  • The onset of hypertrophic pyloric stenosis in the postoperative course of esophageal atresia with tracheoesophageal fistula is rarely reported. The diagnosis could be delayed due to its mimicking symptoms of other postoperative complications including gastroesophageal reflux or anastomotic stricture. We present an infant who had surgery for esophageal atresia with tracheoesophageal fistula. He had never fed since birth. The infant presented with an increased amount of orogastric tube drainage and consistently distended gastric air on simple abdominal X-ray. Abdominal ultrasonography showed hypertrophic thick pyloric muscle. The diagnosis of pyloric stenosis was confirmed d is rarely reported. The diagnosis could be delayed due to its mimicking symptoms of other postoperative complications including gastroesophageal reflux or anastomotic stricture. We present an infant who had surgery for esophageal atresia with tracheoesophageal fistula. He had never fed. The infant presented with uring surgery, After pyloromyotomy, the patient's condition improved.

A Case of Pseudo-Bartter's Syndrome Due to Hypertrophic Pyloric Stenosis (비후성 유문 협착증에 의한 가성 Bartter 증후군 1례)

  • We report a 3-month old boy admitted to our hospital with Bartter's syndrome like symptoms and laboratory findings, which were vomiting, failure to thrive, hypochloremic and hypokalemic metabolic alkalosis associated with hyperreninemia, hyperaldosteronism and normal blood pressure. However, the urine chloride level was low. Hypertrophic pyloric stenosis was diagnosed through abdominal ultrasonography. Fredet-Ramstedt operation was done after electrolyte correction. After surgery he made a good recovery and gained body weight. The electrolytes maintained within a normal limit without any potassium supplementations after surgery. Differential diagnosis from Bartter's syndrome was made on the basis of a decrease in urine chloride and the non-necessity for potassium supplementation after surgery. It is relatively rare for hypertrophic pyloric stenosis to induce pseudo-Bartter's syndrome. The importance of considering this diagnosis in such cases is discussed.

How Should the Pyloric Submucosal Mass Coexisting with Hypertrophic Pyloric Stenosis Be Treated?: A Case of Pyloric Ectopic Pancreas with Hypertrophic Pyloric Stenosis

  • Co-existing pyloric submucosal masses with hypertrophic pyloric stenosis (HPS) are very rare and treating these lesions is always a problem. A 20-day-old boy presented with recurrent episodes of projectile non-bilious vomiting lasting for 5 days. HPS was suspected due to the presenting age and the symptoms. The sonography demonstrated not only circumferential wall thickening of the pylorus, but also a pyloric submucosal mass. At laparotomy, a 0.8 cm sized pyloric submucosal mass was identified along with a hypertrophied pylorus. Pyloric excision was performed due to the possibility of sustaining the symptoms and malignancy. The pathological report of the submucosal mass was ectopic pancreas. Coexisting pyloric lesions can be diagnosed along with HPS, and surgical excision, not just pyloromyotomy, should be considered in these circumstances. To the best of our knowledge, this is the first case report of pyloric ectopic pancreas and HPS to be diagnosed concurrently.

  • Recently, biomedical-grade texture material gauze has often been used to treat wounds. At this time, it is difficult to remove scratches and pushed gauze; if you remove it with force, the tissue may separate and bleeding may occur again. In this study, we studied a method to apply medical-grade silicone material. Similar to the research result that hypertrophic wounds reduce the thickness of scar marks. Through mini-pig experiments, we evaluated the effects on scar treatment. The test results showed that the silicone cover layer applied to the wound site had a sealing effect on the wound area, skin temperature, and histopathological examination. In conclusion, gel treatment utilizing a biocompatible substance had the effect of minimizing hypertrophic scars.

