상악 전치부 고정성 보철물 수복 시 임시 임플란트를 이용한 임시보철물의 유지력 증가 증례보고 (Reinforcing the retention of provisional restoration using provisional implant on maxillary anterior region: clinical case report)

  • 임시 보철물의 적절한 관리는 보철의 성공에 있어서 중요한 요소이다. 상악 전치 결손부 임시 보철물은 장경간의 결손부가 존재하거나, 타원형의 악궁을 가진 환자에서 자주 탈락하게 된다. 이는 상악 전치부가 구치부에 비해서 수평적인 힘에 더 많이 노출되며, 부가적으로 전방 캔틸레버 효과가 임시 보철물의 유지를 저해하고, 상악 전치부 임시 보철물은 하악의 기능 운동시 적절한 전방 유도를 제공하여야 하기 때문이다. 또한 임시 보철물의 최대 감합위에서 교합접촉은 지점선의 반대편에 위치하게 되어 그 자체로 임시 보철물의 탈락을 야기한다. 본 증례에서는 임시 임플란트를 이용하여 상악 전치부 보철물에 대한 전술한 유해 작용을 피할 수 있었고, 발치 후 치유기간 동안 환자에게 편안함과 만족감을 부여하였으므로 이를 보고하는 바이다.

Patients' knowledge and awareness of dental implants in a Turkish subpopulation

  • PURPOSE. The purpose of this study was to evaluate the patients' knowledge on dental implants in a Turkish subpopulation. MATERIALS AND METHODS. Five hundred twenty seven Turkish adults referred to Yeditepe University Faculty of Dentistry, Istanbul, Turkey, were presented with a questionnaire including 20 questions regarding the level of information and awareness about the dental implants. The data were collected and statistical analyses were performed with Chi square test to compare the descriptive data. RESULTS. Among 527 subjects, 54% were female and 46% were male with a mean age of 42.2 years. The rate of patients' implant awareness was 27.7%. When the patients were questioned about the treatment options for rehabilitation of tooth missing, 60.9% of patients were informed about fixed partial denture, followed by conventional complete denture (32.5%) and removable partial denture (24.9%). Six percent reported that they were very well informed about the dental implants whereas 48.2% were poorly informed. The information sources of the implants were from the dentist (44.5%), printed media (31.6%) and friends and acquaintances (17.3%), respectively. Sixteen percent of the population believed that their implants would last forever. CONCLUSION. The dentists should give more detailed information to the patients about dental implants and tooth-supported fixed partial dentures in the future.

구강안면 근긴장이상을 가진 완전 무치악 환자에서 구치부 지르코니아 교합면을 갖는 상악 총의치와 하악 임플란트 지지 고정성 보철물의 수복 (Maxillary complete denture with posterior zirconia occlusion and mandibular implant support fixed prostheses in completely edentulous patients with orofacial dystonia)

  • 구강안면 근긴장이상(Orofacial dystonia)은 자신의 의지와 상관없이 불수의적으로 얼굴, 입술, 혀 및 턱 주변을 불규칙적이거나 반복적으로 움직이는 신경운동학적 장애로 틱장애로 불리기도 한다. 이러한 증상을 갖는 무치악 환자는 총의치 사용의 어려움과 더불어 언어 장애 및 연하곤란, 구강안면의 통증까지 기능적이고 심미적인 문제점들을 겪게 된다. 본 증례에서는 비교적 젊은 나이에 완전 무치악 상태를 경험하게 된 구강안면 근긴장이상 환자를 위해, 양측 구치부 지르코니아 교합면을 갖는 상악 총의치와 하악 임플란트 지지 고정성 보철물의 수복치료를 시행하고 지속적인 미소훈련을 시행하여 안면근육의 심미적인 개선을 도모하고자 하였다. 치료의 결과, 환자는 저작기능과 심미성이 향상된 것 뿐만 아니라, 심리적 안정감을 회복하여 정상적인 일상생활이 가능해진 것에 대해 매우 만족하였기에 이를 보고하고자 한다.

