재발한 경추 신경장성 낭종의 전방경유 치험 - 증 례 보 고 - (Anterior Surgical Approach in Recurrent Cervical Neurenteric Cyst - Case Report -)

  • Intraspinal neurenteric cyst is rare congenital lesion derived from disorder of notochord formation. Most of these are located ventral to the spinal cord and conventional posterior approach is considered to be effective method as initial treatment modality. This cyst can recur, but the risk of reccurence after partial removal through long term follow-up has not been determined. We experienced one case of cervical neurenteric cyst which recurred after partial removal through laminectomy. The magnetic resonance imaging and postmyelography computerized tomography revealed an intradural extramedullary cystic lesion anterior to the cervical cord at the fifth cervical vertebra level. We performed anterior cervical corpectomy and cyst was totally removed. The patient's neurological symptom was improved postoperatively. Neurenteric cyst located ventrally to the cervical spinal cord should be removed through anterior route for direct visualization of the relationship between the cyst wall and the spinal cord.

Heterotopic Ossification

  • Heterotopic ossification (HO) is the formation of mature bone in soft tissue where bone normally does not exist. Although HO is among the most common complications after orthopedic surgery, it is not familiar to oral and maxillofacial surgeons. Here we report rare cases of HO. When a patient presents atypical osseous lesions, HO as well as similar lesions such as osteoma, osteochondroma should be considered in the provisional diagnosis. Three-dimensional reconstruction of preoperative computerized tomography imaging improves surgical success.

Primary extracranial meningioma presenting as a forehead mass

  • Meningioma originates from arachnoid cap cells and is the second most common intracranial tumor; however, it can also be found in an extracranial location. A very rare primary extracranial meningioma without the presence of an intracranial component has also been reported. Primary extracranial meningiomas have been found in the skin, scalp, middle ear, and nasal cavity. A computerized tomography or magnetic resonance imaging scan is necessary to determine the presence or absence of an intracranial meningioma, and a biopsy is essential for diagnosis. We report a case of primary extracranial meningioma located in the forehead skin of a 51-year-old male.

Epilepsy Surgery of the Cerebral Paragonimiasis

  • Objective : The authors investigate appropriate evaluation and surgical methods in treatment of the cerebral paragonimiasis accompanying epilepsy. Methods : Thirteen patients with the cerebral paragonimiasis accompanying epilepsy were included for this study. Preoperative evaluation methods included history taking, skin and serologic tests for Paragonimus westermani, neurologic examinations, computerized tomography, magnetic resonance imaging, amytal test, PET or SPECT, and video-EEG monitoring with depth and subdural grid electrodes. Seizure outcome was evaluated according to Engel's classification. Results : Surgical methods were temporal lobectomy including lesions in six, lesionectomy in five, and temporal lobectomy plus lesionectomy in two. Postoperative neurological complications were not noticed, and seizure outcomes were class I in 12 patients [92%], class II in one [8%]. Conclusion : In patients with a cerebral paragonimiasis accompanying epilepsy, further evaluation methods must be done to define the epileptogenic zone, and complete resection of the epileptogenic zone with different surgical methods should be performed for seizure control.

CT영상의 3차원 재구성 및 표현에 관한 연구 (A Study on the 3D Reconstruction and Representation of CT Images)

  • Many three-dimensional object modeling and display methods for computer graphics and computer vision have been developed. Recently, with the help of medical imaging devices such as computerized tomography, magnetic resonance image, etc., some of those object modeling and display methods have been widely used for capturing the shape, structure and other properties of real objects in many medical applications. In this paper, we propose the reconstruction and display method of the three-dimensional object from a series of the cross sectonal image. It is implemented by using the automatic threshold selection method and the contour following algorithm. The combination of curvature and distance, we select feature points. Those feature points are the candidates for the tiling method. As a results, it is proven that this proposed method is very effective and useful in the comprehension of the object's structure. Without the technician's responce, it can be automated.

Acute Ischemic Stroke: Current Management and Role of the Nurse Practitioner

  • Purpose: This paper reviews current diagnostic evaluation, treatment, nursing considerations, and the nurse practitioner’s (NP) role in acute ischemic stroke care. Methods: National guidelines and extensive literature on acute stroke care were reviewed and a relevant clinical case was introduced. Results: Computerized tomography (CT) of the head without contrast is the initial brain imaging procedure for patients with an acute stroke. Magnetic resonance imaging (MRI) can be an alternative test. Restoration of cerebral perfusion to the affected area is a key therapeutic strategy for ischemic stroke. A number of treatment strategies such as thrombolysis, anticoagulation, antiplatelet, and surgical treatment can be selected to improve blood flow to the ischemic region. The NP on the stroke team is involved with immediate stroke management including neurological assessment, ensuring adequate oxygenation, blood pressure management, activity, and diet. Discharge planning with the patient, family teaching and coordination of follow up care should also be implemented early in the hospitalization. Conclusion: The nurse practitioner is one of the cardinal members on the stroke team, and must be updated with current treatment and management guidelines.

