A Clinical Case Report of a Cervical Herniated Intervertebral Disc Patient with Hyperventilation Syndrome Developed after Magnetic Resonance Imaging (자기공명영상 촬영 후 발생한 경추 추간판탈출증 환자의 과호흡증후군 호전 1례)

  • Objectives : The purpose of this study is to investigate the clinical application of acupuncture treatment to a cervical herniated intervertebral disc patient with hyperventilation syndrome developed after magnetic resonance imaging(MRI). Methods : A patient with cervical herniated intervertebral disc got hyperventilation syndrome after taking magnetic resonance imaging. To relieve symptoms, we took paperbag breathing first aid, but it had no effect. So we used acupuncture therapy with strong stimulation. To measure the outcome of the patient's improvement, we observed change of vital sign and verbal numerical rating scale(VNRS). Results : After 8 minutes of acupuncture treatment, the patient had significant improvement in change of vital sign and verbal numerical rating scale(VNRS). Conclusions : In emergency situation, acupuncture treatment as a first aid has a positive effect to control hyperventilation syndrome.

A Study on the Relationships between Breathing Disorders and Pathological Patterns Based on the Cold-Heat, Phlegm-, Yin Deficiency-, Lao Juan (勞倦)-Pattern Questionnaires and the Nijmegen Questionnaire (호흡실조와 증형간 연관성 연구: 한열, 담음, 음허, 노권 및 네이메헨 설문을 중심으로)

  • Objectives: The purpose of this study was to examine the relationship between pathological patterns and hyperventilation syndrome, using pathological pattern and Nijmegen questionnaires. Methods: 33 healthy adults were asked to complete the Cold-Heat-, Phlegm-, Yin deficiency, and Lao Juan (勞倦)-pattern questionnaires, the Chalder fatigue scale, and the Nijmegen questionnaire at Kyung Hee University Hospital. We performed Pearson correlation analyses between the pathological pattern questionnaires and the Nijmegen questionnaire. The questionnaires were composed of several factors. Therefore, each factor and Nijmegen questionnaire score were also analyzed. Results: All of the pattern questionnaire scores had a positive correlation with the Nijmegen questionnaire score. The phlegm pattern, in particular, and the LaoJuan (勞倦) questionnaire scores had high correlation coefficients. The coefficient for the phlegm pattern was 0.856 and the coefficient for the LaoJuan (勞倦) pattern questionnaire was 0.855. Conclusions: The results mean that the pathological pattern questionnaires could be one of the reference materials to evaluate hyperventilation syndrome. Furthermore, improvement of pathological patterns may be helpful for treating hyperventilation syndrome, together with conventional therapies including breathing training.

  • The purpose of this study was to investigate the clinical application of oriental medical treatment to a patient with hypeventilation syndrome developed after traffic accident. Hypeventilation syndrome is a kind of psychosomatic disorder. Typical complaints are tachypnea, palpitation, paresthesia, muscle spasm and tetany. We treated the patient with herbal medication, physical therapy, acupuncture and moxibustion therapy, etc. In acute hyperventilation attack, we used bloodletting therapy(+宣, shihhsuan), cupping therapy(B40, B57), emergency treatment with paper bag rebreathing and herbal medication in order to tranquilize. After 2 months of oriental medical treatment, we obtained good improvement in patient's state. This results suggest that oriental medical treatment has the positive effects on a patient with hyperventilation syndrome.

  • The oculo-cerebro-renal syndrome of Lowe (Lowe syndrome) is an X-linked recessive disorder involving the eyes, nervous systems, and kidneys. The clinical manifestation of this syndrome is characterized by congenital cataracts, glaucoma, seizure disorder, psychomotor growth retardation, hypotonia, renal tubular acidosis, aminoaciduria, rickets, and osteoporosis. We report a 5-year old boy underwent general anesthesia for the treatment of multiple dental carries. During intraoperative period, marked metabolic acidosis was noted and such acidosis was partially corrected by hyperventilation. We suggest that patients with Lowe's syndrome should be attention and treated to possible anesthetic hazards such as metabolic acidosis due to renal tubular dysfunction, rise of intraocular pressure in patient with glaucoma, the fragility of the bone structures due to rickets and osteoporosis.

Development of the Nijmegen Questionnaire in Korean : Cross-Cultural Translations and Verification of Content Validity (한국어판 네이메헨 설문지(Nijmegen Questionnaire) 개발 - 횡문화적 번역 및 내용타당도 검증)

  • Objectives Nijmegen Questionnaire is one tool used internationally to evaluate hyperventilation syndrome. However, there is a necessity of developing Korean version of the Nijmegen Questionnaire, as the questionnaire of the original one are written in English, marking it hard to apply for Korean patients. So as a first step, we conducted a cross-cultural translation of the Nijmegen Questionnaire into Korean and verification of content validity. Methods We translated the Nijmegen Questionnaire into Korean up to guidelines of cross-cultural adaptation. We conducted a survey with 45 subjects to get content validity, using the translated questionnaire. Results About the translated Nijmegen Questionnaire, 28 out of 45 subjects replied that they had no difficulty understanding them, while 17 were pointed out ambiguous items and 7 offered ideas about ambiguous expressions of them. Upon further examination of five Korean Medical doctors, one sentence was additionally modified in the translated version. Conclusions We translated and adapted cross-culturally the original Nijmegen Questionnaire to develop a Korean version in accordance with internationally accepted guidelines. Then we conducted a survey for content validity with the translated questionnaire and gathered opinions from those questioned. After going through some examining and correcting procedures based on the opinions, we finalized the Korean version of Nijmegen Questionnaire. It will also require a follow-up verification process to prove reliability and validity of the final version of the Korean version of Nijmegen Questionnaire.

