Serial Magnetic Resonance Images of a Right Middle Cerebral Artery Infarction : Persistent Hyperintensity on Diffusion-Weighted MRI Over 8 Months

  • A lesion that is hyperintense on diffusion-weighted imaging (DWI) and hypointense on the apparent diffusion coefficient (ADC) map is a characteristic magnetic resonance imaging (MRI) finding in acute ischemic infarction. In some cases, however, these findings can persist for a few months after infarct onset. It is thought that these finding reflect the different evolution speeds of the infarcted tissue. We report a patient with a right middle cerebral artery territory infarction with persistent hyperintensity on DWI and hypointensity on the ADC map for over 8 months. To our knowledge, this is the most persistent case of hyperintensity lesion on DWI and the serial MRI images of this patient provide important information on the evolution of infarcted tissue.

백질변성이 섬망의 심각도 및 경과에 미치는 영향 (The Effect of Leukoaraiosis on the Severity and Course of Delirium)

  • 연구목적 뇌 자기공명영상에서 발견된 백질변성의 소견은 인지기능 저하와 관련이 있다고 알려져 있지만 이의 중요성은 명확하게 밝혀진 바 없다. 본 연구는 뇌 자기공명영상에서 관찰되는 백질변성이 섬망의 심각도나 경과에 미치는 영향을 규명하기 위해 시행되었다. 방 법 섬망 삽화 전후에 뇌 자기공명영상이 촬영된 42명의 섬망 환자에 대하여 반정량적인 시각평가척도를 이용하여 뇌실주위 백질 고강도신호와 심부백질 고강도신호를 평가하였다. 이 고강도신호의 등급과 섬망평가척도 점수 간의 상관 관계를 분석하였고, 등급이 높은 군과 낮은 군으로 나누어 군에 따른 섬망평가척도 점수의 변화 차이를 추가로 분석하였다. 결 과 후두엽의 심부백질 고강도신호 등급과 섬망평가척도의 총점, 심각도 항목, 인지 항목 및 비인지 항목의 합과 양의 상관관계를 보였다. 또한 첫 평가와 추적 평가를 비교하였을 때 뇌실주위 백질 고강도신호의 등급이 낮은 군이 높은 군보다 섬망평가척도의 인지 항목 점수가 유의미한 경사로 가파르게 감소하였다. 결 론 뇌 백질 고강도신호 등급에 따라 섬망의 심각도와 호전 속도에 차이를 보이는 것은 뇌 백질변성이 대뇌 기능 연결성의 단절을 초래하여 유발한 결과로 볼 수 있다. 본 연구는 뇌 자기공명영상에서 발견된 고강도신호의 등급의 평가를 통해 섬망의 심각도와 경과를 예측해 볼 가능성이 있다는 점에서 임상적으로 유용할 수 있다.

Primary Occipital Malignant Melanoma

  • Primary intracranial melanoma is uncommon. These tumors most commonly occur at the temporal lobe, cerebellum and cerebellopontine angle. We report a case of intracranial malignant melanoma of the occipital lobe in a 60-year-old man who presented with headache and visual disturbance. The mass showed hyperintensity on T1-weighted images and hypointensity on T2-weighted magnetic resonance images. He underwent gross total removal of tumor and received radiotherapy. Follow-up imaging studios showed neither recurrence nor any signs of residual disease for 4 months.

Relationship between Abnormal Hyperintensity on T2-Weighted Images Around Developmental Venous Anomalies and Magnetic Susceptibility of Their Collecting Veins: In-Vivo Quantitative Susceptibility Mapping Study

  • Objective: A developmental venous anomaly (DVA) is a vascular malformation of ambiguous clinical significance. We aimed to quantify the susceptibility of draining veins (χvein) in DVA and determine its significance with respect to oxygen metabolism using quantitative susceptibility mapping (QSM). Materials and Methods: Brain magnetic resonance imaging of 27 consecutive patients with incidentally detected DVAs were retrospectively reviewed. Based on the presence of abnormal hyperintensity on T2-weighted images (T2WI) in the brain parenchyma adjacent to DVA, the patients were grouped into edema (E+, n = 9) and non-edema (E-, n = 18) groups. A 3T MR scanner was used to obtain fully flow-compensated gradient echo images for susceptibility-weighted imaging with source images used for QSM processing. The χvein was measured semi-automatically using QSM. The normalized χvein was also estimated. Clinical and MR measurements were compared between the E+ and E- groups using Student's t-test or Mann-Whitney U test. Correlations between the χvein and area of hyperintensity on T2WI and between χvein and diameter of the collecting veins were assessed. The correlation coefficient was also calculated using normalized veins. Results: The DVAs of the E+ group had significantly higher χvein (196.5 ± 27.9 vs. 167.7 ± 33.6, p = 0.036) and larger diameter of the draining veins (p = 0.006), and patients were older (p = 0.006) than those in the E- group. The χvein was also linearly correlated with the hyperintense area on T2WI (r = 0.633, 95% confidence interval 0.333-0.817, p < 0.001). Conclusion: DVAs with abnormal hyperintensity on T2WI have higher susceptibility values for draining veins, indicating an increased oxygen extraction fraction that might be associated with venous congestion.

