The Effect of Norepinephrine on Immune function in elderly people (노르에피네프린이 노인의 면역기능에 미치는 영향)

  • Purpose: The purpose of this study was to examine how norepinephrine affects immunity in patients over age 65. Methods: We enrolled 25 male and female subjects age 65 or older. A low frequency electroacupuncture (EA) device was used to stimulate acupoint Hogu (L14). The 2 Hz frequency EA was applied to the acupoint for 20 minutes. Leukocyte subtypes-including neutrophils, lymphocytes, monocytes, eosinophils, and basophil-were then measured. The immunoglobulins IgG and IgM were also quantified. The data were finally analyzed using Wilcoxon singed-rank test and regression test as part of the SPSS WIN v. 10.1 program. Results: As norepinephrine levels decreased after EA stimulation, neutrophil, lymphocyte, and monocyte levels increased, and eosinophil and neutrophils levels decreased. Neutrophil and monocyte levels did not change to a statistically significant degree, but eosinophil levels showed a statistically significant decrease (p<0.05). Immunoglobulin IgG showed a statistically significant increase (p<0.05). Conclusion: This study showed that norepinephrine does affect immunity in persons over the age of 65. This indicates that there is an interaction between the nervous system and the immune system, and interaction that plays a crucial roles in the body's immune resistance and homeostasis.

  • Most boys afflicted with panhypogammaglobulinemia, also known as X-linked agammaglobulinemia, remain healthy during the first 6 to 12 months of life because of protection by maternally transmitted IgG antibodies. Thereafter, they repeatedly acquire infections with high-grade pathogens, such as pneumococci, streptococci, and hemophilus unless given antibiotics or immunoglobulin replacement therapy. We experienced a case of panhypogammaglobulinemia in a 4 years old boy. He had been suffered from recurrent upper respiratory tract infection, otitis media and pneumonia since late infancy. He was admitted due to right pleural effusion with pneumonia, and streptococcus pneumoniae was isolated from pleural fluid and blood cultures. His immune status revealed panhypogammaglobulinemia and deficiency in mature B lymphocyte. He was treated with appropriate antibiotics therapy, but showed poor responses. He was transferred to department of thoracic surgery, and received minithoracotomy (decortication) operation. He was successfully treated with operation, antibiotics, and IV gammaglobulin infusions. Now he is being followed with periodic IV gammaglobulin replacement therapy.

  • Management of chronic wound has been a great problem to many surgeons because the wound is usually associated with an underlying disease of the patient. Without accurate diagnosis and treatment of the disease, the wound can not be healed. Systemic vasculitis is a rare systemic disease which causes inflammation and obstruction of the vessels. This autoimmune disease involves multiple organs and may inflict skin wound spontaneously without traumas. It would improve or aggravate the wound in proportion to the activities of the disease. Our experience is a case of 28-year-old female who has had chronic ulcers on her right foot, especially on the great toe for 1 year. Although she had several operations of sympathectomy, debridement and artificial dermal graft, her wound was not improved. She has been diagnosed as systemic vasculitis during the evaluation for histopathology and cause of fever and pancytopenia. After medical treatments, she had the operation of adipofascial turnover flap coverage and skin graft, and the wound was improved without any complication or relapse. The diagnosis and treatment of the underlying disease should be ahead of the management of chronic wound.

  • Monoclonal antibody to progesterone was produced using the antigen $11{\alpha}$-hydroxyprogesterone hemisuccinate conjugated to bovine serum albumin. Hybridomas secreting antibody to progesterone were detected by radioimmunoassay and enzyme-linked immunosorbent assay and cloned in soft agar. Two stable monoclonal antibodies which were highly specific to progesterone were obtained, so it may be advantageously used to study on several physiological functions of progesterone including immunological research.

