Toe-in Gait, Associated Complications, and Available Conservative Treatments: A Systematic Review of Literature

  • Purpose: Toe-in gait is defined as a style of walking in which the foot turns inward. It may be caused by an increase in femoral bone anteversion, tibia torsion, and metatarsus adductus. There are some conservative treatment approaches used to correct this condition. This review aimed to determine the effects of the toe-in gait on joint loading, kinematics, and kinetic parameters while walking. Moreover, it sought to determine the efficiency of various conservative treatments used to correct the condition. Materials and Methods: A literature search was conducted in the following databases: PubMed, Institute for Scientific Information (ISI), Web of Science database, EBSCO, and Embase, using the following keywords in toe, toe-in, toeing, in-toe, pigeon toe, and conservative treatment published between 1950 and 2021. The quality of the studies was evaluated using the Down and Black tool. Results: A total of 13 papers on the impact of toe-in gait on joint contact force, kinematics, kinetic parameters, and conservative approaches to management were found. The quality of the studies varied between a score of 11 and 22. The toe-in gait influences the joint contact forces and kinematics of the joints, especially the hip and pelvis. The effects of conservative treatment on the toe-in gait appear to be controversial. Conclusion: As the toe-in gait influences the joint contact force, it may increase the incidence of degenerative joint diseases. Therefore, treatment is recommended. However, there is no strong evidence on the efficacy of conservative treatments, and there are no recommendations for the use of these treatments in subjects with toe-in gait.

Pre-Coding Method for Underwater Digital Communications in a Multipath Channel (다중 전달 경로 채널에서의 수중 디지털 통신을 위한 선 처리 기법)

  • Signals in an underwater channel get distorted by multipath propagation. In this paper, pre-coding method is suggested which helps comprehending the signals with minimum equalization. The signals are transformed based on the knowledge of the impulse response of the channel. Proposed pre-coding method is tested by simulations based on the ray theory and through water tank experiments. In weak multipath environment, in case of an SNR of about 20 dB, BER is $10^{-3}{\sim}10^{-4}$, while in strong multipath environment, similar BER is achieved with SNR of about 30 dB. In order for the pre-coding method to be used for underwater vehicles, channel prediction method utilizing the waveguide invariant is suggested and tested.

  • 1) The passage "Sila seongdae so-seongdae cheonha daepyeong nahu-deog (新羅盛(聖)代 昭盛代 天下大平 羅侯德)" of the Korean Dynasty , corresponds to the primary passage of Cheonyong Rang Manghaesa(처용랑 망해사)>. These passages are ironic expressions of King Heongang's reign. This passage can therefore be interpreted as having the meaning of "People talk about King Heongang who ruined the kingdom. The bright politics of Silla's loyalty is the virtue of King Heongang." 2) "Cheoyong aba isi insaeng-ae sangbuleo hasilandae isi insaeng-ae samjaepalnan-i ilsi somyeol hasyatda (處容 아바 以是人生애 相(常)不語시란 以是人生애 相(常)不語시란 三災八難이 一時消滅샷다)." This was said by Cheoyong(처용), who was unable to reveal his anger while witnessing his wife being raped by Yuk-sin(疫神). This passage means, "Father Cheoyong(처용), in this world if a man does not express anger about the absurdity of the world, he can escape from catastrophe," or, "Father Cheoyong (처용), because in this world you had not expressed anger about the absurdity of world, you could escape from catastrophe." 3) In "maa-man maa-man haniyeo sibi jegug-i moda jiseo syeon aeu cheoyongbi-hal maa-man haniyeo (마아만 마아만 니여 十二諸國이 모다 지 셰온 아으 處容아비 마아만 니여)," the most likely interpretation of Maa is "마아(麻兒)," which means "scarecrow." In this way, this passage emphasizes that Cheyong(처용) is not a scarecrow, but a representative of the eyes of all people in all kingdoms. 4) In "meoja oeyaja logliya ppallina nae singohal maeyara (머자외야자 綠李야 리나 내 신고 야라", meoja (머자) is the double arranging shape of meotda(멎다), and oeyaja(외야자) is the double arranging shape of oeda(외다); those characters mean "ugly" and "wrong for each other." Additionally, Rokri(綠李) can be regarded as an administrator in blue or black clothes who is performing an errand of the yuksin(疫神).

