인공치아용 세라믹 임플란트 상부구조물의 반복하중 피로특성 (Hertzian contact fatigue of dental ceramic implant abutment)

  • 유사구강 분위기인 인공타액 조건하에서 인공치아용 상부구조물 재료 중 하나인 3Y-TZP를 500~3000 N의 하중 조건으로 $10^6$ 횟수까지 헤르찌안 반복하중을 가하여 그 피로특성을 압입손상과 강도저하현상으로 관찰하였다. 500 N의 하중에서 $5\times10^5$ 반복 횟수까지 피로실험결과, 강도저하 및 균열현상은 관찰되지는 않았지만 반복하중 값이 증가함에 따라 링 균열에서 방사상 균열로 전이되었을 때 급격한 강도 저하현상이 관찰되었다. 또한 강도 저하 현상은 유사 구강분위기하에서 반복 하중 접촉시 발생한 균열을 통해 침투한 인공타액의 화학적 침식으로 가속화 되었다.

Effect of abutment screw length and cyclic loading on removal torque in external and internal hex implants

  • PURPOSE. The purpose of this study was to evaluate the effects of abutment screw length and cyclic loading on the removal torque (RTV) in external hex (EH) and internal hex (IH) implants. MATERIALS AND METHODS. Forty screw-retained single crowns were connected to external and internal hex implants. The prepared titanium abutment screws were classified into 8 groups based on the number of threads (n = 5 per group): EH 12.5, 6.5, 3.5, 2.5 and IH 6.5, 5, 3.5, 2.5 threads. The abutment screws were tightened with 20 Ncm torque twice with 10-minute intervals. After 5 minutes, the initial RTVs of the abutment screws were measured with a digital torque gauge (MGT12). A customized jig was constructed to apply a load along the implant long axis at the central fossa of the maxillary first molar. The post-loading RTVs were measured after 16,000 cycles of mechanical loading with 50 N at a 1-Hz frequency. Statistical analysis included one-way analysis of variance and paired t-tests. RESULTS. The post-loading RTVs were significantly lower than the initial RTVs in the EH 2.5 thread and IH 2.5 thread groups (P<.05). The initial RTVs exhibited no significant differences among the 8 groups, whereas the post-loading RTVs of the EH 6.5 and EH 3.5 thread groups were higher than those of the IH 3.5 thread group (P<.05). CONCLUSION. Within the limitations of this study, the external hex implants with short screw lengths were more advantageous than internal hex implants with short screw lengths in torque maintenance after cyclic loading.

비귀금속 주조 맞춤형 지대주를 이용한 상악 전치부 임플란트 보철수복 증례 (Maxillary anterior fixed implant prosthesis using customized nonprecious metal casting abutment: a case report)

  • 임플란트를 이용하여 심미 영역인 상악 전치부를 심미적, 기능적으로 수복하고 유지 관리하는 것은 어려운 일이다. 임플란트 주위염으로 인하여 치조골의 흡수, 치간유두의 수축 등과 같은 임플란트 주위 조직 문제와 금속 지대주의 노출로 인한 심미적 문제가 발생할 수 있기 때문이다. 본 증례에서는 기성지대주의 단점을 보완 가능한 맞춤형 지대주를 전통적인 제작 방식인 주조 방식으로 Co-Cr 합금을 이용하여 제작하고, 시멘트 유지형 보철물을 제작한 증례에 대해 보고하고자 한다.

Finite element analysis in static and dynamic behaviors of dental prosthesis

  • In recent years, implants have gained growing importance in all areas of medicine. The success of the treatment depends on many factors affecting the bone-implant, implant-abutment and abutment-prosthesis interfaces. In this paper, static and dynamic behaviors of the dental prosthesis are investigated. Three-dimensional finite element models of dental prosthesis were constructed. Dynamic loads in 5 sec applied on occlusal surface. Therefore, FEA was selected for use in this study to examine the effect of the static and dynamic loads on the stress distribution for an implant-supported fixed partial denture and supporting bone tissue.

In vitro performance and fracture resistance of novel CAD/CAM ceramic molar crowns loaded on implants and human teeth

  • PURPOSE. To investigate the fatigue and fracture resistance of computer-aided design and computer-aided manufacturing (CAD/CAM) ceramic molar crowns on dental implants and human teeth. MATERIALS AND METHODS. Molar crowns (n=48; n=8/group) were fabricated of a lithium-disilicate-strengthened lithium aluminosilicate glass ceramic (N). Surfaces were polished (P) or glazed (G). Crowns were tested on human teeth (T) and implant-abutment analogues (I) simulating a chairside (C, crown bonded to abutment) or labside (L, screw channel) procedure for implant groups. Polished/glazed lithium disilicate (E) crowns (n=16) served as reference. Combined thermal cycling and mechanical loading (TC: $3000{\times}5^{\circ}C/3000{\times}55^{\circ}C$; ML: $1.2{\time}10^6$ cycles, 50 N) with antagonistic human molars (groups T) and steatite spheres (groups I) was performed under a chewing simulator. TCML crowns were then analyzed for failures (optical microscopy, SEM) and fracture force was determined. Data were statistically analyzed (Kolmogorow-Smirnov, one-way-ANOVA, post-hoc Bonferroni, ${\alpha}=.05$). RESULTS. All crowns survived TCML and showed small traces of wear. In human teeth groups, fracture forces of N crowns varied between $1214{\pm}293N$ (NPT) and $1324{\pm}498N$ (NGT), differing significantly ($P{\leq}.003$) from the polished reference EPT ($2044{\pm}302N$). Fracture forces in implant groups varied between $934{\pm}154N$ (NGI_L) and $1782{\pm}153N$ (NPI_C), providing higher values for the respective chairside crowns. Differences between polishing and glazing were not significant ($P{\geq}.066$) between crowns of identical materials and abutment support. CONCLUSION. Fracture resistance was influenced by the ceramic material, and partly by the tooth or implant situation and the clinical procedure (chairside/labside). Type of surface finish (polishing/glazing) had no significant influence. Clinical survival of the new glass ceramic may be comparable to lithium disilicate.

