This study evaluated the Comparison of the Effectiveness of Pelvic Floor Muscle exercise and Biofeedback treatment for Genuine Stress Incontinence I assigned 60 participants to 2 groups : 30 to the pelvic floor muscle exercise group and 30 to the biofeedback group. Treatment protocol lasted for 6 weeks. Peak pressure, and duration time of pelvic muscle contraction were evaluated by a perineometer. Lower urinary symptoms, sexual matter and life style scores were achieved by using Jackson's scale. The treatment efficacy of the pelvic floor muscle exercise is compared with the biofeedback group and the main results of the comparison are as follows : 1. Pelvic muscle contraction 1) The peak pressure in the biofeedback group was significantly increased(P=0.000). 2. The frequency and quantity of incontinence 1) The frequency of incontinence in the biofeedback group was significantly decreased(P=0.000). 2) The quantity of incontinence in the biofeedback group was significantly decreased(P=0.000). 3. The lower urinary symptoms Daily frequency(P=0.000), nocturia(P=0.000), urgency(P=0.000), bladder pain(P=0.000), unexplained incontinence(P=0.048), wearing protection(P=0.022), changing outer clothing(P=0.005), hesitancy(P=0.008), intermittent stream(P=0.000), abnormal strength of stream(P=0.004), retention(P=0.000), incomplete emptying(P=0.000), and inability to stop mid stream(P=0.006) of the lower urinary symptoms in the biofeedbatk group were significantly decreased. 4. The sexual matters The dry vagina (P=0.004) and pain during sexual Intercourse (P=0.002) in the biofeedback group was significantly decreased. 5. The life style The fluid intake restriction(P=0.007), affected daily task(P=0.003), avoidance of places & situation(P=0.003), interference in Physical activity (P=0.002), interference in relationship with other people(P=0.01), and feeling about the rest of life with urinary symptom(P=0.000) in the biofeedback group were significantly decreased. In conclusion, the biofeedback treatment was more effective than the pelvic floor muscle exercise in genuine stress incontinence.
본 조사는 요실금 자각 증상이 있는 중년여성을 대상으로 요실금의 주관적 정도와 통증이 그에 따른 삶의 질에 미치는 영향을 알아보고자 하였다. 2010년 2월부터 5월까지 병원을 외래로 방문한 만성 근골격계 여성 환자를 대상으로 요실금 자각 증상이 있는 114명에게 요실금 관련 삶의 질 도구로 설문조사를 실시하였다. 전체적인 삶의 질은 보통($61.36{\pm}13.2$점)이라고 느꼈으며, 요실금과 관련한 통증은 허리통증으로 나타났다. 통증정도는 VAS 4~7의 중간통증으로 나타났으며, 요실금으로 인해 삶의 질 요인 중 행위의 회피 제한과 심리사회적으로 영향을 받는 것으로 나타났다. 조사로 인해 중년여성의 요실금과 통증이 삶의 질에 실질적인 영향을 미침으로 나타나 실질적인 중재 개발에 기초를 제공할 수 있을 것이라 생각된다.
Purpose: This study was done to examine the effect of a urinary incontinence management program on physical symptoms, daily life problems, and sexual life problems for women with mixed urinary incontinence. Methods: This study was conducted using a one group pre-test and post-test design. For the survey, 28 women having mixed urinary incontinence were chosen by convenience sampling and agreed to participate in this study. Their physical symptoms (urinary symptoms, maximum vaginal contraction pressure, and duration of vaginal contraction), daily life problems, and sexual life problems were measured. For descriptive statistics and Wilcoxon signed-rank test were used with SPSS Win 14.0. Results: There were statistically significant differences in the mean scores of physical symptoms (urinary symptoms and maximum vaginal contraction pressure), daily life problems, and sexual life problems. Conclusion: This urinary incontinence management program showed improvements in physical symptoms, daily life, and sexual life of women in the sample. The study results indicate that for effective urinary incontinence management programs nurses should recognize the importance of continual treatment and also develop coping strategies that have sensitivity and are appropriate for patients' daily and sexual problems.
본 연구에서 방광 내벽과 외벽사이의 거리를 측정하여 뇨량을 계산하는 시스템에 적용하기 위한 방광용 초음파 변환기를 설계하고 개발하였다. 뇨량 측정 시스템은 방광에서 오줌을 배출하기 전의 양을 측정함으로써 뇨실금 환자의 치료 과정을 도와주게 된다. 이 실험의 결과로, 뇨량을 더 정확하게 측정할 수 있고 또한 뇨실금 방지 시스템을 개발할 수 있다. 본 연구를 통하여 개발되는 시스템은 뇨실금과 야뇨증 환자를 더 오래 더 건강한 삶을 살 수 있도록 해줄 수 있을 것이다.
Purpose: This study was performed to investigate the effects of lower urinary tract symptoms on depression and quality of life among women in urban areas. Methods: The participants in this study were 306 women in P city. Data were collected using a questionnaire from May 1 to June 6, 2008. Lower urinary tract symptoms (LUTS) were measured with BFLUTS-SF and categorized into voiding, filling and incontinence. The data were analyzed through t-test, one-way ANOVA, Fisher exact test, and multiple regression. Results: Age, filling and incontinence, and all of the subcategories of LUTS explained 16% of variance in depression. Older women, LUTS with incontinence, filling and incontinence, and all the problems including filling, voiding and incontinence lowered the quality of life. Conclusion: In order to develop nursing interventions for women with urinary tract symptoms, comprehensive understanding of LUTS is needed.
