Increase of Plasma IL-12/p40 Ratio Induced by the Combined Therapy of DNA Vaccine and Lamivudine Correlates with Sustained Viremia Control in CHB Carriers

  • We previously reported that $IFN-{\gamma}$ producing T cell responses induced by the combined therapy of DNA vaccine and lamivudine for one year are important for the induction of sustained virological response (SVR). However, $IFN-{\gamma}$ production is not sufficient to predict sustained viremia control in chronic hepatitis B (CHB) carriers treated. Methods: Twelve CHB carriers were intramuscularly immunized 12 times at a 4-week interval with 8mg of HBV DNA vaccine during the standard lamivudine treatment (100mg/daily/1 year). The level of cytokines during and after the combined therapy in plasma of all 12 CHB carriers treated was determined by each ELISA kit. Six out of 12 CHB carriers revisited the clinic, and their HBV DNA levels were examined. Results: The combined therapy increased plasma IL-12 and IL-12/p40 ratio during the treatment (baseline vs. peak level: $41.8{\pm}8.3$ vs. $163.1{\pm}29.2\;pg/ml$; p<0.01 and $0.96{\pm}0.25$ vs. $3.58{\pm}0.86$; p<0.01, respectively), and the peak level of plasma IL-12 and IL-12/p40 ratio was evoked at 6 to 10 months during the combined therapy. In particular, CHB carriers with SVR had two and three-fold higher level of the peak plasma IL-12 and plasma IL-12/p40 ratio than non-virological responders (NVRs), respectively ($218.0{\pm}41.4$ vs. $108.1{\pm}28.6\;pg/ml$; p=0.09 and $5.35{\pm}1.38$ vs. $1.80{\pm}0.29$; p<0.05, respectively), while p40 level was consistent during the combined therapy. In addition, there was no significant temporal correlation between the peak IL-12/p40 ratio and the elevation of serum alanine amino-transferase (ALT) in this study, contrast to $IFN-{\alpha}$ therapy which induced peak IL-12 level following ALT flares. Conclusion: Our results indicate that the combined therapy induces the increase of plasma IL-12 and IL-12/p40 ratio, which are associated with long-term SVR in CHB carriers.

IL-12 Regulates B7-H1 Expression in Ovarian Cancer-associated Macrophages by Effects on NF-κB Signalling

  • Background and Aim: B7-H1, a co-inhibitory molecule of the B7 family, is found aberrantly expressed in ovarian cancer cells and infiltrating macrophage/dendritic-like cells, and plays a critical role in immune evasion by ovarian cancer. IL-12, an inducer of Th1 cell development, exerts immunomodulatory effects on ovarian cancer. However, whether IL-12 regulates B7-H1 expression in human ovarian cancer associated-macrophages has not been clarified. Therefore, we investigated the effects of IL-12 on the expression of B7-H1 in ovarian cancer-associated macrophages and possible mechanisms. Methods: PMA induced THP-1-derived macrophages or human monocyte-derived macrophages were treated with recombinant IL-12 (rIL-12) or infected with adenovirus carrying human IL-12 gene (Ad-IL-12-GFP) for 24 h, then cocultured with the SKOV3 ovarian cancer cell line for another 24 h. Macrophages were collected for real-time PCR and Western blot to detect the expression of B7-H1, and activation of the NF-${\kappa}B$ signaling pathway. Moreover, supernatants were collected to assay for IL-12, IFN-${\gamma}$ and IL-10 by ELISA. In addition, monocyte-derived macrophages treated with IFN-${\gamma}$ were cocultured with SKOV3 and determined for the expression of B7-H1. Furthermore, the expression of B7-H1 in monocyte-derived macrophages was also evaluated after blocking NF-${\kappa}B$ signaling. Results: The expression of B7-H1 was significantly upregulated in monocyte-derived macrophages treated with rIL-12 or Ad-IL-12-GFP compared with the control groups (p<0.05), accompanied by a remarkable upregulation of IFN-${\gamma}$ (p<0.05), a marked downregulation of IL-10 (p<0.05) and activation of NF-${\kappa}B$ signaling. However, the upregulation of B7-H1 was inhibited by blocking the NF-${\kappa}B$ signaling pathway (p<0.05). Expression of B7-H1 was also increased (p<0.05) in monocyte-derived macrophages treated with IFN-${\gamma}$ and cocultured with SKOV3. By contrast, the expression of B7-H1 in THP-1-derived macrophages was significantly decreased when treated in the same way as monocyte-derived macrophages (p<0.05), and IL-10 was also significantly decreased but IFN-${\gamma}$ was almost absent. Conclusions: IL-12 upregulates the expression of B7-H1 in monocyte-derived macrophages, which is possible though inducing the secretion of IFN-${\gamma}$ and further activating the NF-${\kappa}B$ signal pathway. However, IL-12 downregulates the expression of B7-H1 in THP-1-derived macrophages, associated with a lack of IFN-${\gamma}$ and inhibition of expression of IL-10.