구순구개열 환자에서 Locator® 유지장치가 장착된 milled titanium bar를 이용한 가철성 임플란트 피개 국소의치의 보철수복증례 (Removable implant-supported partial denture using milled bar with Locator® attachments in a cleft lip & palate patient: A clinical report)

  • 본 증례는 구순구개열 환자에서 상순의 반흔, 상악 잔존치조제의 과도한 흡수, 협소한 구개 및 상악골의 열성장을 동반한 3급 악간관계로 인해 통상의 가철성 국소의치 수복에 한계를 보이기 때문에 상악 무치악 부위에 4개의 임플란트를 식립 후, CAD/CAM 시스템으로 밀링한 titanium bar를 이용한 상악 가철성 임플란트 피개 국소의치를 계획하였다. 통상의 주조방법은 주조 후 수축을 하게 되지만 CAD/CAM 시스템으로 밀링한 titanium bar는 높은 정밀성과 경제성 그리고 가벼운 장점이 있다. 하지만 실제 임상에서 milled bar는 아주 균등하고 정확한 마찰결합을 얻기 힘들며 의치의 반복적인 착탈에 의해 마찰 유지력이 감소 할 수 있다. 이에 milled bar와 함께 사용할 수 있는 다양한 보조 유지장치(e.g. friction pin, $ERA^{(R)}$, $CEKA^{(R)}$, magnetics, $Locator^{(R)}$ 등)를 같이 사용함으로써 부가적인 유지력, 경제성 및 교체의 용이성 등을 추가적으로 얻을 수 있다. 본 증례에서 milled titanium bar의 상부면에 drill and tapping 방법으로 $Locator^{(R)}$ 유지장치를 연결하였다. milled bar에 screw hole을 형성하고 3개의 $Locator^{(R)}$ 유지장치를 20 Ncm으로 체결하여 추가적인 유지력을 확보하였다. 보철수복 후, 환자가 원하는 심미적인 안모, 저작 기능 및 의치 유지에 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.

하악 임플랜트 유지형 피개의치의 안정성과 하악골 응력분포에 대한 3차원 유한요소법적 연구 (3D FINITE ELEMENT ANALYSIS OF OVERDENTURE STABILITY AND STRESS DISTRIBUTION ON MANDIBULAR IMPLANT-RETAINED OVERDENTURE)

  • Statement of problem: Recently there are on an increasing trend of using implants-especially in edentulous mandible of severly alveolar bone recessed. Purpose: The aim of this study was to analyze the displacement and stress distribution of various mandibular implant-retained overdenture models supported by two implants in interforaminal region under the occlusion scheme load. Material and method: FEA models were made by the 3D scanning of the edentulous mandibular dentiform. The three models were named as Model M1, M2, and M3 accord ing to the position of implants: M1, Lt. incisor area, M2, Canine area, and M3, 1st Premolar area. Inter-implant angulation model was named as M4. Conventional complete denture was named M5 and used as a control group. Ball implant and Gold matrice were used as a retentive anchors. The occlusion type loads were applied horizontally over each tooth. Results: 1. In mandibular implant retained overdenture Canine Protected Occlusion type load resulted in higher levels of stress to the implants and female matrices than other types of loads. 2. The overdenture model M1, with implants in lateral incisor areas resulted in lower stress concentration to the implants and female matrices than other models. 3. In mandibular implant retained overdenture the stresses of the implant and female matrice were lower in mesially inclined implant than these of parallel installed implant. Conclusion: Lateral incisor areas could be the best site for the implants in mandibular implant-retained overdenture. The mandibular implant retained overdenture models mentioned above showed to the lowest stress to the implants and female matrices.

시멘트 유지형 임플란트 보철물의 유지력에 시멘트의 종류와 열순환이 미치는 영향에 관한 연구 (EFFECTS OF VARIOUS CEMENTS AND THERMOCYCLING ON RETENTIVE STRENGTHS OF CEMENTED IMPLANT-SUPPORTED PROSTHESES)

