STATEMENT OF PROBLEM. Primary stability at the time of implant placement is related to the level of primary bone contact. The level of bone contact with implant is affected by thread design, surgical procedure and bone quality, etc. PURPOSE. The aim of this study was to compare the initial stability of the various taper implants according to the thread designs, half of which were engaged to inferior cortical wall of type IV bone(Group 1) and the rest of which were not engaged to inferior cortical wall(Group 2) by measuring the implant stability quotient(ISQ) and the removal torque value(RTV). MATERIAL AND METHODS. In this study, 6 different implant fixtures with 10 mm length were installed. In order to simulate the sinus inferior wall of type IV bone, one side cortical bone of swine rib was removed. 6 different implants were installed in the same bone block following manufacturer s recommended procedures. Total 10 bone blocks were made for each group. The height of Group 1 bone block was 10 mm for engagement and that of group 2 was 13 mm. The initial stability was measured with ISQ value using Osstell $mentor^{(R)}$ and with removal torque using MGT50 torque gauge. RESULTS. In this study, we found the following results. 1. In Group 1 with fixtures engaged to the inferior cortical wall, there was no significant difference in RTV and ISQ value among the 6 types of implants. 2. In Group 2 with fixtures not engaged to the inferior cortical wall, there was significant difference in RTV and ISQ value among the 6 types of implants(P < .05). 3. There was significant difference in RTV and ISQ value according to whether fixtures were engaged to the inferior cortical wall or not(P < .05). 4. Under-drilling made RTV and ISQ value increase significantly in the NT implants which had lower RTV and ISQ value in Group 2(P < .05). CONCLUSIONS. Without being engaged to the inferior cortical wall fixtures had initial stability affected by implant types. Also in poor quality bone, under-drilling improved initial stability.

The use of resonance frequency analysis (RFA) provides a possibility to clinically measure implant stability and osseointegration. The implant stability quotient (ISQ) value of RFA is well known that influenced by effective abutment length and stiffness of the implant in the surrounding tissues. Among these factors stiffness is not accurately defined histologically yet. And the purpose of this study was to find the histolgical relationship of RFA. 17 implants in 3 beagle dogs were used for this study. Among these implants 10 were survived for 7 months, 4 were survived for 3 months and 3 were immediate status after placement. Resonance frequency analyses were conducted and the dogs were sacrificed. Percentage of the bone to implant contact (BIC) in the interface, percentage of the mineralized bone (bone area) within the threads of the implant, and marginal bone level were measured under light microscopy. The correlation between resonance frequency and histomorphometric measurements were analysed and following results were obtained. 1. There was statistically significant correlation between ISQ value and BIC on healed implants. But ISQ value and BIC of all implants were not significantly correlated. (P<0.01) 2. Significant correlation between ISQ value and bone area was not found in this study. 3. There was statistically significant correlation between ISQ value and marginal bone level on all implants as well as on healed implants. (P<0.01).

Purpose: This study was aimed to evaluate the effect of rhPMP-2 coated implants on alveolar ridge augmentation in dogs. Materials and methods: Six Beagle dogs were used in this study. Six 8.0 mm long anodized surface titanium implants were placed 5 mm into the mandibular alveolar ridge following 6 month of healing period after extraction. Each animal received three implants coated with rhBMP-2 and three uncoated control implants using the randomized split-mouth design. Radiographic examinations were undertaken immediately at implant placement (baseline), at weeks 4 and 8 after implant placement. The amount of bone augmentation was evaluated by measuring the distance from the uppermost point of the coverscrew to the marginal bone. Implant Stability Quotient (ISQ) values were measured immediately at implant placement and 8 weeks after implant placement. For the statistical analysis, Man-Whitney ranksum test and Wilcoxon signed rank test of SPSS 12.0 software were used (P=.05). Results: The BMP group exhibited radiographic vertical bone augmentation about $0.6{\pm}0.7$ mm at 8 weeks later while controls showed bone loss about $0.4{\pm}0.6$ mm. There was significant difference among the rhBMP-2 group and controls in bone level change (P<.05). The ISQ values were significantly higher in the BMP-2 group than the control group at 8 weeks later (P<.05), while there was no significant difference at surgery. Conclusion: Within the limitation of this study, the rhBMP-2 coated on anodized implant could stimulate vertical alveolar bone augmentation, which may increase implant stability significantly on completely healed alveolar ridge.

The aim of this study was to evaluate implant stability placed in the maxillary sinus which was augmented with bovine bone mineral(Bio-$Oss^{(R)}$) mixed with autogenous bone from the maxillary tuberosity. Maxillary sinus floor augmentation with the mixture of bovine bone mineral and autogenous maxillary tuberosity bone was performed in 30 maxillary sinuses, and 68 implants were placed at the time of sinus graft. After 6 months of implant placement abutments were connected and implant stability quotient(ISQ) was measured by radio frequency analysis(RFA). In addition, bone level changes was evaluated by taking periapical radiograph. During surgical procedures, no complication was observed, and all patients healed uneventfully. At 6 months the implant showed stable ISQ values. The marginal bone level changes around the fixtures was stably maintained through out the follow up period. This study confirmed that maxillary sinus floor augmentation with mixture of bovine bone mineral and maxillary tuberosity bone could be reliable for bone regeneration in subantral space.