폐유육종증의 활동성 지표로서 IL-12의 효용성에 관한 연구 (The Value of Interleukin-12 as an Activity Marker of Pulmonary Sarcoidosis)

  • 배 경: 유육종증은 원인 모르게 비건락성 상피성 육아종이 전신에 발생하는 질환으로서 그 경과가 매우 다양하여 많은 환자들은 자연적으로 치유되기도 하나 일부 환자들은 계속 진행하여 호흡부전으로 사망하기도 하며 이러한 다른 결과가 초래되는 기전이나, 그 결과를 예측하는 지표는 아직 알려진 것이 없다. 유육종증은 병변내에서는 CD4+ T 임파구가 수적으로 증가할 뿐 아니라 그 작용이 과다하게 증가하며 이러한 병변내에서의 CD4+ T 임파구의 과다한 작용은 정확한 원인은 아직도 모르지만 이들이 oligoclonality를 가진 것으로 미루어 어떤 항원의 지속적인 자극에 기인할 것으로 추측하고 있다. 근래에 CD4+ T 임파구가 주로 IL-2 및 IFN-$\gamma$을 분비하고 지연성 과민 반응에 기여하는 Th1 세포와 IL-4, IL-5, IL-10을 분비하여 알레르기 반응에 관여하는 Th2 세포의 두 종류가 있다는 것이 밝혀졌고, 유육종증 병변 내 CD4+ T 임파구는 IL-2 및 IFN-$\gamma$를 분비하는 Th1 세포가 주인것으로 생각되고 있다. 또한 최근 미분화 T 세포들을 Th1 세포쪽으로 분화를 유도하는 데에 IL-12가 중요한 역할을 한다는 것이 알려졌으며, Mmoller등은 폐유유종증 환자에서 기관지폐포세척액(Bronchoalveolar lavage fluid : BALF) 내 IL-12가 증가한 것을 관찰하였다. 그러므로 폐유육종증의 병변의 계속적인 진행, 또는 자연치유 여부의 결정에 IL-12가 관여할 것을 추측할 수 있다. 이에 저자들은 IL-12가 유육종증의 활동성 지표로 사용될 수 있는지를 알아보기 위하여 본 연구를 시행하였다. 대 상: 조직학적으로 확진된 폐유육종증 환자 26명(남자: 10명, 여자: 16명, 연령: $38.7{\pm}10.6$세)과 정상 대조군 11명을 대상으로 진단 시 얻은 BALF 내 IL-12양과 BALF내 폐포대식세포 (Alveolar macrophage : AM)를 24 시간 배양 시 배양액 내로 분비되는 IL-12양을 ELISA 방법으로 측정 비교하였다. 폐유육종증 환자들중 12명이 임상적으로 비활동성이었고, 14명은 활동성이었다. 결 과: 폐유육종증 환자들의 BALF내 총세포수, AM 총수, 임파구백분율 및 총수, CD4/CD8 비가 대조군에 비해 유의하게 증가하였으나, 활동성 및 비활동성 환자군 사이에 유의한 차이는 없었다. BALF내 IL-12 농도는 폐유육종종 환자에서 $49.3{\pm}9.2$ pg/ml로 정상대조군의 $2.5{\pm}0.4$ pg/ml 보다 유의하게 (p<0.001) 높았고, 활동성 유육종증에서는 $70.3{\pm}14.8$ pg/ml로 비활동성 환자들의 $24.8{\pm}3.1$ pg/ml 보다 유의하게 높았다(p=0.001). BALF내의 IL-12 농도는 BALF내 AM백분율 (p<0.001), 임파구 백분율 (p<0.001) 및 임파구 총수 (p<0.001)와도 유의한 상관관계를 보였다. 또한 BALF내의 IL-12 농도는 최근에 유육종증의 활동성의 지표로 알려지고 있는 혈청 및 BALF내 sICAM-1 농도(각각 p<0.001, p=0.001), AM의 ICAM-1 발현도와도 (p<0.001) 유의한 상관관계를 보였다. AM에서의 IL-12분비량은 폐유육종증 환자들에서 $206.2{\pm}61.9$ pg/ml로 정상대조군의 $68.3{\pm}43.7$ pg/ml 보다 높았으며 (p<0.008), 활동성 및 비활동성 환자군간 유의한 차이는 없었다. 결 론: 이상의 결과로 BALF 내 IL-12 농도는 폐내 염증 세포의 침윤 정도를 반영하며, 유육종증의 활성도를 예측하는 지표로서 유용할 것으로 사료되었다.