ELECTRON MICROSCOPIC STUDY OF THE IRRADIATION EFFECT ON THE RAT MANDIBULAR CONDYLE (방사선조사가 하악과두에 미치는 영향에 관한 전자현미경적 연구)

  • In attempt to determine radiation effect on the mandibular condyle of the growing rat, 27 white female rats (Sprague-Dawley) were divided into 3 groups and irradiated respectively 5Gy, 10Gy, 20Gy using MK Cell Irradiator. Mandibular condyles from rats on the day of 1, 7, 14 after the irradiation day were obtained, sectioned sagittally and examined by light microscopy, and thereafter middle portion through anteroposterior direction on the sagittal plane was selected to examine the ultrastructural change by transmission electron microscopy. The obtained results are followings. 1. In the proliferative zone some cells showed little organelles in case of 5Gy irradiation, in addition the number of degenerative cells increased and in case of 10Gy irradiation, and in case of 20 Gy irradiation total number of cells decreased. 2. In the hypertrophic zone, narrowing of width and partial disorder in hypertrophic process were noted in case of 5 Gy irradiation, and more prominent narrowing of width and more irregular disorder in hypertrophic process in case of both 10Gy and 20Gy irradiation. 3. In the upper hypertrophic zone some chondrocytes seemed to be dying and the polarity of nuclei could not be seen, if any. 4. The periodic observation showed the severest change at day 7 and the signs of recovery at day 14 after irradiation.

Case-Control Study on Effects of Alcohol Intake and Smoking to Gastritis of Korean Adult Men (한국 성인남자에 있어서 흡연 및 음주가 위염에 미치는 영향에 대한 환자-대조군 연구)

  • We performed case-control studies for the 1,138 Korean adult men. According to the results of UGIS, we classified the gastritis into the erosive gastritis, superficial gastritis, and hypertrophic gastritis. And then, we selected controls among non-gastritis group after matching. Alcohol intake and smoking history was obtained by questionnaire. And we observed the effects of alcohol intake and smoking to the each gastritis. The results obtained were as follows : 1. Smokers had a risk of getting erosive gastritis more than twice(2.6) than non-smokers and there was dose-response relationships between smoking and erosive gastritis. Alcohol intake, however, had no significant relation with the erosive gastritis. 2. Both smoking and alcohol intake had no significant relations with superficial gastritis. 3. Smoking had no significant relation with hypertrophic gastritis, but dividing between those who drank more than 100gm of alcohol a week and those who drank less, we obtained the odds ratio of 3.4 suggesting that there existed a significant relation between moderate or excessive alcohol intake and hypertrophic gastritis. 4. Among the gastritis patients, those who had erosive gastritis smoked most heavily, and those who had hypertrophic gastritis drank most excessively.

AN EXPERIMENTAL STUDY ON THE MANDIBULAR CONDYLAR GROWTH FOLLOWING LATERAL DISPLACEMENT IN RABBIT (하악골 측방변위가 가토의 하악두에 미치는 영향에 관한 실험적 연구)

  • The purpose of this study was to evaluate the effect of the lateral displacement on the mandibular condylar growth in the rabbit. The experimental animals were twenty White NewZealand rabbits of 4-week old. Ten of them was used as control group, and experimental animal was composed of remaining ten. Laterodeviation appliance was made of cast base metal and appliance was cemented with resin in permanent fashion. Experimental group were sacrificed at 1, 2, 4, 6, 8 weeks form beginning of the experiment. Both of temporomandibular joint were prepared for histologic study. The conclusions are: 1. In control group, there was slight increase of proliferative zone and hypertrophic zone at 2-week control animal and slight reduction at 4-week. 6-week and 8-week control animal were similiar to 1-week control animal. 2. In right mandibular condyle of experimental group, 2-week experimental animal showed marked increase of proliferative zone and hypertrophic zone at posterior surface of condylar head. In 8-week experimental animal marked increase at anterior surface of anticular surface is observed. 3. In left mandibular condyle of experimental group, proliferative zone and hypertrophic zone were reduced at 1-week experimental animal and slight increase at 2-week. Proliferative zone and hypertrophic zone were reduced at 4-week experimental animal and were slightly increased at 6 week. 4. After 8 weeks, right and left condyle were not different in experimental group. The condylar cartilage was stabilized 8 weeks after the experiment. No marked traumatic change was seen, but minute focal bleeding was seen at articular cavity in 1-week, 2-week and 4-week experimental animal. 6-week and 8-week experimental animal did not show bleeding tendency in articular cavity.

