Application of Remote Sensing and Geographic Information System in Forest Sector (원격탐사와 지리정보시스템의 산림분야 활용)

  • Forest accounts for almost 64 percents of total land cover in South Korea. For inventorying, monitoring, and managing such large area of forest, application of remote sensing and geographic information system (RS/GIS) technology is essential. On the basis of spectral characteristics of satellite imagery, forest cover and tree species can be classified, and forest cover map can be prepared. Using three dimensional data of LiDAR(Light Detection and Ranging), tree location and tree height can be measured, and biomass and carbon stocks can be also estimated. In addition, many indices can be extracted using reflection characteristics of land cover. For example, the level of vegetation vitality and forest degradation can be analyzed with VI (vegetation Index) and TGSI (Top Grain Soil Index), respectively. Also, pine wilt disease and o ak w ilt d isease c an b e e arly detected and controled through understanding of change in vegetation indices. RS and GIS take an important role in assessing carbon storage in climate change related projects such as A/R CDM, REDD+ as well. In the field of climate change adaptation, impact and vulnerability can be spatio-temporally assessed for national and local level with the help of spatio-temporal data of GIS. Forest growth, tree mortality, land slide, forest fire can be spatio-temporally estimated using the models in which spatio-temporal data of GIS are added as influence variables.

Auditory brainstem response (ABR) results in NICU graduates (신생아 집중치료실 퇴원아의 청성뇌간반응(ABR) 결과에 미치는 위험인자 및 경과)

  • Purpose : Hearing loss is one of the most common birth defects, and early detection and intervention positively impact language/speech and cognitive development. It has been reported that NICU graduates have a high incidence of hearing loss. So we investigated the incidence, risk factors and clinical outcome of hearing loss in NICU graduates. Methods : This study involved neonatal auditory brainstem response (ABR) testing of newborn infants who graduated from the NICU of Kyungpook National University Hospital during a 3-year period (between July 2002 and June 2005) and subsequent follow-up of these infants. Results : ABR evaluations were performed on 474 infants. Of these infants, 64 showed abnormal ABR (13.5 percent). Of 128 ears from these 64 infants, two ears (1.6 percent) and 10 ears (7.8 percent) were classified as severe and profound hearing loss, respectively. The infants with abnormal ABR had higher incidence of prematurity, low birth weight, very low birth weight, neonatal asphyxia, cranio-facial malformation and amikacin treatment over 15 days (P<0.05). In infants with hyperbilirubinemia, the peak level of serum bilirubin, duration of phototherapy and exchange transfusion were not associated with the higher incidence of hearing loss. Follow-up ABR evaluation was performed on 15 infants with abnormal ABR at $8.8{\pm}4.4months$. In follow-up ABR, 80.0% showed improvement or normalization of threshold sensitivity. Conclusion : NICU graduates exhibit high risk for hearing loss. Systemic and effective hearing assessment program is needed for these high risk infants.

Characterization of Aerosol Composition, Concentration, and Sources in Bukhansan National Park, Korea (북한산국립공원 내 초미세먼지 농도 및 화학적 특성)

  • To improve understanding of the physico-chemical characteristics of aerosols in the national park and comparing the air pollution between national park and the urban area nearby national park, the aerosol characterization study was conducted in Bukhansan National Park, Seoul, from July through September 2017. Semi-continuous measurements of $PM_{2.5}$ using PILS (Particle Into Liquid System) coupled with IC (Ion Chromatography) and TOC (Total Organic Carbon) analyzer allowed quantification of concentrations of major ionic species($Cl^-$, $SO_4{^{2-}}$, $NO_3{^-}$, $Na^+$, $NH_4{^+}$, $K^+$, $Mg{^{2+}}$ and $Ca{^{2+}}$) and water soluble organic carbon (WSOC) with 30-minute time resolution. The total mass concentration of $PM_{2.5}$ was measured by T640 (Teledyne) with 5-minute time resolution. The black carbon (BC) and ozone were measured with a minute time resolution. The timeline of aerosol chemical compositions reveals a strong influence from urban area (Seoul) at the site in Bukhansan National Park. Inorganic aerosol composition was observed to be dominated by ammoniated sulfate at most times with ranging from $0.1{\sim}32.6{\mu}g/m^3$ (6.5~76.1% of total mass of $PM_{2.5}$). The concentration of ammonium nitrate, a potential indicator of the presence of local source, ranged from below detection limits to $20{\mu}g/m^3$ and was observed to be highest during times of maximum local urban (Seoul) impact. The total mass of $PM_{2.5}$ in Bukhansan National Park was observed to be 10~23% lower than the total mass of $PM_{2.5}$ in urban area (Gireum-dong and Bulgwang-dong, Seoul). In general, ozone concentration in Bukhansan National Park was observed to be similar or higher than urban sites in Seoul, suggesting additional biogenic VOCs with $NO_x$ from vehicle emission were to be precursors for ozone formation in Bukhansan National Park.