Endoscopic Treatment of Chronic Subdural Hematoma Combined with Inner Subdural Hygroma

  • Objective : A chronic subdural hematoma (CSDH) is a collection of bloody fluid located in the subdural space and encapsulated by neo-membranes. An inner subdural hygroma (ISH) is observed between the inner membrane of a CSDH and the brain surface. We present six cases of CSDH combined with ISH treated via endoscopy. Methods : Between 2011 and 2022, among the 107 patients diagnosed with CSDH in our institute, six patients were identified as presenting with CSDH combined with ISH and were included in this study. Preoperative computerized tomography (CT) and magnetic resonance imaging (MRI) were performed simultaneously, and endoscopic surgery for aspiration of the hematoma was performed in all cases of CSDH combined with ISH. Results : The mean age of patients was 71 years (range, 66 to 79). The patients were all male. In two cases, the ISH was not identified on CT, but was clearly seen on MRI in all patients. The inner membrane of the CSDH was tense and bulging after draining of the CSDH in endoscopic view due to the high pressure of the ISH. After fenestration of the inner membrane of the CSDH and aspiration of the ISH, the membrane was sunken down due to the decreasing pressure of the ISH. There was one recurrence in post-operative 2-month follow up. The symptoms improved in all patients after surgery, and there were no surgery-related complications. Conclusion : CSDH combined with ISH can be diagnosed on imaging, and endoscopic surgery facilitates safe and effective treatment.

괴사성 근막염의 고찰 (Consideration of Necrotizing Fasciitis)

  • Purpose: A necrotizing fasciitis is a rare, but insidiously advancing fatal soft tissue infection characterized by extensive fascial necrosis. Diagnosis & treatment of this disease are difficult. Necrotizing fasciitis tend to begin with constitutional symptoms of fever and chills. Quite a many lab studies and imaging studies such as standard radiography, computerized tomography can be used, but nothing can confine the extent of affected tissue. Aggressive surgical interventions are often required because of large skin and soft tissue deformity. However, many patients with necrotizing fasciitis are not healthy enough to overcome aggressive surgical intervention. Methods: Since 2000, we treated 10 patients with necrotizing fasciitis. In 4 patients, we used magnetic resonance imaging(MRI) as a tool for diagnosis as soon as necrotizing fascitiis was doubted. We treated patients with delayed coverage with Alloderm$^{(R)}$ & split thickness skin graft or delayed wound closure in as many cases as possible. Results: In 4 patients using preoperative MRI, diagnosis could be made in earlier stage of the disease compared to other patients. Our treatment modality was debridement and coverage with Alloderm$^{(R)}$ & split thickness skin graft. We could reconstruct deformities without significant limitation of movement in 7 cases. Conclusion: We diagnosed and treated 10 necrotizing fasciitis with MRI and Alloderm$^{(R)}$ graft, and results were good.

A cosmic ray muons tomography system with triangular bar plastic scintillator detectors and improved 3D image reconstruction algorithm: A simulation study

  • Purpose: Muons are characterized by a strong penetrating ability and can travel through thousands of meters of rock, making them ideal to image large volumes and substances typically impenetrable to, for example, electrons and photons. The feasibility of 3D image reconstruction and material identification based on a cosmic ray muons tomography (MT) system with triangular bar plastic scintillator detectors has been verified in this paper. Our prototype shows potential application value and the authors wish to apply this prototype system to 3D imaging. In addition, an MT experiment with the same detector system is also in progress. Methods: A simulation based on GEANT4 was developed to study cosmic ray muons' physical processes and motion trails. The yield and transportation of optical photons scintillated in each triangular bar of the detector system were reproduced. An image reconstruction algorithm and correction method based on muon scattering, which differs from the conventional PoCA algorithm, has been developed based on simulation data and verified by experimental data. Results: According to the simulation result, the detector system's position resolution is below 1 ~ mm in simulation and 2 mm in the experiment. A relatively legible 3D image of lead bricks in size of 20 cm × 5 cm × 10 cm used our inversion algorithm can be presented below 1× 104 effective events, which takes 16 h of acquisition time experimentally. Conclusion: The proposed method is a potential candidate to monitor the cosmic ray MT accurately. Monte Carlo simulations have been performed to discuss the application of the detector and the simulation results have indicated that the detector can be used in cosmic ray MT. The cosmic ray MT experiment is currently underway. Furthermore, the proposal also has the potential to scan the earth, buildings, and other structures of interest including for instance computerized imaging in an archaeological framework.

전정구형낭 청력에 관한 연구 (A Study on Vestibulosaccular Hearing)

  • The aims of this study are to consider auditory physiological characteristics and to confirm audiological evaluation and interpretation in regards to cases of sensorineural hearing loss that observe an abnormal AB gap. Vestibulosaccular hearing occurs when there is an abnormally large air-bone gap (AB gap) in sensorineural hearing loss, also known as pure cochlear conductive hearing loss. Generally, an AB gap is caused by damage to the external and/or middle ear. In conductive hearing loss, loss of air condition hearing occurs due to a loss of resonance in the outer ear and/or impedance mismatching in the middle ear. Most of these types of hearing loss can be treated medically and surgically. However, there is no medical treatment for an AB gap in sensorineural hearing loss and hearing loss can worsen gradually or suddenly. In addition, many studies have reported that head trauma makes hearing loss even more serious. Therefore, in order to differentiate between conductive hearing losses, it is important to check whether or not there is an enlarged vestibular aqueduct by means of temporal bone computerized tomography and/or magnetic resonance imaging.
