Early hypopharyngeal cancer treated with different therapeutic approaches: a single-institution cohort analysis

  • Purpose: Early hypopharyngeal squamous cell carcinoma (HPSCC) is a rarely diagnosed disease, for which the optimal treatment has not been defined yet. We assessed patterns of failure and outcomes in early HPSCC treated with various therapeutic approaches to identify its optimal treatment. Materials and Methods: Thirty-six patients with stage I (n = 10) and II (n = 26) treated between January 1992 and March 2014 were reviewed. Patients received definitive radiotherapy (RT) (R group, n = 10), surgery only (S group, n = 19), or postoperative RT (PORT group, n = 7). All patients in both the R and PORT groups received elective bilateral neck irradiation. In the S group, 7 patients had ipsilateral and 8 had bilateral dissection, while 4 patients had no elective dissection. Results: At a median follow-up of 48 months, the 5-year locoregional control (LRC) rate was 65%. Six patients had local failure, 1 regional failure (RF), 3 combined locoregional failures, and 2 distant failures. There was no difference in 5-year LRC among the R, S, and PORT groups (p = 0.17). The presence with a pyriform sinus apex extension was a prognosticator related to LRC (p = 0.01) in the multivariate analysis. Patients with a bilaterally treated neck showed a trend toward a lower RF rate (p = 0.08). Conclusion: This study shows that patients with early stage HPSCC involving the pyriform sinus apex might need a tailored approach to improve LRC. Additionally, our study confirms elective neck treatment might have an efficacious role in regional control.

하인두암 환자에서의 수술 후 방사선치료의 결과 (The Results of Postoperative Radiotherapy for Hypopharyngeal Carcinoma)

  • 목적: 하인두암의 치료에서 수술 후 방사선치료에 대한 치료결과 분석을 통하여 그 임상적 의의와 한계를 알아보고, 방사선치료와 관련된 예후인자들을 확인하여 향후 새로운 치료방침을 세우는데 기본 자료를 마련하고자 연구를 시행하였다. 대상 및 방법: 1988년부터 1999년까지 부산대학교병원에서 원발성 하인두 편평상피세포암으로 진단되어 수술 후 방사선치료를 시행받은 64명을 대상으로 후향적 분석을 실시하였다. 모든 환자들은 근치적 목적의 종양절제술을 시행한 뒤에 통상적인 분할조사법을 이용하여 원발 부위와 경부 림프절 부위에 수술 후 방사선치료를 실시하였다. 결과: 5년간의 전체 생존율과 질병 관련 생존율은 각각 42.2%42.2%와 51.6%였고, 병기별 5년간 질병 관련 생존율은 Stage I, II, III, IV에서 각각 100%, 80%, 62.5%, 41.5%였다. 예후인자에 대한 단변랑 분석에서 전체 병기, T 병기 및 N 병기, 2차성 원발암 발생 유무, 절제연 침범 여부, 림프절의 외막 침범 여부, 총 방사선량 등이 의미가 있었으며, 다변량 분석에서는 T 병기 및 N 병기, 절제연 침범 여부, 림프절 외막 침범 여부 등이 유의한 예후인자로 판명 되었다. 결론: 절제 가능한 하인두암 환자에서 수술 및 보조적 방사선치료는 후두나 인두의 기능장애 가능성에도 불구하고 생존율이나 국소 제어율 면에서 좋은 치료 결과를 보였다 그러나 상당히 진행된 하인두암의 치료에서는 국소제어나 환자의 삶의 질과 관련하여 기존의 수술 및 방사선 병합치료의 한계를 확인할 수 있었는데, 최근 연구되고 있는 화학-방사선 병합요법이나 세기조절 방사선치료 등과의 비교 연구가 예후인자 관련 연구와 동반해서 진행되어야 할 것이다.

진행성 두경부암에서 선행항암요법과 방사선요법의 치료성적 (Neoadjuvant Chemotherapy and Radiotherapy in the Treatment of Advanced Head and Neck Cancer : Protocol Based Study)

  • Background and Objectives: Standard treatment of locally advanced laryngeal, hypopharyngeal, and some oropharyngeal cancers includes total laryngectomy. In an attempt to preserve the larynx through induction chemotherapy, we designed induction chemotherapy followed by definitive radiation in patients with potentially respectable head and neck cancer to determine whether organ preservation is feasible without apparent compromise of survival. Materials and Methods: The twenty-six patients diagnosed advanced head and neck squamous cell carcinoma, Stage III or IV (AJCC 2002) and performed organ preservation protocols in Ajou university hospital from 1994 to 2001 were included in this study. Results: Neoadjuvant chemotherapy showed an overall response rate of 84.6% and a complete remission (CR) rate was 59.1% following neoadjuvant chemotherapy and radiation. Seven of thirteen patients were able to preserve their larynges for more than two years by chemotherapy and radiation. There were no treatment related mortality after 2 cycles of induction chemotherapy. Conclusion: Although Organ preservation protocol through neoadjuvant chemotherapy and radiation need more controlled randomized study, it was considered alternative treatment modality in advanced head and neck cancer.

