A Review of Randomized Controlled Trials on the Treatment Effects of Herbal Medicine for Hyperemesis Gravidarum (임신오조의 한약 치료 효과에 대한 무작위 대조군 임상 연구 고찰)

  • Objectives: This study aimed to anaylze randomized controlled trials on the effectiveness of herbal medicine for hyperemesis gravidarum. Methods: We searched the randomized controlled trials that intervented oral herbal medicine treatment on hyperemesis gravidarum retrieved using seven domestic and foreign databases. Literature search was conducted on April 19, 2021. Results: 185 studies were searched, and 7 were finally selected based on inclusion and exclusion criteria. All subjects were treated with western symptomatic treatment and in treatment group, additional oral herbal medicine was administered. Although the evaluation index was different for each study, all the indexes in the treatment group were significantly improved compared to the control group. Conclusions: Herbal medicine treatment for hyperemesis gravidarum showed significant effectiveness compared to western symptomatic treatment. In the future, additional studies are needed to evaluate the safety of herbal medicine treatment.

  • Background: We had treatment for two cases of Hyperemesis Gravidarum . they had difficulty in taking herbal medcine(Gamibosengtang), because of bitter taste and increased nausea and vomiting. So We used acupuncture treatment for increasing of taking herbal medicine and reduction of nausea and vomiting. Methods: The patients estimated their dgree of chief complain on a visual analogue scale and change of oral intake and change of frequency of vomiting. Result: Herbal treatment andacupuncture treatment was efficacious for Hyperemesis Gravidarum. Conclusion: More study fo herbal medicine is needed because of bitter taste.

  • Wernicke's encephalopathy is an acute-onset illness characterized by mental confusion, ataxia and ophthalmoplegia due to a thiamine deficiency. Immediate administration of thiamine prevents progression of the disease and reverses brain lesions. We have experienced a case of Wernicke's encephalopathy associated with hyperemesis gravidarum. As Wernicke's encephalopathy is mostly associated with chronic alcoholism, the possibility of Wernicke's encephalopathy may be ignored in young women. We emphasize the need for thiamine supplementation in the patient requiring intravenous alimentation such as hyperemesis gravidarum.

  • Hyperemesis gravidarum is a severe and intractable form of nausea and vomiting in pregnancy. It is a diagnosis of exclusion and may result in weight loss; nutritional deficiencies; and abnormalities in fluids, electrolyte levels, and acid-base balance. The peak incidence is at 8-12 weeks of pregnancy, and symptoms usually resolve by week 16. Interestingly, nausea and vomiting of pregnancy is generally associated with a lower rate of miscarriage. The cause of severe nausea and vomiting in pregnancy has not been identified. Extreme nausea and vomiting may be related to elevated levels of estrogens or human chorionic gonadotropin. Once the diagnosis is made, treatment consists of mainly supportive care. Until vomiting is controlled, these patients should be maintained as NPO. We treated 1 patient who had hyperemesis gravidarum and visited Kyung-Won Incheon Oriental Hospital by Herbal medication, acupuncture, moxa and nega therapy and got good result from them.

  • Objectives: To identify effects and safety of Korean traditional herbal medicine and auricular acupuncture for hyperemesis gravidarum patients. Methods: Korean traditional herbal medicine and auricular acupuncture were applied to 4 hyperemesis gravidarum patients, and we investigated changes of symptoms, continuance of pregnancy, and whether they delivered or not. Results: Hyperemesis gravidarum symptom was improved in 3 patients, and 1 patient showed complete recovery. Among them, 2 patients had children by natural childbirth, 1 patient had a repeat cesarean due to previous cesarean section, and 1 patient has been maintaining pregnancy well for 25weeks. Conclusions: It is confirmed that symptoms of hyperemesis gravidarum patients were improved by Korean traditional herbal medicine and auricular acupuncture, and these medical interventions are safe.

  • Objectives : The purpose of this study was to analyze the characteristics of the pulse energy in women who complained mild hyperemesis gravidarum. Methods : We analyzed the values of pulse energy on 14 patients who suffered mild hyperemesis gravidarum. The patients’ symptoms were assessed by PUQE (Pregnancy-Unique Quantification of Emesis scale). Statistical analysis was performed using SPSS 18.0 for window program, one way ANOVA, pearson correlation coefficient. Results : The results were as follows. 1. There were no statistically significant pulse energy differences. 2. The Chon pulse energy showed negative correlations with each Kwan and Cheok. 3. PUQE score showed positive correlations with right Chon pulse energy and negative correlations with right Kwan pulse energy. Conclusions : The result suggested that PUQE score changes be positive related with spleen pulse energy and negative related with lung pulse energy.