  • 상악 완전 무치악에 6개 이상의 임플란트가 식립된 경우 임플란트-지지형 피개의치 제작이 가능하다. 임플란트-지지 피개의치에 사용되는 다양한 부착장치 중에 milled-bar의 경우 의치의 회전을 허용하지 않고, 안정성이 뛰어나며, 여러 개의 임플란트를 연결 고정하는 효과를 가진다는 장점이 있다. 본 증례에서는 전치부에 심한 골소실을 동반한 상악 완전 무치악 환자에서 세 가지 다른 시스템으로 구성된 7개의 임플란트 상부에 CAD/CAM 맞춤형 지대주와 시멘트 접착형의 milled bar를 이용한 피개의치를 제작하여 상실된 심미와 기능을 효과적으로 회복하였다.

Screw joint stability according to abutment screw materials

  • Statement of problem. There have been previous studies about instability according to screw material by means of calculating preload in tightening screw or recording of the torque necessary to loosen screw after tightening screw. Purpose. The purpose of this study was to evaluate screw joint stability through the analysis of fitness at the mating thread surfaces between implant and screw after tightening screws made of different materials. Material and methods. In this study, screws were respectively used to secure a cemented abutment to a hexlock implant fixture; teflon coated titanium alloy screw and titanium alloy screw(Steri-Oss), gold-plated gold-palladium alloy screw and titanium alloy screw(Implant Innovation), gold screw and titanium screw(AVANA Dental Implant System). Each abutment screw was secured to the implant with recommended torque value using a digital torque controller. Each screw was again tightened after 10minutes. All samples were cross sectioned with sandpaper and polished. Then samples were evaluated with an scanning electron microscope analysis. Results. In titanium alloy screw, irregular contact and relatively large gap was present at mating thread surface. Also in teflon-coated titanium screw, incomplete seating and only partially contact was present at the mating thread surface. In gold-plated gold-palladium alloy screw, relatively close and tight contact without the presence of large gap was present by existing of gold coating at the mating thread surfaces. In gold alloy screw, relatively small gap between the mating components was seen. Conclusions. This result suggested that gold plated gold-palladium alloy screw and gold alloy screw achieved a greater degree of contact at the mating thread surfaces compared to titanium alloy screw and teflon-coated titanium alloy screw.

임플란트 지대주 종류와 동적하중이 고정체와 지대주의 미세간극에 미치는 영향 (The Effect of Types of Abutment and Dynamic Loading on Microgap between Implant Fixture and Abutment)

  • 이전부터 사용된 티타늄, 금합금 지대주 및 최근 사용이 급증한 지르코니아 지대주와 임플란트의 계면에 대한 비교 연구와 동적하중이 계면에 미치는 영향에 관한 연구가 많지 않은 실정이다. 본 연구에서 지대주 종류와 동적 하중에 따른 미세간극의 차이를 주사전자현미경 분석을 통하여 비교평가 하였다. USII ${\phi}3.75{\times}11.5mm$ (Osstem Co., Seoul, Korea) 티타늄 임플란트에 티타늄 지대주, 지르코니아 지대주, 주조 금합금 (UCLA plastic) 지대주를 각 군당 7개씩, 30Ncm의 조임회전력으로 연결하였다. 25-250N의 sine형 동적하중을 장축 방향에서 $30^{\circ}$ 경사로 $10^5$회 시행하였다. 하중 전후에 계면의 미세간극을 순측, 구개측, 근심측, 원심측 부위에서 측정하였고, 그 결과 동적 하중으로 인한 통계적 유의성은 없었다. 지대주 종류에 따른 미세간극을 비교한 결과, 동적하중 시행 전후 모두 UCLA 군과 티타늄 군, UCLA 군과 지르코니아 군 사이에 통계적으로 유의한 차이를 보였다(p<0.05). 티타늄 군과 지르코니아 군 사이에는 유의한 차이를 보이지 않았다. UCLA 지대주는 정밀 연삭된 티타늄, 지르코니아 지대주 보다 더 큰 미세간극을 보였고 $10^5$회 동적하중은 미세간극에 영향을 미치지 않았다.
