  • Optical power splitters and/or couplers are important components for optical signal distribution between channels both in wavelength division multiplexing(WDM) systems and photonic integrated circuits(PICs). Since polarization is usually not known after propagation in an optical fiber, passive WDM components have to be polarization insensitivity, Compared to alternatives such as directional couplers or Y-junction splitters, splitters based on multimode interference(MMI) have found a growing interest in recent yens because of their desirable characteristics, such as compact size, low excess loss, wide bandwidth, polarization independence, and relaxed fabrication tolerances$^{(1)}$ . These devices have been fabricated in polymers, silica, or III-V semiconductor materials. A1 $\times$ 4 MMI power splitter on InP materials that were suitable for application in the 1.55-${\mu}{\textrm}{m}$ region$^{(2)}$ . However, the fabrication process of the structure is too complicated and the photolithography tolerance is very tight. Also, a 1 $\times$ 16 InGaAsP/InP MMI power splitter with an excess loss of 2.2dB and a splitting ratio of 1.5dB was demonstrated by using deep etching$^{(3)}$ . The deep etching of the sidewalls through the entire guide layer of the slab waveguide resulted in a number of drawbacks$^{(4)}$ . (omitted)

Clinical Study on Effect of Carthmi- Flos Herbal acupuncture therapy to Low back pain patient (요통환자(腰痛患者)의 홍화약침치료(紅花藥針治療)에 대한 임상적(臨床的) 연구(硏究))

  • Objective : To compare the effect of Carthmi- Flos herbal acupuncture theraphy and acupucture treatment to Low back pain patient. Methods : 70 patients with Low back pain are evaluated by being divided Carthmi- Flos herbal acupuncture theraphy Group(HAG, 30 peples) and non- Carthmi- Flos herbal acupuncture theraphy Group(NHAG, 40 peoples) at Ik-San Oriental medical hospital in wonkwang university from the first November 2000yr to 31th December 2001yr. Results : 1. In the cause of Low back pain, the most of HAG is acute sprain(9cases, 30%) and NHAG is non-inducement(13 cases, 22.5%). 2. In the analysis of the radiation result, HIVD and Degenerative change respectively were 22 cases(50%) in HAG and 29 cases(52.8 %) in NHAG. 3. The effect of treatment by Duration as follows :Two groups were the most improved highly in acute stage and they were improved lowly in chronic stage(6 month). 4. The effect of treatment by condition of patients was follows: After treatment, Grade IV & III were decreased 69.5% in HAG and 60% in NHAG. 5. In the distribution of treatment progression : 28 cases(93.3%) were recoverd in HAG but 30 cases(90%) in NHAG. Conclusion : These results shows that the effect of treatment by Carthmi- Flos herbal acupuncture treatmen is exellent by relaxing contracted muscles, strengthening weakened ligaments and improving inflammatory parts.

Surgical Management of Unruptured Intracranial Aneurysms (비파열 뇌동맥류의 수술적 치료)

  • Objective : The purpose of this report is to assess the morbidity and mortality associated with clipping of intracranial unruptured aneurysms. Methods : At the authors' institution between May 1989 and December 1998, a total of 128 unruptured aneurysms in 110 patients were treated with surgical clippings. The medical records and neuroimaging studies of the patients were reviewed retrospectively. Results : The main locations of the aneurysms were : middle cerebral artery 31%, internal carotid-posterior communicating artery 28%, anterior communicating artery 16%, paraclinoid 6.5%, internal carotid-anterior choroidal artery 7%, posterior circulation 7%. Forty three percent of the aneurysms were symptomatic and 57% asymptomatic. The overall outcome of the surgery was : Glasgow outcome scale(GOS) I 86%, GOS II 6%, GOS III 4.3%, GOS IV 0% and GOS V(death) 3.5%. The operative risk is higher for large to giant aneurysms, and for aneurysms in posterior circulations. Patients with non-giant aneurysm in anterior circulation showed no mortality, but morbidity of 8.2%, and in posterior circulation : 25% of mortality and 75% of morbidity. Patients with giant anterior circulation aneurysm have 22% of mortality and 22% of morbidity. For patients with giant posterior circulation aneurysm, mortality and morbidity were 25% and 25%, respectively. The postoperative deaths were related to occlusion of the major parent artery in 3 cases(75%). The postoperative morbidity was related to occlusion of artery(9/13), intraoperative rupture(3/13), and cranial nerve injury(1/13). Conclusion : This report documents 3.5% mortality and 13% of morbidity in the clipping surgery for unruptured intracranial aneurysms, and the relatively low risk of surgical clipping in non-giant and those located in anterior circulation. The natural history, especially risk of bleeding, of the unruptured intracranial aneurysms is still controversial. However, with respect to surgical results, unruptured non-giant aneurysm located in anterior circulation should be operated in patients with low risk.

