Haplotype Diversity and Durability of Resistance Genes to Blast in Korean Japonica Rice Varieties

  • Blast disease caused by the fungal pathogen, Magnaporthe oryzae, is one of the most damaging diseases in rice. The use of resistant varieties is an effective measure to control the disease, however, many resistant varieties were broken down to their resistance effects by the differentiating of new virulent isolates. This study was done to analyze the haplotypes of 31 microsatellite markers linked to five major R genes and two QTLs and to identify the alleles for the putatively novel genes related to durable resistance to blast in 56 Korean japonica and four indica varieties. The 31 microsatellite markers produced 2 to 13 alleles(mean = 5.4) and had PICi values ranging from 0.065 to 0.860(mean=0.563) among the 60 rice accessions. Cluster analysis based on allele diversities of 31 microsatellite markers grouped into 60 haplotypes and ten major clusters in 0.810 genetic similarity. A subcluster IV-1 grouped of early flowering varieties harboring Piz and/or Pi9(t) on chromosome 6 and Pita/Pita-2 gene on chromosome 12. The other subcluster V-1 consisted of four stable resistance varieties Donghae, Seomjin, Palgong and Milyang20. The analysis of putative QTLs associated with seven blast resistance genes using ANOVA and linear regression showed high significance to blast resistance across regions and isolates in the markers of two genes Piz and/or Pi9(t) and Pita/Pita-2. These results illustrate the utility of microsatellite markers to identify rice varieties is likely carrying the same R genes and QTLs and rice lines with potentially novel resistant gene.

Radiotherapy of Uterine Cervical Cancer Using Fletcher-Suit-Delclos Cesium Applicator (Fletcher-Suit-Delclos Cesium Applicator를 이용한 자궁경부암의 방사선치료)

  • From Nov. 1983 through Feb. 1986, 35 patients of uterine cervical cancer were treated by external radiation therapy and intracavitary radiation therapy using Fletcher-Suit-Delclos applicator. Age of the patients ranged from 32 to 70 years (median age: 53 years). All patients had follow up from 9 to 34 months and median follow up of 20 months.4 patients were in stage I, 25 were in stage II, 5 were in stage III and 1 was in stage IV. Overall regression rate was $80\%$ and uncorrected actuarial 2 year survival rate was $88\%$. The incindence of rectal complications were analyzed. There was no rectal complication in the patients who received less than 7000 rad maximal rectal dose, but 2 out of 17 patients who received more than 7000 rad developed moderate degree (grade 2) of rectal complication. In viewing of our results, Fletcher-Suit-Delclos applicator (3M) seemed to be an appropriate instrument for intracaviary radiation therapy in the patients of uterine cervical cancer.

Impact of Cardio-Pulmonary and Intraoperative Factors on Occurrence of Cerebral Infarction After Early Surgical Repair of the Ruptured Cerebral Aneurysms

  • Objective: Delayed ischemic deficit or cerebral infarction is the leading cause of morbidity and mortality after aneurysmal subarachnoid hemorrhage (SAH). The purpose of this study is to reassess the prognostic impact of intraoperative elements, including factors related to surgery and anesthesia, on the development of cerebral infarction in patients with ruptured cerebral aneurysms. Methods: Variables related to surgery and anesthesia as well as predetermined factors were all evaluated via a retrospective study on 398 consecutive patients who underwent early microsurgery for ruptured cerebral aneurysms in the last 7 years. Patients were dichotomized as following; good clinical grade (Hunt-Hess grade I to III) and poor clinical grade (IV and V). The end-point events were cerebral infarctions and the clinical outcomes were measured at postoperative 6 months. Results: The occurrence of cerebral infarction was eminent when there was an intraoperative rupture, prolonged temporary clipping and retraction time, intraoperative hypotension, or decreased $O_2$ saturation, but there was no statistical significance between the two different clinical groups. Besides the Fisher Grade, multiple logistic regression analyses showed that temporary clipping time, hypotension, and low $O_2$ saturation had odds ratios of 1.574, 3.016, and 1.528, respectively. Cerebral infarction and outcome had a meaningful correlation (${\gamma}$=0.147, p=0.038). Conclusion: This study results indicate that early surgery for poor grade SAH patients carries a significant risk of ongoing ischemic complication due to the brain's vulnerability or accompanying cardio-pulmonary dysfunction. Thus, these patients should be approached very cautiously to overcome any anticipated intraoperative threat by concerted efforts with neuro-anesthesiologist in point to point manner.

