Hypertrophic osteodystrophy in a Jindodog (진도개에 발생한 비대성골이영양증)

  • 전지 파행을 호소하여 의뢰된 약 2개월령의 진도개 암캐에서 비대성골이영양증이 진단되었다. 내원 2일 전부터 파행과 침울 증상을 보인 이 개의 이학적인 검사 결과 grade II/IV 정도의 좌측 전지파행이 관찰되었으며, 좌측과 우측의 앞발목관절의 미약한 종대가 인정되었다. 좌측발목관절을 촉진한 결과 요골과 척골의 골단부위를 촉진할 때 심한 통증을 호소하였으며, 우측 앞발목관절의 촉진에서도 미약한 통증이 인정되었다. 좌측과 우측 앞발목관절의 x-ray 검사에서 양측 전지 원위 요골 및 척골의 성장절 바로 위쪽 골간단에 성장절과 나란히 존재하는 불규칙한 방사선투과성의 선이 관찰되었고, 이 부위의 바로 아래쪽과 위쪽에는 방사선투과성이 감소되어 있었다. 이상의 결과 비대성골이영양증으로 진단되었다. ketoprofen과 amoxicillin을 투여하고, 운동을 철저히 제한한 결과 이 개의 증상은 빠르게 호전되었으며, 치료 2일 후 파행증상이 완전히 소실되었고, 증상이 재발되지 않았다. 이 보고에서는 진도개에 발생한 비대성골이영양증을 처음으로 소개하면서 이 질환으로 원인, 진단 및 치료에 대해 문헌적으로 고찰되었다.

A modified method for corner mouth lift in scarprone patients

  • Corner mouth lift is in increasing demand among young patients in East Asia. Although various surgical methods exist for corner mouth lift, many patients are reluctant to undergo surgery due to fears of postsurgical scarring. We present a new technique aimed at reducing postoperative scarring. The technique involves triangular excision of the corner of the mouth and incision of the commissure with transposition of a lateral vermilion flap to lift the corner of the mouth. The muscle around the corner of the mouth is dissected to release tension. The corner mouth lift was successfully performed in all patients. It can be effectively used even in patients, such as young Asian women, who are susceptible to hypertrophic scars.

  • Surgical treatment is possible for the obstructive form of hypertrophic cardiomyopathy and transaortic left ventricular septal myectomy and myotomy has been the procedure of choice. If coexisting intrinsic mitral valve disease exists, mitral valve replacement has been performed. But abnormal systolic anterior motion of anterior mitral leaflet[SAM] with intrinsic normal mitral valve disease is the typical feature of IHSS and we prefer not to replace mitral valve. 3 patients underwent transaortic myotomy and myectomy for IHSS with mitral regurgitation. 2 patients of them have coexisting intrinsic mitral valve diseases such as mitral valve vegetation and chorda rupture. Concomittent mitral valve replacement were performed. 1 patient shows SAM of mitral anterior leaflet but has intrinsic normal mitral valve morphologically and transepicardial echocardiogram and direct monitoring of pressure gradient during the operative procedure gives better information for subsided mitral regurgitation. Post operative course during the 12 months follow-up was uneventful.

  • Elbow hygroma and hypertrophic osteodystrophy were diagnosed in Doberman referred to Veterinary Teaching Hosptial of Chungbuk national University. Physical examination, plain radiography, and bone scan were performed in the patient. The radiography revealed soft tissue swelling on the caudal region of the elbow and irregular radiolucent zone involving the metaphysis subjacent and parallel to the distal radial and ulnar physes. The bone scan of the forelimbs revealed increased uptake in region where the soft, fluid-filled mass on the elbow was present. But the radiolucent areas (the distal radial and ulnar physes) were seen normal bone uptake. Therefore, hypertrophic osteodystrophy ruled out the diagnosis. $^{99m}Tc-MDP$ scan is suitable to routine clinical use for soft tissue inflammation and bone lesions detection.

Pathological findings of lymphocystis in cultured flounders (양식넙치에 발생한 lymphocystis의 병리학적 소견)

  • This experiment was done to observe the pathological findings of lymphocystis naturally occurred in the cultured flounders in the southern sea of Korea. Anatomical sign of lymphocystis was characterized by the presence of wart-like nodules on the fins, body surface and mouth. Dense clustering of hypertrophic cells originated from fibroblasts was observed in the lesions. Infected hypertrophic cells had a thick hyaline capsule, large vesiculated nucleus with irregular rims and large nucleolus, and large ribbon-shaped basophilic inclusions at the peripheral zone of the cytoplasm. Hexagonal virus particles with the two layers of capsid were scattered throughout the cytoplasm and were absent from the inclusions.

  • Idiopathic hypertrophic cranial pachymeningitis (ICHP) is diffuse inflammatory process of the dura mater. ICHP can produce similar presentation with Tolosa-Hunt syndrome (THS) if it involves cavernous sinus. A-29-year old male with persistent headache and no definite neurologic dysfunction was noted. Two weeks later, he complained of ophthalmoplegia, and his symptoms were thought to be manifestations of THS. Brain magnetic resonance images revealed diffuse thickened, enhanced pachymeninges in left tentorium. The patient was diagnosed with IHCP. We report a IHCP patient who showed very similar presentation as THS.

  • The Wnt signaling network, which is composed of Wnt ligands, receptors, antagonists, and intracellular signaling molecules, has emerged as a powerful regulator of cell fate, proliferation, and function in multicellular organisms. Over the past two decades, the critical role of Wnt signaling in embryonic cartilage and bone development has been well established, and much has been learnt regarding the role of Wnt signaling in chondrogenesis and cartilage development. However, relatively little is known about the role of Wnt signaling in adult articular cartilage and degenerative cartilage tissue. This review will briefly summarize recent advances in Wnt regulation of chondrogenesis and hypertrophic maturation of chondrocytes, and review data concerning the role of Wnt signaling in the maintenance and degeneration of articular chondrocytes and cartilage.

Prenatally Diagnosed Gastric Duplication - Case report - (산전진단된 위중복증 - 증례 보고-)

  • Gastric duplication is a rare anomaly which account for only 3.8% of all gastrointestinal duplication. Gastric duplications are usually cystic lesion without communication with lumen. Most frequent presentation is an abdominal mass with vomiting, mainly diagnosed within the first year of life. Surgical removal is necessary in all cases, and optimal timing for surgery is the time that diagnosis is made. However, prenatally diagnosed gastric duplication is getting more common, and determining timing for surgery is not easy due to absent or minimal symptoms just after birth. We experienced prenatally diagnosed gastric duplication in a female newborn baby that gastric duplication was suggested in $24^{th}$ week of gestational age through prenatal ultrasonogram. Surgical removal was done at 3 months after birth, and showed good results. We think that natural history of gastric duplication and prevalent age of surgical disease which is similar to gastric duplication such infantile hypertrophic pyloric stenosis should be considered when timing of surgery on prenatally gastric duplication is decided.

  • Hypertrophic pulmonary osteoarthropathy classically consists of clubbing of the fingers and toes, periosteal new bone formation in the tubular bones, painful swelling of limbs, arthralgia and arthritis. Its pathogenesis is uncertain, but it is usually associated with malignant or inflammatory intrathoracic disease. We recently experienced a patient with the characteristic findings of this rare disease on bone scintigraphy using $^{99m}Tc-MDP$(methylene diphosphonate).
