요코가와흡충 감염 흰쥐에 있어서 장병변 및 장상피내 림프구 (IEL) 증감에 대한 경시적 관찰 (Chronological observation on intestinal histopathology and intraepithelial Iymphocytes in the intestine of rats infected with Metagonimus yokogawai)

  • 요코가와흡충 감염시 장상피내 림프구(inkaepithelial Iympho%rotes: IEL)의 증감과 장 병변의 상관관계를 알아보기 위하여 흰쥐(Sprague-Dawley)에 피낭유충을 실험 감염시킨 다음 5일 부터 70일까지 소장 점막에서 IEL 수와 위치, V/C ratio(villus/crypt ratio; 융모/선와의 비) 등을 관찰하였다. 피낭유충은 은어(Plecoglossus altivelis)에서 획득하여 흰쥐 15마리에 각각 3,000개씩 경구 감염시켰고 동일계 흰쥐 3마리를 비감염 대조군으로 사용하였다 감염 5일, 10일, 15일, 24 일 및 70일에 각각 3마리씩 희생시킨 다음 소장 여러 부위에서 조직을 획득하여 절편으로 제작하였고, Giemsa 염색한 후, 병리학적 관찰과 함께 IEL수 및 위치의 변동을 경시적으로 관찰하였다. 응모 위축과 선와 증식. V/C ratio의 감소 등을 특징으로 하는 장병변은 감염 후 5일에서 15일 사이에 가장 심하였으며 감염 24일째부터는 점차 회복되었다. IEL은 감염 초기에 일시적으로 증가하였으나, 장병변이 진행되면서 대조군보다 감소하였다가, 장병변의 회복과 함께 다시 증가하였다 대조군에서 IEL은 대부운 장상피층의 기저부에 위치하였으나, 감염 초기에는 많은 IEL이 장상괴세포의 핵주변부와 상층부에 분포하는 점이 특이하였다 이와 같이 요코가와흡충 감염시 나타나는 IEL수 및 위치의 변동은 장병변의 진행 과정과 밀접한 관련이 있음을 알 수 있었다.

일측폐장절제가토에 있어서 steroid가 제 II 형 폐포세포의 기능에 미치는 영향 (Effect of Betamethasone on Pulmonary Surfactant Activity in Unilateral Pneumonectomized Rabbits)

  • 일측폐장을 절제한 뒤 대상성증식기의 폐장을 태아폐장증식의 모델로 삼고 이때 steroid가 대상성증식중의 폐장내 제II형폐포세포의 폐포표면활성물질의 분비에 미치는 영향을 알아보기 위한 본 실험의 결과를 요약하면 다음과 같다. 1. 잔류폐장의 무게 및 체중에 대한 폐장무게의 비는 폐절제후 betamethasone을 투여한 군에서 폐절제군에 비해 감소하는 경향을 보였다. 2. 폐세척액내의 인지질인의 함량은 폐절제후 betamethasone을 투여한 군에 있어서는 무처치대조군에 비해서는 유의한(p<0.01) 증가를 보였고 폐절제군에 비해서는 증가하는 경향을 보였다. 3. TLC(thin layer chromatography)를 통해 폐세척액내의 인지질의 조성을 조사한 결과, 폐절제후 betamethasone을 투여한 군은 무처치대조군 및 폐절제군과는 다른 양상을 보였다. 이상의 실험결과로 미루어 steroid는 태아폐장의 대상성증식기에 폐포표면활성물질의 분비를 증가시키며 동시에 제II형폐포세포에서 분비되는 폐포표면활성물질의 주성분인 인지질의 조성에도 변화를 일으켜 폐장의 융압률을 증가시킨다고 사료된다.

Expression of Transcription Factor FOXC2 in Cervical Cancer and Effects of Silencing on Cervical Cancer Cell Proliferation

