells, and the same concentration range of safflower seed extracts showed significant effect on periodontal ligament cells. In the recovery on PDGF-BB receptor expression which was depressed by $IL-1{\beta}$, $125-250{\mu}g/ml$ of safflower blossom extracts and $250-500{\mu}g/ml$ of safflower seed extracts showed significant increasing effect on MG63 cells, and $500{\mu}g/ml$ of safflower blossom extract and $250-500{\mu}g/ml$ of safflower seed extracts showed significant effect on periodontal ligament cells. In chemotactic response, among all tested group, safflower seed extracts only were chemotactic to MG63 cells and periodontal ligament cells in concentration range of $125-500{\mu}g/ml$. Also in the view of bone regeneration in rat calvarial defect model, the only group that was orally administrated 0.35g/kg, day of safflower seed extract showed significant new bone formation. These results suggested that safflower extracts might have a potential possibilities as an useful drug for adjunct to treatment for regeneration of periodontal defect.

백서 두개골 결손부에서 항생제를 함유한 키토산 차단막의 골재생 유도 효과 (The Bone regenerative effects of tetracycline blended chitosan membranes on the calvarial critical size defect in Sprague dawley rats)

  • The major goals of periodontal therapy are the functional regeneration of periodontal supporting structures already destructed by periodontal disease as well as the reduction of signs and symptoms of progressive periodontal disease. There have been many efforts to develop materials and therapeutic methods to promote periodontal wound healing. There have been increasing interest on the chitosan made by chtin. Chitosan is a derivative of chitin made by deacetylation of side chains. Chitosan has been widely studied as bone substitution and membrane material in periodontology. Many experiments using chitosan in various animal models have proven its beneficial effects. Tetracycline has been considered for use in the treatment of chronic periodontal disease and gingivitis. The aim of this study is to evlauate the osteogenesis of tetracycline blended chitosan membranes on the calvarial critical size defect in Sprague Dawley rats. An 8mm surgical defect was produced with a trephine bur in the area of the midsagittal suture. The rats were divided into five groups: Untreated control group versus four experimental group. Four types of membranes were made and comparative study was been done. Two types of non-woven membranes were made by immersing non-woven chitosan into either the tetracycline solution or chitosan-tetracycline solution. Other two types of sponge membranes were fabricated by immersing chitosan sponge into the tetracycline solution, and subsequent freeze-drying. The animals were sacrificed at 2 and 8 weeks after surgical procedure. The specimens were examined by histologic analyses. The results are as follows: 1. Clinically the use of tetracycline blended chitosan membrane showed great healing capacity. 2. The new bone formations of all the experimental group, non-woven and sponge type membranes were greater than those of control group. But, there was no significant difference between the experimental groups. 3. Resorption of chitosan membranes were not shown in any groups at 2 weeks and 8 weeks. These results suggest that the use of tetracycline blended chitosan membrane on the calvarial defects in rats has significant effect on the regeneration of bone tissue in itself. And it implicate that tetracycline blended chitosan membrane might be useful for guided tissue regeneration.

홍화씨 분획 추출물이 치주인대 섬유아세포와 MC3T3-E1 세포에 미치는 영향 (The effect of safflower seed fraction extract on periodontal ligament fibroblast and MC3T3-E1 cell in vitro)

