The Outcomes of Proximal Humerus Fractures with Medial Metaphyseal Disruption Treated with Fibular Allograft Augmentation and Locking Plate

  • Background: Proximal humerus fracture is considered to be the third most common fracture for patients aged 65 years or older. Conservative treatment has been known to treat most of humerus fracture. However, fractures with severe displacement or dislocation may require surgical treatment. Intramedullary fibular allograft with a locking plate is frequently used in patients accompanying medial metaphyseal disruption. In this study, author intends to evaluate clinical and imaging results based on patients who underwent surgical treatment using fibular allograft with a locking plate. Methods: This study is conducted prospectively at Wonju Severance Christian Hospital, targeting patients who previously underwent surgical treatment using open reduction and intramedullary fibular allograft with a locking plate between 2011 and 2015. A total of 26 patients were evaluated on the following: postoperational clinical assessment measuring Constant score, American Shoulder and Elbow Society (ASES) score, and the Disabilities of the Arm, Shoulder and Hand (DASH) score. Postoperational imaging assessments are evaluated via measuring the neck-shaft angle. The study subject were Neer classification type 3, 4 proximal humerus fracture cases with disrupted medial hinge and having cortical comminution in the region of the surgical neck. Results: The average period of progression was 22.5 months, and the average age of patients was 72.6 years. At the final follow-up, the average Constant, average ASES, and average DASH scores were 80.1, 78.5, and 20.6 respectively. The average neck-shaft angle was $127.5^{\circ}$. Conclusions: In conclusion, fibular allograft augmentation with a locking plate showed satisfying results in both clinical and imaging studies.

Hemiarthroplasty for Distal Humerus Fracture: A Systematic Review and Meta-analysis for Functional Outcome

  • Background: Treatment of distal humerus fractures in osteoporotic elderly patients is often challenging. For non-reconstructible fractures with open reduction and internal fixation, total elbow arthroplasty (TEA) is an acceptable alternative. However, the relatively high complication rates and lifelong activity restrictions make TEA less ideal for elderly or low-demand patients. Efforts to identify or develop alternate procedures that benefit relatively young, high-demand patients have resulted in increased interest in hemiarthroplasty. This systematic review reports the clinical outcomes of hemiarthroplasty for distal humeral fractures. Methods: We systematically reviewed the databases of PubMed, Ovid MEDLINE, and Cochrane Library. All English-language studies published before June 2017 were considered for possible inclusion. Search terms included 'distal humerus fracture' and 'hemiarthroplasty'. Studies reporting outcomes (and a minimum of 1 year clinical follow-up) in human subjects after hemiarthroplasty (Latitude system) for distal humeral fractures were assessed for inclusion. Patient demographics, clinical and radiographic outcomes, and complications were recorded, and homogenous outcome measures were analyzed. Results: Nine studies with a total of 115 patients met the inclusion criteria. Among the included studies, the weighted mean follow-up time was 35.4 months. Furthermore, the weighted mean of the postoperative range of motion ($107.6^{\circ}$ flexion-extension, $157.5^{\circ}$ for pronation-supination) and functional outcomes (Mayo elbow performance scores: 85.8, Disabilities of the Arm, Shoulder and Hand score: 19.6) were within the acceptable range. Conclusions: Our study indicates that hemiarthroplasty is a viable option for comminuted distal humerus fracture. Satisfactory functional outcomes were observed in most patients.

상완골 근위부 골절 또는 골절 및 탈구에서의 상완골 두 치환술 (Hemiarthroplasty for Fractures or Fracture-Dislocations of the Proximal Humerus)

  • Purpose : To evaluate functional results and study factors influencing results after humeral hemiarthroplasty for comminuted fractures of the proximal humerus. Materials and Methods: We reviewed 12 patients with average follow-up period of 23 months(12 to 42). The proximal humeral fractures consisted of one anatomical neck fracture, five three-part and six four-part fractures or fracture-dislocations. Constant score and Compito et al' evaluation system were used for clinical assessment. Statistical analysis was done with the Wilcoxon test. Results: Six cases of anatomical neck, three-part fractures or fracture-dislocations showed 61 points on the average Constant score and three unsatisfactory results on the Compito et al' evaluation; Six of four-part fractures or fracture-dislocations showed 51 points and three unsatisfactory results. There were 59(two unsatisfactory) for five non-associated dislocation and 53(four unsatistactory) for seven associated dislocation; 66(one unsatisfactory) for three under 60 years and 41(five unsatisfactory) for nine over 60 years; 67(two unsatisfactory) for seven tuberosity union and 41(four unsatisfactory) for five tuberosity non-union or resorption; 58(four unsatisfactory) for ten early operation within two weeks and 43(all unsatisfactory) for two delayed operation; and 18(unsatisfactory) for one case of a accompanied rotator cuff tear. Conclusion: We concluded that tuberosity union was important factor influencing the results. Also, we thought that age, timing of operation and accompanied rotator cuff tear could be influenced to results.