Antitumor Activity of Lentivirus-mediated Interleukin -12 Gene Modified Dendritic Cells in Human Lung Cancer in Vitro

  • Objectives: Dendritic cell (DC)-based tumor immunotherapy needs an immunogenic tumor associated antigen (TAA) and an effective approach for its presentation to lymphocytes. In this study we explored whether transduction of DCs with lentiviruses (LVs) expressing the human interleukin-12 gene could stimulate antigen-specific cytotoxic T cells (CTLs) against human lung cancer cells in vitro. Methods: Peripheral blood monocyte-derived DCs were transduced with a lentiviral vector encoding human IL-12 gene (LV-12). The anticipated target of the human IL-12 gene was detected by RT-PCR. The concentration of IL-12 in the culture supernatant of DCs was measured by ELISA.Transduction efficiencies and CD83 phenotypes of DCs were assessed by flow cytometry. DCs were pulsed with tumor antigen of lung cancer cells (DC+Ag) and transduced with LV-12 (DC-LV-12+Ag). Stimulation of T lymphocyte proliferation by DCs and activation of cytotoxic T-lymphocytes (CTL) stimulated by LV-12 transduced DCs pulsed with tumor antigen against A549 lung cancer cells were assessed with methyl thiazolyltetrazolium (MTT). Results: A recombinant lentivirus expressing the IL-12 gene was successfully constructed. DC transduced with LV-12 produced higher levels of IL-12 and expressed higher levels of CD83 than non-transduced. The DC modified by interleukin -12 gene and pulsed with tumor antigen demonstrated good stimulation of lymphocyte proliferation, induction of antigen-specific cytotoxic T lymphocytes and antitumor effects. Conclusions: Dendritic cells transduced with a lentivirus-mediated interleukin-12 gene have an enhanced ability to kill lung cancer cells through promoting T lymphocyte proliferation and cytotoxicity.