  • Objective: Forkhead box C2 (FOXC2) is a member of the winged helix/forkhead box (Fox) family of transcription factors. It has been suggested to regulate tumor vasculature, growth, invasion and metastasis, although it has not been studied in cervical cancer. Here, we analyzed FOXC2 expression in cervical tissues corresponding to different stages of cervical cancer development and examined its correlation with clinicopathological characteristics. In addition, we examined the effects of targeting FOXC2 on the biological behavior of human cervical cancer cells. Methods: The expression of FOXC2 in normal human cervix, CIN I-III and cervical cancer was examined by immunohistochemistry and compared among the three groups and between cervical cancers with different pathological subtypes. Endogenous expression of FOXC2 was transiently knocked down in human Hela and SiHa cervical cells by siRNA, and cell viability and migration were examined by scratch and CCK8 assays, respectively. Results: In normal cervical tissue the frequency of positive staining was 25% (10/40 cases), with a staining intensity (PI) of $0.297{\pm}0.520$, in CIN was 65% (26/40cases), with a PI of $3.00{\pm}3.29$, and in cancer was 91.8% (68/74 cases), with a PI of $5.568 {\pm}3.449$. The frequency was 100% in adenocarcinoma (5/5 cases) and 91.3% in SCCs (63/69 cases). The FOXC2 positive expression rate was 88.5% in patients with cervical SCC stage I and 100% in stage II, showing significant differences compared with normal cervix and CIN. With age, pathologic differentiation degree and tumor size, FOXC2 expression showed no significant variation. On transient transfection of Hela and SiHa cells, FOXC2-siRNA inhibition rates were 76.2% and 75.7%; CCK8 results showed reduced proliferation and relative migration (in Hela cells from $64.5{\pm}3.16$ to $49.5{\pm}9.24$ and in SiHa cells from $60.1{\pm}3.05$ to $44.3{\pm}3.98$) (P < 0.05). Conclusion: FOXC2 gene expression increases with malignancy, especially with blood vessel hyperplasia and invasion degree. Targeted silencing was associated with reduced cell proliferation as well as invasion potential.

Clinical and molecular biological aspect of the hyaluronidases: basis and clinical overview for oriental medical application

  • Components of extracellular matrix and the matrix-degrading enzymes are some of the key regulators of tumor metastasis and angiogenesis. Hyaluronic acid (HA), a matrix glycosaminoglycan, is known to promote tumor adhesion and migration, and its small fragments are angiogenic. Until now, we have compared levels of hyaluronidase, an enzyme that degrade HA, in normal adult prostate, benign prostate hyperplasia and prostate cancer tissues and in conditioned media from epithelial explant cultures, using a substrate (HA)-gel assay and ELISA-like assay (Kim et al., unpublished results). The present review described an overall characterization of hyaluronidases and its application to human diseases. The hyaluronidases are a family of enzymes that have, until recently, deed thorough explication. The substrate for these enzymes, hyaluronan, is becoming increasingly important, recognized now as a major participant in basic processes such as cell motility, wound healing, embryogenesis, and implicated in cancer progression. And in those lower life forms that torment human beings, hyaluronidase is associated with mechanisms of entry and spread, e.g. as a virulence factor for bacteria, for tissue dissection in gas gangrene, as a means of treponema spread in syphilis, and for penetration of skin and gut by nematode parasites. Hyaluronidase also comprises a component of the venom of a wide variety of organisms, including bees, wasps, hornets, spiders, scorpions, sh, snakes and lizards. Of particular interest is the homology between some of these venom hyaluronidases and the enzyme found in the plasma membrane of mammalian spermatozoa, attesting to the ancient nature of the conserved sequence, a 36% identity in a 300 amino acid stretch of the enzyme protein. Clearly, hyaluronidase is of biological interest, being involved in the pathophysiology of so many important' human disorders. Greater effort should be made in studying this family of enzymes that have, until recently, been overlooked. Also, oriental medical application of the hyaluronidase will be discussed with respect to inhibition and suppression of inflammation and malignacy.

갑상선 종양에서 VEGF(Vascular Endothelial Growth Factor)의 발현과 신생혈관생성 (Expression of Vascular Endothelial Growth Factor and Angiogenesis in the Thyroid Tumor)

  • Background and Objectives: Angiogenesis is the process of new blood vessel development from preexisting vessel. Angiogenenesis has been considered to be essential for the growth and expansion of a solid tumor. Vascular endothelial growth factor (VEGF), known as one of the most important vascular permeability factors, induces proliferation of endothelial cells, stiumulates angiogenesis, and increases vascular permeability. Several recents reports have documented that VEGF overexpression is associated with poor clinical outcomes in many maligmancies. The aims of this study were to determine whether microvessel density and VEGF expression are related to clinicopathologic factors such as age, sex, tumor size, tumor stage, and prognostic factors and to evaluate the relationship between VEGF expression and angiogenesis in benign and malignant thyroid tumors. Materials and Methods: The subjects were 65 patients (27 with papillary carcinoma, 27 with adenomatous hyperplasia, 11 with follicular adenoma) who underwent thyroidectomy from 1995 to 2001. Imuunohistochemistry was used to detect VEGF expression and microvessel density (MVD) in paraffin-embedded thryoid tumor specimens. Results: The intensity of the VEGF expression did not show stastically difference between benign and malignant thyroid tumors. There was no apparent correlation between VEGF expression and age, tumor size, T stage or scores of the AGES, AMES and MACIS systems. The neo-microvessel density was higher in the maligant tumor than the benign tumors. Also, higher neo-microvessel density was associated with metastases of the lymph nodes and scores of the AMES and AGES systems. Conclusion: Our results suggest that neo-microvessel vessel density may be a significant prognostic factor in the thyroid papillary carcinoma. But the VEGF expression does not appear to be an significant independent prognostic factor for thyroid papillary carcinoma.