  • Rett syndrome is an X-linked dominant, progressive neurodevelopmental disorder, with a prevalence estimated to be one in 10,000-15,000 girls, which is thought to be the second most common genetic causes of mental retardation in females after Down syndrome. Patients with classic Rett syndrome show an apparently normal neonatal period, followed by developmental regression and deceleration of head growth, accompanied by gradual loss of speech and purposeful hand use, and development of microcephaly, seizures, autism, ataxia, intermittent hyperventilation and stereotypic hand movements. After regression between infancy and the fifth year of life, the clinical course stabilizes and patients usually survive into adulthood. It was recently discovered that Rett syndrome is caused by mutations in the methyl-CpG binding protein 2(MECP2) gene. Diagnosis of Rett syndrome is clinically difficult before three years of age, especially in atypical cases, but molecular analysis of the MECP2 gene could assist correct diagnosis in some patients. Recently, we diagnosed a case of Rett syndrome in a two year-old girl by mutational analysis of the MECP2 gene and want to report this case with brief review of literature.

How to cope with medical emergencies in the Dental Clinic (치과진료실에서 흔한 긴급상황의 처치)

  • Conceptually, the emergency is classified as an urgency and an emergency. The Urgency is not immediately life threatening, but could become so if not resolved promptly. So, it requires prompt patient care same as in the emergency situation. However, the emergency is immediately life threatening and requires immediate action, such as calling 119 and basic life support. Most medical emergencies in the dental clinic cases are urgencies. The incidence of true emergencies is approximately 1/1,000,000. Adequate managing medical urgencies are important because the chance of encountering medical emergencies in the dental clinic is high and higher these days especially because of rapidly aging Korean society. Many dentists indicate that many dentists feel difficulties to recognize and treat medical problems. This paper reviews the concept of medical emergencies and how to cope with commonly occurring urgencies in the dental clinic such as loss of consciousness, hypoglycemia, hyperventilation syndrome. The best treatment for medical urgencies and emergencies in the dental clinic is prevention. Also, it is required to make preparation for emergency situations such as CPR education for dentists and being well-acquainted with equipment and drugs for the emergency care as well as to systemic medical evaluation, patient monitoring, and sedations for controlling patient's anxiety and pain. In this paper, simple algorithms based on guidelines for common urgencies in the dental clinic are suggested. In conclusion, every dentist has competencies to do the urgency care adequately and basic life support. Also, advanced cardiac life support is strongly recommended when sedation is performed in the clinic.

A Study of the Role and Treatment of Scalene Muscle in Breathing Pattern Disorder (호흡 양상 장애에 대한 사각근의 역할과 치료법에 대한 소고)

  • There are studies on breathing pattern disorder (BPD), but the causes of BPD are still complex, and various studies are ongoing. This study reviewed several studies to investigate the possibility that pathological changes in the scalene muscles may be one of the causes of dyspnea, and that treatment of them may improve respiratory disorders. Anatomically, the scalene muscles are located between the cervical vertebrae and the transverse process of the ribs and act as a respiratory muscle. If there is a problem or excessive in its role, it can cause chest breathing or oral breathing. These problems may further affect respiratory diseases such as hyperventilation syndrome, obstructive disease, restrictive disease, and respiratory disorders. According to the results of previous studies, it seems that manual therapy or exercise therapy for the scalene muscles can contribute to the treatment of BPD.

  • Anti-programmed cell death-1 (PD-1) humanized monoclonal antibody inhibits PD-1 activity by binding to the PD-1 receptor on T-cells and blocking PD-1 ligands and induces immune tolerance of cancer cells. It has been widely used for various kinds of cancer treatment. However, many immune-related adverse events (irAEs) have been reported because it modulates our immune system. In this case study, we reported a case of 42-year-old woman with Hashimoto's thyroiditis who showed rapid aggravation of thyroid goiter and acute hyperventilation syndrome after treatment with PD-1 inhibitor as a neoadjuvant chemotherapy for breast cancer.

A familial case report of paroxysmal kinesigenic dyskinesia in three brothers (한 가족 3형제에게서 발견 된 발작성 운동이상증 1례)

  • Paroxysmal kinesigenic dyskinesia (PKD), previously referred to as movement-provoked seizures, is a rare neurological condition that is characterized by short duration dystonic or choreoathetotic movements precipitated by sudden movement, a change in position or hyperventilation. It can be difficult to distinguish this syndrome from seizures. We reported on three brothers in one family all of whom developed abnormal involuntary dystonic or choreoathetotic movement with a tingling or stiffness sensory aura. Evaluations of the patients included general physical examinations, endoclinologic, metabolic studies, chromosomal analysis, video electroencephalograms and brain MRI imaging. All of these studies were normal except for an arachnoid cyst found in one patient. All symptoms showed excellent response to oxcarbamazepine ($Trileptal^{(R)}$) or carbamazepine. Use of the video electroencephalogram can help differentiate familial PKD from seizures.
