Postoperative Clinical Outcomes of Colonic Stent Placement as Bridge-to-surgery vs. Emergency Surgery in Left-sided Malignant Colonic Obstruction

  • Background/Aims: Colonic stenting as a bridge to elective surgery is an alternative for emergency surgery in patients with acute malignant colonic obstruction. However, since its benefits are uncertain, we aimed to establish whether it has better clinical outcomes. Methods: The patients with acute malignant left-sided colon obstruction enrolled from January 2009 to December 2018 in National Health Insurance Service Ilsan Hospital. The patients were enrolled to undergo colonic stenting as a bridge to elective surgery or emergency surgery. The following oncological outcomes were assessed: incidence of complete remission, disease progression, local recurrence, and systemic recurrence. Results: Out of 40 patients, 33 received self-expanding metallic stent (SEMS) as a bridge-to-surgery, and 7 underwent emergency surgery. More stoma was made in case of emergency surgery with statistical significance (p < 0.001). There were no significant differences in complete remission rate in curable left-sided malignant colonic obstruction between SEMS as a bridge-to-surgery and emergency surgery. Complete remission was achieved for 3 patients (42.9%) in the non-stent group and 27 patients (81.8%) in the stent group. There was no statistically significant difference in oncologic outcomes between the two groups (p = 0.069). According to multi-variate analysis, advanced TNM stage, Adjuvant chemotherapy, and SEMS bridge-to-surgery were significantly associated with disease-free survival. Disease-free survival rate differed significantly between the two groups (p = 0.024). Conclusions: SEMS as a bridge-to-surgery might be an effective strategy and reduce stoma formation in acute malignant left-sided colon obstruction.

A Systematic Review of Nursing Interventions in Patients with Extracorporeal Membrane Oxygenation (ECMO) (체외막산소공급(ECMO) 치료 환자의 간호 중재에 대한 체계적 고찰)

  • Purpose : This study aimed to systematically review the effectiveness of nursing interventions for patients receiving extracorporeal membrane oxygenation (ECMO). As the use of ECMO increases in critical care settings, it is important to understand how nursing interventions affect patient outcomes, survival, and complication rates. Methods : This systematic review followed the preferred reporting items for systematic reviews and meta-analysis guidelines. A literature search was performed using terms related to ECMO and nursing interventions in several international electronic databases including CINAHL, Embase, MEDLINE, and Web of Science. Studies were screened and selected according to predefined eligibility criteria, focusing on those that evaluated the impact of nursing interventions on adult. Data extraction and risk-of-bias assessment were independently performed by two researchers. Results : A total of 647 studies were identified, and seven met the inclusion criteria for qualitative analysis. The included studies demonstrated that high-quality nursing care significantly improves clinical outcomes and reduces complications in patients receiving ECMO. Effective nursing interventions included prone positioning combined with ECMO for patients with acute respiratory distress syndrome, meticulous infection control, comprehensive and continuous nursing protocols, skilled nursing, and multidisciplinary management. These interventions have been shown to improve oxygenation, reduce complications, such as bleeding, manage blood pressure, and enhance overall clinical outcomes. Conclusion : High-quality nursing interventions are critical to improve survival and reduce complications in patients receiving ECMO. Implementing a multidisciplinary approach and comprehensive nursing protocols, including infection control and psychological support, is essential for the effective management of these patients. The findings of this study provide a foundation for the development of practical guidelines and educational programs to improve the quality of care for patients undergoing ECMO, ultimately enhancing the effectiveness of ECMO treatment and patient outcomes.

  • This study investigated the labor market outcomes between IT graduates and non-IT graduates in terms of employment, wage, and work period through Monte-Carlo and Calibration method. The empirical result of the movement from work to unemployment implied that IT major graduates have stable work period irrespective of continuous employment, and but the number of work period of non-IT graduates decreases. It also showed that IT related department graduates got relatively paid more than non-IT graduates and IT major graduates was 0.8% higher wage premium than non-IT graduates.

  • Information Technology represents substantial financial investment for government organization. However, it seems that the real goal of IT investment, which is to make government more efficient, effective, and responsible through IT systems, has not been seriously aware of in the government organizations. As a result, despite of large investment in IT over many years, substantial benefits accrued to the work, such as process innovation and work efficiency, have not actually occurred. In this paper, it is argued that, since IT benefits will not be realized without any effective benefits realization efforts, it is needed in IT evaluation to put more emphasis on the efforts. Therefore, it is suggested that appropriate IT evaluation can possibly work as a guideline for deriving the desired benefits through the IT investment. In this study, some of the well known approaches were reviewed about what IT evaluation and IT benefits realization efforts should be. Current evaluation method was analyzed from the perspective of benefits realization. On the basis of the analysis, this study leads to suggestions that improve the current ways of evaluating IT performance, helping government focus on realizing practical IT benefits at work.

What Kind of Job Characteristics are Important for Improving Students' Competencies in Co-op Education : KoreaTech IPP Case

  • The purpose of this study was to examine the effect of on-site training environments and goals on learning outcomes of trainees. For this purpose, the study analyzed survey data collected from 752 students who participated in KOREATECH's Industry Professional Practice (IPP) on-site training program from 2017 to 2018. The following summarizes some important findings from the study results: First, on examining whether the effect of job characteristics on the academic competencies differs according to the characteristics of the group, it was found that the factors that affect the whole group were job content, coaching, and welfare benefits, but the effect of job scope was statistically significant. Second, on examining whether the effect of job characteristics on job competencies differs according to the characteristics of the group, it was found that the factors that affect the whole group were job content, coaching, and job scope, but the effect of welfare was statistically significant. Analyzing by training goals, it was found that the major influencers were job content in the Recruitment-intended group; and job content, coaching and job scope in the Training-intended group. Based on the results, the implications of the study and follow-up studies are suggested.

