Comparison of Amphotericin B and Itraconazole as Empirical Antifungal Therapy in Children with Acute Leukemia with Neutropenic Fever (발열을 동반한 호중구감소 상태의 급성백혈병 환아에서 경험적 항진균제로 투여한 Amphotericin B와 Itraconazole의 효과와 이상 반응 비교)

  • Purpose : Fungal infection is one of the important causes of morbidity and mortality in patients with hematologic malignancies. Amphotericin B(ABV) and itraconazole(ITZA) have been used as the standard empirical antifungal therapy in neutropenic patients with acute leukemia who have persistent fever that does not respond to antibiotic therapy. ABV is an antifungal drug associated with side effects such as fever and chills, symptoms which may be mediated by pro-inflammatory cytokines such as interleukin-$1{\beta}$(IL-$1{\beta}$) and tumor necrosis factor-${\alpha}$(TNF-${\alpha}$). We assessed modulation of these pro-inflammatory cytokines as well as the anti-inflammatory cytokines(IL-4, IL-1Ra) by ABV and ITZA. Methods : From March 2004 to February 2005, a total of 30 episodes from acute leukemia patients with febrile neutropenia were analyzed for this study. They were randomly allocated to receive intravenous ABV or ITZA for 14 days. Clinical responses were evaluated at the completion of therapy, and cytokine IL-$1{\beta}$, TNF-${\alpha}$, IL-4, and IL-1Ra were measured for determination to know the correlation between two antifungal agents and inflammatory cytokines. Results : Empirical antifungal agents were given to 37 patients(ABV 20, ITZA 17), and 30 patients(ABV 15, ITZA 15) were evaluable for efficacy. White blood cell and absolute neutrophil count in the group treated with ITZA increased early days of treatment, so the duration of neutropenia in ITZA group is shorter. Serum creatinine level is lower in ITZA group than in ABV group but this is not statistically significant. There was no significant difference in response rate between two groups. The IL-$1{\beta}$ was increased in ABV treatment group and the ratio of IL-1Ra/IL-$1{\beta}$ is markedly decreased in ABV treatment group while increased in ITZA group. Conclusion : ITZA and ABV have at least equivalent efficacy as empirical antifungal therapy in neutropenic children with acute leukemia. However ITZA is associated with significantly less toxicity in clinical and molecular aspects.

  • Object : This study is designed to confirm the effect of Traditional korean Medicine on oculomotor nerve palsy patient. Method : We treated with acupuncture therapy, electro-acupuncture therapy, herbal therapy, physical therapy, moxibustion therapy. And we evaluated a patient's eyelid ptosis, eyelid levator function, eyeball movement, eyeball pain. Result : After Traditional korean Medical treatment, ptosis and pain caused by ophthalmoplegia were improved. Conclusion : This study shows Traditional korean Medicine has a good effect on oculomotor nerve palsy.

  • Background and Purpose. Physical therapists are at risk for work-related musculoskeletal disorders (WMSDs). Little understand the characteristics of work in physical therapist or the effort they take to prevent injury. The purpose of this study was to investigate the characteristics of work in physical therapist and the efforts for prevent injury Subjects. Standardized questionnaire was administered to 103 physical therapist in Pusan association. Methods. An questionnaire was given to each subject. Questions investigated the characteristics of work in physical therapist, efforts and caring of injury prevention. Chi-square Test used to assess the association of general characteristics with the characteristics of work in physical therapist. AVONA and T-test used to assess the association of efforts of prevent injury in independent variable Results. Physical therapist was reported a higher prevalence of WMSDs in most body areas. These attributes sometimes resulted in behaviors that contributed to the development of their WMSDs.