Prevalence of Distress and Its Influence on Quality of Life in Breast Cancer Patients (유방암 환자에서의 디스트레스의 유병률 및 디스트레스가 삶의 질에 미치는 영향)

  • Objectives : Early detection and proper management of distress are known to be important for efficient care in cancer patients. The aim of this study was to investigate the prevalence of distress and to evaluate its influence on quality of life in subjects with breast cancer. Methods : A total of 95 patients were recruited from a breast cancer patient community of a general hospital in Seoul. The Hospital Anxiety and Depression Scale(HADS), the Insomnia Severity Index(ISI), the Impact of Event Scale-Revised(IES-R) and the Functional Assessment of Cancer Therapy-Breast(FACT-B) were measured. Results : The prevalence of anxiety, depression, insomnia and posttraumatic stress disorder symptom among breast cancer patients was 26.3%, 26.3%, 13.7%, 24.3%, respectively. Nearly half(47.4%) of total subjects were under one or more significant distress, and they showed significantly lower mean FACT-B total score than patients without distress(82.91 vs. 107.20, p<0.001). Distress was negatively correlated with the quality of life to a significant level even in patients who were in normal range of distress scale scores. Conclusion : These results support the importance of regular screening and proper management of distress for breast cancer patients.

Studies on resveratrol and its metabolite in human urine by GC/MS (GC/MS를 이용한 요 중 resveratrol과 그 대사체에 관한 연구)

  • This study was conducted to define metabolite of the resveratrol by gas chromatography- time-offlight mass spectrometric detection. From these results, we suppose that the structure of metabolite is the result of reduction of double-bond attached by two-phenyl groups. Also, validity of method for determining metabolite of resveratrol and endogenous steroids was tested. The recoveries ranged from 96.47 to 114.74%, and intraand inter-day precision ranged 11.40 - 10.87% and 1.10 - 10.93%, accuracy ranged 80.03 - 119.92% and 80.02 - 119.56%, respectively. Resveratrol and endogenous steroids had correlation coefficients above or equal to 0.996. The method was successfully validated for the determination of resveratrol and endogenous steroids. Urinary samples from volunteers dosed resveratrol were analyzed to confirm a correlation resveratrol and its metabolite. From these results, the highest level of resveratrol and its metabolite was excreted in 10 - 15 hr more slowly than common drug, and conversion rate of metabolite was higher in woman than that in man. In addition, endogenous steroids were shown same the highest level of 10 - 15 hr. For estrone and estradiol, sensitivity was relatively higher in female than in man. And there were no significant changes of excretion patterns in the other endogenous steroids. Thus, we assumed that activation of resveratrol has impact on woman than man.

Factors that Influence Awareness of Breast Cancer Screening among Arab Women in Qatar: Results from a Cross Sectional Survey

  • Background: Breast cancer is the most common cancer among women in the State of Qatar. Due to low participation in breast cancer screening (BCS) activities, women in Qatar are often diagnosed with breast cancer at advanced stages of the disease. Findings indicate that low participation rates in BCS activities are significantly related to women's low level of awareness of breast cancer screening. The objectives of this study were to: (1) determine the factors that influence Qatari women's awareness of breast cancer and its screening activities: and (2) to find ways to effectively promote breast cancer screening activities among Arabic speaking women in Qatar. Materials and Methods: A multicenter, cross-sectional quantitative survey of 1,063 (87.5% response rate) female Qatari citizens and non-Qatari Arabic-speaking residents, 35 years of age or older, was conducted in Qatar from March 2011 to July 2011. Outcome measures included participant awareness levels of the most recent national recommended guidelines of BCS, participation rates in BCS activities, and factors related to awareness of BCS activities. Results: While most participants (90.7%) were aware of breast cancer, less than half had awareness of BCS practices (28.9% were aware of breast self-examination and 41.8% of clinical breast exams, while 26.4% knew that mammography was recommended by national screening guidelines. Only 7.6% had knowledge of all three BCS activities). Regarding BCS practice, less than one-third practiced BCS appropriately (13.9% of participants performed breast self-examination (BSE) monthly, 31.3% had a clinical breast exam (CBE) once a year or once every two years, and 26.9% of women 40 years of age or older had a mammogram once every year or two years). Awareness of BCS was significantly related to BCS practice, education level, and receipt of information about breast cancer and/or BCS from a variety of sources, particularly doctors and the media. Conclusions: The low levels of participation rates in BCS among Arab women in this study indicate a strong need to increase awareness of the importance of breast cancer screening in Qatari women. Without this awareness, compliance with the most recent breast cancer screening recommendations in Qatar will remain low. An increased effort to implement mass media and public health campaigns regarding the impact of breast cancer on women's health and the benefits of early detection of breast cancer must be coupled with an enhanced participation of health care providers in delivering this message to Qatar population.

