내시경점막하박리술의 적응증을 넘어선 조기위암의 미세침습 치료 후 장기 추적 결과 (Long-term Outcome after Minimally Invasive Treatment for Early Gastric Cancer beyond the Indication of Endoscopic Submucosal Dissection)

  • Background: 최근 제안된 조기위암에 대한 미세침습 치료법은 내시경점막하박리술(endoscopic submucosal dissection, ESD)혹은 종양 부위를 전층 절제(endoscopic full-thickness gastric resection, EFTGR)을 통해 위 절제를 최소화하고 동시에 복강경을 통해 감시림프절 절제(sentinel lymph node dissection, SLND)를 시행함으로써 그 가능성을 제시했었고, 이들을 각각 ESN (endoscopic submucosal dissection with laparoscopic sentinel lymph node dissection), Hybrid-NOTES (endoscopic full-thickness gastric resection with laparoscopic sentinel lymph node dissection)라고 명명하였다. 본 고에서는 림프절 전이 위험성이 높은 조기위암에 대해 이 두 치료법을 시행받은 환자들의 장기 추적 결과를 평가하고자 한다. Methods: 이 후향적 연구는 2009년 1월부터 2013년 5월 중에 ESN 혹은 Hybrid-NOTES를 시행받은 환자들이 포함되었다. Results: 총 42명의 환자들이 연구에 포함되었고, ESN 21명, Hybrid-NOTES 21명이 각각 등록되었다. ESN을 시행받은 21명 중 4명, Hybrid-NOTES를 시행받은 5명은 이 미세침습수술 후 추가 수술을 받아야 했다. 아직까지 이 수술 후 사망 환자는 없지만 ESN을 시행받은 1명의 환자에서 림프절 전이가 발견되어 항암치료를 받고 있다. Conclusion: 조기위암의 미세침습 수술인 ESN 혹은 Hybrid-NOTES는 좋은 장기 추적 결과를 보여주었다. 이 방법들은 향후 림프절 전이 위험성이 높은 조기위암에서 ESD와 gastrectomy 사이에 속한 환자들에게 좋은 치료법이 될 수 있을 거라고 생각된다.

Significance of rescue hybrid endoscopic submucosal dissection in difficult colorectal cases

  • Background/Aims: Hybrid endoscopic submucosal dissection (ESD), in which an incision is made around a lesion and snaring is performed after submucosal dissection, has some advantages in colorectal surgery, including shorter procedure time and preventing perforation. However, its value for rescue resection in difficult colorectal ESD cases remains unclear. This study evaluated the utility of rescue hybrid ESD (RH-ESD). Methods: We divided 364 colorectal ESD procedures into the conventional ESD group (C-ESD, n=260), scheduled hybrid ESD group (SH-ESD, n=69), and RH-ESD group (n=35) and compared their clinical outcomes. Results: Resection time was significantly shorter in the following order: RH-ESD (149 [90-197] minutes) >C-ESD (90 [60-140] minutes) >SH-ESD (52 [29-80] minutes). The en bloc resection rate increased significantly in the following order: RH-ESD (48.6%), SH-ESD (78.3%), and C-ESD (97.7%). An analysis of factors related to piecemeal resection of RH-ESD revealed that the submucosal dissection rate was significantly lower in the piecemeal resection group (25% [20%-30%]) than in the en bloc resection group (40% [20%-60%]). Conclusions: RH-ESD was ineffective in terms of curative resection because of the low en bloc resection rate, but was useful for avoiding surgery.

Efficacy and Safety of ClearCutTM Knife H-type in Endoscopic Submucosal Dissection for Gastric Neoplasms: A Multicenter, Randomized Trial

  • Purpose: Endoscopic submucosal dissection (ESD) is an effective treatment for early gastrointestinal neoplasms. However, this is a time-consuming procedure requiring various devices. This study aimed to evaluate the efficacy and safety of the ClearCutTM Knife H-type, which is an integrated needle-tipped and insulated-tipped (IT) knife. Materials and Methods: Between July 2020 and September 2021, 99 patients with gastric epithelial neoplasms scheduled for ESD at three tertiary care hospitals were randomly assigned to H-knife (ClearCutTM Knife H-type) or IT-knife (conventional IT knife) groups. Procedure times, therapeutic outcomes, and adverse events were analyzed. Results: A total of 98 patients (50 in the H-knife group and 48 in the IT-knife group) were analyzed. The median total procedure time was 11.9 minutes (range, 4.4-47.2 minutes) in the H-knife group and 12.7 minutes (range, 5.2-137.7 minutes) in the IT-knife group (P=0.209). Unlike the IT-knife group, which required additional devices in all cases, no additional devices were used in the H-knife group (P<0.001). En-bloc resection was performed for all lesions in both groups. The incidence of adverse events was not significantly different between groups (4.0% in the H-knife group vs. 8.3% in the IT-knife group; P=0.431). Conclusions: The newly developed hybrid device, the ClearCutTM Knife H-type, had comparable efficacy to the conventional IT knife for gastric ESD.