Megakaryocyte-Derived IL-8 Acts as a Paracrine Factor for Prostate Cancer Aggressiveness through CXCR2 Activation and Antagonistic AR Downregulation

  • Prostate cancer is the fifth leading cause of cancer-related mortality in men, primarily because of treatment resistance, recurrence, and metastasis. In the present study, we investigated the role of paracrine interleukin-8 (IL-8) in the antagonistic expression of IL-8 and androgen receptor (AR), and the contribution of IL-8 to prostate cancer aggressiveness. In hormone-responsive LNCaP cells that do not express IL-8, recombinant IL-8 treatment significantly increased expressions of IL-8, CXC chemokine receptor 2 (CXCR2), matrix metalloproteinase (MMP)-2/9, Snail, and vimentin. IL-8 treatment significantly decreased AR and E-cadherin expression. IL-8-induced gene expression changes were suppressed by navarixin, a CXCR1/2 inhibitor, and gallein, a Gβγ inhibitor. In PC-3 androgen-refractory prostate cancer cells, IL-8 knockdown reduced expressions of CXCR2, MMP-2/9, Snail, and vimentin, and increased AR and E-cadherin expressions at the mRNA and protein levels. Co-culture with MEG-01 human megakaryocytic cells secreting high levels of IL-8 induced gene expression changes in both LNCaP and PC-3 cells, similar to those induced by IL-8 treatment. The altered gene expressions were accompanied by significant activation of transcription factor Snail in LNCaP and PC-3 cells. Treatment with the CXCR blocker navarixin inhibited the invasion of PC-3 cells but not LNCaP cells. However, invasion induced by MEG-01 was inhibited by navarixin in both LNCaP and PC-3 cells. The collective findings demonstrate that IL-8 enhances CXCR2 expression, which antagonistically regulates AR expression. More importantly, through changes in IL-8/CXCR2-regulated gene expression, IL-8 induces antiandrogen therapy resistance and epithelial-mesenchymal transition in prostate cancer.

Hijikia fusiforme Attenuate the Mast Cell-mediated Inflammatory Response through NF-kB Pathway Regulation

  • Hijikia fusiforme (HF), a member of brown algae family, exerts various pharmacological effects, including preventing arteriosclerosis and menopausal disorders. This study aimed to elucidate the ameliorative effect of HF on skin inflammation. We investigated the antioxidant and anti-inflammatory effect of HF extract to evaluate its potential as a functional materials. The antioxidant activity of HF was determined using 2,2-diphenyl-1-picrylhydrazy (DPPH) scavenging and 2,2'-azino-bis(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS+) scavenging. To determine the pharmacological mechanism of HF in inflammatory reaction, we evaluated the effects of HF on interleukin (IL)-8, IL-6 and tumor necrosis factor (TNF)-α production and nuclear factor-κB (NF-κB) activation in activated- human mast cells (HMC)-1. Results showed that HF had the high DPPH and ABTS+ radical scavenging activity, and it suppressed IL-8, IL-6 and TNF-α production in a concentration-dependent manner. Moreover, HF significantly ameliorated NF-kB activation in activated-HMC-1. Hence, these results provide evidence that HF's potential for skin inflammation therapy.

Torque Curves and Cardiovascular Response to Isometric Exercise at the Elbow Joint in Normal and Hemiplegic Subjects (정상인과 편마비 환자의 주관절 등척성 운동시 우력양상과 심혈관계에 미치는 영향)

  • The isometric torque of the elbow flexor and extensor muscles were measured for 6 seconds at a joint angle of 90$^{\circ}$ , in 10 normal subjects (control group) and 10 hemiplegic subjects(patient group), using the Cybex NORMTM System. The peak torque, the time to peak torque were measured for each exercise. In addition, heart rate and blood pressure were recorded simultaneously at rest and immediately following exercise completion at 1 and 3m mutes. Statistical analysis was performed using SPSS 8.0 for Windows software and mean and standard deviations were calculated. The results are as follows. 1) In the patient involved group. the isometric values for flexors and extensors were significantly lower than in the normal nondominant group(p<.05). 2) The extensor to flexor strength ratio in the isometric mode was 121.0% in the patient involved group compared with 78.7%in the normal nondominant group, a significant difference(p<.05). 3) The mean increment ratio was increased 19.0% for systolic blood pressure and 25.2% for disatolic blood pressure in the patient group. 4) The mean increment ratio was increased 36.0% heart rate in the patient group.

