Intraoperative Radiation Therapy of locally Advanced Gastric Cancers - Case report - (국소진행 위암의 술중조사 2례)

  • Stomach cancer is the most popular tumor in Korea but the prognosis following extensive surgery and chemotherapy has not improved for many years. Conventional external radiotherapy also has some limitation, namely, a cancerocidal dose can not be delivered to tumors because of low radiation tolerance of adjacent critical sensitive organs. In order to overcome these limitations of curative surgery and external radiotherapy, intraoperative radiotherapy was proposed in many centers and the results were excellent. We treated two cases of locally advanced gastric cancers with IORT by using NEC 18 MeV Linear Accelerator after standard subtotal resection of the stomach. After treatment, the patients are in good conditions so far, but in order to evaluate the effectiveness of IORT, a study will be performed on the survival rates between patients treated by IORT and those treated by surgery alone.

  • Total 28 patients with resectable, locally advanced gastric cancer were entered in our prospective randomized study from June 15, 1988 to Sep. 15, 1990 in Yeungnam University Hospital. This study consisted of curative resection, IORT, external irradiation and combination of chemotherapy. Twenty-four of 28 patients were treated with single dose of 1500 cGy with 9 MeV election intraoperatively. External irradiation of $4300\sim4500$ cGy with 180 cGy per fraction, 5 days per week was started within 4th weeks of postoperative days. Various chemotherapy with or without external irradiation were added for reducing hematogenous and/or peritoneal dissemination and determination of complication of each arm. Duration of follow up was $4\sim31$ months. No serious complication related with radiation were reported compare to resection and chemotherapy only group. Although our follow up period is too short to draw any conclusion, IORT appears to improve local control, hopely further survival. Continuous follow up should be needed for evaluation of real therapeutic gain such as complication vs. improved survival.

  • Customers' requirements written in a natural language are rewritten to modeling language in development phases. In many cases, those who participate in development cannot understand requirements written in modeling language. This paper proposes the translation technique from the requirement model which is written by REED(REquirement EDitor) tool into a natural language in order to help for the customer understanding requirement model. This technique consists of three phases: $1^{st}$ phase is generating the IORT(Input-Output Relation Tree), $2^{nd}$ phase is generating the RTT(Requirement Translation Tree), $3^{rd}$ phase is translating into a natural language.

The Results of Intraoperative Radiotherapy for Stomach Cancer (위암의 수술 중 방사선치료의 결과)

  • Purpose: We retrospectively analyzed the long-term results of radical surgery and intraoperative radiation therapy (IORT) in patients with stomach cancer. Materials and Methods: From 1988 to 1994, 51 patients were treated with curative surgery and IORT. Postoperative external beam radiotherapy (EBRT) was administered to 30 patients, while adjuvant chemotherapy was administered to 35 patients. A dose of 15 Gy was irradiated with a 9 MeV electron beam as the IORT and a median dose of EBRT was 43.2 Gy (range, 7.2 to 45 Gy). The follow-up period ranged from 1~254 months, with a median follow-up period of 64 months. Results: The median age of all the patients was 58 years (range, 30 to 71 years). The distribution of pathologic stage (American Joint Committee on Cancer [AJCC] 2002 tumor-note-metastasis [TNM]) was as follows: 13 stage I (25.5%), 10 stage II (19.6%), 25 stage III (49.0%), and 3 stage IV (5.9%). Distant metastases occurred in 11 patients (10 in the peritoneum and 1 in bone), including one patient with concurrent local recurrence (anastomosis site). The 5-year locoregional control, disease free survival and overall survival rates were 94.7%, 66.5%, and 51.7%, respectively. For the multivariate analysis, age, TNM stage, and EBRT were significant prognostic factors for overall survival, and only TNM stage for disease free survival. Conclusion: We could have achieved a high loco-regional control rate in patients with locally advanced stomach cancer by adding IORT to radical surgery. However, the benefit of IORT on survival remains to be elucidated.