Evaluation of sleep quality and stress response in patients requiring dental prosthetic treatment (치과보철치료를 요하는 환자에서의 수면질과 스트레스 반응 평가)

  • Purpose: The purpose of this study is to investigate the stress response and sleep state of the new patients who visit the hospital for prosthodontic treatment due to tooth loss to make reference to further treatment direction. Materials and methods: Examinations, consultations, and surveys were conducted on 236 patients (94 males, 142 females, and 24-86 years old), who newly registered at the department of prosthodontics at Pusan national university hospital, for 2 years from 2018. The number of remaining teeth, edentulous arch, possibility of mastication, and future prosthetic treatment directions were recorded. The visual analogue scale (VAS), stress response inventory (SRI) and Insomnia severity index (ISI) were used as tools to measure patient's subjective discomfort, stress response, and sleep status. Results: Among the entire prosthodontic patient group, the average value of 'fatigue' was significantly higher among the 7 items of SRI, and the value of 'aggression' was significantly lower (P<.001). There are a significant difference between the edentulous and full dentition in the oral discomfort value measured by VAS (P=.004). In cases where less than 20 teeth remain, mastication is impossible, and patients who are planning treatment with removable prostheses, VAS value and 'depression' and 'fatigue' values in SRI were higher than those in the reverse case (P<.05). The patients showed severe level in VAS value have high value of 'tension', 'somatization', 'depression', 'fatigue'and 'frustration'in SRI than non-serious patients (P<.05). Clinical insomnia occurred in 11.4% of all patients, and women had significantly lower sleep quality than men (P=.044). Patients with insomnia showed significantly higher scores on the VAS value and all 7 SRI items than those of normal sleepers (P<.05). Conclusion: The new prosthetics had high 'fatigue' value among the 7 items of SRI. The oral condition, number of residual teeth and mastication function and clinical insomnia affected oral discomfort and the stress response.

투자대상 벤처기업의 선정을 위한 전문가시스템 개발

  • 오늘날 기술집약적인 벤처기업들에 대한 관심이 집중되고 있다. 소수의 진취적인 벤처기업들이 기술개발 및 신상품 개발 등 두드러진 활약을 보이고 있기 때문이다. 그러나 실제 이 벤처기업의 성공 가능성은 그렇게 높지 않다. 특히 벤처기업 환경이 아직 미약한 국내의 경우 위험부담이 훨씬 더 크다. 이러한 벤처기업 환경에서 투자대상 벤처기업을 선정하는 것은 매우 전략적인 의사결정이다. 일반적으로 일반 벤처투자가들은 관심이 있는 산업에 해당하는 기업의 사업계획서와 기초적인 관련 정보를 토대로 투자여부를 결정한다. 그렇지만 실제로는 이와 같은 분석에 필수적으로 요구되는 정보가 불확실할 뿐만 아니라 기술분야에 대한 전문적 지식도 부족하기 때문에 투자 여부를 결정하는 것은 매우 복잡하고 어려운 문제이다. 그러므로 투자대상 벤처기업의 선정을 효과적으로 지원해주는 체계적인 접근이 필요하다. 특히 벤처 사업과 관련된 기술 동향 및 수준 등에 관련된 전문 지식과 경험이 체계적으로 제공되어야 하고 또한 벤처 투자가의 개인적 경험과 판단이 평가 프로세스에 직접적으로 반영될 수 있어야 한다. 이에 본 연구에서는 전문가의 지식과 경험을 체계화하고 투자가의 개인적 판단을 효과적으로 수용할 수 있는 전문가시스템의 접근방법을 제시하고자 한다. 투자대상 벤처기업의 선정을 위한 전문가시스템을 구축하기 위해 본 연구에서는 다양한 정보수집 과정을 거쳤다. 우선 벤처 투자와 관련된 기존 문헌을 심층 분석하였으며 아울러 벤처 투자 업계에서 활약중인 전문 벤처캐피탈리스트들과의 수차례 인터뷰를 통해 벤처기업 평가의 주요 요인과 의사결정 과정을 파악할 수 있었다. 이러한 과정을 통하여 본 연구에서는 벤처 투자의 90%를 차지하는 정보통신분야에 속한 기법 중에서 투자대상 벤처기업의 선정을 위한 전문가시스템을 구축중이다.의 밀도를 비재무적 지표변수로 산정하여 로지스틱회귀 분석과 인공신경망 기법으로 검증하였다. 로지스틱회귀분석 결과에서는 재무적 지표변수 모형의 전체적 예측적중률이 87.50%인 반면에 재무/비재무적 지표모형은 90.18%로서 비재무적 지표변수 사용에 대한 개선의 효과가 나타났다. 표본기업들을 훈련과 시험용으로 구분하여 분석한 결과는 전체적으로 재무/비재무적 지표를 고려한 인공신경망기법의 예측적중률이 높은 것으로 나타났다. 즉, 로지스틱회귀 분석의 재무적 지표모형은 훈련, 시험용이 84.45%, 85.10%인 반면, 재무/비재무적 지표모형은 84.45%, 85.08%로서 거의 동일한 예측적중률을 가졌으나 인공신경망기법 분석에서는 재무적 지표모형이 92.23%, 85.10%인 반면, 재무/비재무적 지표모형에서는 91.12%, 88.06%로서 향상된 예측적중률을 나타내었다.ting LMS according to increasing the step-size parameter $\mu$ in the experimentally computed. learning curve.   • Bleaching of vital and nonvital teeth is becoming a more commonly used treatment in the dental office and at home. To improve appearance and remove discoloration, the teeth of specific patients are treated with a variety of bleaching agents. The typical bleaching agents contains carbamide or hydrogen peroxide as the active component. The purpose of the review article was to summarize and discuss the available information concerning the effect of peroxide releasing bleaching agent on dental restorative materials and restorations. Information from all original scientific full papers or reviews listed in PubMed or ISI Web Science were included in the review. Bleaching may exert a negative influence on restorations and restorative materials. Advice is provided based on the current literature to minimize the impact of bleaching treatment on restorative materials and restorations.