Purpose: The aim of this study was to compare and analyze the peri-implant tissue conditions and prospective clinical outcomes associated with 2 types of hydroxyapatite (HA)-coated implants: (1) fully HA-coated implants and (2) partially HA-coated implants with resorbable blast medium on the coronal portion of the threads. Methods: Forty-four partially edentulous patients were randomly assigned to undergo the placement of 62 HA-coated implants, and were classified as the control group (partially HA-coated, n=30) and the test group (fully HA-coated, n=32). All patients had chronic periodontitis with moderate crestal bone loss around the edentulous area. The stability and clinical outcomes of the implants were evaluated using the primary and secondary implant stability quotient (ISQ), as well as radiographic, mobility, and peri-implant soft tissue assessments around the implants. The Wilcoxon signed-rank test and the Mann-Whitney test were used to evaluate differences between and within the 2 groups, with P values <0.05 considered to indicate statistical significance. Results: The fully HA-coated implants displayed good retention of crestal bone, and insignificant differences were found in annual marginal bone loss between the 2 types of HA-coated implants (P>0.05). No significant differences were found in the survival rate (group I, 100%; group II, 100%) or the success rate (group I, 93.3%; group II, 93.8%). The fully HA-coated implants also did not significantly increase the risk of peri-implantitis (P>0.05). Conclusions: The fully HA-coated implants did not lead to an increased risk of peri-implantitis and showed good retention of the crestal bone, as well as good survival and success rates. Our study suggests that fully HA-coated implants could become a reliable treatment alternative for edentulous posterior sites and are capable of providing good retention of the crestal bone.
Purpose: This experiment examined the effect of different surface treatment on the osseointegration and stability of implants. Material and methods: In this study, 40 each of machined, SLA and RBM implants, which are the most commonly used implants, were implanted into the tibia of 20 normal rabbits using $OsseoCare^{TM}$. The rabbits were sacrificed after 1 week, 4 weeks, 8 weeks and 12 weeks for implant stability analysis, removal torque analysis, histologic and histomorphometric analysis. Result : ISQ showed significant difference between Machined and RBM at first week and at 4 weeks. There was significant difference between Machined and both SLA and RBM(p<0.05) but after 8 weeks there were no significant difference between each group. In the removal torque, RBM showd significantly higher values than SLA and Machined surface at 1st week. At 4th and 12th week, there was significant difference between Machined and SLA, RBM(p<0.05). In the bone to implant contact variable, there was no significant difference between each surface treatment method. In the Machined surface group, there was no significant difference between each time interval. but in SLA group, there were significant differences between the 1st week and 12th week and in RBM group, there were significant differences between the 1st week and 8th, 12th week and between 4th and 12th week(p<0.05). The bone area showed significantly higher values in SLA and RBM compared to Machined surface 1st and 8th week and significantly higher values in SLA than Machined surface at the 4th week(p<0.05). Conclusion: The roughened surface of implants showed positive effect in the early stages of implantation and assisted in bone formation After the bone formation stage, there was no statistical difference between Machined and roughened surface groups. In dental implantation, where initial stability is critical to the success of implants, the use of roughened surface implants should assist in reducing the healing period after implantation.

Purpose: The aim of this study was to determine the impact of different compressive forces on deproteinized bovine bone mineral (DBBM) particles covered by native bilayer collagen membrane (NBCM) during alveolar ridge preservation (ARP) in the molar area, and to identify any histomorphometric and clinical differences according to the compressive force applied. Methods: Sockets were filled with DBBM after tooth extraction, and different compressive forces (30 N and 5 N, respectively) were applied to the graft material in the test (30 N) and control (5 N) groups. The DBBM in both groups was covered with NBCM in a double-layered fashion. A crossed horizontal mattress suture (hidden X) was then made. A core biopsy was performed using a trephine bur without flap elevation at the implant placement site for histomorphometric evaluations after 4 months. The change of the marginal bone level was measured using radiography. Results: Twelve patients completed the study. The histomorphometric analysis demonstrated that the mean ratios of the areas of new bone, residual graft material, and soft tissue and the implant stability quotient did not differ significantly between the groups (P>0.05). However, the mean size of the residual graft material showed a significant intergroup difference (P<0.05). Conclusions: The application of 2 compressive forces (5 N, 30 N) on particulate DBBM grafts during open-healing ARP in the posterior area led to comparable new bone formation, implant feasibility and peri-implant bone level.
