A Comparative Study of Symptoms in Patients with Hwabyung according to Yin-Yang Personality Types (음양 성격 유형에 따른 화병 환자의 증상 비교 연구)

  • Objectives: To compare symptoms and characteristics among patients diagnosed with Hwabyung according to their yin-yang personality types. Methods: A structured clinical interview was conducted to diagnose Hwabyung, followed by a cross-sectional study involving a total of 118 Hwabyung patients (95 females). Various measures including the Sasang Personality Questionnaire (SPQ), Hwabyung Comprehensive Test (HCT), HwaByung Scale (HBS), State-Trait Anger Expression Inventory (STAXI), Beck Depression Inventory-II (BDI-II), Beck Anxiety Inventory (BAI), Eogul Scale (ES), and Patient Health Questionnaire-15 (PHQ-15) were administered. Results: SPQ-total showed significant negative correlations with HBS-personality and STAXI-control, and a significant positive correlation with STAXI-out. SPQ-behavior showed significant positive correlations with HBS-symptom, STAXI-trait, STAXI-in, STAXI-out, BDI-II, BAI, ES, and PHQ-15. SPQ-cognition exhibited significant negative correlations with HBS-personality, STAXI-control, and ES. SPQ-emotion had significant negative correlations with HBS-personality and STAXI-in and a significant positive correlation with STAXI-out. SPQ-Yin demonstrated a significantly higher HBS-personality than SPQ-Yang. SPQ-Yang exhibited significantly higher STAXI-out than SPQ-Yin, whereas SPQ-Yin showed significantly higher STAXI-control and STAXI-in than SPQ-Yang. Although differences in depression, anxiety, resentment, and physical symptoms between yin and yang were not statistically significant, variations in interpretation criteria were observed. Conclusions: Anger expression patterns and physical/psychological symptoms among Hwabyung patients are different based on their yin-yang personality types. These findings can inform the prediction of symptoms in Hwabyung patients according to their yin-yang personality types and aid in establishing appropriate treatment plans.

Portal vein reconstruction in pediatric liver transplantation using end-to-side jump graft: A case report

  • Attenuated portal vein (PV) flow is challenging in pediatric liver transplantation (LT) because it is unsuitable for classic end-to-end jump graft reconstruction from a small superior mesenteric vein (SMV). We thus introduce a novel technique of an end-to-side jump graft from SMV during pediatric LT using an adult partial liver graft. We successfully performed two cases of end-to-side retropancreatic jump graft using an iliac vein graft for PV reconstruction. One patient was a 2-year-old boy with hepatoblastoma and a Yerdel grade 3 PV thrombosis who underwent split LT. Another patient was an 8-month-old girl who had biliary atresia and PV hypoplasia with stenosis on the confluence level of the SMV; she underwent retransplantation because of graft failure related to PV thrombosis. After native PV was resected at the SMV confluence level, an end-to-side reconstruction was done from the proximal SMV to an interposition iliac vein. The interposition vein graft through posterior to the pancreas was obliquely anastomosed to the graft PV. There was no PV related complication during the follow-up period. Using a jump vascular graft in an end-to-side manner to connect the small native SMV and the large graft PV is a feasible treatment option in pediatric recipients with inadequate portal flow due to thrombosis or hypoplasia of the PV.

SKT + EKE (2:1) protects oxidative stress induced-liver damage (산화적 스트레스에 대한 생간건비탕가음양곽(生肝健脾湯加淫羊藿) (2:1)의 간보호효과)

  • Objective : Saengkankunbi-tang (SKT) is used as a traditional Korean herbal formula for treatment of liver diseases. We investigated the hepatoprotective effects of SKT plus Epimedium koreanum Nakai (EKE) against arachidonic acid (AA) + iron-mediated cytotoxicity in HepG2 cells and carbon tetrachloride (CCl4)-mediated acute liver damage in mice. Methods : The cyto-protective effects of SKT + EKE were determined by MTT assay, western blot and fluorescence activated cell sorting analysis. In mice, blood biochemistry and western blot were assessed in CCl4-induced acute hepatic damage. The animal groups included vehicle-treated control, CCl4, SKT (200 mg/kg/day), EKE (100 mg/kg/day), SKT (200 mg/kg/day) + EKE (100 mg/kg/day) and silymarin (200 mg/kg/day). Results : In HepG2 cells, pretreatment with SKT + EKE significantly suppressed cytotoxicity induced by AA + iron and reduced the expression of proteins related to apoptosis. In addition, pretreatment with SKT + EKE significantly prevented the increase in H2O2 production, GSH depletion, and lower mitochondrial membrane potential induced by AA + iron. In CCl4-induced liver damage mice, the administration of SKT + EKE prevented the liver damage by inhibition of hepatocyte damage and expression of apoptosis proteins in liver. More importantly, in vitro and in vivo assay, SKT + EKE showed significant effect compare with SKT alone or EKE alone in all parameters. Conclusions : These results indicated that SKT + EKE could protect against oxidative stress-induced liver damage, and SKT + EKE is more effective than SKT alone or EKE alone.