알리인 투여 후 혈소판의 방사선 방어기전 연구 (A Study on Radio-Protection Mechanism of Platelet Cells After Injection of Alliin)

  • 거대핵세포(megakaryocyte)에서 유래된 혈소판은 백혈구와 더불어 방사선에 민감하여 가장 먼저 감소하므로 방사선장해의 지표로 활용된다. 따라서 혈소판이 부족하게 되면 점상출혈(dot hemorrhage)이 계속되고 혈구 수의 감소와 면역기능 저하로 이어진다. 특히, 전신조사(whole body irradiation)로 4~6 Gy를 받을 경우, 3주 후 혈소판이 최하로 감소되어 출혈과 빈혈에 의한 조혈기계 장해의 사망원인이 된다. 따라서 본 연구는 혈소판 장해의 메카니즘을 밝히고 방어효과를 알아보고자 하였다. 실험에 사용한 방어물질은 마늘 성분의 알리인(Alliin)을 사용하였으며, rat의 꼬리 정맥에 투여한 후 전자현미경(TEM)으로 관찰하였다. 연구 결과, 5 Gy 방사선조사(Irradiation) 후 10일 경과된 혈소판 조직에서는 세포막이 심하게 손상되어 풍선모양으로 부풀어 올랐으며, 당질층(glycocalyx)이 비후되었다. 조사 후 20일 경과된 혈소판에서는 소기관들의 형태를 알아볼 수 없을 정도로 손상되었으며, 세포질의 내용물이 간질부위로 유출되어 손상이 매우 심하였다. 또한 과립들의 형태도 관찰되지 않았다. 30일 경과된 혈소판에서는 가운데 부분에서 공포(vacuole)가 생기고 조직의 손상이 관찰되었다. 하지만 Alliin을 정맥 투여한 실험군에서는 과립(granules)들의 형태가 대조군과 같이 일정하게 유지되고 있었으며, 세포막의 손상도 확인되지 않았다. 따라서 Alliin의 혈관투여시 혈소판의 방사선방어 효과를 확인할 수 있었다.

실험적 납 중독견의 임상병리학적 및 조직병리학적 소견 (Clinicopathological and Histopathological Findings of Experimental Lead Poisoning in Dogs)

  • The study of present study was to determine the valuable laboratory tests for the differential diagnosis of lead poisoning in dogs. Sixteen mongrel dogs were divided into 2 experimental groups (A and B) and a control group (C). The A and B groups were administered orally 2 mg and 20 mg of lead per kilogram of body weight for 49 days, respectively. In addition to clinical observation, blood, urine and hair samples were collected on appointed day and examined for hematological changes, lead content of serum, whole blood and hair, and urinary $\delta$-aminolevulinic acid concentrations. All dogs were necropside on 49th day and examined for the lead content and histological changes of organs. The results obtained were summarized as follows: The group B showed digestive and nervous signs, and weight loss. The group A showed no significant hematological changes except polychromatophilla on the 7th day. But group B showed polychromatophilia as well as mild anemia and nucleated erythrocyte on the 7th and 35th day. Basophlic stippling erythrocytes were observed in some of the group B on the 14th day. The lead content of whole blood was increased significantly in both A and B groups on the 21the day. The urinary $\delta$-aminolevulinic acid content was increased in both A and B groups on the 7th day. The hair lead content of A and B groups was increased significantly on the 49th and 21th day, respectively. The lead contents of organs including liver, kidney, spleen, muscle and bone were increased significantly in group B. Histopathologic changes were characterized by hemorrhages, necrosis and intranuclear inclusion body in the epithelial cells of convoluted tubles of kidney, cloudy swelling and degeneration and/or necrosis of liver, enlargement of Virchow-Robin space, and swelling of endothelial cells and hyperplasia of the pericytes of brain. From these results it may be concluded that examination of nucleated erythrocyte/polychromatophilia, urinary $\delta$ -aminolevulinic acid, and whole blood and hair lead contents is a reliable clinico-pathological diagnostic methods, and that examination of the Virchow-Robin space, endothelial cells and pericytes of brain as well as intranuclear inclusion body in the epithelial cells of convoluted tubles of kidney is valualble postmortem diagnostic methods for lead poisoning in dogs.

