  • Unlike conventional transmission and distribution lines, catenary system for operating electric railway vehicles are composed of multi-conductor groups (feeder line, contact wire, messenger wire, protection wire) and are used for railway employees, public or passengers in the station yards. Electric shock hazards are exposed and electric shocks such as death or serious injury are occurring in electric railway vehicles, railway high-voltage distribution lines, and catenary system. In order to analyze the types of electric shock accidents on railway by systematic approach method, we modeled 'unsafe behavior classification' method using swiss cheese model. Based on this method, we derived the type of electric shock accidents about railway accidents during the last 5 years by analyzing the frequency of occurrence of human errors and unsafe acts, laws and regulations related to violations, and so on.

  • Occupational fatal injury rate per 10,000 population of Korea is still higher among the OECD member countries. To prevent fatal injuries, the causes of accidents including human error should be analyzed and then appropriate countermeasures should be established. There was an severe converter furnace accident resulting in five people death by chocking in 2013. Although the accident type of the furnace accident was suffocation, many safety problems were included before reaching the death of suffocation. If the safety problems are reviewed throughly, the alternative measures based on the review would be very useful in preventing similar accidents. In this study, we investigated the converter furnace accident by using human error analysis and accident scenario analysis. As a result, it was found that the accident was caused by some human errors, inappropriate task sequence and lack of control in coordinating work by several subordinating companies. From the review of this case, the followings are suggested: First, systematic human error analysis should be included in the investigation of fatal injury accidents. Second, multi man-machine accident scenario analyis is useful in most of coordinating work. Third, the more provision of information on system state will lessen human errors. Fourth, the coordinating control in safety should be performed in the work conducting by several different companies.

  • Human errors performed during the operations have a dominant portion of the accidents. But the systematic human error evaluation methodology universally accepted is not developed yet. One of the difficulties in performing human reliability analysis is to evaluate the performance shaping factors which represent the characteristics and the circumstances in the discriminate manner. For assessing a specific human action more exactly, it is necessary to consider all of the PSFs at the same time which make an effect on the human action. In this paper, dynamic influence diagrams are introduced to model simultaneously their effects on the specific human action. And the human actions and their subsequent PSFs are categorized and classified as the complementary works. A new human error evaluation methodology using influence diagrams is developed. This methodology involves the categorization of PSFs and the PSFs quantification. The applied analysis results for the example task are shown for representative purposes. It is shown that this approach is very flexible in that it can be applied to any kind of actions.

  • Among many automotive manufacturing processes, an assembly process has the smallest automatization percentage. As the correct assembly of the supplied components according to its specification is directly related with the quality of the final product, it is of the utmost importance to reduce the possible human errors in the assembly line. However, due to the lack of an effective verification system for an assembly line, currently, there is huge cost loss caused by the errors in seat assembly every year. Therefore, in this paper, to overcome the limit of the current verification method and to prevent the occurrence of assembly errors caused by mismatching assembling components or its specifications, a verification system and its method is proposed employing the RFID technology. The system is for automatic detection of the assembly errors, so that it can reduce the time and effort needed for verification and maximize the work efficiency. Also, by preventing the assembly errors in advance, the productivity can be increased through a quality innovation.
