건강보험 치석제거 수진에 영향을 미치는 요인 (Affecting Factors on National Health Insurance Coverage Dental Scaling Experience)

  • 연구 대상자의 건강보험 치석제거 수진에 영향을 미치는 요인을 알아 보기 위하여, 2014년 2월 10일부터 2014년 2월 28일까지 수도권(서울, 경기 인천)에 거주하는 만 20세 이상 60세 미만의 성인 238명을 대상으로 설문조사하여 다음과 같은 결론을 얻었다. 건강보험 치석제거에 대한 인식을 조사한 결과 제도에 대한 개선점은 연간횟수가, 건강보험 치석제거에 대한 인지도는 보통이다가 그리고 건강보험 치석제거 제도에 대한 지지도는 그렇다가 가장 많은 분포를 차지하였다. 건강보험 치석제거를 받지 않은 이유로는 시간이 없어서가 가장 많았다. 건강보험 치석제거 수진 의향은 여자가, 정기적 구강검진을 받는 경우, 구강보건교육 경험 이 있는 경우, 건강보험 치석제거 경험이 있는 경우에 유의미하게 높았다. 로지스틱 다중회귀분석결과, 건강보험 치석제거 수진에 영향을 미치는 요인은 소득 (OR, 3.55; p<0.05), 건강보험 치석제거 경험 (OR, 3.06; p<0.05), 정기적 구강검진 (OR, 2.63; p<0.05) 순으로 나타났다. 이상의 결과로 볼 때, 건강보험 치석제거의 수진율을 높이기 위해서는 건강보험 치석제거에 대한 홍보 및 구강보건교육을 통하여 새로운 제도의 인지도를 높이고, 정기적인 구강검진으로 건강보험 치석제거의 중요성을 강조할 필요가 있다.

  • 치위생(학)과 간 지역사회치위생학의 교육과정 개발 및 표준화를 위한 근거 마련 등의 연구 활동이 시급하다고 판단되는 바, 본 연구는 이에 대한 첫 단계로서 치위생(학)과 학생들의 지역사회구강보건 교과목의 현장 실습 만족도 조사를 실시하여 학문체계 확립과 발전을 위한 기초자료로 제공하고자 한다. 구체적인 연구 목표는 다음과 같다. 첫째, 치위생(학)과 학생들의 보건소 실습 만족도를 파악하고 둘째, 만족도에 영향을 주는 인자를 조사하여 교과목의 질 향상에 기여하고자 한다. 마지막으로, 보건소 실습 기간의 적절성을 파악하여 교육의 효율성을 높이고자 한다. 수도권에 위치한 일개 치위생학과 학부생 105명을 대상으로, 경기지역에 위치한 11개 현장실습 보건소에 대하여 실습 후 만족도를 평가하였다. 11개 보건소의 만족도 분석결과, 7점 만점에 전체 평균 5.3점으로 나타났으며 최저 3.7점, 최고 6.8점으로 나타났다. 만족도에 영향을 준 긍정적인 요인은, '편한 실습환경'이 55.2%로 가장 많이 응답하였으며, '접근성(46.7%)', '많이 배울 수 있음 (31.4%)', '기타(6.7%)' 순으로 나타났다. 이러한 만족도의 요인은 학년별로 다른 분포를 보였다. 교육의 수혜자를 대상으로 이들의 지역사회 구강보건실습 현황 및 만족도를 조사하는 것은, 사회치위생 학문의 발전과 지역사회현장실습의 표준화를 위한 초기 근거자료로서의 의의를 가질 것으로 판단된다.

Evaluation of the Quality of Occupational Health and Safety Management Systems Based on Key Performance Indicators in Certified Organizations

  • Background: Occupational Health and Safety Management Systems are becoming more widespread in organizations. Consequently, their effectiveness has become a core topic for researchers. This paper evaluates the performance of the Occupational Health and Safety Assessment Series 18001 specification in certified companies in Iran. Methods: The evaluation is based on a comparison of specific criteria and indictors related to occupational health and safety management practices in three certified and three noncertified companies. Results: Findings indicate that the performance of certified companies with respect to occupational health and safety management practices is significantly better than that of noncertified companies. Conclusion: Occupational Health and Safety Assessment Series 18001-certified companies have a better level of occupational health and safety; this supports the argument that Occupational Health and Safety Management Systems play an important strategic role in health and safety in the workplace.

스케일링 건강보험 급여화사업 이후의 스케일링에 관한 인식에 영향을 미치는 요인 (Factors affecting the recognition about scaling after health insurance benefit business for scaling by national health insurance corporation)

  • Objectives: The purpose of this study was to examine the recognition about scaling after health insurance benefit business for scaling by national health insurance corporation for the general public. Methods: In order to examine the recognition about scaling after health insurance benefit business for scaling by national health insurance corporation for the general public, study subjects were selected from those who are over 20 years old, living in Seoul and Gyeonggi province since December 2016. Among 300 selected subjects, data of 279 subjects were used for the analysis which is 93% of selected data, except 21 data with insufficient responses. Results: Results of examined factors affecting the recognition about scaling after health insurance benefit business for scaling by national health insurance corporation for the general public show that the recognition about scaling after national health insurance benefit is affected by female (p<0.05), past smoker (p<0.01), exercise more than 3 times a week (p<0.05) and 1-2 times a month (p<0.001) in order. Conclusions: It is necessary to prepare a plan to properly inform the national health insurance benefit for dental scaling and to raise the recognition about the necessity of scaling.

