Comparison of the Usefulness of Diagnostic Tests for Rheumatoid Arthritis (류마티스 관절염 진단을 위한 검사의 유용성 비교)

  • Rheumatoid arthritis (RA) is a chronic inflammatory disease, which is mainly characterized by disease of joints affected with synovial hyperplasia, pathological immune response, and progressive destruction; all of which represent an important social health problem. These provide new insights in its pathogenesis of rheumatoid arthritis diagnosis and disease progression in molecular changes. This review focuses on new serological and immunological markers which seem to be useful in early diagnosis and prognosis of rheumatoid arthritis. Therefore, such tests are widely conducted for serological biomarkers and the developments with such immunological factors to identify patients who are at risk for disease progression. This evidence of the disease based on laboratory medicine could provide the best outcome for patients. Finally, data from recent studies will help to refine the ultimate usefulness of this novel approach for early diagnosis, treatment, and helping clinicians to optimize therapy by using this approach.

Bile duct changes in rats reinfected with Clonorchis sinensis

  • This study describes an evaluation of the sonographic, cholangiographic, pathological, and immunological findings, and the protective effect shown by rats reinfected with Clonorchis sinensis. Eight experimental rat groups were, namely, a normal control, a primary infection control, a reinfection I (reinfection 7 week after treatment following 3-week infection), a reinfection II (reinfection 2 week after treatment following 8-week infection), a reinfection III (exploration of the intrahepatic bile ducts 1 week after reinfection 4 week after treatment following 4-week infection), a superinfection, a secondary infection control, and an infection following immunization group. Sonographic and cholangiographic findings showed moderate or marked dilatation of the bile duct confluence in the primary infection control, reinfection II, and secondary infection control groups. Juvenile worms survived in the intrahepatic bile ducts 1 week after reinfection following treatment in the reinfection III group. It was concluded that reinfecting juvenile worms found during the first week following reinfection failed to survive or grow further. Anatomical, pathophysiological, or immunological changes may induce protection from reinfection in rats.

  • The term allergy was originally defined by Clemens Yon Pirquet as ‘an altered capacity of the body to react to foreign substance’, which was an extremely board definition that included all immunological reactions. Allergy is now defined in a much more restricted manner as ‘disease following an immune response to an otherwise innocuous antigen’. Allergy is a member of a class of immune responses that have been termed hypersensitivity reactions; these are harmful immune responses that produce tissue injury and may cause serious disease.   • The recent appearance of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has affected millions of people around the world and caused a global pandemic of coronavirus disease 2019 (COVID-19). It has been suggested that uncontrolled, exaggerated inflammation contributes to the adverse outcomes of COVID-19. In this review, we summarize our current understanding of the innate immune response elicited by SARS-CoV-2 infection and the hyperinflammation that contributes to disease severity and death. We also discuss the immunological determinants behind COVID-19 severity and propose a rationale for the underlying mechanisms.

  • Biochemical, enzymatic, immunological, and morphological evidences on ceramide and other lipids lamellae formation, assembly of component proteins of CE and expression and distribution of TGases provided better understanding of CE formation. Pathophysiology of some of the genetic disease is beginning to be clarified. Hope we can utilize the information gained in understanding CE formation would expedite the development of patient treatment processes.

  • Background: Rheumatoid arthritis is an autoimmune disease characterized by a chronic inflammatory process, primarily involving the synovial membrane of peripheral j oints, where T cell activation is found. To address the superantigen stimulation in rheumatoid arthritis, T cell clonality and the expression of activation markers were analyzed. Methods: To detect TCRB V usage, inverse PCR and sequencing were done. Monoclonal antibodies were used for flow cytometric analysis of TCRBV8 or TCRBV5. As results, a restricted usage of TCRBV3 gene was detected in synovial lymphocytes from one rheumatoid arthritis patient. However, preferential usage for TCRB V8, which may be one indicator for stimulation by staphylococcal superantigen, was not obvious although general activation of T cells was found as high DR+ percentage in synovial T cells. These data show specific antigen rather than superantigen might involve the pathogenesis of rheumatoid arthritis.

