Purpose: A novel attachment system for implant-retained overdentures (IRODs) with novel material combinations for improved mechanical resilience and prosthodontic success (Novaloc) has been recently introduced as an alternative to an existing system (Locator). This study investigated whether differences between the Novaloc and Locator attachment systems translate into differences in implant survival, implant success, and patient-centered outcomes when applied in a real-world in-practice comparative setting in patients restored with mandibular IRODs supported by 2 interforaminal implants (2-IRODs). Methods: This prospective, intra-subject crossover comparison compared 20 patients who received 2 intra-foraminal bone level tapered implants restored with full acrylic overdentures using either the Locator or Novaloc attachment system. After 6 months of function, the attachment in the corresponding dentures was switched, and the definitive attachment system type was delivered based on the patient's preference after 12 months. For the definitive attachment system, implant survival was evaluated after 24 months. The primary outcomes of this study were oral health-related quality of life and patient preferences related to prosthetic and implant survival. Secondary outcomes included implant survival rate and success, prosthetic survival, perceived general health, and patient satisfaction. Results: Patient-centered outcomes and patient preferences between attachment systems were comparable, with relatively high overall patient satisfaction levels for both attachment systems. No difference in the prosthetic survival rate between study groups was detected. The implant survival rate over the follow-up period after 24 months in both groups was 100%. Conclusions: The results of this in-practice comparison indicate that both attachment systems represent comparable candidates for the prosthodontic retention of 2-IRODs. Both systems showed high rates of patient satisfaction and implant survival. The influence of material combinations of the retentive system on treatment outcomes between the tested systems remains inconclusive and requires further investigations.
Purpose: This study aimed to provide the basic data for improving patient satisfaction and dental services by examining the satisfaction of implant patients who have become important customers of dental treatment due to the increased demand for implants and identifying the factors affecting satisfaction. Methods: Out of 145 questionnaires, excluding 15 questionnaires that were unfaithfully responded to or difficult to analyze, 130 questionnaires were used for the analysis of the satisfaction of the dental patients with implant treatment in 19 dental hospitals and clinics in Gyeonggi-do, South Korea. Results: Implant satisfaction was as high as 3.20 (±0.56) for masticatory function satisfaction, 3.19 for psychological function satisfaction (±0.59), and 3.01 (±0.58) for social function satisfaction. Conclusion: Consistent patient care is important because an implant is not a one-shot cure but ends over a period of several months to several years. It is meaningful in that this study measured the satisfaction of implant prostheses evaluated from a patient's point of view and examined various factors and needs affecting patient satisfaction to provide the basic data for research that can contribute to improving the satisfaction of implant patients.
Purpose: Common complications of immediate breast reconstruction with implant are capsular contracture, malposition of implant, hematoma and seroma. Especially, the most severe complication is implant exposure caused by inflammation or necrosis of skin flap margin of mastectomy site. This article reports the experience of cases of salvage in such an exposure of implant. Methods: From July, 2002 to Feb., 2009, sixty-five patients who underwent immediate breast reconstruction with implant were retrospectively analyzed. Exposure of implant was happened in 5 of 65 patients and they were treated at out patient district. Two of five patients were reconstructed with saline implnt and all of them underwent the enveloping of the implant with AlloDerm$^{(R)}$ and Serratus muscle flap. Remaning three patients were reconstructed with silicone implant and all of them underwent the enveloping of the implant with AlloDerm$^{(R)}$. Results: In the group of patients who underwent reconstruction with saline implant, implant exposure was found in one patient due to partial necrosis of the margin of skin flap and debridement and primary repair were done. In the other one patient, dressing with antibiotic ointment were done. And debridement and primary repair were proceeded. In the group of patients who underwent reconstruction with silicone implant, implant exposure was found in one patient. After removal of the implant, tissue expansion was done and a new silicone implant was inserted. Implant exposure were found in the other two patients, antibiotics ointment application and primarily repaired. Conclusion: It was the common knowledge that the exposed implant should be removed. But salvage of the exposed implants may be possible with proper treatment. Four of five patients (80%) with exposed breast implant were salvaged with conservative management.
Purpose: Breast implant ruptures and displacement are problematic complications after augmentation mammoplasty. The authors report a patient whose cohesive silicone gel implant ruptured and migrated into the pleural cavity after augmentation mammoplasty. Methods: A 23-year-old female had received augmentation mammoplasty at a local clinic a week before visiting our hospital. When the patient's doctor performed a breast massage on the sixth postoperative day, the left breast became flattened. The doctor suspected a breast implant rupture and performed revision surgery. The implant, however, was not found in the submuscular pocket and no definite chest wall defect was found in the operative field. The doctor suspected implant migration into the pleural cavity, and after inserting a new breast implant, the doctor referred the patient to our hospital for further evaluation. The patient's vital signs were stable and she showed no specific symptoms except mild, intermittent pain in the left chest. A CT scan revealed the ruptured implant in the left pleural cavity and passive atelectasis. Results: The intrapleurally migrated ruptured implant was removed by video-assisted thoracic surgery (VATS). There were no adhesions but there was mild inflammation of the pleura. No definite laceration of the pleura was found. The patient was discharged on the first day after the operation without any complications. Conclusion: Surgeons should be aware that breast implants can rupture anytime and the injury to the chest wall, which may displace the breast implant into the pleural cavity, can happen during submuscular pocket dissection and implant insertion.
