Comparison of implant stability measurements between a resonance frequency analysis device and a modified damping capacity analysis device: an in vitro study

  • Purpose: A stability-measuring device that utilizes damping capacity analysis (DCA) has recently been introduced in the field of dental implantology. This study aimed to evaluate the sensitivity and reliability of this device by measuring the implant stability of ex vivo samples in comparison with a resonance frequency analysis (RFA) device. Methods: Six implant beds were prepared in porcine ribs using 3 different drilling protocols to simulate various implant stability conditions. Thirty-six pork ribs and 216 bone-level implants measuring 10 mm in height were used. The implant beds were prepared using 1 of the following 3 drilling protocols: 10-mm drilling depth with a 3.5-mm-diameter twist drill, 5-mm drilling depth with a 4.0-mm-diameter twist drill, and 10-mm drilling depth with a 4.0-mm-diameter twist drill. The first 108 implants were external-connection implants 4.0 mm in diameter, while the other 108 implants were internal-connection implants 4.3 mm in diameter. The peak insertion torque (PIT) during implant placement, the stability values obtained with DCA and RFA devices after implant placement, and the peak removal torque (PRT) during implant removal were measured. Results: The intraclass correlation coefficients (ICCs) of the implant stability quotient (ISQ) results obtained using the RFA device at the medial, distal, ventral, and dorsal points were 0.997, 0.994, 0.994, and 0.998, respectively. The ICCs of the implant stability test (IST) results obtained using the DCA device at the corresponding locations were 0.972, 0.975, 0.974, and 0.976, respectively. Logarithmic relationships between PIT and IST, PIT and ISQ, PRT and IST, and PRT and ISQ were observed. The mean absolute difference between the ISQ and IST values on a Bland-Altman plot was -6.76 (-25.05 to 11.53, P<0.05). Conclusions: Within the limits of ex vivo studies, measurements made using the RFA and DCA devices were found to be correlated under a variety of stability conditions.

임플란트 식립 부위의 골밀도 평가에 관한 연구 (The study of bone density assessment on dental implant sites)

  • Introduction: Bone density is one of the important factors for the long term success of endosseous implants. The bone density varies from site to site and from patient to patient. A preoperative evaluation of the bone density is quite useful to oral surgeons for planning dental implantation. More accurate information on the bone density will help surgeons identify suitable implant sites, thereby increase the success rate of dental implantation. This study examined the correlation between the bone density measured preoperatively by computed tomography (CT) and the implant primary stability measured by resonance frequency analysis. Furthermore, the effects of the implant sites, gender, age and generalized systemic disorder patients on the bone density and primary implant stability were examined. Materials and Methods: One hundred and fourteen patients were selected. None of the patients had undergone a tooth extraction or bone graft history in the previous year. Preoperatively, the patients underwent CT scanning to evaluate the Hounsfield unit (HU), and resonance frequency analysis (RFA) was used to evaluate the implant primary stability at the time of implant installation. All implants were 4.0 mm diameter and 11.5 mm length US II. All patients were recorded and the HU and implant stability quotient (ISQ) value were evaluated according to the sites, gender and age. Results: The highest HU values were found in the mandibular anterior site ($827.6{\pm}151.4$), followed by the mandibular molar site ($797{\pm}135.1$), mandibular premolar site ($753.8{\pm}171.2$), maxillary anterior site ($726.3{\pm}154.4$), maxillary premolar site ($656.7{\pm}173.8$) and maxillary molar site ($621.5{\pm}164.9$). The ISQ value was the highest in the mandibular premolar site ($81.5{\pm}2.4$) followed by the mandibular molar site ($80.0{\pm}5.7$), maxillary anterior site ($77.4{\pm}4.1$), mandibular anterior site ($76.4{\pm}11.9$), maxillary premolar site ($74.2{\pm}14.3$) and maxillary molar site ($73.7{\pm}7.4$). The mean HU and ISQ value were similar in females and males. (HU: P=0.331, ISQ: P=0.595) No significant difference was also found in the age group respectively. However, the correlation coefficients between the variables showed a closed correlation between the HU and ISQ value. Conclusion: These results showed close correlation between the bone density (HU) and primary stability value (ISQ) at the time of implant installation (Correlation coefficients=0.497, P<0.01). These results strengthen the hypothesis that it might be possible to predict and quantify the initial implant stability and bone density from a presurgical CT diagnosis.