  • The importance of milk for the growth and health of a newborn offspring is well known. Milkcontains immunoglobulin G (Ig G), Ig A, lactoperoxidase, lactoferin, cytokines, and growth factors.Osteopontin, one of the multifunctional proteins, is secreted by macrophages, T cel, and epithelial cells.bovine milk have not been clarified. The aim of this study was to observe the expression of osteopontin,in bovine milk during the lactation period or bovine mamary glands..Western blot analysis detected thatosteopontin was expressed in bovine milk whey and mamary glands. The expression level of osteopontinin colostrum whey was higher than those in early milk and mature milk whey. Immunohistochemistryshowed that osteopontin was detected in the glandular epithelium and epithelial cels of intralobular ductof mamary glands. These findings suggest that osteopontin transiently shows high expression in colostrumand plays a potential role in the immunological development of breast-fed calves.

  • Cryptococcosis is a rare in normal child and the majority of cases usually occur in patients with defective cell-mediated immunity. Infection is acquired by inhalation of organisms from the environment and disseminated via the blood stream to any organ of the body. We experienced a 7 year old girl who presented with fever, both cervical lymphadenopathy, hepatomegaly under the impression of lymphoma. However lymph node biopsy revealed cryptococcal budding and culture of lymph nodes yielded cryptococcus neoformans. The radiologic finding showed huge, multiple cervical lymph node enlargement spreading to mediastinum and abdomen. The immune fuction in term of T cell, B cell, serum immunoglobulin, complement and neutrophil function tests was normal. The patient was treated with amphotericin B and flucytosine for 6 weeks and responded to the treatment well. We report this case with brief review of the related literatures.

  • Reye syndrome is a rapidly progressive encephalopathy with hepatic dysfunction, which often begins several days after apparent recovery from a viral illness, especially varicella or influenza A or B. Salicylate use was identified as a major precipitating factor for the development of Reye syndrome. With the recommendation to avoid use of salicylates in children, Reye syndrome has virtually disappeared in recent years. We report a case of Reye syndrome in a 5-month-old infant who had been treated with intravenous immunoglobulin and aspirin under the diagnosis of Kawasaki disease, and showed symptoms of sudden onset of irritability, rigidity, decreased activity, vomiting, poor appetite, lethargy, liver dysfunction without jaundice, coagulopathy, and hyperammonemia.

  • Staphylococcal toxic shock syndrome (TSS) is a severe systemic illness caused by toxins produced by Staphylococcus aureus. We report a case of staphylococcal TSS in a 16 month-old boy who presented with high fever, vomiting, skin rash, and shock after a burn injury. He was managed with intravenous vancomycin, fresh frozen plasma, and intravenous immunoglobulin. Methicillin-resistant S. aureus (MRSA) was isolated from the burn wound site and anterior nostril of the patient. In addition, the MRSA isolate was genetically characterized.

  • A girl aged 21 months with $Henoch-Sch\"{o}nlein$ purpura(HSP) developed heavy proteinuria with hematuria 8 days after the appearance of purpuric rash, swelling and tenderness of both ankle joints. Her clinical and laboratory features demonstrated nephrotic and nephritic syndrome. The percutaneous renal biopsy revealed diffuse mesangial proliferative glomerulonephritis. Unlike usual HSP nephritis, immunoglobulin A deposition was not detected in the mesangium or the capillary of the glomeruli. Instead, numerous subepithelial electron-dense deposits('humps') mimicking acute poststreptococcal glomerulonephritis were found.

  • In this study, we investigated the effect of Phellinus baumii (PB) on immediate-type allergic reaction and inflammatory cytokine secretion. PB inhibited compound 48/80-induced systemic reactions in mice. PB inhibited compound 48/80-induced plasma histamine release. In addition, PB also inhibited the immunoglobulin (Ig) E-mediated local allergic reaction. Furthermore, PB decreased the phorbol 12-myristate 13-acetate plus calcium ionophore A23187-stimulated tumor necrosis $factor-\alpha$ and interleukin-6 secretion in human mast cells. These results indicate that PB may be beneficial in the treatment of immediate-type allergic reactions.