Association Between the (GT)n Polymorphism of the HO-1 Gene Promoter Region and Cancer Risk: a Meta-analysis

  • Background: Several studies have previously focused on associations between the (GT)n repeat polymorphism of the heme oxygenase-1 (HO-1) gene promoter region and risk of cancers, but results are complex. We conducted the present meta-analysis to integrate relevant findings and evaluate the association between HO-1(GT)n repeat polymorphism and cancer susceptibility. Materials and Methods: Published literature was retrieved from the PubMed/MEDLINE, EMBASE and ISI Web of Science databases before November 2013. For all alleles and genogypes, odds ratios were pooled to assess the strength of the associations using either fixed-effects or random-effects models according to heterogeneity. Subgroup analysis was conducted according to ethnicity and histopathology. Results: A total of 10 studies involving 2,367 cases and 2,870 controls were identified. The results showed there was no association between HO-1 (GT)n repeat polymorphism and the cancer risk both at the allelic and genotypic level. However, in the stratified analysis, we observed an increased risk of squamous cell carcinoma in persons carrying the LL genotype and the LL+LS genotype as compared with those carrying the SS genotype. When the LS and SS genotypes were combined, the odds ratio for squamous cell carcinoma in LL-genotype carriers, were also significantly increased. No publication bias was observed. Conclusions: The LL genotype and L-allele carrying genotypes (LL+LS) of HO-1 (GT)n repeat polymorphism are potential genetic factors for developing squamous cell carcinoma. More large and well-designed studies are required for further validations.

  • Objective : Premenstrual syndrome (PMS) and premenstrual dysphoric disorder (PMDD) is known to reduce the quality of sleep and is associated with various psychiatric disorders. The aim of this study was to investigate the relationship of PMS with depression, sleep qulity and sleep pattern in Korean female adolescents. Methods : Out of 867 high school female students, 400 female subjects were included in this study. All participants completed self-report questionnaires that included demographic variables, shortened premenstrual assessment form (SPAF), patient health questionnaire-9 (PHQ-9), Pittsburgh sleep quality index (PSQI), Epworth sleepiness scale (ESS), insomnia severity index (ISI) and Korean translation of composite scale (CS). A total of 394 individuals who completed the questionnaires were analyzed. Results : As score of SPAF, 143 subjects was defined PMDD group. They had more prevalence of eveningness, more depressive symptom, poorer sleep quality, and more excessive sleepiness than non-PMDD groups. Although the PMDD groups showed more menstrual dysmenorrhea, no difference was found in bleeding duration and cycle. PMS symptom had correlations with depression, poorer sleep quality and excessive sleepiness during the daytime. There was no difference in sleep duration between two groups, but PMS might have a relationship with eveningness of chronotype. Conclusions : Among Korean female high school students, the PMS might have an association with depression and poor sleep quality, and it might lead to daytime impairment due to excessive sleepiness. An appropriate evaluation of PMS could be helpful in determining the mental health condition of adolescents.

