Effect of 2-Methylaminoethyl-4,4'-Dimethoxy-5, 6, 5' ,6'-Dimethyl­enedioxybiphenyl-2-Carboxylic Acid-2'-Carboxylate Monohydro­chloride (DDB-S) on Indocyanine Green (ICG) Clearance in Rats

  • The clearance of ICG, a known hepatic blood flow marker was investigated in rats in order to examine whether DDB-S influences hepatic blood flow. The effect of DDB-S on the protein binding and blood-to-plasma partition of ICG was measured. The steady-state plasma concentration of ICG was monitored before and after co-administration of various concentration of DDB-S, and ICG clearance was estimated from the steady-state concentration and the infusion rate of ICG. There was no significant difference in protein binding and blood-to-plasma partition of ICG with and without addition of DDB-S (10, 20, and 40 ${\mu}g/mL)$. When ICG was infused into DDB-S pretreated rats, the steady-state concentrations of ICG decreased and the calculated ICG clearance increased. However, no dose-dependency of ICG Css on DDB-S Css was observed. Since DDB-S did not affect the protein binding and blood-to-plasma partition of ICG, the increased clearance of ICG with co-administration of DDB-S seems to be due to the increased hepatic blood flow by DDB-S.

Indocyanine Green Excretion Test in Korean Black Goats (한국 흑염소에 있어서 Indocyanine Green 배설시험)

  • This experiment was performed in order to establish a proper method to determine the half life of indocyanine green(ICG T$\frac{1}{2}$) and its fractional clearance rate(KICG) and investigate the applicability of indocyanine green ( ICG ) excretion test in hepatotoxicity experiment in Korean black goats. The results were as follows : 1. Maximum absorbance of ICG in plasma was at 810 nm in this experiment. 2. The coefficient of correlation between the results obtained by standard method and potassium cyanide method was 0.99 and the regression equation between two methods was y=0.9996 x+0.0065. 3. As the disappearance curve of ICG plotted in semi-log graph revealed linear pattern at least for 6 minutes after injection, the postinjection blood samples were decided to collect at 2 and 6 minutes after ICG injection. 4. ICG T$\frac{1}{2}$ and KICG values were not affected by dose level of ICG. 5. When 0.25 mg of ICG per kg body weight was administered the normal data of ICG T$\frac{1}{2}$ and KICG in Korean black goa were 1.468${\pm}$0.197 minutes(mean${\pm}$SD) and 0.482${\pm}$0.076/minutes respectively. 6. After administration of carbon tetracholride. the ICG T$\frac{1}{2}$ started to increase acutely from day 1, revealed the peak at day 3, and then returned almostly but not completely to preinjection level at day 14. The ICG T$\frac{1}{2}$ value was suggested to be a sensitive indicator of hepatic injury in Korean black goats.

Indocyanine Green Videoangiography for Confirmation of Bypass Graft Patency

  • Objective : The aim of the study is to determine the efficacy of indocyanine green (ICG) videoangiography for confirmation of vascular anastomosis patency in both extracranial-intracranial and intracranial-intracranial bypasses. Methods : Intraoperative ICG videoangiography was used as a surgical adjunct for 56 bypasses in 47 patients to assay the patency of intracranial vascular anastomosis. These patients underwent a bypass for cerebral ischemia in 31 instances and as an adjunct to intracranial aneurysm surgery in 25. After completion of the bypass, ICG was administered to assess the patency of the graft. The findings on ICG videoangiography were then compared to intraoperative and/or postoperative imaging. Results : ICG provided an excellent visualization of all cerebral arteries and grafts at the time of surgery. Four grafts were determined to be suboptimal and were revised at the time of surgery. Findings on ICG videoangiography correlated with intraoperative and/or postoperative imaging. Conclusion : ICG videoangiography is rapid, effective, and reliable in determining the intraoperative patency of bypass grafts. It provides intraoperative information allowing revision to reduce the incidence of technical errors that may lead to early graft thrombosis.

Indocyanine green excretion test and changes of plasma enzyme activites in dogs (개에서의 indocyanine green 배설시험 및 혈장효소 활성치의 변화)

  • This experiment was carried out to establish a proper method of indocyanine green(ICG) excretion test for a applicable liver function test in dogs. The half life(T1/2), fractional clearance rate(KICG) and retention rate after injection of ICG with or without administred carbon tetrachloride($CCl_4$) were also invested. The results obtained were as follows ; 1. The maximum absorbance of ICG in plasma was at 810nm. 2. Half life and fractional clearance rate when administered 0.25 and 0.50mg of ICG per Kilogram body weight were $6.33{\pm}0.58$ minutes and $0.11{\pm}0.99$/minute in the former, $10.01{\pm}1.0$ minutes and $0.07{\pm}0.007$/minute in the latter, respectively. The ICG removal rate was exponentially linear for the first 15 minutes after injection both, of 0.25 and 0.50mg of ICG. 3. One day following the administration of $0.0042m{\ell}\;CCl_4$ kilogram body weight which injected 0.50mg of ICG, half life was more longer and fractional clearance rate was significantly reduced than that of ICG single injection. 4. Plasma retention rate when 15, 30, 45 minutes after injection dose of 0.25 and 0.50mg ICG per Kilogram body weight, $14.7{\pm}4.8$, $5.1{\pm}3.1$, $2.6{\pm}1.6%$ in the former, $26.9{\pm}1.8$, $11.1{\pm}2.4$, $4.8{\pm}1.3%$ in the latter, respectively. However, after administration of $CCl_4$, plasma retention rate of ICG at a dose of 0.50mg, it was $39.3{\pm}0.9$, $16{\pm}2.9$, $10.7{\pm}0.1%$, respectively. 5. Plasma enzyme(AST, ALT, r-GTP) activities administered with $CCl_4$ were increased, but there was no change which injected any dose of single ICG injection. From these results, ICG excretion test to dog is applicable to evaluation of liver function in both clinical and research.

