One Case of Fleural Effusion by Treatment of Chungpesagan-tang and Mannitol in Cerebral Infartion (청폐사간탕(淸肺瀉肝湯)과 Mannitol투여(投與)의 뇌경색환자(腦硬塞患者)에서 흉막삼출증병발보고(胸膜渗出症倂發報告))

  • The patient who has DM, Hypertension, Arrithemia, Arteral fibrillation, Heart failure attacted cerebral infartion and he was took treatment by chungpesagan-tang and mannitol at the same time for 4 days and only chungpesagan-tang for 3 days. General condition improved but at seventh day cold sweating and hypothermia appeared. So we take chest x-ray. The result is fleural effusion. Chungpesagan-tang is the method of Purgative action so heart function is injured in circulation. In the results we consider that the medication of chungpesagan-tang and mannitol at the same time is bad influence on heart and induce ftueral effusion.

Primary hypothyroidism in Jindo dog (진도견에서 발생한 후천성 갑상선 기능저하증)

  • A four-year-old female Jindo dog was admitted to the hospital because of bilaterally symmetrical alopecia and inappentence. Systemic abnomalities detected on physical examination were obesity, bradycardia, exercise intolerance, mental dullness and hypothermia. Dermatologically symmetrical alopecia on the back, trunk and dorsal tail, and ventral hyperpigmentation were observed. Also face was very tragic. The level of basal $T_4$ was 0.01ug/dl. In TSH stimulation test $fT_4$ value was 0.08ng/dl. After treatment was initiated with L-thyroxine the dog's general condition was improved at 4 weeks.

The Effect of Oxygenated Crystalloid Cardioplegia for Myocardial Protection (심정지액의 oxygenation이 심근보호에 미치는 영향)

  • Current methods of myocardial protection has been improved with cardioplegia and hypothermia. We compared St. Thomas hospital cardiopleic solution without oxygenation[Group I], with oxygenation[Group II] and with oxygenation with filter[Group III] under isolated working heart model. Heart rate recovery was more significantly improved in Group II than Group I after 15 minutes of recovery time [p<0.05]. Maximal systolic pressure shows no difference as it increases with time. Cardiac ouputs were not diffrent between groups. Recovery time was shorter significantly in group II and group III than group I [p<0.05] Use of filter showed no difference. As a conclusion, oxygenated cardioplegic solutions improves ability to protect the heart against ischemia and it is manifested by improved recovery time and heart rate.

Influences of Prolonged Extracorporeal Circulation on Organ Function in Dogs (장시간의 체외순환이 생체에 미치는 영향)

  • Influences on organ function were studied in animals during prolonged extracorporeal circulation with a bubble type of oxygenator. More than six hours of total cardiopulmonary bypass was performed under mild hypothermia by means of an extracorporeal circulation system in five dogs. Obtained results were summarized as follows. 1. The renal function was not so impaired seriously until four hours of extracorporeal circulation. However, there was more serious impairment of renal function in this study when extracorporeal circulation was carried out for a period of five hours or more. 2. There was gradual hepatic damage during extracorporeal circulation and the damage was more significant after bypass for a period of five to six hours. 3. There was a significant decrease in serum K during bypass, irrespective of the pump oxygenator prime with a high K solution. The reason for this is complex and due to many factors, however, it was evidently related to serum glucose levels during extracorporal circulation.

  • Human atria play an important role in extracellular homeostasis through release of atrial natriuretic peptide. To evaluate the relationship between plasma level of atrial natriuretic peptide (ANP) and many changes which can develop during extracorporeal circulation, we studied 16 patients undergoing, 12 cardiac operation and 4 thoracic operation. Plasma level of ANP in cardiac patients group was significantly higher and more changeable than thoracic patients group. After aortic cross clamp release, blood was filled at right atrium and right atrial pressure was rapidly increased. At the same time, plasma level of ANP was rised suddenly. Increase of ANP level was correlated (p<0.05) with the increase of total bypass time, but was not correlated statistical with aortic cross clamp time. ANP level did not fall rapidly after aortic cross clamp while both atria were completely empty. This result was explained by intraoperative hypothermia at that time, which can inactivate plasmal endopeptidase and catalytic receptors of ANP. The ANP level of atrial fibrillation group in cardiac patients were generally higher than normal sinus group, but there was no statistical correlation.