Treatment Outcome with Brachytherapy for Recurrent Nasopharyngeal Carcinoma

  • Background: To evaluate the treatment outcome and major late complications of all patients with recurrent nasopharyngeal carcinoma (NPC) treated with intracavitary brachytherapy (ICBT) in Hospital Kuala Lumpur. Materials and Methods: This retrospective study was conducted at the Department of Radiotherapy and Oncology, Hospital Kuala Lumpur, Malaysia. All patients with histologically confirmed recurrent NPC in the absence of distant metastasis treated in the period 1997-2010 were included in this study. These patients were treated with ICBT alone or in combination with external beam radiotherapy (EBRT). Treatment outcomes measured were local recurrence free survival (LRFS), disease free survival (DFS) and overall survival (OS). Results: Thirty three patients were eligible for this study. The median age at recurrence was 56 years with a median time to initial local recurrence of 27 months. Majority of patients were staged as rT1-2 (94%) or rN0 (82%). The proportion of patients categorised as stage III-IV at first local recurrence was only 9%. Twenty one patients received a combination of ICBT and external beam radiotherapy while 12 patients were treated with ICBT alone. Median interval of recurrence post re-irradiation was 32 months (range: 4-110 months). The median LRFS, DFS and OS were 30 months, 29 months and 36 months respectively. The 5 year LRFS, DFS and OS were 44.7%, 38.8% and 28.1% respectively. The N stage at recurrence was found to be a significant prognostic factor for LRFS and DFS after multivariate analysis. Major late complications occurred in 34.9% of our patients. Conclusions: Our study shows ICBT was associated with a reasonable long term outcome in salvaging recurrent NPC although major complications remained a significant problem. The N stage at recurrence was a significant prognostic factor for both LRFS and DFS.

Effects of dietary fiber levels on cecal microbiota composition in geese

  • Objective: This study shows the effects of dietary fiber levels on cecal microbiota composition in geese at day 70 according to pyrosequencing of the 16S ribosomal RNA gene. Methods: A total of 468 1-day-old healthy male Yangzhou goslings with similar body weight were randomly divided into 3 groups with 6 replicates per group and 26 geese per replicate. Geese were fed diets with fiber levels of 2.5% (low fiber level diet, Group I) and 6.1% (Group III) during days 1-70, respectively, or 4.3% for days 1-28 and 6.1% for days 29-70 (Group II). Results: Low fiber level diet decreased body weight, average daily gain during, increased lower feed conversation rate of geese during day 1 to 70 (p<0.05). Low fiber level diet decreased the total operational taxonomic units, Chao1 index and Shannon index, whereas increased the Simpson index of cecal microbiota in geese at day 70. Low fiber level diet decreased the relative abundance of Bacteroidetes, Firmicutes, Bacteroides, and Paraprevotella in cecum of geese at day 70. The similarity of cecal microbiota between low fiber level diet group and other groups was smaller. Conclusion: This study indicates that the low fiber level diet decreased diversity of microbiota, and relative abundance of some beneficial microbiota in cecum of geese at day 70, implying that the low fiber level diet has negative influence on performance by altering the diversity and population of cecal microbiota in geese.

Biological improvement of reclaimed tidal land soil (III) (해안간척지 토양의 생물학적 토성 개량에 관한 연구 3)

  • Two kinds of organic materials, powders of Salicornia and Oryza sativa L. wre added as a source of organic matter to the suspensions of saline soils(soil : water = 1:3) to be 4 per cent of dried saline soil grams. And then, the samples were incubated at $28^{\circ}C$ to improve the decomposition of organic materials by soil microflora. Resutls of this experiment are summarized as followings : 1) The pH of soil suspension showed its highest value on the second or third week after the treatments, which were similar to those of the soil microflora. Results of this experiment are summarized as follwings : 1) The pH of soil suspension showed its highest value on the second ot third week after the treatments, which were similar to those of the soil microflora populations. 2) Salinity increased up to the second week and the highest value of it appeared in the samples which were treated with the powder of Salicornia. In general, the salinity of all samples decreased on the third week after the treatment. The fact was assumed the possibilities of desalination from saline soil by the microbial actions. 3) Soil microflora such as bacteria, actinomycetes and fungi, were determined its populations soil microflora is to act as decomposer in soil. Both of the bacteria and actinomycytes population showed in the third weak after the tratment. In general, 30-years old of saline soil contained microbial population much more than those of 5-year old of saline soil. Salicornia powder favored the increase of both the bacteria and fungal population, and Oryza sativa L. seemed to have been a great role in increase of actinomycetes. Especially, fungal population of the untreated soil suspension contained higher microbial populations more than those of the soils treated with both of the organic materials. 4) Sugar contents of soil suspesions decreased remarkably on the first week after the treatments. 5) The amount of nitrate had similar tendency to those of population changes of soil microflora. Total acidity decreased continuously and the amount of potassium showed its highest value on the third week treatment. However, the amount of phosphorus was determined to be insignificant.