  • It was effective for a dosage of herbal medicine to treat hyperemesis gravidarum, threatened abortion and prevent hard labor. We prescribed Kakambosengtang for 15 patients of hyperemesis gravidarum which had weakness of the spleen and stomache, Kakamdalsengsam gyoaesamultang for 10 patients of threatened abortion which had weakness of vital energy and blood, Kakamdalsengsan for 12 patients of preventing hard labor. The result was that the whole syndromes improved. And the healthy baby was born. According to the above mentioned results, it is valid that the sick pregnant women would rather take herbal medicine rightly than avoid herbal medicine blindly.

  • Objectives: Hyperemesis gravidarum (HG) is defined as severe nausea and vomiting during pregnancy (NVP) with weight loss, ketonuria, and electrolyte imbalance. Many NVP or HG patients get hard to take herbal medicine since the smell and taste of it. Therefore, the distilled herbal medicines are often used in clinical practice. A 32-year-old, 14-week pregnant female was admitted for 3 weeks at a Korean medicine hospital. She was experiencing HG accompanied by 19.61% (10 kg) weight loss, ketonuria, and electrolyte imbalance. The patient received Bosaeng-tang gagambang in its distilled form, acupuncture, electro-acupuncture, sticker needles, cupping, moxibustion, and hydration during admission. The severity and frequency of NVP, weight loss, and ketonuria significantly improved without any side effects. Before the therapy, she admitted to two obstetrics and gynecology hospitals, 1 week each, with no improvement. This case presents the therapeutic potential of Korean medicine including herbal medicine in its diluted form for HG and NVP.

Investigation of Clinical Research of Acupuncture Treatment on Hyperemesis Gravidarum in Medline (Medline 검색을 통한 임신오저에 대한 국외 침치료의 연구 동향 분석)

  • Purpose: The purpose of this study is to examine acupuncture treatment suggested on the papers of hyperemesis gravidarum(HG) in medicine journals and to establish the future direction. Methods: The papers which were associated with acupuncture and HG published between 2000 and 2012 in Medline were searched. Results: 14 studies met the condition. They were 13 randomized controlled trial (RCT)s and 1 clinical study. 13 studies revealed that acupuncture has efficacies in HG. 1 study was shown to have no statistically significant efficacy. 2 studies revealed that acupuncture was safe treatment, and 1 study revealed that acupuncture was reliable treatment. Conclusions: From the paper research, acupuncture treatment seems to be effective on HG. However, due to the lack sufficient research of HG, it would be necessary to fulfill further study about the period of acupuncture treatment on HG and the selection of acupoint through well-designed study and establish standard of evidence of them.

Analysis for 23 studies of patients that used Herbal Medicine during Pregnancy (임신(姙娠)중 한약복용 환자에 관한 논문 23례 분석)

  • Purpose: This study was designed to establish safety and efficacy of using herbal medicine during pregnancy. Methods : We searched through 23 sources, which had been previously published frome 1988 to 2007, concerned patients that used herbal medicine during pregnancy. We searched diseases for that used herbal medicine, kinds of herbal medicine taken, the period of herbal medicine taken, the duration of herbal medicine taken and results of herbal medicine taken. Results: 1. Whaltae(habitual abortion) was the most of diseases that used herbal medicine during pregnancy. Second disease was hyperemesis gravidum. Third disease was Taegibulan (fetal restlessness). 2. The most of Herb medicines of used for Whaltae was Kyoesamultang and Anjeonecheontang. The most of Herb medicines of used for hyperemesis gravidum was Bosaengtang. The most of Herb medicines of used for Taegibulan was Kyoesamultang. 3. The time of herbal medicine taken mostly was the first period of pregnancy. The rate was 78%. 4. The duration of herbal medicine taken mostly was $18.58{\pm}14.24day$ at mean. 5. The rate of normal delivery was 85% and herbal medication during pregnancy does not cause any deformity to the newborn and of patients used herbal medicine. 6. The patients of taken herbal medicines were recovered from their symptoms and prevented miscarriage. The rate of recovery was 87%. Conclusion: The results of this study show that herbal medication during pregnancy is effective to cure disease and maintain the pregnancy. It is thought that have to actively try to treat diseases and symptoms on pregnancy by using herbal medicine.