A Study on Microbial Community Diversity and Antibiotic Resistance in Public Waters in Gwangju (광주지역 공공수역의 미생물 군집 다양성 및 항생제 내성에 관한 연구)

  • Background: As pollutants caused by non-point sources flow into rivers, river water quality monitoring for fecal pollution is becoming increasingly important. Objectives: This study was conducted to investigate the distribution of microbial communities in the Yeongsangang River water system and sewage treatment plants in Gwangju and to evaluate their antibiotic resistance. Methods: In the experiment, samples were distributed to five selective media at each point and then cultured for 18 to 24 hours. When bacteria were observed, they were sub-cultured by size and shape and identified using MALDI-TOF MS equipment. When identification was completed, 17 types of antibiotic susceptibility tests were performed using VITEK II equipment, focusing on gram-negative dominant species among the identified strains. Results: During the study period, a total of 266 strains were isolated from 39 samples. Gram-positive bacteria were 37 strains in four genera, or 13.9% of the total, and Gram-negative bacteria were 229 strains in 23 genera, or 86.1% of the total. Antibiotic susceptibility testing of 23 strains, the major dominant species, showed that one strain (4.3%) was resistant to only one antibiotic, and two strains (8.7%) were 100% susceptible to the 17 antibiotics tested. The other 20 strains (87.0%) were multidrug resistant bacteria resistant to two or more antibiotics. There were various types of multidrug resistance. Among them, penicillin and cephalosporin series showed the highest resistance. Conclusions: Based on the results of this study, it was found that the bacterial community structure changed according to regional and environmental factors, and it was judged that continuous research such as genetic analysis of antibiotic-resistant bacteria present in natural rivers is necessary.

  • Intervertebral disc herniation is frequently encountered in radiological practice. Sequestered disc herniation occurs when the disc material undergoes degeneration and completely loses continuity with the parent nucleus pulposus. Sequestered discs can reside within and outside the spinal canal, exerting a mass effect on adjacent structures, compressing nerve pathways, and eliciting a range of clinical symptoms. In particular, sequestered discs within the dura cannot be identified without durotomy. Therefore, precise preoperative localization is crucial for surgical planning. On MRI, the signal intensity of the sequestered disc may vary due to independent degeneration processes. Additionally, most sequestered disc fragments show varying degrees of peripheral enhancement depending on the degree of angiogenesis and granulation around the isolated tissue. In this article, we review various imaging findings and the location of the sequestered disc to provide patients with an accurate diagnosis and appropriate treatment direction.

Danggwisu-san ameliorates acute inflammatory responses via NF-κB and MAPK pathway (당귀수산(當歸鬚散)의 in vitro 및 in vivo에서의 염증억제효과)

  • Objective : Danggwisu-san (DGSS) is an herbal formula that has been mainly used in the East Asia for the treatment of bruise, sprain and external injury. The cause of this pain is that Qi and blood become tangled and do not circulate well. DGSS can improve the tangled situation and make it well-circulated. The present study evaluated the anti-inflammatory effects of DGSS on Raw 264.7 cells and in rats with paw edema. Methods : Cell viability was measured using the MTT assay. The amount of nitric oxide (NO) production was measured the amount of nitrite content in the cultured medium using Griess reagent. The amount of tumor necrosis factor-α, monocyte chemoattractant protein 1, interleukin (IL)-1βand IL-6 in the cultured supernatant were measured by ELISA kit. Proteins expression were detected by Western blot. Furthermore, the effect of DGSS on acute inflammation was observed in rat paw edema model. Results : The DGSS ameliorates the lipopolysaccharide-activated changes in NO production, iNOS expression and pro-inflammatory cytokines. Additionally, DGSS significantly suppressed expression of p-JNK, p-ERK and nuclear NF-κB. As expected, in rat paw edema study, 1.0 g/kg of DGSS significantly reduced the carrageenan-induced paw edema and iNOS expression for 1-4 h. Moreover, administration of 1.0 g/kg (4 days) of DGSS used in this study did not show any significant change on ALT and AST. Conclusion : These results demonstrate that DGSS has anti-inflammatory effects in vitro and in vivo. Therefore, this present study can put scientific evidences up for the anti-inflammatory effect of DGSS.