정신분열병 환자에서 Interleukin-12와 Transforming Growth Factor Beta 1의 치료 전후의 변화 (Changes of Interleukin-12 and Transforming Growth Factor Beta 1 before and after Antipsychotic Treatments in Schizophrenic Patients)

  • 목 적: 많은 연구에서 정신분열병에서 염증반응체계의 활성화와 사이토카인의 변화가 병태생리학적 및 원인적 역할을 하는 것으로 보고되어 왔으며, 여기에는 type 1 Thelper cell(Th1), type 2 T helper cell(Th2), type 3 T helper cell(Th3)의 조절 이상이 제시되고 있다. 본 연구에서는 정신분열병 환자에서 항정신병 약물 치료 전후로 Th1 사이토카인인 interleukin-12(IL-12), Th3 사이토카인인 transforming growth factor-${\beta}1$(TGF-${\beta}1$)의 혈장 농도를 측정하였다. 방 법: 23명의 정신분열병 환자군과 31명의 정상대조군에서 IL-12와 TGF-${\beta}1$ 농도를 측정하였고 정신분열병 환자군에서는 8주간 항정신병 약물로 치료 후 다시 IL-12와 TGF-${\beta}1$의 농도를 측정하였다. 또한 정신분열병 환자군에서 치료전과 8주간 치료 후, 2차례에 걸쳐 Brief psychiatric rating scale(BPRS)를 측정하였다. 결 과: 치료전 IL-12 농도와 TGF-${\beta}1$ 농도 모두 정상대조군보다 환자군에서 유의하게 높게 나타났다. 8주간의 치료 후 TGF-${\beta}1$ 농도는 유의하게 감소하여 정상대조군의 농도와 차이를 보이지 않게 된 반면, IL-12의 농도는 유의하지 않은 감소를 보였다. BPRS 점수의 변화 및 IL-12 및 TGF-${\beta}1$의 농도의 변화 사이에는 유의한 상관관계가 없었다. 결 론: 정신분열병의 병태생리학에 사이토카인의 이상이 관여할 수 있으며, TGF-${\beta}1$이 중요한 역할을 하는 것으로 생각된다.

일황련(日黃連)이 치과병태(痴果病態)모델에 미치는 영향(影響) (The Effects of Coptis japonica Makino(CJM) Extract on the Alzheimer's Disease Model)

  • This experiment was designed to investigate the effect of Coptis japonica Makino(CJM) on the Alzheimer's disease. The effects of CJM extract on $IL-1{\beta}$, IL-6, amyloid precursor proteins (APP), acetylcholinesterase(AChE), glial fibrillary acidic protein(GFAP) mRNA of PC-12 cell treated by $A{\beta}$ plus $rIL-1{\beta}$ and AChE activity of PC-12 cell lysate treated by $A{\beta}$ plus $rIL-1{\beta}$ and behavior of memory deficit rats induced by scopolamine and mice glucose, uric acid, AChE activity of memory deficit rats induced by scopolamine were investigated, respectively. The results were summarized as follows ; 1. CJM extract suppressed $IL-1{\beta}$, IL-6 mRNA in PC-12 cell treated by $A{\beta}$ plus $rIL-1{\beta}$ 2. CJM extract suppressed APP, AChE, GFAP mRNA in PC-12 cell treated by $A{\beta}$ plus $rIL-1{\beta}$ 3. CJM extract suppressed AChE activity in cell lysate of PC-12 cell treated by $A{\beta}$ plus $rIL-1{\beta}$ 4. CJM extract group showed significantly inhibitory effect on the memory deficit of mice induced by scopolamine in the experiment of Morris water maze. 5. CJM extract increased glucose, decreased uric acid and AChE significantly in the serum of the memory deficit rats induced by scopolamine. According to the above results, it is suggested that CJM extract might be usefully applied for prevention and treatment of Alzheimer's disease and memory deficit symptom.

