충돌 증후군과 회전근 개 파열을 위한 이학적 검사들의 진단적 가치에 대한 분석 (Diagnostic Accuracy of Physical Examinations in Impingement Syndrome and Rotator Cuff Tear)

  • Purpose: The aim of this study was to investigate the diagnostic accuracy of Neer, Hawkins provocative tests and supraspinatus manual muscle test for the assessment of impingement syndrome, partial tear and small complete tear of De rotator cuff. Materials and Methods: Seventy-one female and 115 male patients were included in the study. Patients were divided into four groups of no impingement, impingement without tear, partial tear and small complete tear of the rotator cuff, which were confirmed by sonogram, magnetic resonance imaging and surgery. Neer and Hawkins provocative tests and supraspinatus manual muscle test were performed respectively. SAS 6.12 version was used in statistical analysis. Results: We found that Neer test had 94% sensitivity, 54% specificity for impingement without tear and 89% sensitivity, 78% specificity for partial tear and 96% sensitivity, 23% specificity for small tear. Hawkins test revealed 95% sensitivity, 54% specificity for impingement without tear and 93% sensitivity, 78% specificity for partial tear and 100% sensitivity, 23% specificity for small tear. Supraspinatus manual muscle test revealed 27% sensitivity, 94% specificity for impingement without tear and 29% sensitivity, 82% specificity for partial tear and 48% sensitivity, 82% specificity for small tear. Conclusion: Neer and Hawkins tests have high sensitivity, low specificity for impingement syndrome, partial and small tear. Supraspinatus manual muscle test had low sensitivity and high specificity. However this test was not effective to differentiate the partial and small rotator cuff tear. We thought that more effective provocative test should be designed to detect the partial and small rotator cuff tear.

The Effects of Scapular Stabilization Training using the Different Exercise Prop for Impingement Syndrome of Factory Workers

  • Objective: The purpose of this studyaimed to investigate the effects of scapular stabilization training using the different exercise prop for impingement syndrome of factory workers. Design: A randomized controlled trial Methods: Twenty shoulder impingement syndrome subjects were randomly divided into two groups of ten subjects each. Inclusion criteria was presence of positive sign in at least two of Neer test, Hawkins test, Yocum test, Jobe test and Speeds test. Andexclusion criteria, those with surgical history of rotator cuff, those with disease in upper limb other than shoulder impingement syndrome, those who took anti-inflammatory drugs over the past 12 months were excluded from the sample. One group was performed scapula stabilization exercise using elastic bands, while dumbbell group performed the exercise using dumbbell. Both groups performed the exercise after applying the general physical therapy. After applying five days a week for a total of six weeks, range of motion of shoulder joint, muscle strength and pain and function level were evaluated. Results: After the intervention, in both groups, all dependent variables at the 6-week post-test significantly improved compared with the pre-test (p<0.05). In addition, elastic band exercise group showed a significant differences in SPADI function score and SPADI total score compared to the dumbbell group(p<0.05). Conclusions: The elastic band training may be effective methodology using low-to-moderate intensity resistance for SPADI scores.

봉우리밑충돌증후군 환자의 날개 어깨뼈 평가에 대한 신뢰도 검사 및 날개 어깨뼈와 어깨뼈 안정성에 대한 상관성 연구 (Reliability Test for Winged Scapula and Correlation between Winged Scapula and Scapular Stability in Patients with Subacromial Impingement Syndrome)

  • Purpose: This study investigated the differences in winged scapula between the symptomatic side and asymptomatic side, the reliability of the winged scapula test, and the correlation between winged scapula and the stability of the scapula in patients with subacromial impingement syndrome. Methods: Twenty-four patients with unilateral subacromial impingement syndrome participated in this study. Winged scapula was compared between the symptomatic and asymptomatic sides using a scapulometer. The reliability of the scapulometer was assessed by calculating the intraclass correlation coefficient (ICC), standard error of measurement (SEM), and minimal clinically important difference (MCID). The correlation between winged scapula and the results of the upper quarter Y-balance test were also obtained. Conclusion: Winged scapula was significantly greater on the symptomatic side compared with the asymptomatic side (p < 0.05). High to excellent ICCs were obtained for the winged scapula test, and SEM and MCID values were obtained for winged scapula (SEM: 0.2-0.3 cm, MCID: 0.6-0.8 cm); however, winged scapula and the results of the upper quarter Y-balance test were not correlated. Conclusion: The scapulometer is useful for measuring winged scapula in patients with subacromial impingement syndrome.