Hepatic Angiomyolipoma Presenting as a Hyperintense Lesion During the Hepatobiliary Phase of Gadoxetic Acid Enhanced-MRI: a Case Report

  • Gadoxetic acid-enhanced magnetic resonance imaging (MRI) has been widely used to detect and characterize focal hepatic lesions. Because gadoxetic acid is a hepatocyte-specific contrast agent, its patterns during hepatobiliary phase enhancement provide useful information for differential diagnoses of focal hepatic lesions. Hepatic angiomyolipoma (AML) is a rare mesenchymal hepatic neoplasm composed of blood vessels, epithelioid cells, and varying amounts of adipose tissue components. Hepatic AMLs usually show marked hypointensity during the hepatobiliary phase of gadoxetic acid-enhanced MRI as hepatic AMLs are devoid of hepatocytes and fibrotic components. The present study describes a patient with hepatic AML and an atypical imaging feature. This tumor showed hyperintensity during the hepatobiliary phase of gadoxetic acid-enhanced MRI, mimicking hepatocellular tumors such as hepatocellular adenoma. The hepatobiliary hyperintensity of this lesion was likely due to multifocal entrapped hepatocytes resulting from an intrasinusoidal growth pattern of tumor cells and insufficient hepatic parenchymal enhancement during the hepatobiliary phase of gadoxetic acid-enhanced MRI.

Computed Tomographic and Magnetic Resonance Imaging Features of Oral Melanoma in a Dog

  • Oral melanoma is the most common type of oral tumor in dogs. In this report, computed tomography (CT) and magnetic resonance imaging (MRI) were performed to diagnose a right oral pigmented mass in an 8-year-old dog. The oral mass appeared as a homogeneous soft tissue density parenchyma on pre-contrast CT images, and with heterogeneous enhancement on post-contrast images. Bone destruction of the right mandibular body around the mass and mild enlargement of the right mandibular lymph node were also found. On MRI, the bulky oral mass showed mixed hyperintensity and isointensity compared to the adjacent muscle, where irregular hyperintensity on T1-weighted images corresponded to hypointensity on the T2-weighted images. Based on the physical examinations and imaging results, melanoma was suspected and confirmed via fine-needle aspiration. These unique MRI signals were due to the high paramagnetic melanin content in the tumor, therefore MRI examination could be useful for diagnosis of melanoma.

Peritumoral Brain Edema in Meningiomas: Correlation of Radiologic and Pathologic Features

  • Objective: The primary objective of this study was to perform a retrospective evaluation of the radiological and pathological features influencing the formation of peritumoral brain edema (PTBE) in meningiomas. Methods: The magnetic resonance imaging (MRI) and pathology data for 86 patients with meningiomas, who underwent surgery at our institution between September 2003 and March 2009, were examined. We evaluated predictive factors related to peritumoral edema including gender, tumor volume, shape of tumor margin, presence of arachnoid plane, the signal intensity (SI) of the tumor in T2-weighted image (T2WI), the WHO histological classification (GI, GII/GIII) and the Ki-67 antigen labeling index (LI). The edema-tumor volume ratio was calculated as the edema index (EI) and was used to evaluate peritumoral edema. Results: Gender (p=0.809) and pathological finding (p=0.084) were not statistically significantly associated with peritumoral edema by univariate analysis. Tumor volume was not correlated with the volume of peritumoral edema. By univariate analysis, three radiological features, and one pathological finding, were associated with PTBE of statistical significance: shape of tumor margin (p=0.001), presence of arachnoid plane (p=0.001), high SI of tumor in T2WI (p=0.001), and Ki-67 antigen LI (p=0.049). These results suggest that irregular tumor margins, hyperintensity in T2WI, absence of arachnoid plane on the MRI, and high Ki-67 LI can be important predictive factors that influence the formation of peritumoral edema in meningiomas. By multivariate analysis, only SI of the tumor in T2WI was statistically significantly associated with peritumoral edema. Conclusion: Results of this study indicate that irregular tumor margin, hyperintensity in T2WI, absence of arachnoid plane on the MRI, and high Ki-67 LI may be important predictive factors influencing the formation of peritumoral edema in meningiomas.

고양이에서 척수림프종의 발생례 (Spinal Lymphoma in a Cat)

  • A 3.5kg, 3-year-old castrated male Japanese domestic cat withpelvic limb ataxia and dysuria was referred to the Veterinary Medical Center of the Tokyo University. On the neurologic examination findings, both pelivic had a LMN paresis. The cat was FeLV positive and FIV negative. Radiographic findings did not identify the spinal lesions. In magnetic resonance images(MRI) of the lumbar spinal cord, the cat had the lesions in the lumbar(L) 1 and L3, characterized by hyperintensity on a transverse T2-weighted and T1-weighted images, and contrast enhancement was evident. The mass removed by dorsal laminectomy. Histopathological examination of the mass revealed spinal lymphoma. Three weeks after the surgery, the cat administered chemotherapy protocol for lymphoma by current protocol. Two weeks after chemotherapy, the cat had a metastasis to bone marrow and died.

Atypical Metronidazole-Induced Encephalopathy in Anaerobic Brain Abscess

  • Metronidazole-induced encephalopathy is a very rare complication of the long standing use of metronidazole. The encephalopathy is bilateral and symmetric in nature. We report on the magnetic resonance imaging (MRI) and clinical course of metronidazole-induced encephalopathy in a 60-year-old female with a persistent anaerobic brain abscess after draining of the abscess. After 3 months of metronidazole administration, the patient complained of dysarthria, tingling sense of all extremities, and left hemiparesis. MRI revealed symmetric hyperintensity lesions in medulla, pons, dentate nuclei of cerebellum, and splenium of corpus callosum, all of which represent typical findings of metronidazole-induced encephalopathy. In addition, asymmetric lesions in midbrain, thalamus, putamen and cerebral subcortical white matter were noted. The patient recovered after discontinuation of metronidazole and the remaining abscess was successfully treated with meropenem and levofloxacine.
