The Effect of the Environmental Nursing Intervention on ICU Delirium, Environmental Stress and Sleep in Patients Underwent Cardiac Surgical Procedures (환경적 간호중재가 개심술 환자의 중환자실섬망증, 환경적 스트레스 및 수면에 미치는 효과)

  • Purpose: To examine the effects of the environmental nursing intervention using eye-shields and earplugs on the incidence of delirium, environmental stress and sleep among the patients with open-heart surgery in the post-cardiac ICU(intensive care units). Method: This study was a non-equivalent control group non-synchronized design. The subjects consisted of 93 patients who received ICU care after the cardiac surgery. Among those, 32 subjects was assigned in one experimental group with eye-shields, 31 in the other experimental group with earplugs, and 30 in the control group. ICU delirium was measured by CAM-ICU, environmental stress by ICUESS, and sleep by two sleep scales developed by Oh, et al. and Kim. The data were analyzed by the SPSS 13.0 program including frequency, percentage, t-test, $x^2$ test, ANOVA and ANCOVA. Results: There are no statistically significant differences in the incidence of delirium among three groups (p=.139). The ICU environmental stress scores from the subjects of two experimental groups demonstrated lower (F=6.731, p=.002) than the control group. Also, the degrees of sleep pattern (p=.000, p=.000) and sleep satisfaction (p=.000, p=.000) were greater than those of the control group. Conclusion: Although the environmental nursing intervention using eye-shields and earplugs did not demonstrate the effects in decreasing the incidence of ICU delirium, it has affirmative effects in decreasing environmental stress and improving the quality sleep among the patients with open-heart surgery while they stay in ICU. Further nursing implications are discussed.

Analysis of Nursing Records for Pain Management in Intensive Care Unit Patients (내·외과계 중환자의 통증간호기록 분석)

  • Purpose: The purpose of this study was to analyze nursing records for pain management in intensive care unit (ICU) patients. Methods: Nursing process for pain management were analyzed retrospectively by 180 ICU patients' nursing records. Instruments consisted of 3 questionnaires (pain assessment, intervention, and evaluation). Results: For assessment, there was different pain intensity between cancer patients (7.95) and non-cancer patients (7.20). Also pain intensity was lower in PCA group (5.08) than in PCA with PRN group (8.27). Common pain site was surgical areas, along with 17 kinds of words expressed for pain, and mean of pain intensity was 7.47 by numeric rating scales (NRS). For intervention, the patients received pharmacologic interventions (99.4%) such as narcotic analgesics (38.3%) intermittently (70.5%) without side effects (94.4%). For evaluation, mean of pain intensity was decreased to 3.14, but a few patients (12.8%) experienced pain over 5 points despite the intervention. Nurses evaluated the degree of pain relief after the intervention in 87.2% of patients. Conclusion: Nurses do assess patients' pain by using objective tool, intervene, and evaluate for effective pain management. Nurses should make an individual approach and record all nursing activities for pain management.

  • Nurses' oral health awareness and oral health status and learn about better oral health care and further explore ways to improve awareness of oral care supplies to patients to oral care can help to establish a basis for is for the sole purpose. Seoul, Gyeonggi, Gyeongsang, Jeolla, Chungcheong located in 228 people in the ICU nurses using a questionnaire on oral health awareness and management behavior were investigated. Using oral hygiene aids to living in a residential home, according to the respondents from the two was significant association (p<0.05), oral hygiene, tooth brushing behavior and the use of aids in the relevance of the educational experience, depending on how brushing brushing, depending on how experienced oral there was significant association with the use products, dental products, depending on the path of tooth brushing training methods used were relevant and significant, depending on the state of your oral health, dental supplies used was associated with a significant (p<0.05).

Perception of Clinical Ladder System, Professional Self-Concept, Self-Efficacy, and Organizational Commitment by the Level of Clinical ladder among Intensive Care Unit Nurses (중환자실 간호사의 경력개발제도에 따른 경력개발제도의 인식, 전문직 자아개념, 자기효능감, 조직몰입)

  • Purpose : This study aimed to verify the effectiveness of the clinical ladder system (CLS) by identifying the perception of the CLS, professional self-concept, self-efficacy, and organizational commitment among intensive care unit (ICU) nurses. Methods : Data were collected through a questionnaire survey of 173 nurses working in 11 ICUs at a tertiary hospital in Seoul. Results : The perception of the CLS, self-efficacy, and organizational commitment showed significant difference according to the CLS level. As a result of the partial Spearman correlation analysis after adjusting for demographic difference according to CLS (age, marital status, education, and clinical career), CLS positively correlated with the perception of the CLS (r=.16, p=.045), professional self-concept (r=.24, p=.001), and self-efficacy (r=.21, p=.007). On the contrary, organizational commitment negatively correlated with the perception of the CLS (r=-.43, p<.001) and self-efficacy (r=-.32, p<.001). Conclusion : The CLS is effective for professional self-concept and self-efficacy. However, specialists 2 nurses, who had high professional self-concept and self-efficacy, showed low level of organizational commitment. Thus, it is necessary to examine the factors of work burden and career plateau that are imposed after promotion and improve experience and planning according to the growth needs. Moreover, regular evaluation of the CLS will require hospital support to ensure that ICU nurses appreciate and actively participate in the support.