상완골 간부 단순 골절에서 최소 침습적 금속판 골유합술을 이용한 치료 (The Treatment of Humerus Shaft Simple Fracture by MIPO Technique)

  • 목적: 상완골 간부 단순 골절의 수술적 치료에서 잠김 압박 금속판을 이용한 최소 침습적 금속판 골유합술의 임상적 및 방사선학적 결과를 분석하였다. 대상 및 방법: 2010년 8월부터 2011년 5월까지 상완골 간부 단순 골절 골절에 대해 최소 침습적 금속판 골 유합술을 통해 수술을 시행하고 전향적으로 12개월 이상 추시한 6례의 환자를 대상으로 하였으며, 임상적 및 방사선학적 골유합 기간 및 술 후 관절 운동 범위와 기능을 확인하였다. 결과: 임상적 및 방사선학적 평균 골유합 기간은 각각 7.2주, 8주였다. 술 후 골 정렬에서 전후상 평균 2.8도, 측면상 내반 2도의 변형을 보였다. 술 후 12개월째 견관절 운동 범위는 전방거상 167도, 외회전 50도, 내회전 수위는 흉추 11번이었고, 평균 VAS는 1.2점, KSS는 91.3점, ASES는 26.5점, UCLA 점수는 31.5점이었다. 결론: 상완골 간부 단순 골절의 수술적 치료에서 최소 침습적 금속판 골유합술은 방사선학적 및 기능적으로 우수한 결과를 보였으며 상완골 간부 단순 골절에서 고려해 볼 수 있는 유용한 치료 방법 중 하나라고 생각된다.

제 1형 신경 섬유종증 환자에서 발생한 재발성 견관절 탈구 및 근위 상완골 골절 - 1예 보고 - (Recurrent Shoulder Dislocation and Proximal Humerus Fracture in Neurofibromatosis Type I - A Case Report -)

  • 제 1형 신경 섬유종증 환자에서 발생한 재발성 견관절 탈구 및 동반된 근위 상완골 골절 1예를 보고하고자 한다. 제 1형 신경 섬유종증으로 진단된 20세 남자로 가벼운 외상 후 발생한 우측 견관절의 탈구와 근위 상완골 골절로 내원하였다. 수상측 주관절부에는 큰 망상형 신경 섬유종들이 있었으며 견관절 탈구는 재발성 병변이었다. 단순 방사선과 컴퓨터 단층 촬영 소견 상 상완골과 견갑골의 골성 변형을 보였고 자기 공명 영상 검사 상 삼각근과 회전근 개에 다발성 신경 섬유종성 변화가 관찰되었다. 관혈적 정복술 및 관절막 재건술을 시행하였으며 근위 상완골 골절은 재건 금속판을 이용하여 고정하였다. 골유합과 만족스런 기능 회복을 보였으며 수술 후 7개월 추시 상 견관절 탈구의 재발은 관찰되지 않았다. 제 1형 신경 섬유종증 환자에서 발생한 견관절 재발성 탈구와 근위 상완골 골절은 드문 합병증이나 질환의 정형외과적 접근 시 고려해야 할 것으로 사료된다.

소아 상완골 과상부 골절의 경피적 K-강선 고정술 (Percutaneous K-wire Fixation of Supracondylar Fracture of Humerus in Children)

  • 1990년 3월에서 1993년 9월까지 영남대학병원 정형외과에서 22례의 소아 상완골 골절의 치료에서 도수정복 후 K-강선 고정술을 시행 하였다. 환자의 연령은 대부분 3세에서 11세 사이였으며, 신전형의 Gartland 2, 3형 전위 골절이 많았다. 22례의 환자중 일년 이상 추시가 가능 하였던 10례에서 미관상의 결과는 모두 만족 스러웠고, 기능상의 결과는 한 례를 제외하고는 모두 만족 스러웠다. 일부 단기 추기 가능하였던 7례의 환자 중에서도 5례가 만족스러운 결과를 보였다. 척골신경 손상이나, 연부조직 손상의 합병증은 없었다. 입원 기간도 11.4일로 다른 치료법에 비하여 단축되어, 도수 정복후 K-강선 고정술은 많은 장점을 가진 좋은 치료법으로 사료된다.