Network Analysis of Prescriptions for Inflammatory Bowel Disease - Preliminary Exploration of Prescriptions Using the K-HERB Database - (염증성 장질환 처방에 대한 네트워크 분석 - K-HERB 데이터베이스를 활용한 예비적 처방 탐색 -)

  • Objectives : The aim of this study was to perform network analysis and analysis using the K-HERB database on inflammatory bowel disease (IBD), to verify the similarity between the derived networks and existing prescriptions, and to explore the possibility of developing new IBD prescriptions preliminarily. Methods : We conducted a comprehensive literature search on July 6, 2024, utilizing databases such as ScienceON, RISS, and OASIS. Clinical studies assessing the efficacy of herbal medicine in treating Crohn's disease and ulcerative colitis were identified and compiled into a structured database. This dataset, which included related prescriptions and herbal formulations, was subsequently analyzed using NetMiner 4 for centrality and Louvain clustering analyses. We then compared the networks derived from the K-HERB database with existing therapeutic prescriptions to assess their similarity. Results : A total of 24 prescriptions and 66 herbs were identified across the surveyed studies on IBD. Paeoniae Radix Alba(白芍藥) emerged as the most frequently utilized herb for both Crohn's disease and ulcerative colitis. Prominent herb combinations included Paeoniae Radix Alba-Angelicae Sinensis Radix (白芍藥-當歸), Angelicae Sinensis Radix-Coptidis Rhizoma (當歸-黃連), and Coptidis Rhizoma-Scutellariae Radix (黃連-黃芩) for ulcerative colitis. Centrality analysis revealed that Poria cocos (茯苓) and Paeoniae Radix Alba (白芍藥) had high centrality in the Crohn's disease, while Angelicae Sinensis Radix (當歸) and Paeoniae Radix Alba (白芍藥) had high centrality in the ulcerative colitis, indicating their prominent roles within the networks. Cohesion analysis resulted in 7 networks for Crohn's disease and 16 networks for ulcerative colitis. After excluding networks with a single herb, three networks related to Crohn's disease and two related to ulcerative colitis were examined using the K-HERB database. Among the 14 derived prescriptions for Crohn's disease and seven for ulcerative colitis, all except Oryeong-san (五苓散) were non-traditional in the context of IBD treatment. Conclusion : This preliminary study may provide a basis for the understanding and application of herbal prescriptions for IBD based on network analysis and the K-HERB database.

Analysis of Vegetation Recovery Trends by Restoration Method in Wildfire-Damaged Areas Using NDVI Mean-Variance plot (NDVI 평균-분산 도표를 활용한 산불피해지 복원 방법별 식생 회복 경향 분석)

  • With the increasing wildfire damage driven by climate change, it is crucial to assess the effectiveness of restoration efforts on a large scale. The majority of forests in Korea are situated in rugged mountainous regions, making it challenging to monitor large-scale wildfires. Consequently, establishing methodologies that use satellite imagery to evaluate restoration effectiveness is essential. This study aims to assess the recovery trends of ecosystems in wildfire-affected areas using NDVI mean-variance plots, which monitor changes in NDVI mean and variance over time through satellite imagery and visually represent the restoration process. The analysis of NDVI mean-variance plots for different restoration methods revealed that landscape restoration had the slowest recovery. This slower recovery is likely due to reduced growth from the complete removal of damaged trees. In contrast to High Severity (HS) areas, Moderate High Severity (MHS) areas showed that commercial afforestation, revegetation, ecological forest treatment led to a more stable recovery state post-disturbance, suggesting that areas with lower wildfire severity may recover more quickly. Furthermore, the recovery trends between artificial and natural restoration showed no significant difference, indicating that natural restoration can have similar restoration effects to artificial restoration in appropriate areas. Therefore, the study emphasizes the need to expand natural restoration areas, considering ecological and economic benefits such as increased biodiversity and genetic resource conservation. This research provides critical baseline data for the formulation and implementation of restoration policies in large-scale wildfire-affected regions and is expected to contribute significantly to the development of effective management strategies and monitoring techniques.

