노인의료전문병원의 급식서비스 인카운터에 대한 중요도-만족도 연구 (The Importance-Satisfaction Study of Hospital Foodservice Encounters at the Elderly Health-Care Facilities)

  • Institutional care is often necessary for the health and well-being of the elderly. Good quality foodservice provided at long-term care facilities not only includes patients satisfaction but also cares for good health, contributing to the stability of foodservice management. The purpose of this study was to assess the importance and satisfaction attributes of foodservice management by hospitalized elderly patients. The data were collected via questionnaire by a one-to-one interview with 194 hospitalized elderly patients in six different hospitals. According to the results of dependent t-tests, overall mean scores for the importance attributes (3.96) and satisfactory attributes (3.83) were significantly different (p<0.001). As indicated by the patients, the recognized importance attributes were the kindness of foodservice personnel (4.19), kind smiles by foodservice personnel (4.16), and kind speaking by foodservice personnel (4.12). The most recognized satisfaction attributes were kindness of foodservice personnel (4.36), bedside meal service by foodservice personnel (4.25), kind speaking by foodservice personnel (4.24), kind smiles by foodservice personnel (4.24), and sanitary uniforms worn by foodservice personnel (4.21). These results suggest that the above encounter attributes (importance-satisfaction) would be useful tools for hospital foodservices to adopt, in order to control foodservice quality and satisfy the nutritional needs of elderly patients.

  • Hospitalized patients' perceptions of the quality of hospital foodservice and their importance were surveyed through questionnaires by 820(men 435, women 385) hospitalized patients in Seoul. The results are as follows: Most respondents agreed with the following foodservice characteristics that meals arrived exactly the same time every day(74.6%), cleanliness of dishes(64.9%), employees leave food within reach(60.2%), and employees who bring meals are cheerful(58.7%). Only 34.2% of respondents agreed to variety of menu; 12.9% of respondents viewed these foodservice characteristics as important in selecting a hospital; cleanliness of dishes(66.4%), variety of menu(55.0%), and varying food item combination(45.9%) were considered important by respondents; appetite, mood, and atmosphere of ward were positively correlated(p<0.001) with rating of the foodservice characteristics, but length of hospitalization was negatively correlated(p<0.001) with them; familiarity with cooking method, varying food item combination, cleanliness of the dishes, and foodservice employees' pleasant greeting were positively correlated(p<0.001) with rating of the taste, nutrition, and fresshness of the food characteristics.

  • Hospitalized patients' perceptions of the quality of hospital food and their importance were surveyed through questionnaires by 820(men 435, women 385) hospitalized patients in Seoul. The results are as follows: Men rated the size of food portion as being smaller, compared with women. The mean rating for nutrition of food was 3.24, freshness 3.15, taste 2.88, temperature 2.82; Among the food characteristics, nutrition was considered important by largest percentage(75.5%) of respondents, the taste ranked second(74.4%), and freshness was 62.2%; Appetite, atmosphere of ward were positively correlated(p<0.001) with rating of the taste, nutrition, and freshness, but length of hospitalization was negatively correlated (p<0.001) with them; Multiple regression analysis showed that appetite made the greatest contribution and length of hospitalization made the second greatest contribution. The third was atmosphere of ward, and the forth mood condition.

입원 진폐증 환자의 삶의 만족도, 사회적 지지, 지각된 건강상태 및 고독감에 관한 연구 (Life Satisfaction, Social Support, Perceived Health Status, and Loneliness of the Hospitalized Patients with Pneumoconiosis)

  • Purpose: The purpose of this study was to investigate the relationship among loneliness, life satisfaction, social support and perceived health status, and further to identify factors affecting loneliness among hospitalized patients with pneumoconiosis. Methods: This study was a cross-sectional descriptive survey. A sample of 205 hospitalized patients with pneumoconiosis was recruited from August 10 to 27, 2011. The data were collected using structured questionnaire and were analyzed with SPSS/WIN 17.0 program. Results: The mean score of loneliness was very high (46.5). The levels of loneliness depended on several factors including the duration of coalworker caregiver, personality, exercise, hobbies and reported complications. The reported loneliness was significantly correlated with life satisfaction (r=-.204, p=.003). family support (r=-.220, p=.002), professional medical team support (r=-.303, p<.001) and perceived health status (r=-.175, p=.012). The influential factors affecting loneliness were professional medical team support, life satisfaction, and personality, perceived health status, complication and family support, which explained about 21.8% of the variance. Conclusion: The results of this study indicate that nursing intervention is needed to reduce loneliness, and to improve the perceived health status, social support and life satisfaction in the hospitalized patient with pneumoconiosis. These results can provide for evidence of nursing intervention to facilitate reduction of loneliness.

