A Case Report on the Use of Korean Medicine Treatment for a Patient with Dysarthria Caused by Progressive Bulbar Palsy (PBP)

  • Objectives : To introduce a rare case of a patient with Progressive Bulbar Palsy (PBP) and suggest the possibility of treatment using electroacupuncture and Korean Medicine. Methods : A 61-year-old man with PBP, complaining of dysarthria, was treated with electroacupuncture and Korean Medicine, from June 16 to July 10, 2015. Improvements in symptoms were measured using the speech mechanism screening test (SMST), measurement of tongue and orbicularis oris motility, and speech handicap index (SHI). Results : The scores of SMST, motility of tongue and orbicularis oris showed a tendency for gradual improvement with 25 days of Korean Medicine Treatment after admission, but conversation was still impossible. In SHI scores, one point increased in the speech domain and one point decreased in the psycho-social domain, and three points increased in the other domain. Conclusion : Electroacupuncture and Korean Medicine Treatment improved dysarthria caused by PBP, but not completely. Korean Medicine Treatment seems effective in the management of accompanying symptoms such as black hairy tongue, dry mouth, and general condition. The symptoms of PBP are similar to those of amyotrophic lateral sclerosis and there is controversy regarding the classification of PBP. The most important aspect of treating a patient with PBP is an early diagnosis and devising appropriate rehabilitation strategies.

  • In the conservation treatment of wood remains, polymers, treatment methods, and treatment period can be varied, depending on the percentage of moisture content and species of trees. Thus, in order to apply the appropriate conservation treatment, a lot of information from the wood remains are required. Despite the information from the wood remains, testing a part of the wood remains is necessary to get information about the polymers. When the wooden coffin made of camphor trees was excavated, it was cracked in the section of radiation, and there were hairlines cracks. Therefore, without any conservation treatment, it is impossible to restore it. This experiment is for the conservation treatment of the wooden coffin excavated from Songhyun-dong tombs No 7, Changnyeong. For this experiment, I used camphor trees from Jejudo. The purpose of this experiment is to asses the possibility of preliminary testing materials before the conservation treatment on the wooden coffin. After the camphor trees from Jejudo are artificially aged by Fenton's reagent, they are As a result of oxidation by Fenton, the degree of oxidation is high in proportion to the amount of peroxide and iron sulfate. Among several samples, the sample 'na' made the most similar result to the wooden coffin from Songhyun-dong. If I use this condition to make the sample in polymer tests, it is very helpful to find the most suitable polymer for the conservation treatment of wooden coffin. After the same species of trees are oxidized by Fenton and they become the similar condition, we are able to find suitable methods of conservation treatment. The suitable methods reduce the demage of the remains and make us to get more information. Thus, we are able to carry out conservation treatment more exactly.

  • The objective of this study is to analyze the propriety of the Biotope Area Factor's(BAF) application and propose an improvement plan. The BAP system, initially started by Seoul metropolitan city, is being settled in Korea. The BAF originated from the Biotop $Fl{\ddot{a}}chen$ Faktor(BFF) system of Berlin Germany. It was established as part of the Landscape Plan for the ecological function recovery in the high density built-up area with a sense of environmentally friendly urban management. The study compared the BAF's present condition of Ganddong-Gu, Seoul with Seoul's BAF system. Some problems appeared from the system application. Firstly, it may cause ecological damage if the site ranges are more than current BAF system limits. Secondly, the application of the current BAF system has the possibility of general redevelopment, but the partial improvement considering current standards of the high density built-up area's paving section is impossible. Lastly, division of the application object and application type are not divided well. In addition, the Seoul BAF which is currently applied across the board is based on the destruction of the natural area and low density built-up area. Accordingly, to improve these problems requires a complementary system protecting the ecological function prior to the application of the BAF and with restricted application to high density of BAF system built-up area.