  • Statement of problem : In cemented implant-supported porstheses, it is still controversy what kind of cement to use. However, the effect of thermocycling on retentive strength of cemented implant-supported prostheses has not been well investigated. Purpose : This study was tested to evaluate the effects of various cements and thermocycling on retentive strengths of cemented implant-supported prostheses. Material and methods : Prefabricated implant abutments, height 5mm, diameter 6mm, 3-degree taper per side, with light chamfer margins were used. Ten specimens of two-unit fred partial denture were fabricated. The luting agents used for this study were three provisional luting agents which were Temp bond, Temp bond NE, IRM and four permanent luting agents which were Panavia F, Fuji-cem, Hy-bond Zinc cement, Hy-bond Polycarboxylate cement. 24 hours after cementation. the retentive strengths were measured by the universal testing machine with a cross-head speed of 0.5mm/min. Then cementation procedures were repeated and specimens were thermocycled 1000 times at temperature of $5^{\circ}C$ and $55^{\circ}C$. After thermocycling, the retentive strengths were measured. Results : Before thermocycling, the retentive strengths were decreased with the sequence of Panavia F. Fuji-cem. Hy-bond Zinc cement. Hy-bond Polycarboxylate cement, IRM, Temp bond NE and Temp bond, and there were significant differences among each groups(p<0.05). After thermocycling, the retentive strengths were decreased with the sequence of Panavia F. Fuji-cem, Hybond Zinc cement, Hy-bond Polycarboxylate cement, IRM, Temp bond NE and Temp bond, and there were no significant differences among Panavia F, Fuji-cem and Temp bond NE, Temp bond(p>0.05). The retentive strengths before and after thermocycling showed significant differences in Hy-bond Zinc cement. IRM, Temp bond NE and Temp bond(p<0.05). Conclusion : Within the limitation of this study, thermocycling do not affect the retentive strengths of permanent luting agents but the retentive strengths of temporary cements were reduced significantly after thermocyling.

A STRAIN GAUGE ANALYSIS OF IMPLANT-SUPPORTED CANTILEVERED FIXED PROSTHESIS UNDER DISTAL STATIC LOAD

  • Statement of problem. Unreasonable distal cantilevered implant-supported prosthesis can mask functional problems of reconstruction temporarily, but it can cause serious strain and stress around its supported implant and surrounding alveolar bone. Purpose. The purpose of this study was to evaluate strain of implants supporting distal cantilevered fixed prosthesis with two different cantilevered length under distal cantilevered static load. Material and methods. A partially edentulous mandibular test model was fabricated with auto-polymerizing resin (POLYUROCK; Metalor technologies, Stuttgart, Swiss) and artificial denture teeth (Endura; Shofu inc., Kyoto, Japan). Two implants-supported 5-unit screw-retained cantilevered fixed prosthesis was made using standard methods with Type III gold alloy (Harmony C&B55; Ivoclar-vivadent, Liechtenstein, Germany) for superstructure and reinforced hard resin (Tescera; Ivoclar-vivadent, Liechtenstein, Germany) for occlusal material. Two strain gauges (KFG-1-120-C1-11L1M2R; KYOWA electronic instruments, Tokyo, Japan) were then attached to the mesial and the distal surface of each standard abutment with adhesive (M-bond 200; Tokuyama, Tokyo, Japan). Total four strain gauges were attached to test model and connected to dynamic signal conditioning strain amplifier (CTA1000; Curiotech inc., Paju, Korea). The stepped $20{\sim}100$ N in 25 N increments, cantilevered static load 8mm apart (Group I) or 16mm apart (Group II), were applied using digital push-pull gauge (Push-Pull Scale & Digital Force Gauge, Axis inc., Seoul, Korea). Each step was performed ten times and every strain signal was monitored and recorded. Results. In case of Group I, the strain values were surveyed by $80.7{\sim}353.8{\mu}m$ in Ch1, $7.5{\sim}47.9{\mu}m/m$ in Ch2, $45.7{\sim}278.6{\mu}m/m$ in Ch3 and $-212.2{\sim}718.7{\mu}m/m$ in Ch4 depending on increasing cantilevered static load. On the other hand, the strain values of Group II were surveyed by $149.9{\sim}612.8{\mu}m/m$ in Ch1, $26.0{\sim}168.5{\mu}m/m$ in Ch2, $114.3{\sim}632.3{\mu}m/m$ in Ch3, and $-323.2{\sim}-894.7{\mu}m/m$ in Ch4. Conclusion. A comparative statistical analysis using paired sample t-test about Group I Vs Group II under distal cantilevered load shows that there are statistical significant differences for all 4 channels (P<0.05).