  • A relation between generated test cases and an original requirement is important, but it becomes very complex because a relation between requirement models and requirements are m-to-n in automatic test case generation based on models. In this paper, I suggest automatic generation technique for REED (REquirement EDitor), 1-to-1 requirement modeling tool. Test cases are generated though 3 steps, Coverage Target Generation, IORT (Input Output Relation Tree)Generation, and Test Cases Generation. All these steps are running automatically. The generated test cases can be generated from a single requirement. As a result of applying to three real commercial systems, there are 5566 test cases for the Temperature Controller, 3757 test cases for Bus Card Terminal, and 4611 test cases for Excavator Controller.

A Study on Electron Dose Distribution of Cones for Intraoperative Radiation Therapy (수술중 전자선치료에 있어서 선량분포에 관한 연구)

  • For intraoperative radiation therapy using electron beams, a cone system to deliver a large dose to the tumor during surgical operation and to save the surrounding normal tissue should be developed and dosimetry for the cone system is necessary to find proper X-ray collimator setting as well as to get useful data for clinical use. We developed a docking type of a cone system consisting of two parts made of aluminum: holder and cone. The cones which range from 4cm to 9cm with 1cm step at 100cm SSD of photon beam are 28cm long circular tubular cylinders. The system has two 26cm long holders: one for the cones larger than or equal to 7cm diamter and another for the smaller ones than 7cm. On the side of the holder is an aperture for insertion of a lamp and mirror to observe treatment field. Depth dose curve. dose profile and output factor at dept of dose maximum. and dose distribution in water for each cone size were measured with a p-type silicone detector controlled by a linear scanner for several extra opening of X-ray collimators. For a combination of electron energy and cone size, the opening of the X-ray collimator was caused to the surface dose, depths of dose maximum and 80%, dose profile and output factor. The variation of the output factor was the most remarkable. The output factors of 9MeV electron, as an example, range from 0.637 to 1.549. The opening of X-ray collimators would cause the quantity of scattered electrons coming to the IORT cone system. which in turn would change the dose distribution as well as the output factor. Dosimetry for an IORT cone system is inevitable to minimize uncertainty in the clinical use.

  • With the increase in use of the computer, the VCT syndrome has occured as a new socialJhea/health problem. ErgonomicaI design standards are a for the users to reduce stress and poor physical posture in the human body. In this study. we have suggested design dimensions recommended from previous studies. We also have reviewed users' preference dimenision, and analyzed differences between users' preference dimenision. and the previous design aiteria to verify physical appropriateness. We analyzed how each design dimension was reached and affected the tunan body by objective EMG evaluation. and subjective evaluation of physical discomfort and oorrIort. We have found that keyboard height is very important in a workstation. If the elbow's height is lower than the keyboard's height. it effedS the hand and wrist. If higher, it brings fatigue to the shoulder and neck. As a result of this experiment. we suggested that the height of a keyboard desk for Koreans be 660mm for the fixed type and 540-774mm for the adjustable type. Also other design reoommendations were suggested in the thesis. In ooncIusion, our research will be very important in the database because it provides adjustable ranges to fit user's body types in the various design flekIs.

  • A docking intraoperative electron beam applicator system, which is easily docking in the collimator for a linear accelerator after setting a sterilized transparent cone on the tumor bearing area in the operation room, has been designed to optimize dose distribution and to improve the efficiency of radiation treatment method with linear accelerator. This applicator system consisted of collimator holder with shielded metals and docking cone with transparent acrylic cylinder, A number of technical innovations have been used in the design of this system, this dooking cone gives a improving latral dose coverage at therapeutic volume. The position of $90\%$ isodose curve under suface of 8 cm diameter cone was extended $4\sim7$ mm at 12 MeV electron and the isodose measurements beneath the cone wall showed hot spots as great as $106\%$ for acrylic cone. The leakage radiation dose to tissues outside the cone wall was reduced as $3\sim5\%$ of output dose. A comprehensive set of dosimetric characteristics of the intraoperative radiation therapy applicator system is presented.
