In the case of a patient with a small number of unilateral remaining teeth in the mandible, a restoration with conventional removable partial denture cannot provide sufficient retention and resistance. A harmful stress from the poor stability of denture on remaining teeth occurs due to alveolar bone resorption in edentulous area. Although implant fixed prosthesis can provide multiple benefits over conventional removable partial denture, there are limitations come from anatomic structures and costs. In this case, an implant-assisted removable partial denture (IARPD) with a couple of implants placed in strategic positions can be a treatment modality. The objective of this article is to present a case report of two patients showed satisfactory clinical outcomes during three-year follow-up period describing the fabrication and advantages of removable partial denture assisted by teeth and implants (IARPD) for a patient with a small number of unilateral remaining teeth in mandible using Locator as an implant attachment.

Implant prostheses and removable partial dentures are mainly used as treatment methods for partial edentulous patients who have lost a number of teeth. The implant-assisted removable partial denture (IARPD) is strategically selected. The defect in maxillofacial structure due to osteomyelitis, a type of facial bone infection, causes dysfunction such as mastication, swallowing, and pronunciation, as well as social and psychological effects, so a removable restoration is required to restore the supporting tissue. Design of abutment and partial dentures is an essential factor in the success of treatment. In this case, IARPD, which has superior retention and stability compared to traditional removable partial dentures, can have a good prognosis. In a partial edentulous patient with bone defects due to osteomyelitis treatment, the stability of the denture was secured with IARPD restoration. Moreover, maintenance problem that may occur in the future was minimized by providing an appropriate denture design and occlusal scheme through several provisional restorations. This case can be expected to have a favorable prognosis in the long term.

Traditional denture treatment is often performed in patients with the completely edentulous maxilla and the bilateral edentulous posterior mandible. However, with a long-term prognosis, a complication called 'combination syndrome' may occur in this oral environment. In this case, by applying an implant-assisted removable partial denture (IARPD), it is possible to preserve the alveolar bone and prevent combination syndrome. In this case, when additional implants are placed in the edentulous area, the stress applied to the abutment due to the movement of the removable denture can be reduced, so it is possible to design an esthetic clasp using resin rather than metal, and flexible denture base resin is used. As a treatment for maxillary complete denture and mandibular flexible IARPD, it showed an esthetic and functional long-term prognosis with minimal cost and surgery.

Implant-assisted removable partial denture (Implant-assisted RPD, IARPD), posterior edentulous extension areas of which obtains additional support and retention from implants, is attracting increasing interest. This case report presents a successful treatment on a partially edentulous patient with a severely canted occlusal plane resulted from a long-term use of posterior extended RPD. The full mouth was rehabilitated through a fixed prosthesis on maxilla and IARPD with zirconia occlusal surface on mandible, which allowed to achieve an esthetic occlusal plane with long-term stability and, ultimately, functionally satisfying outcome.

The long-term use of an implant overdenture shows problems, such as wear of attachment, concentrated occlusal forces on the anterior teeth, fracture of artificial teeth and rotation of denture. By transitioning to an Implant-assisted removable partial denture (IARPD) using additional implant placement, the problems can be solved. In this case report, a transition was made from implant overdenture to IARPD utilizing surveyed crowns to distribute occlusal forces concentrated on anterior teeth and to prevent denture rotation in a skeletal Class III patient. Design of definitive prosthesis with appropriate function and aesthetics was determined through several stages of provisional restorations. In this case, appropriate posterior occlusion and maintenance of peri-implant bone level of definitive prosthesis were observed.

Fixed implant prostheses or removable partial dentures are common treatment for partially dentulous patients. Recently, an implant-assisted removable partial denture (IARPD) has been introduced and is widely used. In the case of removable partial dentures using implant attachment and surveyed fixed prostheses, the consideration about parallelism of insertion path between implants and surveyed fixed prostheses is an important factor for success of IARPDs. In this case, she complained of discomfort while using the mandibular removable partial denture. Thus, an IARPD using implant surveyed fixed prostheses was fabricated. Thereafter, more implants were placed by a digital guide surgery to have the same insertion path as the existing surveyed fixed prostheses. Locator attachments were installed to the left and right premolar implants, and the left molar was able to obtain support of the removable partial denture with healing abutment. The clinical results were satisfactory on the aspect of aesthetic and masticatory function.

Implant-fixed prostheses and removable partial dentures are widely used as a traditional treatment method for partially edentulous patients who have lost multiple teeth. Recently, there has been increasing interest in implant-assisted removable partial denture (IARPD), which additionally obtains retention and support using a small number of implants. The IARPDs have higher retention, stability, and aesthetics than conventional removable dentures. It also has the effect of reducing the cost as well as the burden of surgery by placing a small number of implants at the edentulous site. In this case, the patient with a partial edentulous state that has multiple numbers of tooth loss in maxilla initially planned to be treated with implant fixed prosthesis. However, the treatment plan was changed to IARPD using Bar and Locator due to several factors. The clinical results were satisfactory on the aspect of esthetic and masticatory function.

Traditional options for posterior edentulous treatment include removable partial dentures and implant fixed prostheses. Recently, the concept of implant assisted removable partial denture, in which two treatments are fused, has been introduced in consideration of systemic health and patient's needs, costs, residual alveolar bone status and so on. Implant assisted removable partial denture has the advantage of increasing the retention and stability of the denture and improving its esthetics in cases of large bone defects or biomechanical disadvantages. In addition, it is possible to strategically place the implants in a site where the alveolar bone is relatively sufficient, thereby overcome the limit of the conventional removable partial denture design as well as reducing the burden on a wide range of implant surgery. Cost reduction is also expected. In this case, the patient was treated by placing the implant in both premolar sites of the mandible and fabricating the distal extension removable partial denture with the implant fixed prosthesis as an abutment. After delivering the definitive prosthesis, the patient showed satisfaction with the masticatory function and esthetics. and has been regularly followed-up for more than one year. The following 20-months follow-up case report describes the design of an implant-assisted-removable partial denture (IARPD) in which two cementretained implant crowns used to provide support and stability.

Reconstruction of the maxillectomy with an obturator is to restore masticatory, swallowing, phonetic and esthetic problems. Stress created by lateral forces is minimized by the proper selection of an occlusal scheme, elimination of premature occlusal contacts, and wide distribution of supporting area. It should be considered that properly designed retainers reduce the stresses transmitted to the abutment while the obturator is in function. The following clinical report presents palatal obturator treatment with implant assisted removable partial denture (IARPD) design that restores normal function and esthetics in patients who experienced maxillectomy and dental implant failure.

Implant-assisted removable partial denture (IAPRD) can be considered as a simple and cost-effective treatment approach for an edentulous patient with anatomical or financial limitations. Recently, it was reported that the application of IARPD with implant supported fixed prostheses covered by the National Health Insurance Service (NHIS) were increasing. This case report describes the treatment of maxillary fully edentulous patient with anterior four-implant-supported fixed prosthesis and distal extension IARPD. This treatment approach may be advantageous over maxillary implant overdentures in some circumstances. The patient was satisfied with improved function and esthetics in the anterior area and financial benefit from the NHIS. Further long-term clinical studies are needed to establish clinical validity of the treatment approach described in this case report.