호흡곤란증후군과 기관지폐이형성증 예측 인자로서의 미숙아 제대혈 IL-10, IL-12 (Cord blood IL-10, IL-12 in preterm newborns as predictors of respiratory distress syndrome and bronchopulmonary dysplasia)

  • 목 적 : 최근 RDS나 BPD이 폐의 미성숙함 뿐만 아니라 손상과 재생, 염증 반응 기전이 관여할 것이라는 주장과 연구들이 대두되고 있다. 이에 본 연구에서는 미숙아의 제대혈에서 IL-10, IL-12를 측정하여 RDS, BPD 발생의 관련성을 살펴보고자 하였다. 방 법 : 2003년 1월부터 2005년 6월까지 임신 중기부터 이대 목동 병원에서 산전 관리를 받고 보호자 동의하에 제대혈 검사가 가능하였던 재태 기간 34주 미만의 미숙아 40명을 대상으로 하였다. 출생 당시 제대혈을 채취하여 혈청 분리 후 영하 $70^{\circ}C$에 보관하였다가 ELISA법을 이용하여 IL-10, IL-12를 측정하였고, 각 주수 기간에 따라 RDS가 있는 군(RDS군)과 RDS가 없는 군(non-RDS군)으로, 그리고 RDS군내에서 BPD가 있는 군(BPD군)과 BPD가 없는 군(non-BPD군)으로 나누어 IL-10, IL-12 및 임상자료들을 비교하고 분석하였다. 이들의 임상적인 자료는 의무기록을 통하여 후향적인 방법으로 얻었다. 결 과 : IL-10은 재태 연령(r=-0.391, P=0.005)이나 출생체중(r=-0.442, P=0.001)과 역의 상관관계가 있었고, IL-12는 재태 연령이나 출생체중과는 상관관계가 없었다. 30-34주 사이의 미숙아를 대상으로 보았을 때 RDS군에서 non-RDS군에 비해 의미 있게 높은 IL-10수치를 나타냈다(1.0 vs 0.1 pg/mL; P=0.001). 하지만 IL-12는 두 군 간의 유의한 차이가 없었다. BPD군과 non-BPD군을 비교하면 IL-10, IL-12 모두 통계적으로 유의한 차이가 없었다. 결 론 : 제대혈 IL-10 농도는 태아의 성숙도를 예측하는 지표로 사용될 수 있을 것으로 생각되고, 높은 IL-10의 수치로 RDS로의 이환을 예측하는 지표로 사용할 수 있을 것으로 생각된다. 그러나 제대혈 IL-10이 BPD로의 이환을 예측하기는 어려울 것으로 보이며, 이에 대한 연구가 더 필요할 것으로 보인다.

면역기능 증강 신물질에 대한 마우스의 면역학적 및 혈액학적 변화 (Changes of immunostimulatory effects by Immu-Forte on mice)

  • Immu-Forte composed of chitosan, ${\beta}-glucan$, manno-oligosaccharide and pangamic acid was evaluated for its effectiveness as a nonspecific immunostimulator in mice. The effects of Immu-Forte were determined by analysis of cytokines using ELISA and phenotype of leukocyte subpopulations using monoclonal antibodies specific to mouse leukocyte differentiation antigens and flow cytometry. All T cells, all B cells, CD4 T cells, CD8 T cells, macrophages, IL-2, IL-4, IL-12 and IFN-r in Immu-Forte A-treated group increased in 1 months posttreatment and were significantly higher (p < 0.05) than that of control at 1 months posttreatment. All T cells, all B cells, CD4 T cells, CD8 T cells, macrophages and IL-2 in Immu-Forte EX-treated low and middle dose groups increased in 1 months posttreatment and were significantly higher (p < 0.05) than that of control at 1 months posttreatment. In the Immu-Forte soybean-treated group, NK cells and IL-4 were significantly higher in middle dose-treated group, and IL-2, IL-4 and IFN-r were significantly higher in low dose-treated group. In the Immu-Forte F-treated group, all T cells, all B cells, CD4 T cells, CD8 T cells, macrophages, NK cells, IL-2, IL-4, IL-12 and IFN-r in high dose-treated group and all T cells, all B cells, CD4 T cells, CD8 T cells, macrophages, IL-2, IL-4, IL-12 and IFN-r in middle dose-treated group and NK cells, IL-2, IL-4, IL-12 and IFN-r in low dose-treated group were significantly higher (p < 0.05) than that of control at 1 months posttreatment. In conclusion, this study has demonstrated that Immu-Forte had an immunostimulatory effect on mice through proliferation and activation of mouse immune cells.