생약석곡(Dendrobii herba)의 생리활성 성분 규명 (Phytochemical Identification of Bioactive Constituents of Dendrobii herba)

  • Two new phenanthrenes, namely, (1R,2R)-1,7-hydroxy-2,8-methoxy-2,3-dihydrophenanthrene-4(1H)-one (1) and 2,7-dihydroxyphenanthrene-1,4-dione (2), were isolated from the ethyl acetate-soluble fraction of Dendrobii Herba, together with the eleven known compounds, densiflorol B (3), 6,7-dimethoxyphenanthrene2,5-diol (4), dehydroorchinol (5), 1,5,7-trimethoxy-2-phenanthrenol (6), denthyrsinin (7), ephemerantol A (8), lusianthridin (9), moscatilin (10), gigantol (11), 3-[(1E)-2-(3-hydroxyphenyl)ethenyl]-5-methoxyphenol (12) and (-)-syringaresinol (13). Structures of 1 and 2 were elucidated by analyzing 1D and 2D NMR as well as HR-ESI-MS data. The absolute configuration of compound 1 was confirmed by ECD spectroscopic method. In cytotoxicity evaluation using FaDu human hypopharyngeal squamous carcinoma cell line, compounds 3-6, 8, 10 and 12 showed activities, with IC50 values ranging from 2.55 to 17.70 μM. Among them, compound 10 exhibited remarkable cytotoxic activity with IC50 value of 2.55 μM. This is the first report on the anticancer mechanistic study of compound 10 in HNSCC including FaDu cells.

경부식도암 및 하부인두암에서 근치적 전후두인두식도절제술 후 위를 이용한 재건술의 의의 (Use of the Stomach as an Esophageal Substitute after Total Pharyngolaryngoesophagectomy for Treating Cervical Esophageal Cancer or Hypopharyngeal Cancer)

  • 목적: 하부인두암 및 경부식도암 환자에서 근치적 전인두후두식도절제술시 위를 이용한 재건술 후 수술의 결과를 분석하고자 한다. 대상 및 방법: 1991년부터 2006년까지 하인두암 및 경부식도암에서 전인두후두식도절제술을 시행하였고, 재건술을 개흉술 없이, 경열공적으로 위를 이용하여 하인두위 문합술을 시행한 23예를 대상으로 하였다. 수술 후 합병증에 대해서는 신체검사소견과 수술 후 시행한 식도조영술의 결과에 따라 분류하였고, 퇴원 후 외래추적조사 시 환자의 증상 및 신체검사소견상 협착이나 재발의 가능성이 있는 환자에서 시행한 흉부 X-선 검사, 식도내시경 및 식도조영술을 바탕으로 분석하였다. 결과: 수술 후 사망한 환자는 2예(8.8%)였으며, 4예(4.4%)에서 누출이 있었으나, 공장루를 통해 경장영양으로 해결되었으며, 협착은 없었다. 호흡기합병증은 폐렴 1예, 기흉 1예였다. 수술 후에 연하곤란은 없었으나, 2000년 이후 유분부성형술을 시행하지 않으므로, 일시적으로 정체증상을 호소한 환자가 1예 있었다. 결론: 하인두암이나 경부식도암에서 근치적 절제 후 위장관 재건술시 위를 이용하는 것은 충분한 길이를 얻을 수 있어, 원활한 혈액공급이 가능하여 조직괴사나 문합부 누출 및 협착 등의 합병증이 적으며, 술식의 시간이 빠르다. 또한 모든 문합이 경부에서 이루어지므로 문합부의 누출이 발생하더라도 쉽게 치료되어 수술 후 이환율과 사망률이 감소한다는 장점이 있다.

Optimal timing for salvage surgery after definitive radiotherapy in hypopharyngeal cancer

  • Purpose: Use of radiotherapy combined with chemotherapy is increasing in hypopharyngeal cancer. However, many show residual tumor after radiotherapy. Timing for treatment evaluation and salvage therapy is essential. However, optimal timing for salvage surgery has not been suggested. In this study, we tried to evaluate optimal timing for salvage surgery. Methods and Materials: Patients who were diagnosed with hypopharyngeal squamous cell carcinoma between 2006 and 2015 were retrospectively analyzed. All patients received definitive radiotherapy with or without chemotherapy. Response of all treated patients were analyzed at 1, 3, and 6 months after radiotherapy. Any patients with progression before 6 months were excluded. Results: A total of 54 patients were analyzed. Complete remission (CR) rates at 1 month (CR1), 3 months (CR3) and 6 months (CR6) were 66.7%, 81.5%, and 90.7%, respectively. Non-CR at 1 month (NCR1), 3 months (NCR3), and 6 months (NCR6) showed poor locoregional recurrence-free survival rates (1-year rates of 63.7%, 66.7%, and 0.0%, respectively) compared to CR1, CR3, and CR6 (1-year rates 94.3%, 88.0%, and 91.5%, respectively). Particularly significant differences were seen between CR6 and NCR6 (p < 0.001). Of 10 patients with NCR3, 5 showed CR at 6 months (NCR3/CR6). There was no statistical difference in locoregional recurrence-free survival between CR3 and NCR3/CR6 group (p = 0.990). Conclusion: Our data suggest half of patients who did not show CR at 3 months eventually achieved CR at 6 months. Waiting until 6 months after radiotherapy may be appropriate for avoiding additional salvage therapy.