No Effect of Diltiazem on the Hepatic Clearance of Indocyanine Green in the Rats

  • In order to investigate the effect of the pretreatment with various doses of diltiazem (DTZ) on the pharmacokinetics of indocyanine green (ICG) at steady state, especially the hepatic blood clearance due to the change of hepatic blood flow, the following experiments were carried out with ICG, a hepatic function test marker, not metabolized in liver and only excreted in bile. The intravenous bolus injection ($3,780\mu\textrm{g}$/kg) and the constant-rate infusion ($10,100\mu\textrm{g}$/kg/hr) of ICG into the left femoral vein were made in order to check the steady-state plasma concentration ($C_{ss} of $10\mu\textrm{g}$/ml) of ICG at 20, 25 and 30 min. Following a 90-min washout period, the intravenous bolus injection (108, 430, 860 and $1,720\mu\textrm{g}$/kg) and the constant-rate infusion (108, 433, 866 and $1,730\mu\textrm{g}$/kg/hr) of DTZ into the right femoral vein were made and the achievement of the steady-state plasma levels ($C_{ss} of 50, 200, 400 and 800 ng/ml) of DTZ were conformed at 60, 70 and 80 min. During the steady state of DTZ, the intravenous bolus injection ($3,780\mu\textrm{g}$/kg) and the constant-rate infusion ($10,200\mu\textrm{g}$/kg/hr) of ICG into the left femoral vein were made and also the steady-state plasma concentration of ICG was checked at 20, 25 and 30 min. The plasma concentrations of DTZ and ICG were determined using a high performance liquid chromatographic technique. At the steady state, the hepatic blood clearance of ICG was obtained from the plasma concentration and blood-to-plasma concentration ratio ($R_B$) of ICG. The pretreatment with various doses of DTZ did not influence the plasma concentrations, $R_B$ and plasma free fraction ($f_p$) of ICG. So the hepatic blood clearance of ICG was independent of concentration of DTZ. The hepatic blood clearance of ICG could be affected by both hepatic bood flow and hepatic intrinsic clearance. But there was no change of the hepatic blood clearance of ICG between the control and the DTZ-pretreated rats in this study. So it may be suggested that DTZ does not influence hepatic blood flow.

Value of Indocyanine Green Videoangiography in Deciding the Completeness of Cerebrovascular Surgery

  • Objective : Recently, microscope-integrated near infrared indocyanine green videoangiography (ICG-VA) has been widely used in cerebrovascular surgery because it provides real-time high resolution images. In our study, we evaluate the efficacy of intraoperative ICG-VA during cerebrovascular surgery. Methods : Between August 2011 and April 2012, 188 patients with cerebrovascular disease were surgically treated in our institution. We used ICG-VA in that operations with half of recommended dose (0.2 to 0.3 mg/kg). Postoperative digital subtraction angiography and computed tomography angiography was used to confirm anatomical results. Results : Intraoperative ICG-VA demonstrated fully occluded aneurysm sack, no neck remnant, and without vessel compromise in 119 cases (93.7%) of 127 aneurysms. Eight clipping (6.3%) of 127 operations were identified as an incomplete aneurysm occlusion or compromising vessel after ICG-VA. In 41 (97.6%) of 42 patients after carotid endarterectomy, the results were the same as that of postoperative angiography with good patency. One case (5.9%) of 17 bypass surgeries was identified as a nonfunctioning anastomosis after ICG-VA, which could be revised successfully. In the two patients of arteriovenous malformation, ICG-VA was useful for find the superficial nature of the feeding arteries and draining veins. Conclusion : ICG-VA is simple and provides real-time information of the patency of vessels including very small perforators within the field of the microscope and has a lower rate of adverse reactions. However, ICG-VA is not a perfect method, and so a combination of monitoring tools assures the quality of cerebrovascular surgery.