Contributing Factors on Pharmacokinetic Variability in Critically Ill Neonates (신생아중환자의 약동학적 다양성에 영향을 미치는 요인)

  • Neonates have large inter-individual variability in pharmacokinetic parameters of many drugs due to developmental differences. The aim of this study was to investigate the factors affecting the pharmacokinetic parameters of drugs, which are commonly used in critically ill neonates. Factors that reflect physiologic maturation such as gestational age, postnatal age, postconceptional age, birth weight, and current body weight were correlated with pharmacokinetic parameters in neonates, especially preterm infants. Comorbidity characteristics affecting pharmacokinetics in critically ill neonates were perinatal asphyxia, hypoxic ischemic encephalopathy, patent ductus arteriosus (PDA), and renal dysfunction. Administration of indomethacin or ibuprofen in neonates with PDA was associated with the reduced clearance of renally excreted drugs such as vancomycin and amikacin. Therapeutic hypothermia and extracoporeal membrane oxygenation were influencing factors on pharmacokinetic parameters in critically ill neonates. Dosing adjustment and careful monitoring according to the factors affecting pharmacokinetic variability is required for safe and effective pharmacotherapy in neonatal intensive care unit.

  • Saponin, essential oil, fat oil and alkaloidal fraction were fractionated from the ethanol extract of fanax ginseng. Effect of each fraction of Panax ginseng upon the temperature response induced by reserpine, chlorpromazine and nembutal was investigated in mice, so as to secure some hidden facets of each fraction of Panax ginseng acting upon central nervous system. The authors could arrive at some results, that is: (1) Inhibitory effect of Panax ginseng upon temperature decline induced by nembutal and chlorpromazine and potentiating action upon hyperthermia induced by reserpine reside mainly in saponin fraction and slightly in essential oil fraction. (2) The effect of Ginseng saponin on temperature response in nice seems to be related with the liberation of serotonin and histamine.

  • A 25-month-old patient with complete atrioventricular septal defect and Tetralogy of Fallot underwent repair of both anomalies. The diagnosis was established preoperatively by 2D-echocardiography, cardiac catheterization and cardioangiogram, Repair was accomplished using cardiopulmonary bypass and profound hypothermia to 18C, Closing of the atrioventricular septal defect was achieved with the use of two Dacron patchs by an atrial approach alone. Infundibulectomy and outflow tract reconstruction with the transannular pericadial patch containing a monocusp were performed. Upon the postoperative evaluation by 2D-echocardiography, mitral regurgitation was absent, but a tiny dehiscence of ventricular patch and minimal tricuspid regurgitation were noticed.

  • Total anomalous pulmonary venous connection is relatively rare cyanotic congenital heart diaease, which represents 1-4% of all congenital cardiac defects. Generally in the majority cases, severe heart failure and cyanosis develops in the early infancy. Because of high mortality in the untreated infants and surgical risk, there are still many things to be improved. Two patients with total anomalous pulmonary venous connection are presented, which we recently experienced. The one was 10 year old female with supracardiac type drained through left innominate vein, and survived the operation and continuous to do well for 1 year. The other 5 year old female with mixed type (right pulmonary vein drained via coronary sinus and left pulmonary vein through left innominate vein) was operated successfully under hypothermia and extracorporeal circulation, and followed up for 6 months without problem. It was very rare case in the literature.

  • Complete circulatory arrest with profound hypothermia has been an indispensable adjunct to the safe management of selected giant intracranial aneurysms. For the conduct of cardiopulmonary bypass, there are usually two kinds of methods, open and closed chest methods. We could manage one case of huge intracranial aneurysm that was successfully operated under circulatory arrest using the closed chest method, especially with percutaneous insertion of arterial and venous cannulas for cardiopulmonary bypass.