재건된 새 인두의 이차암에 대한 증례 보고서 (Second primary cancer in reconstructed neopharynx: a case report)

  • Background. Ever since the first report of deltopectoral flap in pharyngo-esophageal reconstruction in 1965, various methods of flap reconstruction have been introduced, allowing surgical eradication of tumors that were once thought to be inoperable. Despite these advancement, many literatures emphasize distant metastasis and second primary malignancies as the most important factors that contribute to the low 5-year survival rate of the patients. Specific consensus about defining second primary cancer is still debatable, due to small number of reports regarding second primary tumors arising in flaps used for reconstruction of defects in the head and neck region. Case. We report a case of a 72-year-old male patient who, under the diagnosis of hypopharyngeal cancer, underwent total laryngectomy with partial pharyngectomy, extended right radical neck dissection with extended left lateral neck dissection, right hemithyroidectomy and radial forearm free flap reconstruction on June 16, 2003. After 37 cycles of radiation therapy, the patient exhibited no sign of recurrence. The patient revisited our department on June 14, 2016 with chief complaint of dysphagia that started two months before the visit. Radiologic studies and histology revealed squamous cell carcinoma in neopharynx, one that had been reconstructed with forearm free flap. Conclusion. Until now, only a handful of reports regarding patients with second primary cancer in reconstructed flaps have been described. Despite its rarity, diagnostic criteria for second primary cancer should always kept in consideration for patients with recurred tumor.

Is FDG -PET-CT A Valuable Tool in Prediction of Persistent Disease in Head and Neck Cancer

  • Objectives: To evaluate accuracy of FDG-PET CT in prediction of persistent disease in head and neck cancer cases and to determine prognostic value of metabolic tumor response. Materials and Methods: Between 2009 and 2011, 46 patients with squamous cell carcinoma of head and neck receiving PET-CT were treated with definitive radiotherapy, with or without chemotherapy. There were 29 nasopharyngeal, 11 hypopharyngeal, 3 oropharyngeal and 3 laryngeal cancer patients, with a median age of 50.5 years (range 16-84), 32 males and 14 females. All patients were evaluated with PET-CT median 3-5 months (2.4-9.4) after completion of radiotherapy. Results: After a median 20 months of follow up, complete metabolic response was observed in 63% of patients. Suspicious residual uptake was present in 10.9% and residual metabolic uptake in 26.0% of patients. The overall sensitivity, specificity, positive predictive value and negative predictive value of FDG-PET-CT for detection of residual disease was 91% and 81%, 64% and 96% respectively. Two year LRC was 95% in complete responders while it was 34% in non-complete responders. Conclusions: FDG PET CT is a valuable tool for assessment of treatment response, especially in patients at high risk of local recurrence, and also as an indicator of prognosis. Definitely more precise criteria are required for assessment of response, there being no clear cut uptake value indicating residual disease. Futhermore, repair processes of normal tissue may consume glucose which appear as increased uptake in control FDG PET CT.

Prognostic value of FDG PET/CT during radiotherapy in head and neck cancer patients

  • Purpose: To evaluate the prognostic value of $^{18}F$-fluorodeoxyglucose positron-emission tomography (FDG PET) with computed tomography (CT) before and during radiotherapy (RT) in patients with head and neck cancer. Methods: Twenty patients with primary head and neck squamous cell carcinoma were enrolled in this study, of whom 6 had oropharyngeal cancer, 10 had hypopharyngeal cancer, and 4 had laryngeal cancer. Fifteen patients received concurrent cisplatin and 2 received concurrent cetuximab chemotherapy. FDG PET/CT was performed before RT and in the 4th week of RT. The parameters of maximum standardized uptake value, metabolic tumor volume, and total lesion glycolysis (TLG) of the primary tumor were measured, and the prognostic significance of each was analyzed with the Cox proportional hazards model. Results: Higher TLG (>19.0) on FDG PET/CT during RT was a poor prognostic factor for overall survival (OS) (p = 0.001) and progression-free survival (PFS) (p = 0.007). In the multivariate analysis, TLG during RT as a continuous variable was significantly associated with OS and PFS rate (p = 0.023 and p = 0.016, respectively). Tumor response worse than partial remission at 1 month after RT was another independent prognostic factor for PFS (p = 0.024). Conclusions: Higher TLG of the primary tumor on FDG PET/CT during RT was a poor prognostic factor for OS and PFS in patients with head and neck cancer.