Abdominal Wall Closure Using Artificial Mesh for Patients with an Open Abdomen (복부개방(Open abdomen) 환자에서 인공막(Artificial Mesh)를 이용한 근막봉합술)

  • Purpose: After damage control surgery, abdominal wall closure may be impossible due to increased intra-abdominal pressure (IAP), and primary closure may induce abdominal compartment syndrome. The purpose of this study was to investigate changes in the IAP and the feasibility of abdominal wall closure using artificial mesh. Methods: From July 2010 to July 2011, 8 patients with intra-abdominal hypertension underwent abdominal wall closure using artificial mesh. Medical data such as demographics, diagnosis, operation, IAP, postoperative complications, mortality and length of hospital stays were collected and reviewed, retrospectively. One patient was excluded because of inadequate measurement of the IAP. Results: Seven patients, 4 males and 3 females, were enrolled, and the mean age was 54.1 years old. Causes of operations were six traumatic abdominal injuries and one intra-abdominal infection. The IAP was reduced from $21.9{\pm}6.6mmHg$ before opening the abdomen to $15.1{\pm}7.1mmHg$ after fascial closure. Fascial closure was done on $14.9{\pm}17.5$ days after the first operation. The mean lengths of the hospital and the intensive care unit (ICU) stays were 49.6 days and 29.7 days respectively. Operations were performed $3.1{\pm}1.5$ times in all patients. Two patients expired, and one was transferred in a moribund state. Three patients suffered from complications, such as retroperitoneal abscesses, enterocutaneous fistulas, and bleeding that was related to the negative pressure wound therapy. Conclusion: After abdominal wall closure using artificial mesh, intra-abdominal pressure was well controlled, and abdominal compartment syndrome does not occur. When the abdominal wall in patients who have intra-abdominal hypertension is closed, artificial mesh may be useful for maintaining a lower abdominal pressure. However, when negative pressure wound therapy is used, the possibility of serious complications must be kept in mind.

  • Quality evaluation for food and agricultural products have always been one of the most elusive problems associated with the handling , processing and marketing in a food plant production. In order to detect physical foreign materials in food and agricultural products, non-destructive techniques have been developed for many years. Application of X-ray system to detect physical foreign materials in food and agricultural products could be considered to be a high potential method. Especially , it is impossible to detect internal physical foreign materials by visual inspections. In this study, it was tried to be applied for two different X-ray devices. Soft X-ray system with CdTe sensor and X-ray CT scanner were evaluated for advantage of the detection of non-meltallic foreign materials in food and agricultural products . Though the soft X-ray is not a high energy radiation, it is possible to detect small different density in a material. The CdTe sensor has a high resolution for t e soft X-ray energy region. The density characteristics of foods and foreign material were expressed region. The density characteristics of foods and foreign materials were expressed as a soft X-ray energy spectrum. The energy spectrum was analyzed by a personal computer with a multi-channel analyzer. X-ray CT scanner can provide visual image and analyze by three dimensional information inside food and agricultural products. The X-ray CT scanner using as a medical equipment was used to detect a foreign material. The density characteristics of food and foreign materials in food were tried to be detected by the threshold value on the basis of the CT numbers. The soft X-ray absorption characteristics for acrylin plates and distilled water were obtained and could be found the possibility of detecting a small physical foreign materials such as a plastic wrapping film , a stone and grasshopper in food and agricultural products.

Feasibility study on shale gas wastewater treatment using membrane distillation (막 증발법을 이용한 셰일가스 폐수 처리 가능성 평가)

  • Development of shale gas has drawn increasing attention since it is one of promising alternative energy resources. However, contamination of groundwater and surface water during the extraction of shale gas is becoming a serious environmental issues, which brings the needs to treat wastewater generated from hydraulic fracking. In this study, the feasibility of membrane distillation (MD) for the treatment of shale gas wastewater was investigated using a laboratory scale experimental setup. Flat-sheet MD membranes were used to treat produced water from a shale gas well in the United States. Different configurations such as direct contact MD (DCMD) and air gap MD (AGMD) were compared in terms of flux and fouling propensity. The foulants on the surface of the membranes were examined. The results suggest that MD can treat the shale gas produced water containing more than 200,000 mg/L of total dissolved solids, which is impossible by other technologies such as reverse osmosis (RO) and forward osmosis (FO). In this study, we investigated the possibility of processing and characterization of shale gas produce wastewater using membrane distillation. Laboratory scale membrane distillation experimental device was developed. It was compared the flat-sheet direct contact membrane distillation and flat-sheet air gap membrane distillation. AGMD flux in lower than the flux of DCMD, it was expected that the contamination caused by organic matters.