The Clinical Effect with the Use of Gel Anesthesia within Gingival Sulcus during Scaling

  • Although scaling is the primary method for improving oral health, it is also associated with dental fear. The objective of this study was to empirically verify whether the use of gel anesthetic within the gingival sulcus during scaling relieves pain and improves other factors. A total of 128 patients scheduled to undergo scaling at a dental clinic of a general hospital located in the Gyeonggi Province, between July 2014 and July 2015, were enrolled in the study. The participants underwent scaling following the application of 20% benzocaine gel or placebo gel anesthetic within the gingival sulcus, and the data was collected using a questionnaire. There was a significant difference in the severity of pain, participant satisfaction, perceived sensitivity, overall discomfort, and fear of scaling between the two groups. The two groups were compared in terms of perceived need for gel anesthesia, willingness to pay for anesthesia costs, and willingness to receive scaling in the future. There were significant differences in all the three parameters depending on whether gel anesthesia was used or not. There were significant differences between the two groups in perceived sensitivity immediately after scaling and one day after scaling, with no difference seen one week after scaling. With regards to overall discomfort over time, there were significant differences between the two groups immediately after scaling. Based on these findings, we expect that application of gel anesthetic within the gingival sulcus during scaling will reduce pain, perceived sensitivity, overall discomfort, and fear of scaling with increased satisfaction.

수도권 성인의 정기적 스케일링 수진 의사 결정 관련 요인 (Deciding factors of regular scaling checkup in metropolitan adults)

  • Objectives : The purpose of the study is to investigate the deciding factors of regular scaling checkup in metropolitan adults. Methods : The subjects were 395 adults of 20s to 50s in Seoul and Gyeonggi province from September 25 to October 4, 2012. Data were self-reported questionnaires. Results : Female tried to receive more regular scaling checkup than male. Dental practitioners were acquainted with the dental knowledge and had more scaling checkup. Those who received dental scaling checkup tended to use dental hygiene products and visit the dentists regularly. Those who receiving good dental health services tended to visit the dental clinics more frequently. Higher knowledge and lower fear of dental treatment lead to frequent regular checkup. There were significant positive correlations between satisfaction, kindness, knowledge of scaling and regular scaling checkup. Fear to dental treatment showed the negative correlation. Conclusions : In order to increase intention degree of regular scaling checkup, it is necessary to develop programs for proper oral health behavior and to improve patient care services by dental hygienist.

우리나라에서 사용되는 일부 신나의 구성성분에 관한 연구 (A Study on Composition of Thinners Used in Korea)

  • In this study, 108 thinners were analyzed to identify their composition. The purposes of this study were to provide the data for MSDS and worker exposure levels. Thinners were collected from manufacturing industries, distributors and users. Wide ranges of thinner components were found ; Toluene, xylene, and ethyl benzene were most often found. Next, MIBK, cellosolve acetate, butyl cellosolve, and butyl acetate were found in 20-40 thinners. Others, such as acetone, n-hexane, cyclohexane, heptane, methylcyciohexane, octane and nonane were also found. There were about 5-6 components in each thinner. In the view point of Industrial Hygiene, Benzene was the most important component of thinners, which procures leukemia. Benzene was found from 8 kinds of thinners out of the total 108 surveyed. Thus, Content of benzene in thinners must be evaluated when industrial hygiene surveys are performed. Aromatic hydrocarbons were identified from 71 kinds of thinners out of total 108 and their contents were 10-87%. Alkanes were not contained in automobile painting, spraying, degreasing, urethane and epoxy thinners but lacquer, enamel and coating thinners.

Residential Radon and Lung Cancer Risk: An Updated Meta-analysis of Case-control Studies

  • Background: Numbers of epidemiological studies assessing residential radon exposure and risk of lung cancer have yielded inconsistent results. Methods: We therefore performed a meta-analysis of relevant published case-control studies searched in the PubMed database through July 2011 to examine the association. The combined odds ratio (OR) were calculated using fixed- or random-effects models. Subgroup and dose-response analyses were also performed. Results: We identified 22 case-control studies of residential radon and lung cancer risk involving 13,380 cases and 21,102 controls. The combined OR of lung cancer for the highest with the lowest exposure was 1.29 (95% CI 1.10-1.51). Dose-response analysis showed that every 100 Bq/$m^3$ increment in residential radon exposure was associated with a significant 7% increase in lung cancer risk. Subgroup analysis displayed a more pronounced association in the studies conducted in Europe. Studies restricted to female or non-smokers demonstrated weakened associations between exposure and lung cancer. Conclusions: This meta-analysis provides new evidence supporting the conclusion that residential exposure to radon can significantly increase the risk of lung cancer in a dose-response manner.