Correlation between Results of Preoperative Impingement Test and Clinical Outcomes after Arthroscopic Rotator Cuff Repair

  • Background: The aim of the present study was to determine the correlation between the amount of pain reduction after local anesthetic injection into the subacromial space preoperatively and clinical outcome after arthroscopic rotator cuff repair. Methods: A total of 127 patients who underwent arthroscopic rotator cuff repair and followed up at least 1 year were analyzed retrospectively. Preoperatively, a visual analogue scale (VAS) for pain was measured in all patients before and after the ultrasound guided impingement test. The participants were divied into four groups according to pain reduntion ater impingement test (Group A: >75%, Group B: 50%-75%, Group C: 25%-50%, Group D: <25%). VAS for pain, shoulder range of motion, shoulder isometric strength, ASES score were evaluated preoperatively and at 3, 6, 9, and 12 months postoperatively. Results: After surgery, the amount of pain reduction shows significantly at 3, 6 months in Groups A, B as compared to Groups C, D (p<0.05). Among the range of motion of shoulder joint, forward flexion was significantly improved in Group A at 3 months (p<0.05). The ASES score significantly improved at 3, 6 months in Groups A, B as compared to Group C, D (p<0.05). Conclusions: Preoperative degree of pain reduction after impingement test correlates with the improvement of pain after arthroscopic rotator cuff repair, especially in the early phase. Therefore, the impingement test could be effectively used.

Atomization Improvement of a Liquid Jet with Wall Impingement and its Application to a Jet Engine Atomizer

  • In the present study, capability of improving the liquid atomization of a high-speed liquid jet by using wall impingement is explored, and its application to a jet engine atomize. is demonstrated. Water is injected from a thin nozzle. The liquid jet impinges on a wall positioned close to the nozzle exit, forming a liquid film. The liquid film velocity and the SMD were measured with PDA and LDSA, respectively. It was shown that the SMD of the droplets was determined by the liquid film velocity and impingement angle, regardless of the injection pressure or impingement wall diameter. When the liquid film velocity was smaller than 300m/s, a smaller SMD was obtained, compared with a simple free jet. This wall impingement technique was applied to a conventional air-blasting nozzle for jet engines. A real-size air-blasting burner was installed in a test rig in which three thin holes were made to accommodate liquid injection toward the intermediate ring, as an impingement wall. The air velocity was varied from 41 to 92m/s, and the liquid injection pressure was varied from 0.5 to 7.5 MPa. Combining wall impinging pressure atomization with gas-blasting produces remarkable improvement in atomization, which is contributed by the droplets produced in the pressure atomization mode. Comparison with the previous formulation for conventional gas-blasting atomization is also made, and the effectiveness of utilizing pressure atomization with wall impingement is shown.

Effect of Physiotherapeutic Intervention Using TECAR Therapy on Pain Self-Awareness and Hip Joint Function in Hip Impingement Syndrome: A Case Study

  • PURPOSE: The current case study focuses on identifying the effects of the independent application of TECAR therapy and physiotherapeutic intervention using TECAR therapy on pain self-awareness and hip joint function in patients with hip impingement syndrome caused by nonstructural changes. Subjects: The research subject was a 34-year-old woman struggling with acute pain in her left hip, difficulty in actively moving the hip, and a problem in its overall function. METHODS: The subject's pain awareness and hip joint function were measured using a Visual Analog Scale (VAS) and passive range of motion (PROM), respectively. The experimental intervention was carried out in 24 sessions of 16 minutes each, three times a week, for eight weeks. RESULTS: The VAS score decreased to 0 cm on the post-test from 4.3 cm, 6.5 cm, and 7.2 cm in the pre-test at the rest, standing, and gait positions, respectively. The index of PROM measured hip joint flexion, extension, abduction, adduction, internal rotation, external rotation, and passive straight leg raise. The values increased to 122.5°, 24.5°, 78°, 33°, 65°, 42°, and 96.5° in the post-test compared to 88.5°, 15°, 39°, 21.5°, 23°, 22°, and 46.5° in the pre-test, respectively. CONCLUSION: TECAR therapy and physiotherapeutic intervention using TECAR can help reduce pain and enhance the hip joint function in patients with hip impingement syndrome.