성인의 상완골 과상부 골절의 도수 정복 후 경피적 고정 방식의 치료 결과 (Results of Closed Reduction and Percutaneous Fixation of the Supracondylar Fractures of the Humerus in Adults)

  • Purpose: To evaluate the results of the treatment of the supracondylar fractures of the humerus according to the fixation methods in adults Materials and Methods: Seven patients, aged 55 to 52 years (average,69 years), were reviewed after a mean follow-up of 37 months (range, 11-65 months). According to AO classification all fractures were classified as type A2 (simple transverse supracondylar fracture). Six patients underwent closed reduction and one patient, open reduction after failure of closed reduction. Percutaneous fixation with cannulated screws was performed to the 4 patients, per- cuta)leous fixation with Kirschner wires in 3 patients. All except one patients have associated medical problems. The results were assessed based on the Mayo Elbow Performance Score. Results: All the patients with cannulated screw fixation had stable bony union with excellent ranges of motion (mean: 5-125 degrees). All the three patients who received percuatnaous smooth K-wire fixation had nonunion with poor results, one of them had changed into cannulated screw. and then had good result. Conclusion: Although simple supracondylar fracture is similar to the pediatric fracture in nature, it should be firmly fixed with the method such as threaded cannualted screw rather than the simple fixation with K-wires.

Pulmonary Embolism after Screw Fixation for a Greater Tubercle Fracture of Humerus

  • Pulmonary embolism is a serious complication, which is well known in patients undergoing total hip or total knee arthroplasty or lower extremity fracture surgery. But, there are few literatures concerning pulmonary embolism after upper extremity surgery. Pulmonary embolism after minor upper extremity fracture surgery is extremely rare. We report a case of 66-year-old female patient that developed pulmonary embolism after percutaneous cannulated screw fixation for a greater tubercle fracture of the proximal humerus with literature review.

잠김 압박 금속판을 이용한 상완골 근위부 골절의 치료 (Operative Treatment with Locking Compression Plate (LCP) in Proximal Humerus Fracture)

  • 목적: 잠김 압박 금속판(LCP)을 이용한 상완골 근위부 골절의 수술적 치료에 대한 임상적, 방사선적 치료 결과를 평가해보고자 하였다. 대상 및 방법: 2005년 1월부터 2007년 4월까지 상완골 근위부 골절에 대해 LCP를 이용해 치료받고 12개월 이상 추시가 가능했던 24예를 대상으로 하였으며, 남자는 8예, 여자는 16예이었다. 평균 연령은 68.9세(33~90)이었다. 임상적 결과는 Neer의 기능적 평가 방법을, 방사선적 결과는 골 유합 시기 및 경간각을 이용한 Paavolainen의 방법으로 평가하였다. 결과: 평균 골 유합 기간은 11.9주(8~15)이었다. Neer의 기능적 평가상, 만족 이상의 결과를 보인 경우는 21예(87%)이었고, Paavolainen의 방법에 의한 방사선적 결과가 양호 이상인 경우는 22예(91%)이었다. 3예(13%)에서 금속 실패, 무혈성 골 두 괴사, 관절 강직의 합병증이 각각 발생하였다. 결론: 상완골 근위부 골절에 대해 LCP를 이용한 내고정술은 비교적 양호한 임상적, 방사선적 결과 및 적은 합병증을 보여, 상완골 근위부 골절의 수술적 치료에 유용한 방법 중 하나로 판단된다.

Operative Treatment with Intramedullary Fibular Strut Allograft for Osteoporotic Proximal Humerus Fracture

  • Background: The purpose of this study was to investigate the clinical and radiological outcomes of locking plate fixation with fibular strut allograft to manage unstable osteoporotic proximal humerus fractures. Methods: We retrospectively reviewed 15 patients who underwent open reduction and locking plate fixation with fibular strut allograft for osteoporotic proximal humerus fracture between July 2011 and June 2015. For functional evaluation, we evaluated visual analogue scale (VAS) pain score, American Shoulder and Elbow Surgeons (ASES) score, University of California Los Angeles (UCLA) shoulder score, and active range of motion. For radiological evaluation, shoulder true anteroposterior (AP) and AP in $20^{\circ}$ external rotation, as well as the axillary view were taken at two weeks, six weeks, three months, six months, and one year. And the neck-shaft angle was measured on the AP view in $20^{\circ}$ external rotation view. Results: At the one-year follow-up, mean VAS pain score and all shoulder scores, including ASES score and UCLA shoulder score, exhibited satisfactory clinical outcomes. All patients obtained bone union between three and six months post-procedure. Moreover, the mean immediate postoperative neck-shaft angle was $138^{\circ}{\pm}4^{\circ}$, and at one-year follow-up, the neck shaft angle was $137^{\circ}{\pm}5^{\circ}$. There was no significant difference between the preoperative and postoperative values (p=0.105). Conclusions: For the unstable two-part and three-part osteoporotic proximal humerus fractures with medial calcar comminution, the use of fibular strut allograft with locking plate fixation was effective in maintaining the initial status of reduction and exhibiting the satisfactory functional and radiological outcomes.