Retrospective Evaluation of Risk Factors and Immunohistochemical Findings for Pre-Neoplastic and Neoplastic lesions of Upper Urinary Tract in Patients with Chronic Nephrolithiasis

  • Background: Urinary stones are known predisposing factors for upper urinary tract carcinoma (UUTC) which are commonly detected at advanced stage with poor outcome because of rarity and lack of specific criteria for early detection. Aims and objectives: The main aim was to evaluate the impact of age, gender andstone characteristics on risk of developing UUTC in patients with chronic nephrolithiasis. We also discuss the role of aberrant angiogenesis (AA) and immunohistochemical expression of p53, p16INK4a, CK20 and Ki-67 in diagnosis of pelvicalyceal neoplastic (NL) and pre-neoplastic lesions (PNL) in these patients. Materials and Methods: Retrospective analysis of pelvicalyceal urothelial lesions from 88 nephrectomy specimens were carried out in a tertiary care centre from June 2012 to December 2014. Immunohistochemistry (IHC) was performed on 37 selected cases. Computed image analysis was performed to analyse aberrant angiogenesis. Results: All UUTC (5.7%) and metaplastic lesions were found to be associated with stones. Some 60% were pure squamous cell carcinoma and 40% were transitional cell carcinoma. Odd ratios for developing NL and PNL lesions in presence of renal stone, impacted stones, multiple and large stag horn stones were 9.39 (95% CI 1.15-76.39, p value 0.05), 6.28 (95% CI 1.59-24.85, p value 0.000) and 7.4 (95% CI, 2.29-23.94, p value 0.001) respectively. When patient age was ${\geq}55$, the odds ratio for developing NL was 3.43 (95% CI 1.19-9.88, p value 0.019). IHC analysis showed that mean Ki-67 indices were $3.15{\pm}3.63%$ for non-neoplastic lesions, $10.0{\pm}9.45%$ for PNL and $28.0{\pm}18.4%$ for NL. Sensitivity and specificity of CK20, p53, p16INK4a, AA were 76% and 95.9%; 100% and 27.5%; 100% and 26.5%; 92.3 % and 78.8% respectively. Conclusions: Age ${\geq}55years$, large stag horn stones, multiple stones and impacted stones are found to be associated with increased risk of NL and PNL in UUT. For flat lesions, a panel of markers, Ki 67 index >10 and presence of aberrant angiogenesis were more useful than individual markers.

Cancer Screening Knowledge and Attitudes of Under- and Post-Graduate Students at Kasr Al Ainy School of Medicine, Cairo University, Egypt