  • Recently, use of natural medicine is getting more attention, and some of them are believed to be effective in the treatment of periodontitis. Among them, the seeds of safflower(Carthamus tinctrorius L.) have been proven to be effective through its use in bone diseases such as fracture and osteoporosis. During the last few years, studies using the seeds of safflower gown in Korea have been active, and it has been reported that safflower seed extract increase the proliferation and the alkaline phosphatase(ALP) activity of human periodontal ligament fibroblast(hPDLF), osteoblast, and that they promote the mineralization process. In animal studies, when safflower seed extract were administered orally new bone formation was promoted. Recently, in an effort to find out the most effective osteogenic components, among many components of the safflower seed, various safflower seed fraction extracts were obtained by multistep extraction of the safflower components using various solvents. Among these, saf-M-W fraction extracted by methanol and water was most effective in increasing osteogenic potential of osteoblasts. In this study, the effect of safflower seed fraction extract, saf-M-W, on the growth and differentiation of hPDLF and MC3T3-E1 cell was investigated. The toxicity of saf-M-W on both cells was measured using M'IT(3-(4,5dimethylthiazol-2-y1)-2,5-diphenyl tetrazolium bromide) test, and ALP activity was measured using the colorimetric assay of hPDLF. In addition, in MC3T3-El cells, the expression of ALP, bone sialoprotein(BSP) mRNA was observed using Northern blot, and the mineralized nodule formation Was observed using von Kossa stain and phase-contrast microscope. 1. In concentrations below $10{\mu}g/ml$, saf-M-W didn't show any toxicity on hPDLF and MC3T3-El cell. 2. The change in saf-M-W concentration had no effect on the ALP activity of hPDLF. 3. In MC3T-E1 cells, mRNA expressions of ALP and BSP were greater in the experimental group treated with $10{\mu}g/ml$ concentration of saf-M-W compared with the control group. 4. In MC3T3-El cells, abundance of mineralized nodules were formed in the experimental group treated with $10{\mu}g/ml$ Concentration of saf-M-W, while no mineralized nodule was formed in the control group. These results suggest that safflower seed fraction extract, saf-M-W. didn't show any toxicity on hPDLF and MC3T3-E1 cell at concentrations below $10{\mu}g/ml$ and effectively enhanced the differentiation and osteogenic potential of MC3T3-El cell.

'추서(醜書)'에 대한 새로운 모색 -옥흥화(沃興華)와 증상(曾翔)의 서예 작품을 중심으로 (A New Exploration of Ugly Calligraphy(醜書) - Focused on Calligraphy Works of Ok Heung-hwa(沃興華) and Jeung Sang(曾翔) -)

  • 현대 서예의 유파가 많은데 '추서(醜書)'가 하나의 유파로 봐야 할지에 대해 학자들의 의견이 분분하다. '추서'를 인정받지 않은 사람은 '추서'에서 전통적인 결자 규범이 없다고 본다. 그리고 문제의 식별성도 사라진다고 주장한다. 그러나 '추서'가 존재하고 발전을 이루는 것은 현대 서예가의 끊임없은 탐색과 개혁과 관련된다. 뿐만 아니라 서예들의 미적 인식 능력이 늘어나고 미적 표준도 시대와 함께 변화한다. 이에 따라 '추서'를 인정하는 사람도 많아질 것이다. 시대에 앞장선 서예가들이 전통적인 서예 사상을 줄곧 견지하면서 전인을 배우고 경험을 쌓는다. 그 중에서 전통적인 중용(中庸)사상을 저버리고 '추(醜)'를 세인에게 보이는 서예가들이 있다. 옥흥화(沃興華)와 증상(曾翔) 등 서예가들이 바로 여기에 해당한다. 옥흥화와 증상은 모두 고대 비첩을 골자로, 민간 서예를 혈액으로 하여 나름의 풍격을 창출했다. 본고는 옥흥화와 증상의 작품을 분석함으로써 '추서'의 정의와 형성 과정을 탐색, 연구하고자 한다. 이와 동시에 '추서'의 특징 및 '추서'의 미감을 정리하도록 한다.

QR 코드를 이용한 의료정보 시스템 설계 및 구현 (Design and Implementation of Medical Information System using QR Code)