  • Ileal atresia, a subtype of intestinal atresia, is one of the well-recognized causes of bowel obstruction in newborns. Prenatal diagnosis of intestinal atresia is very important in its management and outcome. Unfortunately, there are few cases of ileal atresia diagnosed prenatally, so more appropriate diagnoses and management plans are needed. As an associated gastrointestinal malformation with ileal atresia, hypertrophic pyloric stenosis is rarely reported. We report one case of postnatally diagnosed ileal atresia associated with hypertrophic pyloric stenosis which was complicated initially by bowel perforation and later by vomiting due to pyloric obstruction. Vomiting in the postoperative period is a common problem. But, if vomiting continues after the operation for ileal atresia, hypertrophic pyloric stenosis should be considered as a possible cause of medically retractable non-bilious vomiting.

Idiopathic Hypertrophic Spinal Pachymeningitis : Report of Two Cases and Review of the Literature

  • Idiopathic hypertrophic spinal pachymeningitis (IHSP) is a rare inflammatory disease characterized by hypertrophic inflammation of the dura mater and various clinical courses that are from myelopathy. Although many associated diseases have been suggested, the etiology of IHSP is not well understood. The ideal treatment is controversial. In the first case, a 55-year-old woman presented back pain, progressive paraparesis, both leg numbness, and voiding difficulty. Initial magnetic resonance imaging (MRI) demonstrated an anterior epidural mass lesion involving from C6 to mid-thoracic spine area with low signal intensity on T1 and T2 weighted images. We performed decompressive laminectomy and lesional biopsy. After operation, she was subsequently treated with steroid and could walk unaided. In the second case, a 45-year-old woman presented with fever and quadriplegia after a spine fusion operation due to lumbar spinal stenosis and degenerative herniated lumbar disc. Initial MRI showed anterior and posterior epidural mass lesion from foramen magnum to C4 level. She underwent decompressive laminectomy and durotomy followed by steroid therapy. However, her conditions deteriorated gradually and medical complications occurred. In our cases, etiology was not found despite through investigations. Initial MRI showed dural thickening with mixed signal intensity on T1- and T2-weighted images. Pathologic examination revealed chronic nonspecific inflammation in both patients. Although one patient developed several complications, the other showed slow improvement of neurological symptoms with decompressive surgery and steroid therapy. In case of chronic compressive myelopathy due to the dural hypertrophic change, decompressive surgery such as laminectomy or laminoplasty may be helpful as well as postoperative steroid therapy.

Recurrent acinic cell carcinoma in the parotid gland with cardiac metastasis and hypertrophic osteoarthropathy (심장 전이와 비후성 골관절증을 동반한 재발성 이하선 선방세포암)

  • Acinic cell carcinoma (ACC) is an uncommon malignant tumor of the salivary glands that is difficult to diagnose. It grows slowly and shows distant metastasis rarely. We experienced a case of recurrent ACC in the parotid gland with cardiac metastasis and hypertrophic osteoarthropathy. The 29-year-old man had been suffering from severe multiple bones and joints pain for 2 months. Ten years earlier, he underwent superficial parotidectomy due to a right subauricular mass. The mass was diagnosed with ACC. After surgery, the tumor recurred twice. Then the patient was diagnosed with cardiac metastasis via positron emission tomography-computed tomography and trans-thoracic echocardiography. He also had hypertrophic osteoarthropathy with multiple bone metastasis. He was given palliative radiotherapy and conservative treatment. ACC in the parotid gland with cardiac metastasis and hypertrophic osteoarthropathy has not yet been reported in literature. From this case, it is recommended to evaluate multiple distant metastasis in the ACC of the parotid gland when joint and bone pain are present.
