합병증을 동반하지 않은 2형 당뇨병 환자에 대한 황련해독탕의 혈당 강하 효과에 대한 유효성과 안전성 : 체계적 문헌 고찰과 메타분석 (Efficacy and Safety of the Antidiabetic effect of Hwangryunhaedok-tang for Type 2 Diabetes Mellitus Patients without Complications: A Systematic Review and Meta-Analysis)

  • Objectives: The purpose of this review is to assess the efficacy and safety of Hwangryunhaedok-tang for type 2 diabetes without complications by systemic review and meta-analysis of randomized controlled trials (RCTs). Methods: We searched the following up to March 31. 2021: PubMed, EMBASE, The Cochrane Library, CNKI, CiNii, KISS, KMBASE, OASIS, ScienceON. Data extraction and assessment of Cochrane's risk of bias (RoB) tool were performed by two independent authors, and if there was disagreement between two researchers, it was resolved through the intervention of a third researcher. Results: A total of seven trials are included in this systemic review. The treatment group (Hwangryunhaedok-tang plus conventional treatment) showed more statistically significant effects than did the control group (conventional treatment only) in fasting plasma glucose level, 2-hour postprandial plasma glucose level, glycated hemoglobin A1c, fasting insulin level, and HOMA-IR. There was no significant difference between the treatment group (Hwangryunhaedok-tang alone) and the control group (metformin alone). Conclusion: Hwangryunhaedok-tang might have efficacy and safety in controlling blood sugar level and improving insulin-resistance. However, the number of studies included in the meta-analysis was insufficient, and the Hwangryunhaedok-tang used in the included studies was not standardized. Also, the quality of the involved studies was generally low. Therefore, further studies are needed to determine the efficacy and safety of Hwangryunhaedok-tang treatment for type 2 diabetes mellitus.

황련해독탕(黃連解毒湯)이 척수 압박손상 흰쥐의 운동기능 장애에 미치는 영향 (Effects of Hwangryunhaedok-tang(Huanglianjiedu-tang) on Locomotor Dysfunction of Contusive Spinal Cord Injury-induced Rats)

  • Objectives : This study was performed to evaluate the effects of Hwangryunhaedok-tang(Huanglianjiedu-tang HHT) water extract on locomotor dysfunction induced by spinal cord injury(SCI) in rats. Methods : SCI was induced by mechanical contusion following laminectomy of 10th thoracic vertebra in Sprague-Dawley rats. HHT was orally given once a day for 14 days after SCI. Neurological behavior was examined with the Basso-Beattie-Bresnahan locomotor rating scale. Tissue damage and nerve fiber degeneration were examined with cresyl violet and luxol fast blue staining. Using immunohistochemisty, cellular damage to neurons and nerve fibers were examined against Bax and MAP-2. As inflammatory response markers, iNOS and COX-2 expressions were also examined. Results : 1. HHT ameliorated the locomotor dysfunction of the SCI-induced rats. 2. HHT attenuated the reduction of motor neurons in the ventral horn of the SCI-induced rat spinal cord. 3. HHT significantly reduced the number of Bax positive cells in the peri-lesion of the SCI-induced rat spinal cord. 4. HHT attenuated the reduction of MAP-2 positive cells in the peri-lesion of the SCI-induced rat spinal cord. 5. HHT significantly reduced the number of iNOS and COX-2 positive cells in the peri-lesion of the SCI-induced rat spinal cord. Conclusions : These results suggest that HHT improves the locomotor dysfunction of SCI by protecting motor neurons from cell death through anti-inflammatory effect.

황련해독탕 추출액의 각막염 원인균에 대한 항균효과 (Antimicrobial Activity of Hwangryunhaedok-tang Extract against Keratitis)

  • Objectives: The goal of this project was to measure the antibacterial effectiveness of Hwangryunhaedok-tang (barberry root decoction for detoxification) and its constituents. All of the active ingredients including the final product were prepared using high-pressure sterilization for use as medicinal eye drops. The varying forms of Hwangryunhaedok-tang (barberry root decoction for detoxification) were used to treat Staphylococcus epidermidis and Enterococcus faecalis which are found in keratitis. Methods: The antibacterial effect was measured by observing the presence or absence of antimicrobial activity when treated with varying concentrations of Hwangryunhaedok-tang extract. The tests were performed using a dosage of $70{\mu}l$ dosages of 100%, 50%, 10% and 1% the extracted solution by the minimum growth inhibitory concentration measurement. Antimicrobial activity was measured by examining the correlation between dosage strength and bacterial activity from $70{\mu}l$ to $10{\mu}l$ at the same concentration. Results: 1. Hwangryunhaedok-tang (barberry root decoction for detoxification), Phellodendri Cortex, and gardenia didn't show any antimicrobial effects against S. aureus, S. epidermidis, or E. faecalis. 2. Barberry root showed antimicrobial effects against S. aureus and S. epidermidis depending on the levels of concentration but didn't show any antimicrobial effects against E. faecalis. 3. Skullcap showed antimicrobial effects against S. aureus and S. epidermidis when a dosage of 100% extract $70{\mu}l$ was used. However, did not show any antimicrobial effects at all against E. faecalis. Conclusions: Hwangryunhaedok-tang (barberry root decoction for detoxification) and its constituents such as barberry root, phellodendri cortex, skullcap, and gardenia, can be used as an alternative to antibiotic medicinal eye drops to treat keratitis. However, further research on effective uses of and efficient extraction methods are needed.

Sub-acute toxicity and effect of Hwangryunhaedok-tang on human drug-metabolizing enzymes

  • Objectives: Hwangryunhaedok-tang (HHT; Huanglianjiedu-tang, Orengedoku-to), a traditional herbal formula, is used for treating inflammation, hypertension, gastritis, liver dysfunction, cerebrovascular diseases, dermatitis and dementia. The objective of this study was to assess the sub-acute toxicity of HHT in Sprague-Dawley (SD) rats, and its effect on the activities of human microsomal cytochrome P450s (CYP450s) and UDP-glucuronosyltransferases (UGTs). Methods: Male and female SD rats were orally administered HHT once daily at doses of 0, 500, 1000 and 2000 mg/kg for 4 weeks. We analyzed mortality, clinical observations, body weight, food consumption, organ weights, urinalysis, hematology, serum biochemistry, and histopathology. The activities of major human CYP450s (CYP1A2, CYP3A4, CYP2B6, CYP2C9, CYP2C19, CYP2D6, and CYP2E1) and UGTs (UGT1A1, UGT1A4, and UGT2B7) were assessed using in vitro fluorescence- and luminescence-based enzyme assays, respectively. Results: No toxicologically significant changes related to the repeated administration of HHT were observed in both male and female SD rats. The no observed adverse effect level (NOAEL) value was more than 2000 mg/kg/day for both sexes. HHT inhibited the activities of human microsomal CYP1A2, CYP2C19, CYP2D6, and CYP2E1, whereas it weakly inhibited the activities of CYP2B6, CYP2C9, CYP3A4, and UGT1A1. In addition, HHT negligibly inhibited the activities of human microsomal UGT1A4 and UGT2B7 with $IC_{50}$ values in excess of $1000{\mu}g/mL$. Conclusions: Our findings indicate that HHT may be safe for repeated administration up to 4 weeks. In addition, these findings provide information on the safety and effectiveness of HHT when co-administered with conventional drugs.