  • 신규 의료기기 개발 기술의 발전으로 다양한 형태로 손쉽게 생체 정보 및 의료 정보를 얻을 수 있는 기술이 증가하고 있다. 이러한 정보 수집 기술과 기기들의 증가로 생체 정보는 일상생활의 라이프로그와 함께 의료서비스의 주요 정보로 활용되고 있다. 그러나 다양한 생체신호의 활용성이 증가하고 있지만 보안적인 측면을 고려하지 않는 문제점을 갖고 있다. 또한, 의료현장에서 환자의 생체신호와 의료영상정보는 개별적인 디바이스에 의해 생성되며, 통합 관리되지 못하는 실정이다. 이러한 문제점을 해결하기 위해서, 본 논문에서는 생체신호와 의사의 소견정보를 포함하여 QR 코드화하고 이와 연계된 의료영상정보와 통합하고자 한다. 이를 위해, 의료영상정보 표준인 DICOM(Digital Imaging and Communication in Medicine)과 기존 생체신호 계측기들로부터 수집된 생체신호를 QR 코드화하여 의료영상정보에 통합한 이미지 파일 스킴을 제시한다. 그리고 시스템 구현 환경은 의료영상기기와 생체신호 수집을 위한 생체신호 계측기 그리고 스마트 디바이스와 PC로 구성하였다. 의료기기나 생체 신호 계측장치로부터 데이터를 전송 받기 위한 의료영상이미지 정보와 생체신호의 ROI 추출을 위하여 .NET Framework를 사용하여 QR 서버 모듈을 윈도우 서버 2008 운영체제에서 운영되도록 구현하였다. QR 서버 모듈의 주요기능은 의료영상기기로부터 생성된 DICOM파일을 파싱하고, 식별 ROI 정보를 추출하여 데이터베이스에 저장하여 관리한다. 또한, EMR, OCS와 같은 환자의 의료정보는 기본 정보 및 긴급상황 시 필요한 ROI 정보를 추출하여 QR코드화 하여 관리한다. 또한 생체 계측 기기로 환자 식별에 사용될 PID (patient identification) 와 함께 생체 정보를 전송 받을 경우 생체 정보의 크기에 따라 이를 해당 환자의 ROI와 함께 QR코드화 하여 관리하며, 생체 정보 파일 또한 저장하여 관리한다. 전송받은 생체정보가 QR코드로 변환할 최대 사이즈 이상일 경우 서버를 통해 생체정보에 접근할 수 있는 URL 정보를 QR코드화 한다. 또한 QR 코드 형태로 제공되는 정보는 .NET 프레임워크가 설치된 PC와 Android기반의 스마트 단말기상에 뷰어 프로그램을 통해 확인함으로 인증된 클라이언트만이 관련 정보를 확인할 수 있도록 하였다. 끝으로 응용 서비스의 수행결과를 통해 기존 의료영상정보와 생체신호 그리고 환자의 건강정보가 통합되어 의료현장에서 적용하는데 적합한 의료정보 서비스를 제공함을 보였다.

척추 수술에 사용되는 나사못의 반복 삽입과 인출이 인장항력에 미치는 영향 : 생체 역학적 연구 (The Effect of Repetitive Insertion and Pullout of Spinal Screws on Pullout Resistance : A Biomechanical Study)

  • 목적 : 최근 척추 수술에 나사못을 사용하는 빈도와 범위가 넓어 지고 있는데 때로 수술 중 한번 삽입하였던 나사못을 다시 사용하는 경우가 있다. 인체 골과 타이타늄의 탄성계수가 크게 차이 나지만 반복 삽입 과정에서 나사못의 이가 손상될 가능성이 있다. 저자들은 나사못의 반복 삽입이 나사못의 인출 저항에 미치는 영향을 조사하였다. 방법 : 각각 6개의 세가지 다른 종류 cortical lateral mass screw, cancellous lateral mass screw and cervical vertebral body screw의 나사못을 시험하였다. 나사못을 인체의 골과 비슷한 밀도의 인공합성골에 삽입하였으며 삽입 중 삽입력을 측정하였고 그 후 Instron(Model TT-D, Canton, MA)을 이용하여 2.4mm의 속도로 인출하여 인장항력을 digital oscilloscope에 기록하였다. 위의 과정을 3회 반복하여 기록한 뒤 나사못을 광학 현미경으로 확대하여 관찰하였다. 결과 : cortical lateral mass screws의 평균 인장항력(1회인장 시험 $185.66N{\pm}42.60$, 2회 인장시험 $167.10N{\pm}27.01$, 3회인장 시험 $162.52N{\pm}23.83$ : p=0.03)과 cervical vertebral body screws ($386.0N{\pm}24.1$, $360.2N{\pm}17.5$ and $330.9N{\pm}16.7$ : p=0.0024)은 반복하여 삽입, 인장 검사 할 때 마다 감소하였으나 cancellous lateral mass screws의 평균 인장항력($194.00N{\pm}36.47$, $219.24N{\pm}26.58$ and $199.49N{\pm}36.63$ : p=0.24)은 감소하지 않았다. 전자현미경 소견에서 나사이의 끝이 무디어지고 표면이 문드러진 것을 관찰할 수 있었다. 결론 : 일부 나사못을 반복하여 삽입한 후 나사못의 인장항력이 감소되었으므로 수술중 여러번 삽입하였던 나사못은 최종 구조물에 사용되지 않아야 한다.