중증 근무력증 환자에서 da Vinci 로봇을 이용한 흉선절제술 -1예 보고- (Robot-assisted Thymectomy with the 'da Vinci' Surgical System in a Patient with Myasthenia Gravis -A case report-)

  • 중증 근무력증의 치료법으로서 흉선절제술은 효과가 입증된 표준 치료법 중 하나로 받아들여지고 있다. 흉선절제 수술법은 종래에 시행되던 흉골절개나 혹은 다른 개흉 수술법에 반해 최근 최소 침습 수술법이 시도되고 있다. 증례는 28세 남자로, 양손의 위약감 및 피로감을 호소하여 검사 결과 흉선 과형성을 동반한 중증 근무력증으로 진단하였고, da Vinci 로봇을 이용하여 흉선절제술을 시행하였다. 수술은 좌우 흉강을 통해 양측으로 접근하여, 우측에서 흉선의 2/3를 절제하고 좌측에서 나머지를 절제하였으며, 소요시간은 각각 1시간, 30분이었다. 환자는 술후 8일째 합병증 없이 퇴원하였다. 최소 침습 흉선절제술로서 da Vinci 로봇을 이용한 수술법이 국외에서 시도되고 있지만 국내에는 아직 보고된 바가 없어, 본원에서 수행한 성공적인 경험을 보고하는 바이다.

파동형 Nd:YAG 레이저조사가 인체 치은조직의 미세구조에 미치는 영향 (The effect of a pulsed-Nd:YAG laser irradiation on microstructure of human gingiva)

  • Since laser therapy has been applied to dentistry, many dental practitioners are very interested in laser therapy on various intraoral soft tissue lesions including gingival hyperplasia and aphthous ulcer. The purpose of the present study was to determine the therapeutic effect and the harmful effect of a pulsed-Nd:YAG laser irradiation on human gingival tissue. In twenty periodontal patients with gingival enlargement, the facial gingival surface of maxillary anterior teeth was randomly irradiated at various power of 1.0W(100mJ, 10Hz), 3.0W(100mJ, 30Hz) and 6.0W(l50mJ, 40Hz) for 60 seconds by contact delivery of a pulsed-Nd:YAG laser(EN.EL.EN060, Italy). Immediately after laser irradiation, the gingival tissues were surgically excised and prepared in size of 1mm3. Subsequently the specimens were processed for prefixation and postfixation, embedded with epon mixture, sectioned in $1{\mu}$ thickness, stained with uranyl acetate and lead citrate, and observed under transmission electron microscope(JEM 100 CXII). Following findings were observed; l. In the gingival specimens irradiated with l.OW power, widening of intercelluar space and minute vesicle formation along the widened intercellular space were noted at the epithelial cells adjacent to irradiated area. 2. In the gingival specimens irradiated with 3.0W power, the disruption of cellular membrane, aggregation of cytoplasm, and loss of intercellular space were observed at the epithelial cells adjacent to irradiated area. 3. In the gingival specimens irradiated with 6.0W power, the disruption of nuclear and cellular membrane was observed at the epithelial cells adjacent to irradiated area. The ultrastructural findings of this study suggest that surgical application of a pulsed-Nd:YAG laser on human gingival tissue may lead somewhat delayed wound healing due to damage of epithelial cells adjacent to irradiated area.

Hunter 증후군 환아의 치과적 관리: 증례보고 (DENTAL MANAGEMENT OF PATIENT WITH HUNTER SYNDROME (MUCOPOLYSACCHARIDOSIS TYPE II) : A CASE REPORT)

  • 뮤코다당증(mucopolysaccharidosis : MPS)은 glycosaminoglycans (GAGs)의 분해에 필요한 라이소좀 효소의 결함으로 인해 야기되는 질병으로 GAGs의 대사산물이 세포의 라이소좀 내에 축적되어 점차적으로 세포, 조직 그리고 기관의 기능 이상을 초래해 육체적, 정신적인 퇴행을 보이며, 심한 경우, 조기에 사망하게 되는 다양한 임상양상을 보이는 질환이다. 임상형은 어떠한 효소가 결핍되느냐에 따라 I형에서 IX형으로 분류한다. 치과적인 증상으로는 맹출 지연, 법랑질 저형성증, 왜소치, 부정교합, 하악 과두의 결함, 치은증식, 그리고 함치성낭 같은 여포성낭이 보고되었다. 뮤코다당증 환자는 심혈관계와 호흡계 기능이 취약하므로 치과 치료를 하는 경우, 전신 상태에 대한 주의사항과 뮤코다당증 환자에서 나타날 수 있는 구강 내 증상에 대해 미리 숙지하고, 의학적으로 환아의 전신 상태 변화에 민감하게 대처해가며 치과 진료를 시행해야 한다. 이 증례는 구강 내 다양한 임상증상을 보이는 제2형 뮤코다당증 환아의 치과 진료 후, 다소의 지견을 얻었기에 이를 보고하는 바이다.