  • Background: Increasing knowledge and awareness of cancer screening significantly influence health promotion behavior which could markedly reduce incidence rates. In many countries, health care providers are the principal source of information concerning cancer screening. This study was carried out to assess the level of knowledge concerning cancer screening among medical students, house officers and residents and to explore their attitude towards cancer screening practices. Materials and Methods: This cross-sectional study was conducted in Kasr Al Ainy Medical School at Cairo University in Egypt, with 300 undergraduate medical students and 150 postgraduates (interns and residents) enrolled. A pre-tested self-administered questionnaire was used to collect data from the study participants regarding personal and education-related information, knowledge about cancer screening and its sources, and attitude towards cancer screening. Results: More than 64% of participants had knowledge scores of ${\leq}10$ points (out of 24). The total knowledge score (out of 6 points) for breast cancer screening increased from $1.9{\pm}1.0$ to $2.3{\pm}1.2$ and $2.4{\pm}1.1$ for $4^{th}$, $5^{th}$ and $6^{th}$ year respectively, interns showed the highest score of $2.6{\pm}1.1$, P= 0.001. Year of enrollment at medical school was a significant positive predictor of acquiring knowledge about cancer screening (post graduate vs. undergraduate students) (OR= 1.30, C.I =1.01-1.63), lack of or none receiving of orientation/training about cancer screening was the sole negative significant predictor for proper knowledge about cancer screening (OR=0.50, C.I=0.31-0.82). Over 92% of students agreed that they had insufficient knowledge about cancer screening, 88.2% appraised the need to have enough knowledge in order to direct/advice patients, relatives and friends, and 93.7% required that the faculty should emphasize the importance of cancer screening in the delivered curricula at medical school. Conclusions: A relatively low to moderate level of knowledge about cancer screening was detected among the selected medical students regardless of their year of enrollment at medical school or their graduation status, which may implicate a negative impact on early cancer detection especially in a low resource country like Egypt.

Usefulness of Preoperative Breast MRI in Breast Cancer Diagnosed After Excisional Biopsy (유방 절제 생검 후 유방암이 진단된 환자에서 수술 전 자기공명영상의 유용성)

  • Purpose : The aim of this study was to evaluate the usefulness of MRI performed after excisional biopsy to diagnose residual cancer and additional lesions. Materials and Methods : A total of 16 patients who had breast cancer diagnosed by excisional biopsy underwent preoperative breast MRI between March 2005 and August 2007 were included. MRI findings were considered positive for residual cancer if focal, thick, or irregular rim enhancement or adjacent enhancing lesion was identified around the postoperative biopsy cavity. And additional lesions separated from biopsy cavity including multifocal, multicentric, or contralateral lesion were evaluated. We evaluated the diagnostic accuracy of MRI, comparing MRI with histopathologic finding, and the impact of MRI on surgical planning. Results : The sensitivity and specificity of MRI for detecting residual disease considering rim enhancement were 85.7%(6/7) and 55.6%(5/9). Additional lesions including multifocal, multicentric, or contralateral lesion were found in 6 patients. In 7 patients, results of MRI findings changed surgical treatment planning. Conclusion : The pattern of rim enhancement on MRI after excisional biopsy is not differential point to evaluate remnant lesion. But MRI has an important role to help the detection of multifocal or multicentric, or contralateral breast malignancies, resulting in beneficial change in surgical treatment planning.

Impact of HER2 and PTEN Simultaneous Deregulation in Non-small Cell Lung Carcinoma: Correlation with Biological Behavior

  • Background: HER2/neu overexpression due to gene amplification is an important factor in breast cancer, modifying the sensitivity to anti-HER2 monoclonal antibody therapy. The clinical significance of HER2 expression in non small cell lung carcinoma (NSCLC) is currently under evaluation. The tumor suppressor gene PTEN negatively regulates the HER2/PI3K/Akt signalling pathway. The purpose of this study was to evaluate the role of simultaneous alteration in HER2 and PTEN protein expression in relation to biological behaviour of NSCLCs. Materials and Methods: Protein expression was determined by immunohistochemistry in sixty-one (n=61) NSCLC cases along with CISH for HER2 gene analysis and detection of chromosome 17 aneuploidy. Patients were followed-up for a period of 34 to 41 months after surgery. Results: HER2 overexpression (2+/3+score) was detected in 17 (27.9%) patients while loss of PTEN expression was observed in 24 (39.3%) cases, low expression in 29 (47.6%) and overexpression in 8 (13.1%). Simultaneous HER2 overexpression and PTEN low/loss of expression were correlated with metastasis (71.4% vs 36.2% p=0.03). Analysis in the subgroup of 22 patients of pTNM stage III with lymph node status N1 or N2 revealed that there was a relationship between the number of positive regional lymph node groups and simultaneous deregulation of the two genes (p=0.04). Multivariate analysis determined that HER2 overexpression was associated with an increasing risk of developing metastases (OR: 4.3; 95%CI: 1.2-15.9; p: 0.03) while PTEN overexpression was associated with lower risk (OR: 0.1; 95%CI: 0.1, 1.0; p: 0.05). Conclusions: Simultaneous HER2/PTEN deregulation is a significant genetic event that leads to a more aggressive phenotype of NSCLC.
