주부의 시간계획 및 관리전략에 따른 관리만족도에 관한연구 (A Study on the Managerial Satisfaction of Housewives' by the Time Planning and Time Management Strategies)

  • The purpose of this study were to investigate the independant influence of variable related to the managerial satisfaction and casual relation between variables. For this purposes the data were collected through the questionnaire whose respondent were 448 housewives. The major results of this study were as follows; 1. The variables influenced independently to the managerial satisfaction were wife's age income perceived time pressure time planning time management strategies. 2. Among all varialbles affecting the managerial satisfaction income time planning and time management strategies had direct effect. And household size employment status and educational level had indirect effect.

A multi-state model approach for risk analysis of pensions for married couples with consideration of mortality difference by marital status

  • Marital status has been identified as an important risk factor affecting adult mortality. Many studies have found that marriage has positive effects on mortality and increases life expectancy. Since most pension contracts providing retirement income are provided to married couples, mortality assumption for actuarial valuation based on the entire population is likely to overestimate the actual mortality of the group of beneficiaries specified in the contracts. This study considered the differences in mortality according to marital status to analyze the length and value of the payments of a typical pension contract for a married couple. The study quantified the effect on actuarial measurements of considering marital status in mortality assumptions with a multi-state model framework using Korean experience mortality data organized by marital status. The results of analysis indicate that considering marital status in mortality assumptions improves mortality risk management.

노인의 사회경제적 지위에 의한 건강불평등: 심리사회적 기제들의 매개효과 분석 (The Relationship between Socioeconomic Status and Health Inequality in Later Life: The Mediation Effects of Psycho-social Mechanisms)

  • 노년기 건강문제는 의료적 관점에서뿐만 아니라, 사회경제적 지위와 같은 사회적 조건과의 상호작용 속에서 해법을 찾을 필요가 있다. 이러한 인식 하에, 본 연구에서는 사회경제적 지위가 노인의 건강불평등에 이르는 메커니즘을 파악하고자 하였다. 구체적으로는 사회경제적 지위(소득 및 교육)와 신체적 건강의 세 차원(의료적 건강/기능적 건강/주관적 건강) 간의 사이에서의 중간기제로서 건강행동, 의료시설 접근성, 사회참여, 사회적 관계망의 매개효과를 규명하는 것이다. 연구모형의 검증을 위해 2014년에 실시된 전국노인실태조사 데이터를 2차 분석하였다. 본 연구의 대상은 65세 이상 남녀 노인 10,451명으로, 매개모형 검증을 위해 Baron & Kenny(1986)가 제안하는 절차에 따라 위계적 다중회귀분석을 실시하였고 매개효과의 유의성 검증을 위해 Sobel test를 실시하였다. 분석결과, 노인의 소득수준과 교육수준의 건강에 대한 효과가 동일하지는 않았고 건강의 하위차원에 따라서도 다소 다른 결과가 도출되기는 했지만, 전반적인 결과의 방향은 노인의 사회경제적 지위는 건강격차를 야기하고 있었으며, 그 메커니즘은 건강행동, 의료시설 접근성, 사회참여, 사회적 관계망을 매개한다는 것이다. 특히 본 연구에서 교육은 소득보다도 건강에 대한 보다 강력한 효과를 나타내었다는 점, 사회참여와 같은 사회통합요인의 효과가 부각되었다는 점, 거주지역을 통제한 이후에도 잔존한 의료시설 접근성의 유의미한 매개효과 등은 본 연구의 결과 중에서도 주목할 만하다. 이러한 결과를 바탕으로 사회경제적 조건으로 인한 건강불평등의 메커니즘을 규명하고 이를 완화시킬 수 있는 대안을 모색할 수 있었다는 점에서 본 연구의 의의가 있다.