In Vivo Enhanced Indocyanine Green-Photothermal Therapy for a Subconjunctival Tumor

  • Indocyanine green (ICG) is a dye approved for use in clinical diagnostics. ICG remains in the intravascular space following intravenous administration, due to its ability to rapidly bind to the plasma proteins, and its therapeutic potential has been studied in well-vascularized cutaneous tumors. Here we have evaluated the clinical response of a subconjunctival tumor to photothermal therapy (PTT) using an ICG-enhanced near-infrared diode laser and its adverse effects, in a rabbit. 22 male New Zealand white rabbits with subconjunctival tumors were enrolled (control group 6, laser-only group 8, laser-with-ICG group 8). Rabbits in the laser-with-ICG group received ICG (twice, 2 mg/kg each time, intravenously) directly followed by irradiation with a diode laser (λ = 810 nm). Rabbits in the laser-only group were irradiated with the diode laser. ICG angiography, ultrasonography, and pathologic examination were performed to evaluate PTT response at specific time points (0, 2, and 4 weeks after PTT). Two weeks after initial treatment, the eight rabbits treated by laser with ICG showed a 100% response rate. There was no clinical response in both laser-only and control groups. ICG-PTT is a potential and effective palliative therapeutic modality for subconjunctival tumors.

Streptococcus Mutans Biofilm Inhibition Effect of Indocyanine Green and Near Infrared Diode Laser (Indocyanine Green과 근적외선 다이오드 레이저의 Streptococcus mutans 세균막에 대한 억제 효과)

  • The purpose of this study was to evaluate the effect of Indocyanine Green (ICG) and near-infrared (NIR) diode laser on Streptococcus mutans biofilms depending on ICG concentrations. S. mutans biofilms were formed on a Hydroxyapatite disk, and 0.5, 1.0, 2.0, 3.0, 4.0, 5.0 mg/mL ICG solutions dissolved in sterile distilled water and a NIR diode laser having a power of 300 mW and a wavelength of 808 nm were applied to the biofilms. The temperature changes of the biofilm surface according to the concentrations of the ICG solution were measured using a 1-channel thermocouple thermometer. Compared to the control group, in the groups with only the 3.0, 4.0, 5.0 mg/mL ICG solution application, and in the groups with the 1.0, 2.0, 3.0, 4.0, 5.0 mg/mL ICG solution application and light irradiation, a statistically significant decrease in the bacterial counts were observed. The temperature increase according to the concentration of the ICG solutions was 9.53℃, 10.43℃, 11.40℃, 12.10℃, 12.67℃, and 13.63℃ in ICG solutions of 0.5, 1.0, 2.0, 3.0, 4.0, and 5.0 mg/mL respectively. This study presents the potential for clinical application of ICG and NIR diode lasers as a new method for preventing dental caries.

Effect of Photothermal Therapy with Indocyanine Green in Multispecies Biofilm (Indocyanine Green을 이용한 광열 치료의 다종 우식원성 바이오필름에 대한 효과)

  • The purpose of this study is to investigate the antibacterial effects of indocyanine green (ICG) and near-infrared diode lasers on multispecies biofilms. Multispecies biofilms of Streptococcus mutans, Lactobacillus casei and Candida albicans were treated with different irradiation time using photosensitizer ICG and 808 nm near-infrared diode laser. Colony forming unit (CFU) was measured, and qualitative evaluation of biofilm was performed with confocal laser scanning microscopy (CLSM). Temperature measurement was conducted to evaluate photothermal effect. In the groups using ICG and diode laser, reduction in CFU was statistically significant, but the difference in antibacterial effect on L. casei and C. albicans with irradiation time was not significant, and similar results were confirmed with CLSM. Groups with ICG and diode laser showed higher temperature elevation than groups without ICG, and results of measured temperature were similar to the range of hyperthermia. In conclusion, ICG and near-infrared diode laser showed antibacterial effects on multispecies biofilms, but studies on protocol are necessary for clinical application.

  • This experiment was carried out to establish a proper method of indocyanine green(ICG) excrection test for a applicable liver function test in three Korean native cattle average weighing about 450kg and dairy cattle parity of 3~5. The results obtained the half life($T^1/_2$), fractional clearance rate(KICG), retention rate and plasma enzyme activities before or after injection of ICG were as follows. 1. The maximum absorbance of ICG in plasma was at 805nm. 2. Average half life and fractional clearance rate following the injection of ICG 0.25mg/kg body weight were $5.53{\pm}1.27$ minute and $0.131{\pm}0.031$/minute in Korean native cattle, $4.55{\pm}0.68$ minute and $0.156{\pm}0.031$/minute in dairy cattle, respectively. The ICG removal rate was exponentially liner for the first 15 minutes after injection both of Korean native cattle and dairy cattle. 3. Average plasma retention rate when 10, 15, 30 minutes after injection was $35.7{\pm}13.9$, $23.2{\pm}7.1$, $10.8{\pm}3.5%$ in Korean native cattle, $26.8{\pm}3.3$, $14.2{\pm}1.2$, $5.5{\pm}2.2%$ in dairy cattle, respectively. 4. Plasma enzyme activities(AST, ALT, r-GTP) were no variation among the before, during and after injection of ICG. From these results, ICG excretion test to cattle is applicable to evaluation of liver funtion in both clinical and research, and adopted the 15 minutes plasma sample as the sample taken at the ideal time for comparative purposes.