  • In the view of oriental medicine, the liver is the general of the army in its function of protecting against the enemy. So this concept is very closely associated to the immunological function. Its relations with immunological function are as follows. 1. The liver produces most of the proteins and converts them with hepatocytes, composes 80% in total reticuloendothelial system with Kuffer cells & endothelial cells and has typical structure of sinusoidal vessels closely related with the blood system. 2. The liver plays an important role in innate immunity with Kuffer cells as well as with the molecules that the liver produces, related to complementary systems. 3. In the embryonic period, the liver is associated with immune associated cell growth and their maturation. After birth, it is associated with removing old red blood cells and with systematically modulating immune system through hormone metabolism. 4. The liver controls the autoimmune disease resulting from immune complex by removing molecules like immune complex. 5. In the processing of blood 19A from the digestive system, the liver has an important role in protecting the body from unnecessary immune responses. 6. In the oriental medical view, liver plays a major role in the immune function by storing blood and dispersing stagnated hepatoqi with the help of the kidneys and spleen.

Immunological Changes on Allergic Response after Beevenom Immunotherapy (봉독 면역요법후의 면역학적 변화에 대한 고찰 -알레르기 질환에 응용 가능성을 중심으로-)

  • Beevenom immunotherapy(BVIT) in allergic patients is a well-established treatment modality for the prevention of systemic anaphylactic reactions caused by insect stings. BVIT is accompanied by increases in allergen-specific IgG, particularly the IgG4 isotype, which blocks not only IgE-dependent histamine release from basophils but also IgE-mediated antigen presentation to T cells. Inhibition of T cells after BVIT also involves decreased induction of the costimulatory molecule ICOS, which, in turn, seems to be dependent on the presence of IL-10, also associated with the inhibited status of T cells after BVIT. Suppression of T cells by IL-10 is an active process, which depends on the expression and participation of CD28. Immune tolerance in specific allergen immunotherapy might be a consequence of decreased Th2 or increased Th1 response of allergen specific T lymphocytes. BVIT shifted cytokine responses to allergen from a TH-2 to a TH-1 dominant pattern, suggesting direct effects on T cells. Many studies showed that severe side effects due to venom immunotherapy are rare. These results suggest that immunological changes after BVIT may be applied to be therapeutic alternative of general allergic diseases including beevenom allergy.

  • Food allergy is a disease caused by an abnormal immunological reaction to specific food proteins. Whole milk and soy beans are the most frequent causes of food allergy, some studies show that 2.2~2.8% of children aged between 1 and 2 year are allergic to milk. It can be classified to acute (urticaria, asthma, anaphylaxis) or chronic (diarrhea, atopic dermatitis) allergy according to clinical symptoms, or to IgE related or non IgE related allergy by an immunological aspect. Generally, allergies invading only the GI tract are mostly due to a non IgE related reaction. These hypersensitive, immunologic reactions of the GI tract, not related to specific IgE for food, present themselves in many ways such as food protein-induced enteropathy, food protein-induced enterocolitis syndrome (FPIES), celiac disease, food induced protocolitis, or allergic eosinophillic gastroenteritis. FPIES is one kind of non IgE related allergic reaction and is manifested as severe vomiting and diarrhea in infants between 1 week and 3 months. We report a case of FPIES in a 40-day old male infant presenting with 3 times of repeated events of watery diarrhea after cow's milk feeding.

Identification of immunological parameters associated with the alveolar bone level in periodontal patients

  • Purpose: The present study was performed to clarify the relationship between periodontal disease severity and selected immunological parameters consisting of serum IgG titer against periodontopathogenic bacteria, the expression of the helper T-cell cytokine by gingival mononuclear cells, and patients' immunoreactivity to cross-reactive heat shock protein (HSP) epitope peptide from P. gingivalis HSP60. Methods: Twenty-five patients with moderate periodontitis had their gingival connective tissue harvested of gingival mononuclear cells during an open flap debridement procedure and peripheral blood was drawn by venipuncture to collect serum. The mean level of interproximal alveolar bone was calculated to be used as an index for periodontal disease severity for a given patient. Each of selected immunologic parameters was subject to statistical management to seek their correlations with the severity of periodontal disease. Results: A significant correlation could not be identified between serum IgG titers against specific bacteria (Porphyromonas gingivalis, Prevotella intermedia, Fusobacterium nucleatum, Actinobacillus actinomycetemcomitans, and Streptococcus mutans) and the severity of periodontal disease. Expression of interleukin (IL)-10 by gingival mononuclear cells was statistically significant in the group of patients who had higher levels of alveolar bone height. However, a similar correlation could not be demonstrated in cases for IL-4 or interferon-$\gamma$. Patients' serum reactivity to cross-reactive epitope peptide showed a significant correlation with the amount of alveolar bone. Conclusions: It was concluded that expression of IL-10 by gingival mononuclear cells and patients' sero-reactivity to the cross-reactive HSP peptide of P. gingivalis HSP60 were significantly correlated with alveolar bone height.