의료비 지출이 종사상 지위 및 소득변화에 미치는 요인연구 (Research on Factors Influencing the Change of the Types of the Occupation and the Income by Medical Expenditure)

  • 본 연구는 의료비 지출이 가입자에게 미치는 경제적 영향을 의료비지출자의 종사상 지위변화와 가구내 소득 소비실태 변화를 중심으로 살펴보았다. 또한 의료비지출에 미치는 요인과 이와 같은 의료비가 종사상 지위변화 및 소득변화에 영향을 미쳤는지 분석하였다. 분석자료는 '한국노동패널'의 4차 연도 부가조사인 '건강과 은퇴' 응답자 가운데 의료비 지출자 4,215명의 자료를 5차 연도와 병합하였다. 분석결과 의료비지출자의 근로소득대비 의료비는 평균 5.5%로 나타났으나, 저지출 집단과 고지출 집단과의 격차가 크게 발생하였다. 또한 상대적 저소득그룹의 의료비 부담이 가구근로소득의 1/3을 차지하여 저소득계층은 의료적으로 취약할 뿐만 아니라 의료비부담이 과중함을 알 수 있었다. 한편 의료비 고지출 집단은 사적이전소득이 높아, 의료비가 발생할 경우 가족 및 친지로부터 의료비 등의 지원이 있는 것으로 보인다. 그러나 의료비가 발생할 경우 금융소득 및 부동산을 처분하여 의료비를 충당하는지에 대해서는 통계적으로 유의한 결과가 나타나지 않았다. 또한 의료비 지출자 가운데 종사상 지위는 14.4%만이 변화하였으며, 의료비 지출의 평균이상 여부가 종사상 지위변화의 주요 요인이 되었다. 즉, 의료비 저지출 집단은 건강이 상대적으로 나쁘지 않음을 의미하여 의료비의 비중이 낮을 뿐 아니라, 이와 같은 요인이 종사상 지위변화에 미치는 영향은 미미한 것으로 보인다. 그러나 의료비지출이 높은 그룹은 건강이 악화되어 종사상 지위까지 변화시킨 것으로 보인다. 나아가 이와 같은 종사상 지위변화는 총소득 변화에 부(-)적인 영향을 미쳐, 종사상 지위가 변화된 경우 총소득이 감소하여 가구내 경제상황이 더 악화된 것으로 보인다. 따라서 우리나라 건강보험은 질병으로 인하여 발생하는 비용과 함께, 경제활동 축소 및 중지에 따라 발생하는 소득손실을 보장하는데 미흡한 것으로 판단된다.

성인 여성의 자궁경부암 및 유방암 조기검진 수검행위에 영향을 미치는 요인 (The Influence Factors on Cervical and Breast Cancers Screening Behavior of Women in a City)

  • This study was aimed at preparing basic data required for establishment of a cancer screening program by examining the status of cancer screenings performed by women in a city and the factors that had an influence on cervical and breast cancers screening behavior of women. In order to find out the status of cervical and breast cancers screening and the factors having an influence on cervical and breast cancers screening behavior, 1,113 women were selected as study area and subjects by a random cluster sampling method, and the subjects were answered by questionnaires. The major results were as follows: 1. In the cases of the breast and the cervical cancers, 16.7% and 55.7% of the subjects turned out to have had one or more screenings respectively in their life-time. Also the rate of screening group of cervical and breast cancers was 16.7%, the rate of screening group of only cervical cancer was 38.9%, and the rate of non-screening group was 44.4%. 2. As to the screening pattern of cervical and breast cancer, there were significantly for age, income, occupation, married status, spouse, the factors associated with health promotion (doctor visiting, exercising), married age, number of children, and breast feeding(p〈0.05). 3. The factors associated with screening behaviors for cervical and breast cancers were age, income, occupation, married status, doctor visiting, exercising, married age, number of children, and breast feeding(p〈0.05). Based on the above-mentioned results obtained by this study, it was anticipated that this study may be playa vital role as basic data for the development and execution of cervical and breast cancers screening program for women in a community. And the analysis, done on the basis of the status of the cancer screening, of the influence factors on cervical and breast cancers screening behavior showed that for the development of a cancer screening program, the factors like age, social-economic class, married status, health promotion behavior, and reproductive characteristics should be considered.