Integrating 3D facial scanning in a digital workflow to CAD/CAM design and fabricate complete dentures for immediate total mouth rehabilitation

  • PURPOSE. To integrate extra-oral facial scanning information with CAD/CAM complete dentures to immediately rehabilitate terminal dentition. MATERIALS AND METHODS. Ten patients with terminal dentition scheduled for total extraction and immediate denture placement were recruited for this study. The patients were submitted to a facial scanning procedure using the in-office PritiMirror scanner with bite registration records in-situ. Definitive stone cast models and bite records were subsequently submitted to a lab scanning procedure using the lab scanner (iSeries DWOS; Dental Wings). The scanned models were used to create a virtual teeth setup of a complete denture. Using the intra-oral bite records as a reference, the virtual setup was incorporated in the facial scan thereby facilitating a virtual clinical evaluation (teeth try-in) phase. After applying necessary adjustments, the virtual setup was submitted to a CAM procedure where a 5-axis industrial milling machine (M7 CNC; Darton AG General) was used to fabricate a full-milled PMMA immediate provisional prosthesis. RESULTS. Total extractions were performed, the dentures were immediately inserted, and subjective clinical fit was evaluated. The immediate provisional prostheses were inserted and clinical fit, occlusion/articulation, and esthetics were subjectively assessed; the results were deemed satisfactory. All provisional prostheses remained three months in function with no notable technical complications. CONCLUSION. Ten patients with terminal dentition were treated using a complete digital approach to fabricate complete dentures using CAD/CAM technology. The proposed technique has the potential to accelerate the rehabilitation procedure starting from immediate denture to final implant-supported prosthesis leading to more predictable functional and aesthetics outcomes.

Survival of 352 titanium implants placed in 181 patients: a 4-year multicenter field study

  • Purpose: The aim of this retrospective chart review was to evaluate the four-year survival rate of a titanium implant system. Methods: A total of 352 sand-blasted, thermally acid-etched titanium implants were inserted into 181 partially or completely edentulous patients. Their cumulative survival rate was evaluated retrospectively. Associated factors, such as the implant distribution and treatment type were included in the evaluation. Results: The implants were equally distributed between the maxilla (52.3%) and the mandible (47.7%). 48 implants (13.6%) were placed in the anterior region and 304 implants (86.4%) in the posterior region. The majority of the implants were inserted into bone of type II and III quality (89.8%) and volume (quantity B and C, 87.2%). Most of the implants (70.7%) were restored as single crowns; 28.7% supported a bridge construction and 0.6% a full denture. Only one implant failed, resulting in a four-year cumulative survival rate of 99.7%. Conclusions: The implant system showed an excellent four-year survival rate. It proved to be a safe and predictable means for restoration of the dentition in partially or completely edentulous patients.

Digital workflow를 활용한 'All-on-4' 임플란트 지지 고정성 보철물 수복 증례 ('All-on-4' fixed implant supported prosthesis restoration using digital workflow: a case report)

  • 구치부 심한 골소실을 보이는 완전 무치악 환자에게 이공 전방에 4개 이상의 임플란트를 식립 후 전악 수복을 하는 'All-on-X' 임플란트 지지 고정성 보철 수복을 고려해 볼 수 있다. 과거에는 이러한 임플란트 고정성 보철 수복시 특별한 기준점 없이 술자의 판단에 의존하여 잔존 치조제에 임플란트를 식립하고 보철 수복 단계에선 이미 식립된 임플란트에 맞춰서 보철물을 제작하는 방식이었지만, 최근 디지털 치의학의 발전으로 진단, 수술, 보철 제작 등 모든 과정에서 계획적이고 예지력 있는 치료가 가능해졌다. 디지털 총의치, 임플란트 서지컬 가이드, 안면스캔 그리고 복제 의치를 통한 인상 채득 등의 디지털 기술들을 사용하여 복잡한 기공 과정을 줄이고, 임시 수복물의 심미와 기능을 최종 보철물로 효과적으로 이행할 수 있다. 상기 환자는 상악 디지털, 하악 'all-on-x' 임플란트 고정성 보철물 수복환자로, 진단부터 최종 수복까지 디지털 기술을 활용하여 기능과 심미성을 만족시키는 보철물을 제작할 수 있었기에 이를 보고하고자 한다.