Purpose: To develop outcome indicators of urinary incontinence to measure quality of care in long term care hospitals in Korea. Methods: The draft indicators of urinary incontinence were developed from a literature review and clinical expert panel. A survey of medical records of 280 patients in 20 hospitals was conducted to test inter-rater reliability. Statistical analysis was done to test risk adjustment criteria, variation between hospitals, and stability of indicators, using assessment data from 77,918 patients in 623 hospitals. Results: The inter-rater reliability of items was high (Kappa range: 0.66- 0.92). Severe cognitive impairment (odds ratio [OR]: 3.15, confidence interval [CI]: 3.03-3.26) and total mobility activities of daily living (ADLs) dependency (OR: 4.85, CI: 4.72-4.98) increased the prevalence of urinary incontinence, thus they proved to be significant criteria to stratify high and low risk groups. The prevalence for low risk showed more substantial variation than the high risk group. The indicators were stable over one month. Conclusion: This study demonstrated the feasibility of outcome indicators of urinary incontinence. Improving the reliability of the patient assessment tool and refining the indicators through validation study is a must for future study.
Purpose: This study was done to examine the effects of an incontinence prevention program on postpartum women. Methods: The study design was a nonequivalent control pretest-posttest design. The subjects were 49 postpartum women with a normal vaginal delivery, 25 in the experimental group and 24 in the control group. Data was collected from lune 1. 2007 to April 30. 2008 at a postpartum women's care center located in Jeonju, Korea. For the experimental treatment, an incontinence prevention program was carried out for 24 weeks. Measures included maximum pressure of pelvic floor muscle contraction and duration of pelvic floor muscle contraction at pre-treatment, 5 weeks postpartum and 24 weeks postpartum. Data was analyzed by Repeated ANOVA using the SPSS/WIN 14.0 program. Results: The mean maximum pressure of pelvic floor muscle contraction (F = 8.95, p < .001) and mean duration of pelvic floor muscle contraction (F = 22.01, p < .001) were significantly different between the groups, and significantly increased as time passed. Conclusion: Practice of an incontinence prevention program is considered an effective intervention for the results of fewer urinary incontinence symptoms in postpartum women.
Purpose: The purpose of this study was to examine the effect of a multifactorial program for urinary incontinence, self-esteem and self-efficacy in older women. Methods: A one-group pretest-posttest with no control group was used. Nineteen women completed weekly 90-min group sessions for eight weeks. The group received a multi-factorial program comprised of Kegel exercise, education and psychosocial programs over 8 weeks. Data on urinary incontinence, self-esteem and self-efficacy were collected twice: before the intervention and 8 weeks after the intervention. Data were analyzed using paired t-test by non-parametric statistics. Results: Total scores for the International Consultation on Incontinence Questionnaire decreased significantly after the intervention (pretest $7.95{\pm}5.48$, posttest $5.15{\pm}4.72$, p<.05). The scores for self-esteem and self-efficacy improved significantly. Conclusion: These results indicate that a multifactorial program for older women is feasible to prevent urinary incontinence in women living in the community.
Purpose: We aimed to compare the prevalence rates and associated symptoms of constipation and fecal incontinence in children and young adults and evaluate how these patient groups cope with these disorders. Methods: A cross-sectional study was performed in which 212 children (8-17 years) and 149 young adults (18-29 years) from the general Dutch population completed a questionnaire about defecation disorders. Results: Constipation occurred in 15.6% of children and 22.8% of young adults (p=0.55), while the prevalence of fecal incontinence was comparable between groups (7%, p=0.91). The symptoms associated with constipation occurred as often in children as in young adults, while most fecal incontinence symptoms occurred more often in young adults. Approximately 43% of children had constipation for more than 5 years, while 26% of young adults experienced constipation since childhood. Only 27% of constipated children and 21% of constipated young adults received treatment (mostly laxatives). For fecal incontinence, 13% of children and 36% of young adults received treatment (mostly antidiarrheal medications or incontinence pads). Conclusion: In contrast to the general belief, the prevalence of defecation disorders and associated symptoms seem to be comparable in children and young adults. Only a few people with defecation disorders receive adequate treatment.
Background: The purpose of this study was to investigate whether postoperative cystography findings can predict early and longterm recovery from incontinence after radical prostatectomy (RP), compared with the other cystography parameters. Methods: I retrospectively reviewed 118 patients who underwent robot-assisted RP (RARP) for localized prostate cancer at single institution between January 2016 and April 2021. One hundred and seven patients were included in the study. Postoperative cystography was routinely performed 7 days after surgery. The bladder neck to pubic symphysis ratio, vesicourethral angle, and bladder neck anteroposterior length (BNAP) ratio (the bladder neck-posterior margin distances divided by the anteroposterior lengths) were evaluated. Continence was defined as cessation of pad use. The association between these variables and urinary incontinence was also analyzed. Results: The urinary incontinence recovery rates 1, 3, 6, and 12 months after RARP were 43.92%, 66.35%, 87.85%, and 97.19%, respectively. Multivariate logistic regression analysis demonstrated that a lower BNAP ratio and wider vesicourethral angle were significantly associated with continence restoration at 1, 3, and 6 months after surgery. In addition, in terms of days of pad usage, lower BNAP ratio, wider vesicourethral angle, and bladder neck preservation were significantly associated with recovery from urinary incontinence within 12 months as assessed by Cox proportional hazard analysis. Conclusion: This study demonstrated that vesicourethral angle and BNAP ratio were independent predictors of early recovery from post-prostatectomy incontinence. I suggest that both the sagittal and coronal views of postoperative cystography help anticipate early continence restoration after RARP.