UNDERSTANDING THE H STATISTIC DURING ROUTINE ANALYSIS OF ANIMAL FATS.

  • During two consecutive years, it was developed global calibrations for the prediction of fatty acids on Iberian pig fat. These equations should analyse well samples of that animal fat because of their high accuracy (SECV/sub C16:0/ = 0.26%; SECV/sub C18:0/ = 0.28%; SECV/sub C18:1/ = 0.26%; SECV/sub C18:2/ = 0.15%) and their broad covering composition range. In some cases, when new samples are predicted H (Mahalanobis distance) values higher than 3 (recommended value for agricultural products by the ISI software) are obtained. However, there are not any obvious factors which tells that samples scanned are very different to the spectral mean of the calibration population. Furthermore, these samples are well predicted according to the SEP values. The objective of the present work is to deepen the understanding of the H statistic when analysing animal fats. Three different validation files were predicted with equations obtained from January '97 to April '98. The Set A has spectra of 20 samples not included on the calibration file and scanned in May of 1998. The Set B has spectra of 20 samples included on the calibration file and scanned again in November '99. The Set C contains 150 spectra of one sample representative of the mean values (for fatty acids composition) of the calibration file. This sample was analysed three times per week during June '99 to July '00. The H mean values for the Set A, Set B and Set C were respectively 1.35, 14.39 and 11.71. These anomalous values for the Set B and C make not sense because Set B contains replicate subsamples of the same samples scanned during calibration development and Set C only contains spectra of one sample which represent the mean spectrum of the calibration files. Results will be shown to demonstrate that small day to day variations are responsible of the high H values. When a PCA and LIB file are created with calibration samples and spectra of the Set C modelling day to day variations, the H values for Set A, Set B and Set C were respectively 1.83, 2.16 and 0.93.

Near Infrared Spectroscopy for Measuring Purine Derivatives in Urine and Estimation of Microbial Protein Synthesis in the Rumen for Sheep

  • The efficiency of the luminal fermentation process influences overall efficiency of luminal production, animal health and reproduction. Ruminant production systems have a significant impact on the global environment, as well. Animal wastes contribute to pollution of the environment as ammonia volatilized to the air and nitrate leached to ground water. Microbial protein synthesis in the rumen satisfies a large proportion of the protein requirements of animals. Quantifying the microbial synthesis is possible by using markers for lumen bacteria and protozoa such as nucleic acids, purine bases, some specific amino acids, or by isotopic $^{15}N,^{32}P,\;and\;^{35}S$ labelled feeds. All those methods require cannulated animals, they are time-consuming and some methods are very expensive as well. Many attempts have been made to find an alternative method for indirect measurement of microbial synthesis in intact animals. The present investigations aimed to assess possibilities of NIRS for prediction of purine nitrogen excretion and ruminal microbial nitrogen synthesis by NIR spectra of urine. Urine samples were collected from 12 growing sheep,6 of them male, and 6- female. The sheep were included in feeding experiment. The ration consisted of sorghum silage and protein supplements -70:30 on dry matter basis. The protein supplements were chosen to differ in protein degradability. The urine samples were collected daily in a vessel containing $60m{\ell}$ 10% sulphuric acid to reduce pH below 3 and diluted with tap water to 4 liters. Samples were stored in plastic bottles and frozen at $-20^{\circ}C$ until chemical and NIRS analysis. The urine samples were analyzed for purine derivates - allantoin, uric acid, xantine and hypoxantine content. Microbial nitrogen synthesis in the lumen was calculated according to Chen and Gomes, 1995. Transmittance urine spectra with sample thickness 1mm were obtained by NIR System 6500 spectrophotometer in the spectral range 1100-2500nm. The calibration was performed using ISI software and PLS regression, respectively. The following statistical results of NIRS calibration for prediction of purine derivatives and microbial protein synthesis were obtained.(Table Omitted). The result of estimation of purine nitrogen excretion and microbial protein synthesis by NIR spectra of urine showed accuracy, adequate for rapid evaluation of microbial protein synthesis for a large number of animals and different diets. The results indicate that the advantages of the NIRS technology can be extended into animal physiological studies. The fast and low cost NIRS analyses could be used with no significant loss of accuracy when microbial protein synthesis in the lumen and the microbial protein flow in the duodenum are to be assessed by NIRS.