성견에서 사다리꼴형 디자인과 미세나사선을 가진 단폭경임플란트의 골유착 평가: 예비연구 (Evaluation of narrow-diameter implant with trapezoid-shape design and microthreads in beagle dogs: A pilot study)

  • Objective: The objective of this study was to evaluate the osseointegration of narrow-diameter implant with trapezoid-shape and to evaluate the effect of coronal microthreads on trapezoid-shape narrow-diameter implant. Materials and Methods: The experimental narrow-diameter implants were classified into two groups according to absence or presence of coronal microthreads: trapezoid-shape narrow diameter implant (TN group) and trapezoid-shape narrow-diameter implant with microthreads (TNM group). They were installed alternately in bilateral mandible in three dogs. After 8 weeks, the animals were sacrificed. Resonance frequency analysis, removal torque test, and histometric analysis were performed. Results: Statistically higher implant stability quotient (ISQ) values were observed in TNM group than in TN group at the time of implant installation. However, significant ISQ values difference was not observed between groups at 8 weeks. Both groups showed significantly increased ISQ values at 8 weeks, compared to the time of implant installation. There was no significant difference between groups in removal torque test. Bone-implant contact ratio also showed no significant difference between groups in total and coronal part. Conclusion: Within the limitation of this study, it could be concluded that the trapezoid-shape design on narrow-diameter implant showed successful ossointegration, and the microthreads on coronal part did not result in significant bone-implant contact and biomechanical stability at 8 weeks.

마그네슘 양극산화 임플란트의 성공률에 관한 전향적 임상연구 (A Prospective Clinical Trial on the Mg Oxidized Clinical Implants)

  • 마그네슘 양극산화 임플란트는 동물실험에서 우수한 골반응을 나타내었지만 이에 대한 임상 연구는 전무하다. 본 연구에서는 마그네슘 양극산화 임플란트의 단기간 성공률 및 안정성, 변연골 흡수량을 평가하고자 하였다. 강릉원주대학교 치과병원의 임상시험위원회의 승인을 거쳐 건강한 부분 무치악 환자에게 마그네슘 양극산화 임플란트를 식립한 후 일반적인 프로토콜에 따라 보철 치료를 시행하였다. 1차 수술, 2차 수술, 보철물 시적, 하중 후 1개월, 3개월, 6개월 평가를 시행하였으며, 매회 내원 시 임플란트 안정지수(Implant stability quotient, ISQ)를 측정하고 구내 치근단 방사선 사진에서 변연골 흡수량을 산출하였다. 반복측정이 있는 분산분석법을 이용하여 통계적 분석을 시행하였다. 총 50명에게 식립된 101개의 임플란트 중 골유착에 실패한 임플란트는 없었으며, 하중 후 6개월 성공률은 100%였다. 평균 ISQ는 시간이 경과함에 따라 증가하였으며 여성보다 남성에서, 상악보다 하악에서 높게 나타났다 (P<.05). 하중 6개월 후 골질이 양호할수록, 고정체 직경이 클수록 높은 ISQ 값을 보였다 (P<.05). 하중 후 6개월간 평균 변연골 흡수량은 0.26 mm로 관찰되었으며 하악보다 상악에서, 여성보다 남성에서 더 많은 흡수가 관찰되었다. 본 연구의 마그네슘 양극산화 임플란트의 6개월 성공률은 100%이며, 낮은 골질에서도 우수한 골유착이 관찰되었다. 안정성과 변연골 수준 또한 만족스러운 결과를 보였지만 더욱 장기적인 임상연구가 필요하다고 사료된다.

골유착 재현 레진 모델에서 골 상태 및 임플란트 형태에 따른 임플란트 안정성에 관한 연구 (Implant stability evaluation according to the bone condition, fixture diameter and shape in the osseointegration simulated resin model)

  • 연구 목적: 자기공명주파수 분석, 페리오테스트, 풀림 토크는 임플란트의 안정성을 측정하기 위한 방법으로 알려져 있다. 이 방법들은 골 상태와 임플란트 형태에 영향을 받는다. 본 연구에서는 골유착이 재현된 아크릴 레진 실험 모델에서 임플란트가 결합된 골의 양을 변화하고 경부의 골 상실양이 증가하였을 경우, 자기공명주파수 분석, 페리오테스트, 풀림 토크 측정에 어떠한 영향을 주는지 알아보고 이들 상호간의 관계를 알아보고자 하였다. 연구 재료 및 방법: 골 유착을 재현하기 위하여 임플란트를 알루미늄 틀에 나사로 고정한 후 자가중합형 아크릴릭 레진의 가루와 용액을 틀 안에 부어서 중합을 시켰다. 임플란트와 결합된 레진의 두께를 조정하여 골 결합 두께를 각각 1, 3, 5, 10 mm인 경우로 재현하였다. 그 후 임플란트 주변 경부 골 상실 양이 1, 3, 5 mm인 경우를 재현하였다. 각각의 재현된 골 조건에서는 4 mm 직경의 직선형 임플란트, 같은 직경의 tapered 형 임플란트, 5 mm 직경의 tapered 형의 3가지 임플란트가 각각 5개씩 사용되었다. 모든 골 조건에서 자기공명주파수 분석과 페리오테스트 측정이 이루어졌으며 골 결합 두께 1, 3 mm 시편에서는 풀림 토크가 측정되었다. 결과: 골 결합 두께가 증가하거나 임플란트 직경이 증가할수록 통계적으로 유의하게 ISQ 값은 증가하였고, 페리오테스트 값은 감소하였다 (P<.001, P<.001). 임플란트 경부에 주위 골 상실 양이 커질수록 ISQ 값은 감소하였고, 페리오테스트 값은 증가하였다 (P<.001). ISQ 값과 페리오테스트 t값은 매우 강한 상관관계를 나타냈다 (r = -0.99, P<.001). ISQ값과 풀림토크는 약한 상관관계를 나타냈으며 (r = 0.52, P<.001), 페리오테스트 값과 풀림 토크도 약한 상관관계를 나타냈다 (r = -0.52, P<.001). 결론: 레진으로 재현한 골의 양이 증가할수록, 임플란트의 직경이 증가할수록, 경부의 골 상실양이 적을수록 임플란트 안정성이 증가함을 확인할 수 있었다. 또한 본 연구에서는 ISQ 값과 페리오테스트 값의 강한 상관관계를 밝힐 수 있었다.

