Preliminary study of presumptive intradural-intramedullary intervertebral disc extrusion in 20 dogs

  • Background: Intradural-intramedullary intervertebral disc extrusion (IIVDE) is a rare condition of intervertebral disc disease. However, the diagnosis of IIVDE is challenging because the prognosis and imaging characteristics are poorly characterized. Objectives: We aimed to describe the clinical and imaging characteristics of tentatively diagnosed IIVDE in dogs to assess the prognostic utility of neurological grade and magnetic resonance imaging (MRI) findings. Methods: Twenty dogs were included in this retrospective cohort study. Results: Nonchondrodystrophic breeds (n = 16) were more predisposed than chondrodystrophic breeds. Most dogs showed acute onset of clinical signs. Neurological examination at admission showed predominant non-ambulatory paraparesis (n = 9); paresis (n = 16) was confirmed more frequently than paralysis (n = 4). Follow-up neurological examination results were only available for 11 dogs, ten of whom showed neurological improvement and 8 showed successful outcomes at 1 month. The characteristic MRI findings include thoracic vertebra (T)2 hyperintense, T1 hypointense, intramedullary linear tracts with reduced disc volume, and cleft of the annulus fibrosus. None of the MRI measurements were significantly correlated with neurological grade at admission. Neurological grade did not differ according to the presence of parenchymal hemorrhage, parenchymal contrast enhancement, and meningeal contrast enhancement. Neurological grades at admission showed a statistical correlation with those observed at the 1-month follow-up (r = 0.814, p = 0.02). Conclusions: IIVDE is a rare form of disc extrusion commonly experienced after physical activity or trauma and most frequently affects the cranial-cervical and thoracolumbar regions of nonchondrodystrophic dog breeds. Neurological score at admission emerged as a more useful prognostic indicator than MRI findings in dogs with suspected IIVDE.

A Cerebral Astroblastoma Mimicking an Extra-axial Neoplasm

  • Astroblastoma is a rarely diagnosed primary brain neoplasm whose histogenesis has been clarified recently. It occurs in children and young adults and presents as a well circumscribed, contrast-enhancing lesion in the cerebral hemisphere. We present a case of 20-year-old woman with an astroblastoma in the left temporal convexity that was treated with total tumor resection alone. We thought the mass was extra-axial neoplasm based on radiological findings of computed tomography and magnetic resonance imaging initially, but later, we obtained angiographic findings suggesting an intra-axial neoplasm. The patient is doing well even two years after surgery. The characteristic radiological and histopathological features of this case are described with a literature review. An astroblastoma should be included in the differential diagnosis of a superficially located tumor presenting with the findings of an extra-axial mass, especially in a young patient.

Imaging Features of Duodenal Adenocarcinoma in a Dog (개에서 발생한 십이지장 샘암종)

  • A seven-year-old, castrated male Basset hound weighing 21.1 kg was presented for investigation of anorexia, lethargy, weight loss, melena and vomiting for 4 months. In laboratory findings, microcytic hypochromic anemia was identified, and other results were within a normal reference range. On the plain radiographs, soft tissue opacity was increased in the descending duodenal region; and on the contrast radiographs, ulcerative changes were identified in the entire segment of descending duodenum. Ultrasonographic findings included increased duodenal wall thickness and duodenal wall layering was lost. Endoscopy revealed irregular mucosal surface and luminal narrowing of the descending duodenum. There was concentrically thickened descending duodenum on the computed tomography. And the wall of the descending duodenum showed heterogenous enhancement after contrast agent injection. On histopathological findings, both chronic inflammation with mucosal proliferation and neoplastic changes with multiple small glandslike structures invading into the submucosa were identified. Based on these findings, presented case was diagnosed as an annular form duodenal adenocarcinoma. After 13 months of supportive medical treatment, the patient was expired. The purpose of this case report was to describe the duodenal adenocarcinoma in a dog.

A Case Report of Idiopathic Bilateral Internuclear Ophthalmoplegia (특발성 양안성 핵간안근마비 1례)

  • Objectives : The aim of this study is to report the effect of Korean medicine treatment on bilateral internuclear ophthalmoplegia without abnormal findings on brain magnetic resonance imaging.Methods : The patient was treated by using acupuncture, electroacupuncture treatment and hominis placenta pharmacoacupuncture. The change of eye movement and strabismus were evaluated by comparison the photographs of the extraocular movements of patient. Strabismus also was evaluated by corneal reflex test.Results : Adduction of both eyes were improved after Korean medicine treatment. The amplitude and frequency of nystagmus showed a tendency to decrease in both eyes.Conclusions : Korean medicine treatment should be effective for the idiopathic bilateral internuclear ophthalmoplegia.