A Case Report of Hamstring shortening after Spinal Fusion, Treated by Chuna Manual Therapy (경근 추나 치료를 적용한 척추유합술 후 슬굴곡근 단축 환자 치험 1례)

  • Objectives : The present study introduces the clinical application of Hamstring Chuna Manual Therapy to a patient who underwent the pain after spinal fusion. Methods : A patient who had the pain after spinal fusion was hospitalized at Bucheon Jaseng hospital of Oriental Medicine for 35 days. During the hospital treatment, the pain was relieved by Hamstring Chuna Manual Therapy. The improvement of the patient was measured by VAS(Visual Analogue Scale) score, SLR(Straight-Leg Raising) test, and walking distance. Results : The values of the patient's VAS score, SLR test, and walking distance measured before and after Hamstring Chuna Manual Therapy presented that the pain was relieved after Hamstring Chuna Manual Therapy. Conclusions : Therefore, this case demonstrates that Hamstring Chuna Manual Therapy is effective treatment method for relieving pain in the spine.

A Case Study of 2 Trigger Finger Patients using Burning Acupuncture Therapy (가열식 화침(火鍼)을 이용한 방아쇠수지 치험 2례)

  • Objectives : The purpose of this case study is to observe the effect of burning acupuncture therapy on the trigger finger. Methods : 2 patients were treated by burning acupuncture therapy to recover from trigger finger. The symptoms were evaluated by visual analog scale(VAS) and Tanaka score. Results : In both cases, VAS decreased to 0 and 1. Patients were evaluated as excellent and good by Tanaka score. Conclusions : Burning acupuncture therapy is effective to trigger finger.

The Clinical Analysis of Traffic Accident Patients Treated by Oriental Medicine (교통사고 후유증 환자의 임상적 분석과 한방치료효과)

  • Objectives : This study was designed to find out the characteristic of patients who had admitted due to traffic accident and to promote the superiority of oriental medical treatment on the sequelae of traffic accident. Methods : We analyzed 332 traffic accident patients statistically according to medical charts, who had been hospitalized from January 1,20000 to October 30, 2004. Results and Conclusions : The analysis showed that there was no significant difference in sex(p<0.05), but the older group and the shorter group of interval time from onset to addmission had the better remedial result(p<0.05). The most common diagnosis that had the better remedial result was lumbar sprain, followed by multiple contusion, cervical sprain, herniated lumbar disc, herniated cervical disc(p<0.05), and the most effective oriental medical treatment was moxibusition, followed by blood cupping therapy, taping therapy, chuna therapy except acupuncture, herbal medication, oriental physical therapy(p<0.05). In using of TENS, ICT, U/S, complex therapy had better remedial result than single therapy(p<0.05). The longer period of treatment group had better remedial result generally(p<0.05).

Focal Muscle Vibration Changes the Architecture of the Medial Gastrocnemius Muscle in Persons With Limited Ankle Dorsiflexion

  • Background: The gastrocnemius tightness can easily occur. Gastrocnemius tightness results in gait disturbance. Thus, various interventions have been used to release a tight gastrocnemius muscle and improve gait performance. Moreover, focal muscle vibration (FMV) has recently been extensively researched in terms of tight muscle release and muscle performance. However, no study has investigated the effects of FMV application on medial gastrocnemius architectural changes. Objects: In this study, we aimed to investigate the effects of FMV on medial gastrocnemius architecture in persons with limited ankle dorsiflexion. Methods: Thirty one persons with <10° of passive ankle dorsiflexion participated in this study. We excluded persons with acute ankle injury within six months prior to study onset, a history of ankle fracture, leg length discrepancy greater than 2 cm, no history of neurological dysfunction, or trauma affecting the lower limb. The specifications of the FMV motor were as follows: a fixed frequency (fast wave: 150 Hz) and low amplitude (0.3-0.5 mm peak to peak) of vibration; the motor was used to release the medial gastrocnemius for 15 minutes. Each participant completed three trials for 10 days; a 30-second rest period was provided between each trial. Medial gastrocnemius architectural parameters [muscle thickness (MT), fiber bundle length (FBL), and pennation angle (PA)] were measured via ultrasonography. Results: MT significantly decreased after FMV application (p < 0.05). FBL significantly increased from its baseline value after FMV application (p < 0.05). PA significantly decreased from its baseline value after FMV application (p < 0.05). Conclusion: FMV application may be advantageous in reducing medial gastrocnemius excitability following a decrease in the amount of contractile tissue. Furthermore, FMV application can be used as a stretching method to alter medial gastrocnemius architecture.