Udenafil Induces the Hair Growth Effect of Adipose-Derived Stem Cells

  • Udenafil, which is a $PDE_5$ inhibitor, is used to treat erectile dysfunction. However, it is unclear whether udenafil induces hair growth via the stimulation of adipose-derived stem cells (ASCs). In this study, we investigated whether udenafil stimulates ASCs and whether increased growth factor secretion from ASCs to facilitate hair growth. We found that subcutaneous injection of udenafiltreated ASCs accelerated telogen-to-anagen transition in vivo. We also observed that udenafil induced proliferation, migration and tube formation of ASCs. It also increased the secretion of growth factors from ASCs, such as interleukin-4 (IL-4) and IL12B, and the phosphorylation of ERK1/2 and $NF{\kappa}B$. Furthermore, concomitant upregulation of IL-4 and IL12B mRNA levels was attenuated by ERK inhibitor or $NF{\kappa}B$ knockdown. Application of IL-4 or IL12B enhanced anagen induction in mice and increased hair follicle length in organ culture. The results indicated that udenafil stimulates ASC motility and increases paracrine growth factor, including cytokine signaling. Udenafil-stimulated secretion of cytokine from ASCs may promote hair growth via the ERK and $NF{\kappa}B$ pathways. Therefore, udenafil can be used as an ASC-preconditioning agent for hair growth.

하고초 약침이 LPS로 유발된 급성염증 백서 모델에 미치는 영향 (Effects of Prunella vulgaris Pharmacopuncture on Lipopolysaccharide-Induced Acute Inflammatory Rat Model)

  • Objectives : To investigate the anti-inflammatory effects of Prunella vulgaris pharmacopuncture in lipopolysaccharide (LPS)-induced inflammatory rat model. Methods : Sprague-Dawley rats were divided into 5 groups; normal control (n=8), LPS control (n=8), LPS+Prunella vulgaris pharmacopuncture at CV4 (CV4, n=8), LPS+Prunella vulgaris pharmacopuncture at ST36 (ST36, n=8), and LPS+Prunella vulgaris pharmacopuncture at CV12 (CV12, n=8). Pharmacopuncture was given every two days for 4 weeks followed by inflammation induction by peritoneal LPS injection (5mg/kg). Proinflammatory cytokines including interleukin-$1{\beta}$ (IL-$1{\beta}$), interleukin-6 (IL-6), tumor necrosis factor-$\alpha$ (TNF-$\alpha$), interleukin-10 (IL-10), thiobarbituric acid reactive substance (TBARS) from blood and liver tissue were compared before and 5 hrs after inflammation induction. Results : In CV4 and CV12 groups, plasma IL-$1{\beta}$, IL-6 and TNF-$\alpha$ levels increased by LPS injection, significantly decreased 5 hrs after injection (p<0.05). For CV12 group, plasma IL-10 concentration significantly increased (p<0.05). Liver IL-$1{\beta}$ and IL-6 levles significantly decreased in CV4 and CV12 groups (P<0.05), while normal and LPS control groups were not significantly different in TNF-$\alpha$ and IL-10 levels. Plasma TBARS concentration was significantly decreased in CV12 group, while there was no significant difference among LPS control and pharmacopuncture groups for liver TBARS concentration. Conclusions : Based on the present findings, Prunella vulgaris pharmacopuncture at CV12 may have a potentially preventive anti-inflammatory effect in an LPS-induced inflammatory rat model.