임프란트 식립시 초기 안정성이 골유착에 미치는 영향 (THE INFLUENCE OF THE INITIAL STABILITY AFTER DENTAL IMPLANT INSTALLATION ON THE OSSEOINTEGRATION)

  • Purpose: The long-term experience of using osseointegrated implants for prosthetic rehabilitation of the edentulous patients shows that high success rates can be predictably achieved. Primary implant stability has been identified to be a prerequisite to achieve osseointegration. In this study, we set up the amount of removed bone so that it differed on implant installation site for each group. The influence of each initial stability on secondary stability and osseointegration was compared with time lapse using resonance frequency analysis and histomorphometric analysis. Materials and methods: A total 27 US $II^{(R)}$ (Osstem, Korea) implants were placed in the mandibular edentulous area of 3 beagle dogs. The implant site was prepared by the conventional technique with drills, and three experimental groups were divided into under-drilling group, normal-drilling group and over-drilling group. The Implant Stability Quotient (ISQ) was measured at intervals of immediately, 4, 8, 12 weeks after placement using $Osstell^{(R)}$ mentor RFA. After the animals were sacrificed, histomorphometric evaluation was executed for measuring BIC and BD. Results: 1) The under-drilled group showed most high ISQ value for whole experiment period. 2) Bone-to-implant-contact(BIC) showed the tendency to be increased gradually as the experiment period passed except the 8 weeks of the normal group. 3) The under-drilled group showed most high bone density(BD) level for whole experiment period, and it was expressed the aspect to be increased gradually according to an experiment period passage in the average of all group. 4) Resonance frequency analysis and histomorphometric analysis are presumed by generally proportional. Conclusions: As this research result, it seems that there are some correlation between resonance frequency analysis and histomorphometric analysis. As are accomplished osseointegration stably so that more superior at the region which the overpressure comes to add, it will be applicable method in clinical field.

Retrospective clinical study of ultrawide implants more than 6 mm in diameter

  • Background: The prognosis of wide implants tends to be controversial. While wider implants were initially expected to result in a larger osseointegration area and have higher levels of primary stability, they were reported to have a relatively high rate of failure. The clinical outcome of ultrawide implants of more than 6 mm in diameter was evaluated through a retrospective study. Methods: The investigation was conducted on patients who had received ultrawide implant (${\geq}6mm$ diameter) placements in Seoul National University Bundang Hospital from January 2008 to December 2013. Complications were investigated during the maintenance period, and marginal bone loss was measured using periapical radiography. Primary stability immediately after the implant placement and second stability after second surgery or during impression were measured using $Osstell^{(R)}$ Mentor (Osstell, Sweden) as an implant stability quotient (ISQ). Results: Fifty-eight implants were placed in 53 patients (30 male, 23 female), and they were observed for an average of $50.06{\pm}23.49$ months. The average ISQ value increased from $71.22{\pm}10.26$ to $77.48{\pm}8.98$ (P < 0.005). The primary and secondary stability shows significantly higher at the mandible than at the maxilla (P < 0.001). However, mean survival rate shows 98.28 %. Average marginal bone loss of 0.018 and 0.045 mm were measured at 12 and 24 months after the loading and 0.14 mm at final follow-up date (mean 46.25 months), respectively. Also in this study, the bone loss amount was noticeably small compared to regular implants reported in previous studies. Conclusions: The excellent clinical outcome of ultrawide implants was confirmed. It was determined that an ultrawide implant can be used as an alternative when the bone quality in the posterior teeth is relatively low or when a previous implant has failed.