Discrepancy of the location of depression on the soft tissue and the bone in isolated zygomatic arch fracture

  • Background: When performing reduction of zygomatic arch fractures, locating the inward portion of the fracture can be difficult. Therefore, this study investigated the discrepancy between the locations of the depression on the soft tissue and bone and sought to identify how to determine the inward portion of the fracture on the patient's face. Methods: We conducted a retrospective review of chart with isolated zygomatic arch fractures of type V in the Nam and Jung classification from March 2013 to February 2022. For consistent measurements, a reference point (RP), at the intersection between a vertical line passing through the end point of the root of the ear helix in the patient's side-view photograph and a transverse line passing through the longest horizontal axis of the external meatus opening, was established. We then measured the distance between the RP and the soft tissue depression in a portrait and the bone depression on a computed tomography (CT) scan. The discrepancy between these distances was quantified. Results: Among the patients with isolated zygomatic arch fractures, only those with a fully visible ear on a side-view photograph were included. Twenty-four patients met the inclusion criteria. There were four types of discrepancies in the location of the soft tissue depression compared to the bone depression: type I, forward and upward discrepancy (7.45 and 3.28 mm), type II, backward and upward (4.29 and 4.21 mm), type III, forward and downward (10.06 and 5.15 mm), and type IV, backward and downward (2.61 and 3.27 mm). Conclusion: This study showed that discrepancy between the locations of the depressions on the soft tissue and bone exists in various directions. Therefore, applying the transverse and vertical distances measured from a bone image of the CT scan onto the patient's face at the indicated RP will be helpful for predicting the reduction location.

Sasang Constitution Classification related to an aspect of distribution GCM(General Coordinative Manipulation) Body Type and Experimental Study based on the character of Static Posture and Dynamic Hyper/Hypo-mobility Pattern (사상의학의 4체질 분류에 따른 각 체질별 전신조정술 체형분포 양상과 그에 따른 정적 자세특성 및 동적 운동증감 양상에 관한 실증적 연구)

  • The Purposes of this study were to find complementary connectible new factors that analyzed correlation relate of Sasang Constitution and GCM Body Type in Static Posture and Dynamic Hyper/Hypo-mobility Pattern. Method of this study was asymtomatic volunteers 232(unmarried man and women), conducted from September 1 to December 31. In this main study progressing step diagnosised first, Constitution of Sasang medicine after being classified into four groups of Soyangin, Taeumin, Soeumin, Taeyangin diagnosis of GCM Body Type and progressed that related Static Posture and Dynamic Hyper/Hypo mobility Pattern. The results are as follows. Distribution of Sasang Medicine Constitution proved to be Taeyangin 13, Soyangin 66, Taeumin 67, Soeumin 86 respectively. Distribution of GCM Body Type proved I Body Type 72(31.0%), II Body Type 54(23.3%), III Body Type 89(38.4%), IV Body Type 17(7.3%). The distribution of Sasang Constitution according to GCM Body Type was that; I Body Type was distributed in the order Soeumin 34.7%(25), Taeumin 31.9%(23), Soyangin 30.6%(22), Soeumin 34.7%(25) is the most people. II Body Type was distribution of in the order Soeumin 42.6%(23), Soyangin 5.9%(14), Taeumin 24.1%(13), Soeumin 42.6%(23) is the most people. III Body Type was distribution of in the order Soeumin 37.1%(33), Taeumin 30.3%(27), Soyangin 28.1%(25), Soeumin 37.1% is the most people. IV Body Type proved high distribution each of Soeumin 29.4%(5) and Soyangin 29.4%(5). In case of main left side posture character of spine and limbs about I Body Type 72 persons with left scapular and ilium forward tilted pattern proved in the order high distribution iliac crest thigh and scapular high 70(97.2%), gluteal fold high and scapular abduction 69(95.8%), lumbar scoliosis 65(90.3%). Also, in case of right side posture character proved in the order high distribution deep gluteal fold 69(95.8%), umbilical deviation 68(94.4%). Incase of main left side posture character of spine and limbs about n Body Type 54 persons proved in the: order high distribution knee hyperextension 50(92.6%), shoulder deviation 49(90.7%) etc. Also, in case of right side posture character proved in the order high distribution pelvic deviation 53(98.1%), iliac crest thigh 52(96.3%), hip flexion and ankle inversion 51(94.4%) etc. In case of main left side posture character of spine and limbs about III Body Type 89 persons proved in the order high distribution shoulder deviation 87(97.8%), scapular abduction 86(96.6%), scapular high 85(95.5%) etc. And in case of right side posture character proved in the order high distribution pelvic deviation and iliac crest thigh 86(96.6%) etc. In case of main left side posture character of spine and limbs about IV Body Type 17 persons proved in the order high distribution pelvic deviation, lumbar sciliosis and lumbar lordosis increase 15(88.2%) etc. Also, in case of right side posture character proved in the order high distribution wrist dorsiflexion 16(94.1%), thickened achilles tendon etc.