Incidence of falls and fall-related characteristics in hospitalized children in South Korea: a descriptive study

  • Purpose: With the recent increase in interest in patient safety, prevention of falls in hospitalized children has become important. This study aimed to identify the incidence rate of falls among hospitalized children and explore fall-related characteristics. Methods: This retrospective descriptive study analyzed the medical information of 18,119 patients aged <18 years admitted to a general hospital in South Korea from electronic medical records and fall event reports between January 1, 2018, and September 30, 2023. The study variables included the general and clinical characteristics of the fall group and fall-related characteristics of the fall events. This study employed descriptive statistics and a chi-square test using IBM SPSS version 26.0. Results: Among the patients, 82 fall events were identified. Therefore, the fall incidence rate was 4.5 falls per 1,000 patients. Furthermore, a statistically significant difference was found concerning the type of injury sustained between children >1 year old and those <1 year old, and most cases had no damage or bruises. More cases were found in which falls occurred two days after hospitalization in winter and summer than on the day or the day after hospitalization, which was a statistically significant difference. In addition, fall prevalence was higher between 8 am and 4 pm, and when no caregiver was present. Conclusion: Based on the results of this study, education and interventions to prevent fall events in infants and toddlers should persist throughout hospitalization, and it is necessary to guide continuous management and observation of the caregivers.

일 중소병원 입원노인환자의 수면양상에 영향을 미치는 요인 (Factors Affecting Sleeping Patterns among Hospitalized Elderly)

  • Purpose: The purpose of this study was to identify the factors affecting sleeping patterns among hospitalized elderly. Methods: A descriptive correlational study was conducted. Participants were 121 hospitalized elderly in a hospital located in a county. A structured questionnaire was used for data collection. Collected data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson's correlation, and stepwise multiple regression. Results: Almost a half of hospitalized elderly in this study suffer from sleep disturbances. As results of univariate analyses, physical symptoms, anxiety, depression, and environmental factors disturbed sleep among hospitalized elderly. However, the result of stepwise multiple regression analysis identified that anxiety and environmental factors were significant predictors of sleeping patterns for hospitalized elderly and these two factors accounted 32.5% of variance of sleeping patterns among hospitalized elderly. Conclusion: Sleep assessment should be conducted on admission to identify sleep difficulties and thereby to improve quality of nursing care. Nurses and other health care personnels should make efforts to decrease anxiety and to eliminate environmental barriers of sleep among hospitalized elderly. Educational programs for nursing staff that offer information about sleep and sleep promoting interventions for the elderly patients are critical. Moreover, sleep promotion intervention program should be developed, applied and evaluated.

Comparison of four nutritional screening tools for Korean hospitalized children

  • BACKGROUND/OBJECTIVES: Several nutritional screening tools were recently developed to screen the risk of malnutrition in hospitalized children, but have not been validated in Asia. We compared four nutritional screening tools for pediatric patients in evaluating nutritional risks in newly hospitalized children. SUBJECTS/METHODS: Medical records of newly admitted pediatric patients between June 2016 and May 2017 at two tertiary hospitals were reviewed. Initial information by nurses and hospital records by doctors on baseline demographic, clinical, and anthropometric data at admission were collected in all subjects. Nutritional risks were evaluated using four nutritional screening tools including the pediatric nutritional risk score (PNRS), the screening tool for the assessment of malnutrition in pediatrics (STAMP), the paediatric Yorkhill malnutrition score (PYMS), and the screening tools for risk of nutritional status and growth (STRONGkids). RESULTS: A total of 559 patients (310 boys and 249 girls, mean age $6.3{\pm}5.5years$) were recruited. Patients in medical and surgical departments were 469 (83.9%) and 90 (16.1%), respectively. The prevalence of patients at risk of malnutrition were 31.1% for low risk, 52.2% for medium risk, and 16.6% for high risk by PNRS; 11.4%, 39.7%, and 48.8% by STAMP; 26.5%, 25.4%, and 48.1% by PYMS; and 35.6%, 58.9%, and 5.5% by STRONGkids. PNRS versus STRONGkids and STAMP versus PYMS showed moderate agreement (kappa = 0.566 and kappa = 0.495, respectively). PYMS and STAMP revealed a relatively high sensitivity of 87.8% and 77.6% for wasting. CONCLUSION: Different nutritional screening tools revealed considerably different results in evaluating nutritional risks in newly hospitalized children. Since pediatric patients are at risk of malnutrition at admission and during hospitalization, screening tools should be applied properly according to the situation of each hospital.

