The Improvement and Completion of Outcome index: A new assessment system for quality of orthodontic treatment

  • Objective: Given the considerable disagreement between the Peer Assessment Rating (PAR) index and the American Board of Orthodontics Cast-Radiograph Evaluation, we aimed to develop a novel assessment system-the Improvement and Completion of Outcome (ICO) index-to evaluate the outcome of orthodontic treatment. Methods: Sixteen criteria from 4 major categories were established to represent the pretreatment malocclusion status, as well as the degree of improvement and level of completion of outcome during/after treatment: dental relationship (arch length discrepancy, irregularity, U1-SN, and IMPA); anteroposterior relationship (overjet, right and left molar position, ANB); vertical relationship (anterior overbite, anterior open-bite, lateral open-bite, SN-MP); and transverse relationship (dental midline discrepancy, chin point deviation, posterior cross-bite, occlusal plane cant). The score for each criterion was defined from 0 or -1 (worst) to 5 (ideal value or normal occlusion) in gradations of 1. The sum of the scores in each category indicates the area and extent of the problems. Improvement and completion percentages were estimated based on the pre- and post-treatment total scores and the maximum total score. If the completion percentage exceeded 80%, treatment outcome was considered successful. Results: Two cases, Class I malocclusion and skeletal Class III malocclusion, are presented to represent the assessment procedure using the ICO index. The difference in the level of improvement and completion of treatment outcome can be clearly explained by using 2 percentage values. Conclusions: Thus, the ICO index enables the evaluation of the quality of orthodontic treatment objectively and consecutively throughout the entire treatment process.

  • In recent domestic automotive industry, applications of computer simulation to the manufacturing of stamping dies for inner and outer body panels which greatly affect durability and aesthetic quality of automobiles, have been increased. Enhancement of die quality, and reduction of total die manufacturing time and consequently manufacturing cost are the visible outcome. However, to successfully apply the result of simulation by a commercial package to the die manufacturing, development of an optimal die manufacturing process is required upon the completion of analysis of forte and shortcomings of available sheet metal forming softwares in the market. Based on the results of numerical analysis of front door outer panel forming, this paper evaluates the applicability of simulation results to the real die making for automotive body panels. Also, it attempts to select an optimal die manufacturing process including design, machining and tryout. Lastly, it discusses the expected effects by adopting the selected process in a real stamping die manufacturing facility.

The Influence of MMPI Characteristics on the Outcome of Cognitive Behavioral Therapy for Panic Disorder (공황장애 환자의 다면적 인성검사 (MMPI) 특성이 인지행동치료 결과에 미치는 영향)

  • Objective : This study was designed to find the characteristics of MMPI that could influence the outcome of cognitive behavioral therapy(CBT) for panic disorder. Methods : 34 patients who met DSM-IV criteria for panic disorder with or without agoraphobia had completed 11 weekly sessions of cognitive behavioral therapy. All the patients were assessed with MMPI before the initiation of treatment. Five self-report measures including Beck Depression Inventory(BDI), Spielberger State-Trait Anxiety Inventory(STAI), Agoraphobic Cognition Questionnarie(ACQ), Body Sensation Questionnaire(BSQ), and Daily Anxiety Selfrating (0 - 8 scales) were also assessed as a pre- and post-treatment assessment. After the completion of treatment, patients were classified by the High End-State(HES) functioning group and the Low End-State(LES) functioning group for the data analysis. Results : 1) The LES group showed significantly higher scores in Hypochondriasis Scale(HS), Depression Scale(D), Hysterical Scale(Hy), Obsessive Scale(Pt), Schizophrenia Scale(Sc) and Validity Scale(F) of MMPI than the HES group. However, these differences gave impressions that the LES group had more severe symptoms rather than that they could be the factors influencing the outcome of CBT. 2) Though, the severity of symptoms of the LES group in the 5 measures of pre-assessment was basically higher than that of the HES group. The fact that both group showed the similar improvement between pre- and post-assessment supported the above interpretation. Conclusion : In regarding the above results, MMPI was not a proper tool that could provide the factors influencing the outcome of CBT. In the future study, the authors need to use a different tool that can find the personality characteristics more directly.

A Study on the Effect of Mind Liberal Arts Course based on Mindfulness for Female University Students (여대생을 대상으로 한 마음챙김기반 마음교양강좌의 효과 연구)

  • The purpose of this study was to confirm the effect of mindfulness education on perceived stress, mindfulness, psychological well-being, and brain waves. The study used a repeated-measures design to examine 42 female university students currently enrolled in mindfulness courses. Subjects participated in the mindfulness course once a week for eight weeks, 50 minutes per session. The main outcome variables, such as perceived stress, mindfulness, and psychological well-being, were measured with a structured questionnaire and brain waves were measured with an electroencephalograph. The same variables were then measured after the completion of the eight-week course. The results demonstrate that perceived stress significantly decreased from 1.85±0.39 to 1.66±0.39 points (t=3.44, p=.001), while, on the other hand, mindfulness increased from 3.10±0.26 points to 3.19±0.32 points (t =-2.78, p=.008). There was also a significant difference in the level of autonomy, a subcategory of psychological well-being (t=-2.70, p=.010). In addition, EEG indicated significant changes in the frequencies of Theta channel 19 and Rapid Alpha channels 7 and 10. These results suggest that further studies are needed to elucidate the effects of mindfulness education through the modification of mindfulness course plans, diversification of teaching methods, and improvement in the educational environment.

The Effects of Rehabilitation Exercise Using a Home Video Game (PS2) on Gait Ability of Chronic Stroke Patients (가정용 게임기를 이용한 재활운동이 뇌졸중 환자의 보행 능력에 미치는 효과)

  • The purpose of this study was investigate the effect of rehabilitation exercise using a home video game(PS2, Sony) on gait ability in the chronic stroke patients. The subjects of this study were 32 patients who underwent stroke for more than 6 months and were assigned to a game-based exercise group (n=16) or a control group (n=16). The Game-based exercise group executed a rehabilitation exercise three times a week during 6 weeks, at the rehabilitation unit. The exercise was performed by PS2 for one hour. Control group maintained their usual life without application of exercise. Outcome measures included gait ability. After the completion of the game-based exercise, 10m walking velocity was improved significantly (p<0.05). Six minute walking distance was increased significantly (p<0.05). These results showed that the rehabilitation exercise using a home video game is effective in the improvement of gait ability in chronic stroke patients. This study suggest that interesting, motivating game-based rehabilitation exercise and effective in recovery of function exercise for chronic stroke patients should be necessary to develop and apply.