  • Background : Idiopathic pulmonary fibrosis(IPF) is a devastating illness for which there is little effective treatment. The key cytokines currently implicated in the fibrotic process are the transforming growth factor-${\beta}_1$(TGF-${\beta}_1$), tumor necrosis factor-$\alpha$(TNF-$\alpha$), endothelin-1(ET-1) and interferon-$\gamma$(IFN-$\gamma$). The rat model for paraquat-induced pulmonary fibrosis was chosen to investigate the role of ET-1 in this disease. Both ET-1 and TGF-${\beta}_1$ expression in lung lesions were examined using immunohistochemical staining. After $Bosentan^{(R)}$ administration, an orally active ET-$l_A$ and ET-$1_B$ receptor antagonist, the degree of pulmonary fibrosis and ET-1 and TGF-${\beta}_1$ expression were analyzed. Method : Sprague-Dawley rats were divided into three groups, the control group, the fibrosis group, and the fibrosis-$Bosentan^{(R)}$-treated group. The animals were sacrificed periodically at 1, 3, 5, 7, 10, 14 days after administering saline or paraquat. The effects between groups were compared with the results of light microscopy and immunohistochemical staining for ET-1 and TGF-${\beta}_1$. The degree of fibrosis was evaluated by H&E and Masson's trichrome staining, which were graded by a computerized image analyzer. The degree of immunohistochemical staining was categorized by a semi-quantitative analysis method. Results : The lung collagen content had increased in the paraquat instillated animals by day 3, and continued to increase up to day 14. A daily treatment by gavage with $Bosentan^{(R)}$ (100mg/kg) did not prevent the increase in collagen deposition on the lung that was induced by paraquat instillation. There were increased immunohistochemical stains of ET-1 on the exudate, macrophages, vascular endothelial cells and pneumocytes in the paraquat instillated group. Furthermore, TGF-${\beta}_1$ expression was higher on the exudate, macrophages, some inflammatory cells, pneumocytes( type I, and II), vascular endothelium and the respiratory epithelial cells around the fibrotic area. After Bosentan treatment, there were no definite changes in ET-1 and TGF-${\beta}_1$ expression. Conclusion : Fibrosis of the Paraquat instillated group was more advanced when compared with the control group. In addition, there was increased ET-1 and TGF-${\beta}_1$ expression around the fibrotic area. A literal study on the Gu-Chang (구창의 문헌연구)

  • Gu-Chang is a disorder characterized by recurring ulcers confined to the oral mucosa. Despite much clinical and research attention, the causes remain poorly understood. In this paper, we will compare Gu-Chang with Recurrent Aphthous Stomatitis(RAS) in order to know what is the similiarity between Gu-Chang and RAS. So we will arrange various oriental and western medical literatures which are important. As a result of arrangement of the causes, symptoms and therapys of Gu-Chang, we can conclude through the studies as follows. 1. The etiologies of Gu-chang are following. In the Sthenia syndrome, there are evil heat of external factor, heat of heart and spleen, insomnia, heat of upper warmer, stress and diet, heat of lung and heart, excessive heat of upper warmer, inappropriate food intake, heat conveyance of organ, heat of stomach merdian, moistured heat of spleen and stomach and stasis of liver energy. In the Asthenia syndrome, there are deficiency of stomach energy, deficiency of upper warmer leading to heat, deficiency of middle warmer leading to cold, deficiency of lower warmer leading to heat, deficiency of middle energy, deficiency of blood, decreased fire and deficiency of soil, yin fire of lower warmer, deficiency of heart yin, deficiency of spleen yin and deficiency of qi and blood. 2. In western medicine the causes of RAS is presumed as local, microbial, systemic, nutritional, genetic, immunologic factors. 3. Once Gu-chang is compared with RAS, in the deficiency of yin leading to hyperactivity of fire, deficiency of yin leading to floating of fire and stasis of liver energy, recurring of Gu-chang is similar to RAS. Although recurring of Gu-chang due to tripple warmer of excessive fire has no recurrance, since there are the degree of Pain, site of lesion, dysphagia etc, it is similar to major RAS. It is may be believed that Sthenia Gu-chang is similar to major RAS, shape of recurring, site of lesion, degrree of Pain and white color of Asthenia Gu-chang are similar to minor RAS, but there is no similarity concerning herpes RAS in the literatures that describe the symptoms. 4. Generally, the treatment of Gu-chang is divided into Asthenia and Sthenia Syndrome. The method of cure to Sthenia syndrome is heat cleaning and purge fire, Asthenia syndrome is nourish yin to lower and adverse rising energy and strength the middle warmer and benefit vital energy. 5. Following is the medication for Sthenia syndrome. Heat of heart and spleen is Do Jok San, Yang Gyek San, Juk Yup Suk Go Tang, evil heat of external factor is Yang Gyek San Ga Gam, Stasis of liver energy is Chong Wi Fae Dok Yum, moistured heat of spleen and stomach is Chong Gi Sam Syep Tang. The medication for Asthenia Syndrome is following. Deficiency of upper warmer leading to heat is Bo Jung Ik Gi Tang, deficiency of middle warmer leading to cold is Bu Ja Lee Jung Tang, deficiency of lower warmer leading to heat is Yuk Mi Ji Hwang Tang, deficiency of yin leading to hyperactivity of fire is Ji Baek Ji Hwang Hwan, deficiency of yin leading to floating of fire is Lee Jung Tang Ga Bu Ja Medicine for external use were Yang Suk San, Boo Wyen San, Rok Po San, Yoo Hwa San ate. 6.