만성질환으로 입원한 노인 환자의 우울, 자아존중감 및 일상생활수행능력(ADL)간 관계연구 (The Relationship among Depression, Self-esteem and ADL in the Case of the Hospitalized Elderly Patients with Chronic Disease)

  • Purpose: This study has aimed to examine the relationship among self-esteem, Activities of Daily Living (ADL) and depression reported by hospitalized elderly patients with chronic diseases. Methods: A descriptive correlation study by means of a self-report questionnaire or face-to-face interview was used to collect data from 119 elderly patients who were hospitalized in a General Hospital from January 5 to February 25, 2010. Their levels of depression was measured using Short-Form Geriatric Depression Scale, self esteem using Jeon's, and activities of daily living using K-ADL. Results: 80.7% of the subjects experienced depression. Depression correlates with self-esteem (r=-.67) and ADL (r=.45). The influencing factors on depression were self-esteem, ADL, subjective health status, and family support satisfaction ($R^2=.57$), while self esteem in itself explained 45% of variance in depression. Conclusion: These findings indicate the importance of early detection of depression, which starts from the admission of patients and the continuing evaluation/management in daily life after discharge to ensure their well-being and quality of life. The development of program empowering self esteem, ADL and subjective health status with adequate family support during hospitalization and in daily life is indispensible.

노인전문병원에 입원한 뇌졸중환자의 복용약물과 관련된 낙상 위험요인 (Medication Use as a Risk Factor for Falls among Hospitalized Stroke Patients)

  • Purpose: To identify the relationship between medication use and falls among hospitalized stroke patients. Method: The medical records of 472 patients with strokes were reviewed using a questionnaire on falling developed by the authors. Frequencies, percentages, means, standard deviations, and t-test and ${\chi}^2$-test, multiple logistic regression analysis were done using the SAS program. Results: The rate for falls by the patients during their stay in the hospital was 14.0%. The length of stay was longer and the morbidity duration of stroke shorter in the fall group than in the non-fall group. The use of sedatives, laxatives, and antidepressants was a significant predictor of falls and was associated with increase likelihood of falling(1.82, 1.81, 1.75 times respectively). Conclusion: In hospitalized stroke patients, there was a significant association between the use of sedatives, laxatives, antidepressants and falls. The number and kinds of ingested drugs was also associated with falls. It is necessary to further analyze the causes of falls based on results of the present study.

포괄간호서비스 병동과 일반병동 입원 환자의 간호만족도와 병원재이용의도 영향요인 비교 (Comparing Satisfaction with Nursing Care and Factors Relevant to Hospital Revisit Intent among Hospitalized Patients in Comprehensive Nursing Care Units and General Care Units)

  • Purpose: The purpose of this study was to compare hospitalized patients in comprehensive nursing care units and general care units as to satisfaction with nursing care and factors influencing their intent to revisit the hospital. Methods: A cross-sectional study was conducted with 178 patients who had been hospitalized in a comprehensive nursing care unit and a general care unit in one hospital. Participants completed self-report questionnaires. Data were analyzed using SPSS 21.0. Results: There was a significant difference between the comprehensive nursing care unit and general care unit for intent to revisit the hospital (p=.036). Factors influencing intent to revisit the hospital for patients in the comprehensive nursing care unit were 'satisfaction with nursing care' (p<.001) and 'use of additional costs for comprehensive nursing care' (p=.041). The factor influencing intent to revisit hospital for patients in the general care unit was 'satisfaction with nursing care' (p<.001). Conclusion: Findings indicate that comprehensive nursing care service in which all care is provided by nursing staff only, without family or private caregivers, increases intent to revisit the hospital. These results can be used as the foundation of reviewing the operation and expansion of comprehensive nursing care service.