Imaging Diagnosis: Biphasic Synovial Sarcoma in a Dog (개에서 발생한 이상성 활막육종 증례)

  • An 11-year-old castrated male, Shih-tzu dog was admitted for progressive weight-bearing lameness and mass on the right hindlimb. Radiography and ultrasonography revealed a large well-marginated multinodular mass with mixed echogenicity. On T1 and T2-weighted magnetic resonance imaging (MRI), the mass has a heterogenous signal intensity similar to or higher than muscle. The masses were extended to the pelvic cavi쇼 through obturator foramen and displaced the rectum dorsally. It was diagnosed as synovial sarcoma which was composed with mesenchymal and epithelial elements on histopathological findings of the multifocal biopsied specimen. MRI was helpful to determine the definitive margin for surgical resection of the mass. The mass was recurred at the 6th month after surgery. On the 10th month, the patient was euthanasiuzed by owner's request.

A Huge Intra-Abdominal Mass Associated with Long-Term Surgical Gauze Retention in a Toy Breed Dog

  • A 1.83 kg, 9-year-old, spayed female Yorkshire Terrier was referred to the Veterinary Medical Teaching Hospital of Konkuk University for evaluation of an intra-abdominal mass with a week history of vomiting and diarrhea. On physical examination, survey radiography, abdominal ultrasonography, and computed tomography, a huge firm mass was identified in the mid-caudal abdomen. Surgical exploration of the abdominal cavity was performed to remove the mass. The encapsulated mass adhered to the mesentery, tail of the spleen, small intestine, omentum, and right lobe of the pancreas was removed using blunt dissection with dry gauze and cotton swabs. Macroscopic and histopathological examination revealed that the mass was foreign-body granuloma consistent with gauze fiber. Plain abdominal radiography demonstrated no remarkable findings 8 months post-operatively. There was no evidence of vomiting, diarrhea, coughing, difficulty breathing, and cyanosis on exertion 13 months post-operatively.

A Case of Treatment of Idiopathic Immune-Mediated Polyarthritis in a Dog (개에서 발생한 특발성 면역매개성 다발성관절염의 치료 증례)

  • A 12-year-old, castrated male, mixed-breed dog was referred to us with a history of depression, lameness, neck and multiple joints pain. Clinical signs had been deteriorating from rear limbs lameness leading to pain on all four limbs lameness for 7 days. Mild leukocytosis and increased C-reactive protein concentration were revealed in blood work. On radiography, degenerative change of humerus and cervical intervertebral space narrowing were found. The results of synovial fluid analysis revealed severe neutrophilic pleocytosis, decreased viscosity, increased turbidity and bacterial culture was negative. The antinuclear antibody test was negative and MRI results revealed mild cervical intervertebral disk disease (IVDD). Based on all tests, we diagnosed this case as idiopathic immune-mediated polyarthritis (IMPA). Prednisolone and mycophenolate mofetil were administered and clinical signs were resolved after 7 days. This case report demonstrated that clinical, diagnostic imaging and synovial fluid analysis findings and successful treatment result with prednisolone and mycophenolate mofetil in canine idiopathic IMPA.

Pulmonary Mineralization Secondary to Iatrogenic Hyperadrenocorticism in a Dog (개에서 발생한 의인성 부신피질기능항진증과 속발성 폐 석회화 진단 1례)

  • An 11-year-old, intact male Shih Tzu presented with obesity, thin hair coat, and abdominal distention. The patient had previously received corticosteroid therapy for dermatitis. Thoracic radiographs showed incidental findings of a generally diffuse bronchointerstitial pattern and markedly radiolucent pleural lines between the middle and caudal lung lobes, but clinical signs related to the thoracic radiographic findings were absent. Echogenicity of the hepatic parenchyma was increased and bilateral adrenal glands were not enlarged on ultrasonography. On computed tomography, the central regions of the lung lobes showed ground-glass attenuation, the peripheral regions of the lung lobes were relatively hypoattenuated compared with the central regions, and hyperattenuated nodules were distributed throughout the pulmonary parenchyma. Iatrogenic hyperadrenocorticism and secondary pulmonary mineralization was diagnosed on the basis of diagnostic imaging and adrenocorticotropic hormone stimulation test.

Pancreatic Exocrine Adenocarcinoma in a Cat (고양이에서 발생한 췌장 외분비 선암종 1례)

  • An 8-year-old, neutered female Korean short hair cat was referred with severe vomiting and anorexia. Abdominal mass effect was seen in the cranial abdomen on radiographs, and a mass with a heterogeneous echogenic pattern was observed medial to the right kidney on ultrasonography. On computed tomography, a large mass with soft tissue attenuation and a contrast-enhanced capsule was seen. In histopathological findings, the acinar structures were lined with irregular cuboidal cells that have pale eosinophilic cytoplasm and round to oval nuclei. Based on these diagnostic imaging and histopathological findings, the mass was diagnosed as pancreatic exocrine adenocarcinoma.

  • Inflammatory myofibroblastic tumor is a rare benign lesion that accounts for 0.04-1% of all lung tumors and usually appears as a solitary pulmonary nodule or mass. Here, we report the case of an endobronchial inflammatory myofibroblastic tumor in a 21-year-old man with a focus on the imaging findings and a review of previous literature.