Statement of problem. Various anodic oxidation techniques can be applied to dental implant surfaces. But the condition for optimal anodized surfaces has not been described yet. Purpose. The purpose of this investigation was to compare an implant that was oxidized by another method with $TiUnite^{TM}$ through resonance frequency analysis and histomorphometry. Material and methods. Turned (control), $TiUnite^{TM}$ and another oxidized fixtures, which used $Ca^{2+}$ solution for anodic oxidation, were placed in the tibiae of 5 New Zealand White rabbits. The bone responses were evaluated and compared by consecutive resonance frequency analysis once a week for 6 weeks and histomorphometry after a healing period of 6 weeks. Results. At the first week, both oxidized implants showed significantly higher implant stability quotient (ISQ) values than the control. No significant differences in resonance frequency analysis were found between the two oxidized groups for 6 weeks. The means and standard deviations of bone-to-implant contact (BIC) ratios were $71.0{\pm}4.2$ for $TiUnite^{TM}$, $67.5{\pm}10.3$ for the $Ca^{2+}$-based oxidation fixture, $22.8{\pm}6.5$ for the control. Both oxidized implants were significantly superior in osseointegration to the turned one. There was, however, no statistically significant difference between the two oxidized implants. Conclusion. $TiUnite^{TM}$ and the $Ca^{2+}$-based oxidation fixture showed superior early bone response than the control with respect to resonance frequency analysis and histomorphometry. No significant differences between the oxidized groups, however, were found in this investigation using the rabbit tibia model.
Purpose: The aim of this study was to determine the influence of anatomical conditions on primary stability in the models simulating posterior maxilla. Methods: Polyurethane blocks were designed to simulate monocortical (M) and bicortical (B) conditions. Each condition had four subgroups measuring 3 mm (M3, B3), 5 mm (M5, B5), 8 mm (M8, B8), and 12 mm (M12, B12) in residual bone height (RBH). After implant placement, the implant stability quotient (ISQ), Periotest value (PTV), insertion torque (IT), and reverse torque (RT) were measured. Two-factor ANOVA (two cortical conditions${\times}$four RBHs) and additional analyses for simple main effects were performed. Results: A significant interaction between cortical condition and RBH was demonstrated for all methods measuring stability with two-factor ANOVA. In the analyses for simple main effects, ISQ and PTV were statistically higher in the bicortical groups than the corresponding monocortical groups, respectively. In the monocortical group, ISQ and PTV showed a statistically significant rise with increasing RBH. Measurements of IT and RT showed a similar tendency, measuring highest in the M3 group, followed by the M8, the M5, and the M12 groups. In the bicortical group, all variables showed a similar tendency, with different degrees of rise and decline. The B8 group showed the highest values, followed by the B12, the B5, and the B3 groups. The highest coefficient was demonstrated between ISQ and PTV. Conclusions: Primary stability was enhanced by the presence of bicortex and increased RBH, which may be better demonstrated by ISQ and PTV than by IT and RT.
Purpose: Resonance frequency analysis, Periotest, and removal torque (RT) test were known as the methods to assess implant stability. The results of these methods are affected by the bone condition, implant diameter and shape. The purpose of this study is to access the meaning and the correlationship of the resonance frequency analysis, Periotest and RT test in osseointegration simulated acrylic resin when the engaged bone thickness and peri-implant bone defect are changed. Materials and methods: To simulate osseointegration, the fixture was fixed to an aluminum mold with a screw. Acrylic resin powder and liquid were poured into the mold for polymerization. The engaged resin thickness with implant was controlled. Simulated cortical bone thicknesses were 1, 3, 5 and 10 mm. Additional 1, 3 and 5 mm peri-implant bone defects were simulated. Three types of implants were used; 4 mm diameter implants of straight shape, 4 mm diameter implants of tapered shape and 5 mm diameter implants of tapered shape. Five fixtures per each type were tested in respective bone condition. Resonance frequency analysis and Periotest were evaluated in all bone conditions. Peak removal torque was measured at simulated cortical bone thicknesses of 1 and 3 mm. The statistical analysis was performed with the Kruskal-Wallis test, Mann-Whitney U test, and Spearman test using a 95% level of confidence. Results: With increasing engaged bone depth, the Implant Stability Quotient (ISQ) values increased and the Periotest values (PTVs) decreased (P<.001, P<.001). With increasing peri-implant bone defect, ISQ values decreased and PTVs increased (P<.001). When the diameter of implant increased, ISQ values increased and Periotest values (PTV) decreased (P<.001). There was a strong correlation between ISQ values and PTVs (r = -0.99, P<.001). Furthermore, the peak removal torque values had weak correlations with both ISQ values and PTVs (r = 0.52, P<.001 ; r = -0.52, P<.001). Conclusion: This study confirmed favorable implant stability with increasing engaged bone depth and implant diameter and decreasing peri-implant bone defect. ISQ values and PTVs showed strong correlation with each other and not with the peak removal torque values.