Investigation into the Use of Complementary and Alternative Medicine and Factors Affecting Use in Korean Patients with Brain Tumors (뇌종양 환자의 보완대체요법 이용에 영향을 미치는 요인)

  • Purpose: The study was done to define complementary and alternative medicine (CAM) use among primary brain tumor patients and factors influencing use. Methods: The study was conducted with convenience sampling in 5 neuro-oncology centers in Seoul and Gyeonggi Province. Data were collected using a 25-item questionnaire developed by the researchers. Results: Of 250 patients approached, 231 (92.4%) agreed to participate. Overall, 26.8% of the respondents used CAM and the average cost for CAM use was 300,000 KRW. More than 72% of CAM users did not disclose CAM use to their health care professionals. The most frequently used therapy was natural products including mushrooms. Logistic regression analysis revealed that an education level of university or higher (OR=2.92, 95% CI=1.56-5.44, p=.001), unemployment (OR=1.99, 95% CI=1.04-3.80, p=.037), and WHO grade III & IV tumors (OR=2.18, 95% CI=1.07-4.72, p=.048) were significant factors influencing CAM use. Conclusion: Three out of ten brain tumor patients spend substantial sum of money for CAM. In these situations, health care professionals should be aware of this phenomenon and provide adequate information and consultation to the patients.

Very low protein diet plus ketoacid analogs of essential amino acids supplement to retard chronic kidney disease progression

  • Background: A very low protein diet (VLPD) with ketoacid analogs of essential amino acids (KA/EAA) administration can remarkably influence protein synthesis and metabolic disturbances of patients with advanced chronic kidney disease (CKD), and may also slow the decline in renal function. Methods: A retrospective cohort study was carried out to monitor renal progression and metabolic and nutritional status among 140 patients with CKD stage III or IV. One group (n = 70) was on a low protein diet (LPD) with 0.6 g of protein intake, and another group (n = 70) was on a VLPD with 0.3 g of protein and KA/EAA supplementation of 100 mg/kg/day for 12 months. Results: At 12-month follow-up, estimated glomerular filtration rate (GFR) significantly decreased from $41.6{\pm}10.2$ to $36.4{\pm}8.8mL/min/1.73m^2$ (P < 0.001) and urine protein increased from $0.6{\pm}0.5$ to $0.9{\pm}1.1g/day$ (P = 0.017) in the LPD group, but no significant changes in estimated GFR and urine protein were found in the VLPD plus KA/EAA group. A significant mean difference in rate of change in estimated GFR ($-5.2{\pm}3.6mL/min/1.73m^2$ per year; P < 0.001) was observed between the two groups. After Cox regression analysis, treatment with VLPD plus KA/EAA significantly protected against the incidence of declining GFR > 10% annually (adjusted hazard ratio, 0.42; 95% confidence interval, 0.23-0.79; P = 0.006) and significant correlations were found between using VLPD plus KA/EEA and increased GFR. Conclusion: VLPD supplementation with KA/EAA is associated with delayed renal progression while preserving the nutritional status in the patients with CKD. Co-administration of VLPD and KA/EAA may prove an effective alternative to conservative management of CKD.

  • Objectives : This study aimed to analyze characteristics of the Korean Medicine users aged 19-39 years, the determinants of Korean Medicine use and effects of Korean Medicine use experience on behavioral intentions among youths. Methods : We studied Korean Medicine Utilization and Herbal Medicine Consumption Survey(2017) data set by complex-sample survey data analysis. In order to derive the characteristics of younger users, Rao-scott χ2 test and t-test were conducted. Through multiple logistic regression model adjusted by general characteristic variables, the determinants and behavioral intentions of the Korean Medicine utilization among youths were derived. Results : Fist of all, young users and non-users have significant differences in socio-demographic characteristics and perception on Korean Medicine. Among the general characteristic variables, region, age, marital status, occupation, chronic disease, and private health insurance plans had an effect on the Korean Medicine use in young people(Model II). And adjusted by general characteristic variables as control variables, awareness of Korean Medicine and perception of safety of herbal medicine were deduced as determinants of Korean Medicine use(Model III). Secondly, Korean Medicine use experience increased intentions to use and recommend Korean Medicine services(Model IV-1, 3). Conclusions : In order to increase the use of Korean Medicine among young people, Korean Medicine institutions should strive to increase users' satisfaction, and public health insurance coverage for Korean Medicine services needs to be expanded.

The use of interdental care products in Korean young adults aged 19-39 years and factors affecting their use : Korean National Health and Nutrition Examination Survey IV-VII (19-39세 한국 청년의 치간관리용품 사용실태 및 각 용품 사용의 영향요인 : 제4기-제7기 국민건강영양조사)

  • Objectives: This study aimed to examine the actual use of interdental care products (ICPs), such as dental floss (DF) and interdental brushes (IDB), among Korean youth, confirm their relevance to periodontal health, and determine the factors that influence the use of each product. Methods: This study included 15,912 young adults aged 19-39 years and data from the Korea National Health and Nutrition Examination Survey (KNHANES) (2007-2018). The usage rate of ICPs according to the characteristics of the subjects for each cycle of KNHANES was presented. Multivariable logistic regression analysis was performed to identify factors affecting the use of ICP. Results: The use rates of DF and IDB gradually increased until the 7th period, reaching 34.8% and 26.8%, respectively. The rate of using more than one ICP also showed a tendency to gradually increase from 25.2% in the 4th period to 50.0% in the 7th period. The use of interdental care products is related to gingivitis and periodontitis. Factors related to the use of ICP were gender, age, education level, frequency of brushing, and dental examination experience. Conclusions: The use of dental floss or interdental toothbrushes was related to periodontal health, but only half of the adults aged 19-39 years used ICP. Therefore, oral health experts should actively encourage the use of DF and IDB in young adults.