Factors Influencing Stress of Nurse who care for patients using a Home Mechanical Ventilator in General Ward (병동 간호사의 가정용 인공호흡기 적용 환자 간호 스트레스 영향요인)

  • Purpose: The purpose of this study was to identify factors associated with stress related to home mechanical ventilator (HMV) care in general ward nurses. Methods: The study participants were 110 general ward nurses. Data on participant characteristics, level of knowledge, education needs, coping ability in emergency situations, confidence, and stress were collected from August 1 to 30, 2018 using a structured questionnaire by web-based surveys. Data were analyzed using SPSS/WIN 20.0 for descriptive statistics and independent t-test, one-way analysis of variance, Pearson's correlation coefficient, and multiple regression analysis. Results: Significant factors associated with stress related to HMV care were ward career, intensive care unit (ICU) career, intensive care room (ICR) career, education experience, and satisfaction level of HMV education. Stress had negative correlations with confidence and positive correlations with education needs. The determining factors affecting stress related to HMV care in the general ward were confidence (${\beta}=-.31$, p=.004), ICR career (${\beta}=-.27$, p<.001), education needs (${\beta}=.24$, p=.005), education frequency (${\beta}=-.18$, p=.040), and ICU career (${\beta}=-.18$, p=.025); their explanation power was about 41.8%. Conclusions: It is necessary to develop HMV care training manuals and guidelines and consider ICU or ICR careers for patient safety.

Study on an Unplanned Extubation Prevention Program (UEPP) for Intubated Patients in Intensive Care Unit (비계획적 발관 예방을 위한 교육프로그램 적용효과)

  • Unplanned extubation prevention program (UEPP) was developed and applied to intubated patients hospitalized in a medical intensive care unit (ICU). The number of unplanned extubation was 29 during the preliminary investigation and 23 cases during the prevention intervention program. The general features of the patients and nurses who experienced UE did not have any significant differences between the two periods, with only the number of new nurses in the ICU being significantly different (p=.039). Although the number of new nursing staff in the ICU was high, the mean rate of unplanned extubation, which is the number of UE days per 1000 intubation days, was 8.8 during the prevention intervention program, which was significantly lower than 12.3, the mean rate of UE during the preliminary investigation (t=-2.333, p=.040). The results provide a prevention program that could contribute to the safety of patients by actively preventing unplanned extubation in intubated patients in ICU.

Nurses' Experiences of Ethical Dilemmas and their Coping Behaviors in Intensive Care Units (중환자실 간호사가 경험하는 윤리적 딜레마와 대처행위)

  • Purpose: This study was aimed to describe ethical dilemmas and types of coping behaviors among nurses who worked in intensive care units (ICUs). Methods: Data were collected by 2 focus group interviews with 12 ICU nurses in an academic affiliated hospital in Seoul, Korea. All interviews were tape-recorded and transcribed, and data were analyzed by modified qualitative content analysis. Results: Three themes emerged from the focus group interviews: "Respect for Persons (2 contents)", "Beneficence (13 contents)", "Justice (1 content)". Coping behaviors against the dilemmas were consultations with the doctors or colleagues, acceptance, guilt, reflection, forgetting, endurance, and frustration. Conclusion: The results of this study help us to understand ethical dilemmas that nurses experienced in ICUs and their coping behaviors. It would be useful to develop education programs for nurses in ICUs to support coping strategies for ethical dilemmas.

  • Purpose: This study was to investigate intensive care unit (ICU) nurse's knowledge and nursing performance on the intensive care unit syndrome. Method: A survey questionnaire was administered to a convenience sample of 123 nurses in a university hospital. Data were collected from August 1st to August 25th, 2009. Data were analyzed using SPSS/WIN 12.0. Results: The mean score for knowledge and nursing performance were66.96 and 74.97 respectively. According to the general characteristics, there were significant differences in knowledge depending on marital status, education level and career length and in nursing performance depending on their age, education level and career length. The correlation between knowledge and nursing performance was a significant positive one. Conclusion: Comprehensive educational programs are needed to decrease accident rates related to ICU syndrome and to improve the health of ICU patients.

  • The purpose of this study is to develop a patient care standard which is the basis of unit based quality assurance. The subjects were 570 nurses of 6 hospitals is Seoul. Patient Care Standards were developed from 3 times of clinical Nurses Association's workshop & the joint meeting of Clinical Nurses Association & the Korean Nurses Academic Socity of Nursing Administration. Respondents were instructed to rate of the 2 types of 5 - point Likert type questionnaire(one is the level of perceived importance, the other is the level of actual performance) Findings of this study were as follows 1. As a results of reliability analysis, each questionnaire ranged from $\alpha$=0.79 to 0.94

  • The Relationship between the Work Environment and Person-centered Critical Care Nursing for Intensive Care Nurses (중환자실 간호사의 근무환경과 인간중심 간호의 관계)

    • Purpose : The purpose of this study was to investigate the effect of the perceived nursing work environment (NWE) on person-centered critical care nursing (PCCN) practices among nurses in intensive care units (ICUs). Methods : A cross-sectional research design was adopted for this study; participants were 126 ICU nurses working in five hospitals in B city. From December 2018 to February 2019, a survey questionnaire on NWE and PCCN were conducted. The collected data were analyzed with a multiple hierarchical regression analysis. Results : The mean score for NWE was $2.98{\pm}0.39$. Among the factors, basic work system was the highest ($3.39{\pm}0.41$), followed by leadership of the head nurse ($3.31{\pm}0.55$), interpersonal relationships ($2.77{\pm}0.62$), and institutional support ($2.58{\pm}0.52$). The mean score for PCCN was $3.52{\pm}0.46$. Among the factors, comfort was the highest ($3.77{\pm}0.62$), followed by respect ($3.55{\pm}0.56$), compassion ($3.43{\pm}0.60$), and individuality ($3.38{\pm}0.56$). The interpersonal relationship (${\beta}=.31$, p=.001) and the number of patients per shift (${\beta}=-.23$, p=.005) had a significant effect on PCCN, accounting for 25.9% of the total variance. Conclusion : These results suggest that it is necessary to enhance the relationship-oriented culture and reduce the workload of nurses in order to improve the quality of nursing care in ICUs.