사회경제적 위치와 유방암 수술 후 총 사망위험과의 관련성 (Association Between Socioeconomic Status and All-Cause Mortality After Breast Cancer Surgery: Nationwide Retrospective Cohort Study)

  • Objectives: This study aims to evaluate and explain the socioeconomic inequalities of all-cause mortality after breast cancer surgery in South Korea. Methods: This population based study included all 8868 females who underwent radical mastectomy for breast cancer between January 2002 and June 2003. Follow-up for mortality continued from January 2002 to June 2006. The patients were divided into 4 socioeconomic classes according to their socioeconomic status as defined by the National Health Insurance contribution rate. The relationship between socioeconomic status and all-cause mortality after breast cancer surgery was assessed using the Cox proportional hazards model with adjusting for age, the Charlson’s index score, emergency hospitalization, the type of hospital and the hospital ownership. Results: Those in the lowest socioeconomic status group had a significantly higher hazard ratio of 2.09 (95% CI =1.50 - 2.91) compared with those in the highest socioeconomic group after controlling for all the identifiable confounding variables. For allcause mortality after radical mastectomy, all the other income groups showed significantly higher 3-year mortality rates than did the highest income group. Conclusions: The socioeconomic status of breast cancer patients should be considered as an independent prognostic factor that affects all-cause mortality after radical mastectomy, and this is possibly due to a delayed diagnosis, limited access or minimal treatment leading to higher mortality. This study may provide tangible support to intensify surveillance and treatment for breast cancer among low socioeconomic class women.

Association between health status and tooth loss in Korean adults: longitudinal results from the National Health Insurance Service-Health Examinee Cohort, 2002-2015

  • Purpose: This study investigated the association between health status and tooth loss based on data from the National Health Insurance Service-Health Examinee Cohort in 2002-2015. Methods: Multivariate Cox proportional hazards regression analyses were applied to a longitudinal retrospective database, which was updated and newly released in 2018, to assess the association between health status and tooth loss while adjusting for potential confounders among sociodemographic and economic factors (sex, age, household income, insurance, and presence of disability), general and oral health status (body mass index [BMI], smoking and drinking status, periodic dental visits and scaling, and brushing before sleep), and comorbid disease (hypertension, diabetes mellitus [DM], and Charlson comorbidity index [CCI]). Results: Among 514,866 participants from a South Korean population, 234,247 (45.5%) participants satisfying the inclusion criteria were analyzed. In the adjusted multivariate analysis, sex, age, household income, insurance, presence of disability, BMI, smoking and drinking status, periodic scaling, tooth brushing before sleep, DM, and CCI showed statistically significant associations with the loss of at least 1 tooth. The risk of experiencing a loss of ${\geq}4$ teeth was associated with an increase in age (in those 50-59 years of age: hazard ratio [HR], 1.98; 95% confidence interval [CI], 1.93-2.03; in those 60-69 years of age: HR, 2.93; 95% CI, 2.85-3.02; and in those 70-79 years of age: HR, 2.93; 95%, CI 2.81-3.05), smoking (HR, 1.69; 95% CI, 1.65-1.73), and DM (HR, 1.43; 95% CI, 1.38-1.48). Conclusions: The results of this study showed that the risk of experiencing tooth loss was related to multiple determinants. DM and smoking were especially significantly associated with tooth loss.

제조업 및 건설업 근로자들의 근로 형태와 COPD발생 간의 연관성 (Association between Employment Status and the Prevalence of COPD for Manufacturing and Construction Workers)

  • Objective: This study aims to analyze the relationship between employment status and the prevalence of chronic obstructive pulmonary disease (COPD) for manufacturing and construction workers. Methods: Subjects, who had or have worked as temporary workers in manufacturing or construction worksites for the longest career years, were extracted from the KNHANES(2007-2009) database. Characteristics of demographical, socio-economic, and smoking history were treated as independent variables. Considering that smoking experience and age are significantly influential in the prevalence of COPD, logistic regressions were analyzed after stratifying subjects according to their smoking experience and age. A two-stage stratified systematic sampling method was applied for the logistic regression analysis. Results: Age, gender, and household income level were associated with the prevalence of COPD in both smoking and non-smoking groups. However, employment status was only significant among smoking group. Smoking experience and age were significantly related to the prevalence of COPD among all age groups. In addition, employment status and household income level were also associated with the prevalence of COPD in young aged group. But gender was the only significant factor among the elder group. Conclusions: This study confirmed that employment status was a major social factor which may be related to the prevalence of COPD. Proper supports such as providing safety measures should be provided to temporary workers to manage their respiratory health.