In this paper, we present a novel algorithm to generate a 3D model of patient-specific orbital implant, which is finally produced by the 3D printer. Given CT (computed tomography) scan data of the defective orbital wall or floor, we compose the depth image of the defect site by using the depth buffering, which is a computer graphics technology. From the depth image, we compute the 3D surface which fills the broken part by interpolating the points around the broken part. By thickening the 3D surface, we get the 3D volume mesh of the orbital implant. Our algorithm generates the patient-specific orbital implant whose shape is accurately coincident to the broken part of the orbit. It provides the significant time efficiency for manufacturing the implant with supporting high user convenience.
Implant-supported restorations were connected to commercially pure titanium endosteal implants 3 months in mandible and 6 months in maxilla after the implants were inserted into patient jaws. Modifications of titanium implant surfaces have reduced the waiting time for osseointegration, resulting in the development of the early loading concept, which is defined at present as a restoration in contact with the opposing dentition and placed at least 1 week after implant placement, but no later than 2 months afterward. Nowadays, immediate loading protocols have also been introduced, using the implants that are designed to enhance initial stability. Immediate loading eliminates the edentulous period of a patient. Although dentists widely accept these concepts of early and immediate loading, they agree the conventional loading concept is still necessary, which describes loading protocols later than 2 months after implant insertion. The timing of loading is determined mainly by the factors of a patient. This paper considers for what dental clinicians should be careful in implant-supported restoration procedures, considering the implant late failure.
The purpose of this study was to provide basic data of education program and consult for dental implant patient. The 210 of dental patients were selected at five dental clinic located in Jin-ju city. Data were collected by self-administrated questionnaire. A SPSS 10.10 for Windows was used for statistical analysis. The results of the study are summarized as follows: 1. The female responser was 56.2%, the rate of age was thirty years old 26.7%, education level was more than university and college 45.7%, job is company unit 15.2%, income was 100~200man won of 48.6%. 2. Recognition of dental implant was that incase of 'yes' is more than sixty years old 77.3%. 3. The information of implant was from neighborhood 29.0%, and when dental consult 24.3%. 4. The responsor who is everyone can take the dental implant operation is 39.5%, can not take is 32.9%. 5. The responsor who is everyone wants implant operation was 45.2%, 'do not' is 17.1%. 6. Using period of implant tooth is 'forever' 40.0%. In limited of twenty years' is 23.8%. 7. When everyone select to dental clinic was because of dentist medical technique is 64.3%, dental treatment cost is 15.9%. In conclusion, it is necessary for the dental implant patient to make dental consult and education program. So, try to study for make basic data with analysis recognition of dental implant patients.

PURPOSE. The aim of this systematic review is to address treatment outcomes of Mandibular implant overdentures relative to implant survival rate, maintenance and complications, and patient satisfaction. MATERIALS AND METHODS. A systematic literature search was conducted by a PubMed search strategy and hand-searching of relevant journals from included studies. Randomized Clinical Trials (RCT) and comparative clinical trial studies on mandibular implant overdentures until August, 2010 were selected. Eleven studies from 1098 studies were finally selected and data were analyzed relative to number of implants. RESULTS. Six studies presented the data of the implant survival rate which ranged from 95% to 100% for 2 and 4 implant group and from 81.8% to 96.1% for 1 and 2 implant group. One study, which statistically compared implant survival rate showed no significant differences relative to the number of implants. The most common type of prosthetic maintenance and complications were replacement or reattaching of loose clips for 2 and 4 implant group, and denture repair due to the fracture around an implant for 1 and 2 implant groups. Most studies showed no significant differences in the rate of prosthetic maintenance and complication, and patient satisfaction regardless the number of implants. CONCLUSION. The implant survival rate of mandibular overdentures is high regardless of the number of implants. Denture maintenance is likely not inflenced substantially by the number of implants and patient satisfaction is typically high again regardless os the number of implants.
The completely edentulous patient has few treatment options in conventional dentistry. When implants are considered, treatment plans range from a 2-implant overdenture to a completely implant-supported prosthesis. Fixed prosthesis is often the preferred selection of the edentulous patient. fixed full-arch cert amo-metal restorations can be a predictable implant treatment modality for the edentulous patient. Implant-supported fixed prosthesis has several advantages: predictability, fixedness, retrievability, improved function, lower maintenance of prosthesis, long-term published success. Edentulous patients with a severely resorbed mandible often experience problems with their dentures. Treatment concepts involving two to four implants for the support of an overdenture have been proposed. There seems to be no need to insert more than two endosteal implants to support an overdenture, however, long-term prospective studies are needed to support this notion. Using short endosseous implants and an overdenture in the extremely resorbed mandible is a justified treatment option because of the relative simplicity and low morbidity of this treatment strategy. Implant-supported overdenture has several advantages: Cost, retrievability, hygiene access, profile and contour control, increased retention and stability, implant installed in a predicted region(ant. mandible).
무치악 환자에서 임플란트를 이용한 보철치료는 임플란트 고정성 보철, 임플란트 융합 국소 의치, 하이브리드 보철물, 임플란트 유지 및 지지 피개의치 등의 다양한 방법이 있다. 본 증례에서는 중등도 이상의 만성 치주염에 이환 되어 전악 발치가 필요한 환자에서 computer guided surgery를 이용하여 계획된 위치에 임플란트를 식립하였다. 상악은 불량한 골질로 초기고정력이 충분하지 않아 지연 부하를 시행하였고 골 유착 기간 동안 임시 의치를 사용하였다. 하악은 안정적인 초기 고정을 얻어 즉시 부하를 이용하여 환자의 불편감을 감소시켰다. 임플란트 일차 안정은 즉시 부하를 얻기 위해 가장 중요한 요소로 여겨지기 때문에 수술 전 반드시 임상적, 방사선학적 평가를 통해 잔존 치조골 양 및 골질을 파악해야 한다.