A CLINICOSTATISTICAL ANALYSIS OF ORAL CANCER PATIENTS FOR RECENT 8 YEARS (최근 8년간 구강암 환자에 대한 임상통계학적 연구)

  • We investigated 248 patients who were diagnosed as malignant tumor in the department of Oral and maxillofacial Surgery of Kyungpook National University from 1999 to 2006, and following results were obtained. 1. Among 248 patients who have malignant tumor, 164 were men and 84 were women, which made the ratio of male to female 1.95:1. 2. The average age of oral cancer patients was 58.3. 3. As of the primary origin site, lower alveolus and gingiva were the greatest with 70 cases(28.2%), followed by tongue(l6.9%), upper alveolus and gingiva(14.9%), palate(13.7%), mouth floor(9.7%), buccal mucosa(4.8%), retromolar trigone(4.4%), Mx. & Mn. bone(3.2%) and lip(2.8%). 4. As of histologic distribution, squamous cell carcinoma was the greatest with 170 cases(68.6%), followed by sarcoma with 17 cases(6.9%), adenoid cystic carcinoma with 17 cases(6.9%), malignant lymphoma with 15 cases(6.0%), mucoepidermoid carcinoma with 13 cases(5.2%), metastatic carcinoma with 6 cases(2.4%) and malignant melanoma with 4 cases(1.6%). 5. Period between recognition of the symptom and the first visit to hospital was less than 3 months for 58.9% of the patients, and more than 3 months for 41% of the patients. 6. Investigation of whether the patients drink or smoke revealed that the number of non-smoking and non-drinking patients was 63 among 170 patients(37.0%) that were able to investigate. The number of patients who smoke only was 29(17.1%) and both drinking and smoking patients were 78(45.9%). 7. In clinical stage order, Stage IV(61.7%) was found th be the largest, followed by stage I(17.2%), stage II(13%) and stage III(7.8%). 8. The 5-year survival rate of the entire oral cancer patients appeared to be 57.7%. The survival rate was higher in younger group and women had higher survival rate but there was no statistical significance to this. In the aspect of stage, the survival rate was Stage I, Stage II, Stage IV and Stage III in decreasing order. The order according to T classification was the same. In N classification, patients with N0 had the highest survival rate and the survival rate decreased in the order of N1 and N2. Survival rate was especially low in patients with N2.

Intracranial Meningiomas, WHO Grade II : Prognostic Implications of Clinicopathologic Features

  • Objective : Intracranial meningiomas are primarily benign tumors with a good prognosis. Although WHO grade II meningiomas are rare (2-10%), WHO grade II meningiomas have higher recurrence and mortality rates than benign. We evaluated the patient recurrence rate and investigated the prognostic factors of WHO grade II meningiomas. Methods : Between 1993 and 2005, 55 patients were diagnosed with WHO grade II meningiomas in our hospital. WHO grade II meningiomas (n=55) were compared with other WHO grades meningiomas (I, n=373; and III, n=20). The patients had a median age of 48.4 years (range, 14-17 years), a male-to-female ratio of 26 : 29, and a mean follow-up time of 45 months (range, 3-175 months). Results : In WHO grade II meningiomas, only the extent of resection was a significant prognostic factor. Post-operative radiotherapy had no significant influence on tumor recurrence (p=0.053). The relative risk of recurrence was significantly higher in WHO grade II meningiomas with incomplete resection (10/27, RR=37%) than in WHO grade II meningiomas with complete resection (4/28, RR=14%) regardless of post-operative radiotherapy. In the incomplete resection group, Simpson grade III or IV had a significantly high risk of recurrence regardless of post-operative RT (n=3, RR=100%) However, if the degree of resection was Simpson grade II, the recurrence rate was similar to the complete resection group even though post-operative RT was not performed. Conclusion : Complete resection was the most powerful independent predictive factor of the recurrence rate in WHO grade II meningiomas. Post-operative adjuvant RT was not a significant factor in this study.