4P Model for Strategic Research Planning: Focusing on the Cases of Korea Research Institute of Chemical Technology

  • The demand for efficient utilization of input resources and productive outcomes is increasing as the government's R&D investments in Government-funded research institutes (GRIs) expand. These changes call for improving research-planning activities, which are defined as a set of activities wherein objectives are established, strategies for acquisition and expenditures of research resources are devised, and utilizations of research outcomes are addressed. This study introduces the integrated 4P analysis model that identifies the relationships among patents, papers, products, and projects. It looks into 4P analysis structure and its efficiency as a research planning means through case studies of the Korea Research Institute of Chemical Technology. This study introduces 4P analysis applied to KRICT, which can be utilized for outcome-oriented research planning of GRIs. At the same time, it investigates into the benefits and implications of 4P analysis. It proffers policy suggestions on such aspects as how research planning of GRIs should go through changes in a strategic and systematic way.

Involvement of Social Media in Disaster Management during the Wenchuan and Ya'an Earthquakes

  • Social media is being increasingly utilized in disaster relief work to identify safety issues, locate displaced-victims, and seek or provide support for those who need help. The presence of social media in disaster management has changed significantly in recent years, as it was not prevalent in the 2008 Wenchuan earthquake, but had become a powerful force in the 2013 Ya'an earthquake. This paper discusses the development of social media in disaster management via making a comparison between how it functioned in the two earthquakes. It examines the following aspects: who are the stakeholders that use social media in the earthquake management; how do they adopt this means in response to the earthquakes; and what are the outcomes of adopting social media with regards to public engagement and collaboration in an emergency event. As Sina Weibo acts as the equivalent of China's Twitter, the methodology relies on an analysis of posts in Weibo. The outcomes primarily show that: (1) authorities, celebrities and the public actively adopted social media for the purpose of information dissemination and resource mobilization; and (2) social media users are both content consumers and content generators in the times of earthquakes. The study concludes that social media as a backchannel communication tool is helpful for government institutions, corporations, and nonprofit organizations to build relationships with their stakeholders in the disaster management cycle. The result will interest academics and emergency management practitioners who are concerned with improving disaster communication.

Definition of 3D Modeling Level of Detail in BIM Regeneration Through Reverse Engineering - Case Study on 3D Modeling Using Terrestrial LiDAR - (역설계를 통해 BIM 구축시에 3D 모델링에 대한 세밀도(LoD) 정립 - 지상 LiDAR 활용한 3D 모델링 연구 중심 -)

  • When it comes to set up the BIM through the reverse engineering, the level of detail(LoD) required for finalized outcomes is different from each purpose. Therefore, it is necessary to establish some concrete criteria which describe the definition of LoDs on 3D modeling for the purpose of each reverse engineering. This research shows the criteria of the 1) positional accuracy, 2) generalization level, 3) scale level, 4) scope of description, and 5) the area available for application by classifying LoD from 1 to 6 on 3D modeling for each purpose of reverse engineering. Moreover, through applying those criteria for the 3D point cloud dataset of building made by terrestrial LiDAR, this research finds out the working hour of 3D modeling of reverse engineering by each LoDs according to defined LoD criteria for each level. It is expected that those findings, how those criteria of LoD on reverse engineering are utilized for modeling-workers to decide whether the outcomes can be suitable for their budget, applicable fields or not, would contribute to help them as a basic information.

  • This paper assesses the evidence of the built environments' impact on patients' and staff's well-being. It also introduces evidence-based design (EBD) and discusses barriers and solutions for promoting EBD. EBD is an informed approach to design by improving design decisions based on best available knowledge. Designers should provide psychologically and physically supportive environments to promote occupants' well-being. There is increasing evidence that a poorly designed environment elicits negative feelings and stress. Better design will lead to better outcomes especially when it applied to a vulnerable population such as patients, children, and the elderly. Due to high demands of promoting patients' health, healthcare providers and professional designers have been searching for evidence to create healing environments. EBD has been well received in healthcare facilities design and is expanding to other types of environmental design such as educational and commercial design. Design firms should consider EBD as a value-added component of design to meet current and future challenges. It is important for designers to realize that engaging in EBD is not a rejection of creativity, but a means by which to scrutinize their design projects. With critical literature reviews on healing environments, there is sufficient evidence that well-designed environments can promote patients' and medical staff's health outcomes.

A Proposed Conception of Technical Solutions to Address the Educational Loss in Mathematics in the Fifth Grade of Primary School

  • The study aimed to provide a proposed conception of technical solutions to address the educational loss in mathematics in the fifth grade of primary school, and the study adopted the descriptive approach. male and female teachers, and to achieve the study objective, the researcher built a tool for the study, a "questionnaire", in which he used the comprehensive inventory method, and the results of the study showed: identifying the learning outcomes that represent an educational loss, and identifying the learning outcomes that are considered essential in teaching and learning mathematics for the fifth grade of primary school. In the event that it is not achieved by students, it is considered an educational loss that may affect the future of students' education and learning. Because it is a basis for later experiences in mathematics in other classes, and the study also found the effectiveness of the proposed visualization of technical solutions provided to address the educational loss in mathematics for the fifth grade: (short electronic tests, YouTube channel, homework, educational platform, electronic worksheets, and communication channels).