급수가열기 충격판 설계변경에 따른 동체감육 완화에 관한 유동해석 연구 (A Study on the Fluid Mixing Analysis for the Shell Wall Thinning Mitigation by Design Modification of a Feedwater Heater Impingement Baffle)

  • Feedwater heaters of many nuclear power plants have recently experienced wall thinning damage, which will increase as operating time progresses. As it is judged that the wall thinning damages have generated due to local fluid behavior around the impingement baffle installed in downstream of the high pressure turbine extraction steam line to avoid colliding directly with the tubes, numerical analyses using PHOENICS code were performed for two models with original clogged impingement baffle and modified multi-hole impingement baffle. To identify the relation between wall thinning and fluid behavior, the local velocity components in x-, y-, and z-directions based on the numerical analysis for the model with the clogged impingement baffle were compared with the wall thickness data by ultrasonic test. From the comparison of the numerical analysis results and the wall thickness data, the local velocity component only in the y-direction, and not in the x- and z-direction, was analogous to the wall thinning configuration. From the result of the numerical analysis for the modified impingement baffle to mitigate the shell wall thinning, it was identified that the shell wall thinning may be controlled by the reduction of the local velocity in the y-direction.

Study on Effects of Pressure Ratio on the Wall-impingement Spray Characteristics of Nitrogen Gas using CNG Injector

  • In this study, an experimental investigation on the effects of the pressure ratio on the wall-impingement spray characteristics of nitrogen gas using a compressed natural gas (CNG) injector was conducted. The transient development of the impingement spray was recorded by a high speed camera with Z-type Schlieren visualization method. The spray behavior under various pressure ratio conditions were analyzed. The experimental results showed that the pressure ratio has positive effect on the development of spray wall-impingement. The effects of the above factor were evaluated in a constant volume chamber at atmospheric conditions. The data from test showed that, with the increase of the pressure ratio, the spray tip penetration (STP) quickly increases before the impingement and gradually increases after the impingement. Additionally, the spray velocity first increases and then sharply decreases on regardless of the injection pressure level. As the spray spreading angle increases, spray area and volume increases rapidly with the increase in STP at the beginning of injection, and finally entered a stable range, has a great correlation with the increase of pressure ratios.

EFI 인젝터에 의한 연료분무의 벽면충돌 특성 (Wall Impingement Phenomena of a Fuel Spray Injected by an EFI Injector)

  • In a port fuel injection system of engine, a large part of fuel injected into an intake port adheres on its wall and inlet valve. Consequently, the wall impinging spray interaction might occur the generation of several harmful phenomena. There are uncontrollable mixture formation, an accidental backfire and unburned hydrocarbons. Therefore, it is important to analyze the fuel behavior during the spray-wall interaction. In this study, splash characteristics of impingement and reflecting or scattering behavior of droplets of fuel injected from EFI nozzle were studied experimentally. A test fuel used is LAWS and its physical characteristics are similar to the conventional gasoline except for the ignition point. Since the liquid film formed immediately after impinging on an impingement plate is unstable, it is easy to cause secondary disintegration. In addition, when the intermittently impingement on the impingement plate with LAWS, the splash ratio is around 0.6. If an injection period becomes longer, liquid film will become thick and the splash ratio will fall bout 10 percent. On the other hand, when the injection period of an intermittent spray is long, the same time lapse as a continuous spray is shown.

과냉각대형액적 충돌예측을 위한 오일러리안 기반 수치 모델링 (Eulerian-based Numerical Modeling for Impingement Prediction of Supercooled Large Droplets)

  • 외부 환경조건에 의한 항공기 위협인자로서 과냉각 대형 액적은 그 중요성이 지속적으로 보고되고 있다. 이러한 대형 액적의 거동은 상대적으로 작은 액적과 달리 그 형태가 변화하며 액적이 표면과 충돌시 파편이 발생하는 등 다양한 물리적 특성을 나타낸다. 이러한 대형 액적의 거동을 시뮬레이션 하기 위해 비정렬 격자계 기반 2차원 압축성 Navier-Stokes 코드와 액적 거동 시뮬레이션 코드를 개발하였다. 또한 대형 액적의 물리적 현상을 모사하기 위해 반경험적 기법에 기반한 액적항력모델과 액적-고체표면 충돌 모델을 기존 액적장 지배방정식의 액적항력계수 및 경계면의 수치적 경계조건으로 적용하였다. 그 결과 풍동 시험과 액적충돌 영역 및 최대 축적율은 매우 유사하게 나타난 반면 NACA23012 익형의 아랫면 주위 축적율의 경향은 풍동 시험보다 다소 크게 나타났다.