Primary implant stability in a bone model simulating clinical situations for the posterior maxilla: an in vitro study

  • Purpose: The aim of this study was to determine the influence of anatomical conditions on primary stability in the models simulating posterior maxilla. Methods: Polyurethane blocks were designed to simulate monocortical (M) and bicortical (B) conditions. Each condition had four subgroups measuring 3 mm (M3, B3), 5 mm (M5, B5), 8 mm (M8, B8), and 12 mm (M12, B12) in residual bone height (RBH). After implant placement, the implant stability quotient (ISQ), Periotest value (PTV), insertion torque (IT), and reverse torque (RT) were measured. Two-factor ANOVA (two cortical conditions${\times}$four RBHs) and additional analyses for simple main effects were performed. Results: A significant interaction between cortical condition and RBH was demonstrated for all methods measuring stability with two-factor ANOVA. In the analyses for simple main effects, ISQ and PTV were statistically higher in the bicortical groups than the corresponding monocortical groups, respectively. In the monocortical group, ISQ and PTV showed a statistically significant rise with increasing RBH. Measurements of IT and RT showed a similar tendency, measuring highest in the M3 group, followed by the M8, the M5, and the M12 groups. In the bicortical group, all variables showed a similar tendency, with different degrees of rise and decline. The B8 group showed the highest values, followed by the B12, the B5, and the B3 groups. The highest coefficient was demonstrated between ISQ and PTV. Conclusions: Primary stability was enhanced by the presence of bicortex and increased RBH, which may be better demonstrated by ISQ and PTV than by IT and RT.

상악결절과 다공성 골 미네랄의 복합이식을 통한 상악동 골이식술 (MAXILLARY SINUS AUGMENTATION WITH BOVINE BONE MINERAL(Bio-$Oss^{(R)}$) AND THE MAXILLARY TUBEROSITY)

  • The aim of this study was to evaluate implant stability placed in the maxillary sinus which was augmented with bovine bone mineral(Bio-$Oss^{(R)}$) mixed with autogenous bone from the maxillary tuberosity. Maxillary sinus floor augmentation with the mixture of bovine bone mineral and autogenous maxillary tuberosity bone was performed in 30 maxillary sinuses, and 68 implants were placed at the time of sinus graft. After 6 months of implant placement abutments were connected and implant stability quotient(ISQ) was measured by radio frequency analysis(RFA). In addition, bone level changes was evaluated by taking periapical radiograph. During surgical procedures, no complication was observed, and all patients healed uneventfully. At 6 months the implant showed stable ISQ values. The marginal bone level changes around the fixtures was stably maintained through out the follow up period. This study confirmed that maxillary sinus floor augmentation with mixture of bovine bone mineral and maxillary tuberosity bone could be reliable for bone regeneration in subantral space.

Biomechanical evaluation of dental implants with different surfaces: Removal torque and resonance frequency analysis in rabbits

  • STATEMENT OF PROBLEM. Macroscopic and especially microscopic properties of implant surfaces play a major role in the osseous healing of dental implants. Dental implants with modified surfaces have shown stronger osseointegration than implants which are only turned (machined). Advanced surface modification techniques such as anodic oxidation and Ca-P application have been developed to achieve faster and stronger bonding between the host bone and the implant. PURPOSE. The purpose of this study was to investigate the effect of surface treatment of titanium dental implant on implant stability after insertion using the rabbit tibia model. MATERIAL AND METHODS. Three test groups were prepared: sandblasted, large-grit and acid-etched (SLA) implants, anodic oxidized implants, and anodized implants with Ca-P immersion. The turned implants served as control. Twenty rabbits received 80 implants in the tibia. Resonance frequencies were measured at the time of implant insertion, 2 weeks and 4 weeks of healing. Removal torque values (RTV) were measured 2 and 4 weeks after insertion. RESULTS. The implant stability quotient (ISQ) values of implants for resonance frequency analysis (RFA) increased significantly (P <. 05) during 2 weeks of healing period although there were no significant differences among the test and control groups (P >. 05). The test and control implants also showed significantly higher ISQ values during 4 weeks of healing period (P < .05). No significant differences, however, were found among all the groups. All the groups showed no significant differences in ISQ values between 2 and 4 weeks after implant insertion (P >. 05). The SLA, anodized and Ca-P immersed implants showed higher RTVs at 2 and 4 weeks of healing than the machined one (P < .05). However, there was no significant difference among the experimental groups. CONCLUSION. The surface-modified implants appear to provide superior implant stability to the turned one. Under the limitation of this study, however, we suggest that neither anodic oxidation nor Ca-P immersion techniques have any advantage over the conventional SLA technique with respect to implant stability.