The Determinants of Undiagnosed Hypertension Among Indonesian Adults: A Cross-sectional Study Based on the 2014-2015 Indonesia Family Life Survey

  • Objectives: This study investigated the determinants of undiagnosed hypertension among Indonesian adults. Methods: This study involved an analysis of secondary data from the 2014 Indonesia Family Life Survey (IFLS) on 5914 Indonesian adults (≥40 years). The determinant variables examined in this cross-sectional study were education level, monthly per capita expenditures (PCE), whether the participant experienced headaches in the morning, and other general health variables. The outcome variable was undiagnosed hypertension, which was defined as participants with hypertension who had not received a hypertension diagnosis from a health professional and had never been prescribed medication for treating hypertension. The data were analyzed using logistic regression. Results: A total of 3322 participants (56.2%) were found to have undiagnosed hypertension. The odds ratios (ORs) and 95% confidence intervals (CIs) of undiagnosed hypertension were significantly higher among those who completed primary school or lower (OR, 1.60; 95% CI, 1.29 to 1.98), had low monthly PCE (OR, 1.28; 95% CI, 1.13 to 1.43), did not report experiencing headaches in the morning (OR, 1.97; 95% CI, 1.76 to 2.21), and reported a general health status of healthy (OR, 2.05; 95% CI, 1.82 to 2.30) than those who had a higher education level, had high monthly PCE, experienced headaches in the morning, and were unhealthy. Conclusions: Education level, monthly PCE, the experience of headaches in the morning, and general health status were associated with undiagnosed hypertension. The monitoring system for detecting undiagnosed hypertension cases must be strengthened. Health promotion is also necessary to reduce the prevalence of undiagnosed hypertension.

Prognostic Role of Circulating Tumor Cells in the Pulmonary Vein, Peripheral Blood, and Bone Marrow in Resectable Non-Small Cell Lung Cancer

  • Background: Studies of the prognostic role of circulating tumor cells (CTCs) in early-stage non-small cell lung cancer (NSCLC) are still limited. This study investigated the prognostic power of CTCs from the pulmonary vein (PV), peripheral blood (PB), and bone marrow (BM) for postoperative recurrence in patients who underwent curative resection for NSCLC. Methods: Forty patients who underwent curative resection for NSCLC were enrolled. Before resection, 10-mL samples were obtained of PB from the radial artery, blood from the PV of the lobe containing the tumor, and BM aspirates from the rib. A microfabricated filter was used for CTC enrichment, and immunofluorescence staining was used to identify CTCs. Results: The pathologic stage was stage I in 8 patients (20%), II in 15 (38%), III in 14 (35%), and IV in 3 (8%). The median number of PB-, PV-, and BM-CTCs was 4, 4, and 5, respectively. A time-dependent receiver operating characteristic curve analysis showed that PB-CTCs had excellent predictive value for recurrence-free survival (RFS), with the highest area under the curve at each time point (first, second, and third quartiles of RFS). In a multivariate Cox proportional hazard regression model, PB-CTCs were an independent risk factor for recurrence (hazard ratio, 10.580; 95% confidence interval, 1.637-68.388; p<0.013). Conclusion: The presence of ≥4 PB-CTCs was an independent poor prognostic factor for RFS, and PV-CTCs and PB-CTCs had a positive linear correlation in patients with recurrence.

Radical surgery for stage IV gallbladder cancers: Treatment strategies in patients with limited metastatic burden

  • Backgrounds/Aims: The present study looked at the role of radical surgery in gallbladder cancers (GBC) with limited metastatic disease. Methods: The retrospective observational study was conducted to screen the database from 1st January 2010 to 31st December 2019. Patients of GBC found to have low-volume metastatic disease upon surgical exploration were included. Results: Of the 1,040 patients operated for GBC, 234 patients had low-volume metastatic disease (microscopic disease in station 16b1 node or N2 disease isolated port-site metastases, or low burden peritoneal disease with deposits less than 1 cm, in adjacent omentum or adjacent diaphragm or Morrison's pouch or a solitary discontinuous liver metastasis in adjacent liver parenchyma) detected intraoperative. Of these, 62 patients underwent radical surgery for R-0 metastatic disease followed by systemic therapy, while the remaining 172 patients did not undergo radical surgery and were given palliative systemic chemotherapy. Patients who underwent radical surgery had significantly superior overall survival (19 months versus 12 months, p < 0.01) and superior progression-free survival (10 months versus 5 months, p < 0.01) when compared to the rest. This difference in survival was more significant amongst patients when operated on after neoadjuvant chemotherapy. Regression analysis showed that a sub-group of patients with incidental GBC with limited metastases showed more favorable outcomes with radical surgery. Conclusions: Authors suggest a possible role for radical treatment of advanced GBC with a limited metastatic burden. Neoadjuvant chemotherapy can be used for preferentially selecting patients of favorable disease biology for curative treatment.