Control System for Ship Collision Avoidance considering the Effect of Wind and Ship's Manoeuvrability

  • The studies on automatic ship collision avoidance system, which have been carried out in the last 10 years, are facing on new situation due to newly developed high technology such as computer and other information system. It was almost impossible to make it used in real navigation field 3-4 years ago because of the absence of any tool to give other ship's information, however recently developed technology suggests new possibility. This study is carried out to develop the automatic ship collision avoidance support system which considers ship's manoeuvrability into it's collision avoidance algorithm. One of the important part in ship collision avoidance system is collision decision module which can calculate collision risk with other ships and act properly to avoid the situation. Many of previous researches are using present ship's dynamic data such as present speed, position and course to calculate collision risk. However when a ship commences avoidance action, the real situation is quite different with one that has been estimated by the ship's initial data due to the ship's manoeuvring characteristic. Therefore it is better to take into account ship's manoeuvring characteristic from the stage of collision decision in ship collision avoidance system. In this study, these effects are included in the developed system. The proposed system are verified its usefulness in numerical simulation environments.

Microbiological Safety of Infant Formula and Baby Food (조제분유와 이유식의 미생물학적 안전성)

  • Powered infant formula and baby food contaminated with Enterobacter sakazakii were reported to cause infection among infants and to be associated with sporadic cases and outbreaks of sepsis, menigitis, cerebritis, and necrotizing enterocolitis. Salmonella contamination of infant formula has also been responsible for multiple outbreaks. Other species of Enterobacteriaceae in powdered infant formula may be causative agents, about which there has been no report. Other pathogenic bacteria have been isolated from powdered infant formula but they were not associated with outbreaks among infant. While Enterobacter sakazakii caused disease in all age groups, premature infants under 28 days old and with birth weight are most sensitive to its infection. Even if low contamination level of the bacteria in powdered infant formula and baby food may not cause infection, the possibility to multiplicate during preparation and storage of reconstituted formula may increase. The etiological factors and pathogenecity of S. sakazakii have not been elucidated. There were wide variability in phenotype and genotype between its strains. S. sakazakii has been isolated from factory facility and surroundings more frequently than Salmonella and thus factory environment should be the source for post-processing contamination of the formula with S. sakazakii. Considering current technology to manufacture power infant formula and baby food it is impossible to sterilize powdered formula but the frequency of outbreak hazard by S. sakazakii can be reduced by pasteurizing the formula base before drying and shortening storage time of the reconstituted formula.

  • During the shipbuilding construction, the repair works will be occurred by various reasons such as the natural disaster, mistakes of engineers or workers, defect of material, and so on. The one of the engineer's responsibilities is to design considering every possibility and to prevent the repair works due to the mistakes during construction stages, but actually it is very difficult and impossible to avoid every mistake. However, it is also the responsibility of engineers to find the best solution for the unavoidable mistakes which is to maintain the capacity of vessel and guarantee the safety for the repair works considering the construction schedules and economic cost. In this paper, it will be introduced the brief of repair works to the structural damage of the moon-pool structure in drillship which is built in our shipyard. For the verification of the structural strength, the fatigue analysis has been carried out based on the guide of Classification society. Furthermore, the modifications of structure design and welding procedure have been applied to avoid the stress concentration in the moon-pool structure.

Scanning Electron Microscopy and Energy Dispersive X-ray Spectroscopy Studies on Processed Tooth Graft Material by Vacuum-ultrasonic Acceleration

  • Purpose: The current gold standard for clinical jawbone formation involves autogenous bone as a graft material. In addition, demineralized dentin can be an effective graft material. Although demineralized dentin readily induces heterotopic bone formation, conventional decalcification takes three to five days, so, immediate bone grafting after extraction is impossible. This study evaluated the effect of vacuum ultrasonic power on the demineralization and processing of autogenous tooth material and documented the clinical results of rapidly processed autogenous demineralized dentin (ADD) in an alveolar defects patient. Methods: The method involves the demineralization of extracted teeth with detached soft tissues and pulp in 0.6 N HCl for 90 minutes using a heat controlled vacuum-ultrasonic accelerator. The characteristics of processed teeth were evaluated by scanning electron microscopy (SEM), and energy dispersive X-ray spectroscopy (EDS). Bone grafting using ADD was performed for narrow ridges augmentation in the mandibular area. Results: The new processing method was completed within two hours regardless of form (powder or block). EDS and SEM uniformly demineralized autotooth biomaterial. After six months, bone remodeling was observed in augmented sites and histological examination showed that ADD particles were well united with new bone. No unusual complications were encountered. Conclusion: This study demonstrates the possibility of preparing autogenous tooth graft materials within two hours, allowing immediate one-day grafting after extraction.