Sensory Information Processing

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Results show the superiority of the proposed method to the conventional method using power spectrum and also its usefulness for the tissue characterization. 30541 T00401030541 ^x This paper describes the implementation of a computerized radial pulse diagnosis by aids of a clinical expert. On this base, we composed of the radial pulse diagnosis system in korean traditional medicine. The system composed of a radial pulse wave detection system and a radial pulse diagnosis system. With a detection system, we detected Inyoung and Cheongu radial pulse wave and processed it. Then, we have got the characteristic parameters of radial pulse wave and also quantified that according to the method of Inyoung-Cheongu Comparison Radial Pulse Diagnosis. We defined the jugement standard of radial pulse diagnosis system and then we confirmed the possibility for realization of automatic radial pulse diagnosis in korean traditional medicine. 30545 T00401030545 ^x Microspheres are expected to be applied to biomedical areas such as solid-phase immunoassays, drug delivery systems, immunomagnetic cell separation. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. 예방적 두강내 방사선 조사후 소세포 폐암 환자의 뇌전이 빈도와 생존율에 대한 연구 (Prophylactic cranial irradiation in limited small-cell lung cancer : incidence of brain metastasis and survival and clinical aspects)

  • 목적 : 소세포 폐암 환자에서 생존율이 향상됨에 따라서 뇌전이의 임상적 중요성이 커지고 있다. PCI 는 뇌전이 빈도를 감소시키며 특히 완전관해된 제한병기 소세포 폐암 환자에서는 생존기간의 연장까지 가져온다는 보고도 있다. 저자들은 완전관해가 된 제한병기 소세포 폐암환자에서 PCI를 시행한 후 뇌전이의 빈도, 생존율 및 임상상을 알아보고자 하였다. 대상 및 방법 : 1989년 3월부터 1999년 9월까지 충남대학교병원에 입원하여 조직학적으로 소세포 폐암으로 확진되고 병기분류상 제한병기로 분류된 후 항암 화학요법 및 방사선치료후 완전관해가 된 환자중 PCI를 시행 받은 42명올 대상으로 후향적분석을 하였다. 항암 화학요법은 초치료로 VPP/CAV교대요법을 6회이상 시행 하였고, 흉부 방사선치료는 동시치료군이 38.1%, 연속치료군이 61.9%였다. 결과 : 대상환자의 88.1%가 남자였고, 중앙연령은 58세였다. 중앙 관찰기간은 18.1개월이었다. 경과 관찰 중에 57.1%에서 전이 또는 재발하였는데, 뇌전이는 14.3%에서 발생하였고, 폐 원발병소에서 재발된 경우가 35.7%로 가장 많았다. 평균 무병생존기간은 1,090일(중앙값 305일), PCI후 뇌전이가 발생하기까지는 평균 2,548일, 뇌전이를 제외한 전이 및 재발되기까지는 평균 1,395일(중앙값 460일)이었다. 생존기간의 중앙값은 21.1개월, 2년생존율은 41.7% 이었다. 전이 및 재발이 발생한 군의 평균 생존기간은 642일(중앙값 489일), 발생하지 않은 군의 평균 생존기간은 2,622일로 유의한 차이가 있었으나, 뇌전이 유무에 따른 생존기간의 차이는 없었다. 사망원인은 원발 병소의 재발 및 전신질환에 의한 사망이 많았고(85.7%), 뇌전이자체에 의한 사망은 14.3%이었다. 결론 : 완전관해가 된 제한병기 소세포 폐암 환자에서 PCI는 뇌전이를 감소시키고 지연시키는 경향이 있었다. 전이 및 재발이 있는 군에서 생존기간이 짧았으나 뇌전이 유무에 따른 생존기간의 차이는 없었고 주된 사망원인이 뇌전이보다는 원발병소의 재발과 전이 및 전산질환에 의한 것이 대부분이어서, 생존율을 높이기위해서는 원발병소의 재발과 전이 및 전신질환에 의한 것이 대부분이어서, 생존율을 높이기 위해서는 원발병소의 재발 및 전이를 감소시킬 수 있는 효과적인 치료방법의 개발 및 전신진환의 적절한 조절이 필요한 것으로 생각된다.
