Pulmonary Pneumatocele in a Pneumonia Patient Infected with Extended-Spectrum β-Lactamase Producing Proteus mirabilis

  • Pulmonary pneumatoceles are air-filled thin-walled spaces within the lung and are rare in adult cases of pneumonia. We report the case of a 74-year-old male who was admitted with a cough and sputum production. He had been treated with oral dexamethasone since a brain tumorectomy 6 months prior. Contrast-enhanced computed tomography (CT) of the chest revealed a large pneumatocele in the right middle lobe and peripheral pneumonic consolidation. Bronchoalveolar lavage was performed; cultures identified extended-spectrum ${\beta}$-lactamase (ESBL) producing Proteus mirabilis. A 4-week course of intravenous ertapenem was administered, and the pneumatocele with pneumonia resolved on follow-up chest CT. To the best of our knowledge, this is the first reported case of pulmonary pneumatocele caused by ESBL-producing P. mirabilis associated with pneumonia.

A Case Report of Facial Nerve Palsy Treated with Korean Medicine Treatment (안면신경마비 환자에 대한 한방 치험 1례)

  • Objective: This study is designed to evaluate the effects of Korean medicine treatment for facial nerve palsy. Method: The patient was administered Korean medicine treatment, including acupuncture, pharmacopuncture, and herbal medicine. We used the House-Brackmann Grading System and Yanagihara's Unweighted Grading System to assess the symptoms of facial nerve palsy. Results: The patient's House-Brackmann grade was reduced from grade IV to grade II, and Yanagihara's grade was increased from 19 to 32. The patient's symptoms, which were facial pain, facial asymmetry, reduced wrinkling forehead, incomplete closing eye, epiphora, and tinnitus, improved during the hospitalization. Conclusion: This study shows that Korean medicine treatment may be an effective treatment for facial nerve palsy.

  • Objectives: The purpose of this clinical case is to evaluate the efficacy of traditional Korean medicine as a treatment for fibromyalgia. Methods: The patient was treated with Korean medicine therapy (herbal medicine, acupuncture, pharmacopuncture, and Chu-na therapy). We measured the status and progress of this patient using a numerical rating scale (NRS), EuroQol-5D, range of movement (ROM), and a special test. Results: After the treatments, the patient's pain was controlled and the NRS score decreased. The overall symptoms of the patient and the EQ-5D score were both improved. The ROM and special test score were also improved. Conclusions: Traditional Korean medicine may have positive effects as a treatment for fibromyalgia.

  • Objectives: This study reports a case of a 66-year-old Korean female with stubborn pruritus and assesses the effectiveness of Korean medical treatment. Methods: A patient was treated with acupuncture, electro-acupuncture, and herbal medicine. We evaluated the improvement in symptoms by changes in global assessment (G/A) and sleep quality. Results: After Korean medical treatment, improvements were observed in G/A, and sleep quality. Skin and excretions turned blue to green, with no abnormal findings on the examination. Conclusions: This study suggests that Korean medical treatment is effective in treating stubborn pruritus caused by prurigo. Further studies are needed.

  • Objective: Our aim was to report a case of hematoma in the lower extremity induced by anti-platelet agents (Astrix, Plavix). Methods: The patient was prescribed anti-platelet agents and a hematoma occurred three times in the lower extremity. Before admission, the hematoma had occurred in the lateral compartment area of left lower extremity ($2{\times}2cm$). We detected a spontaneous hematoma in the lateral compartment area of right lower extremity ($8{\times}4cm$) and we stopped the Astrix medication. We also used herbal medicine (大黃左經湯加味方 (Daehwangjwagyeong-tang-gami)), external therapy of 大黃梔子 (Dae-hwang-Chi-ja) powder, and wet cupping therapy. We then detected a spontaneous hematoma in the posterior compartment area of the left lower extremity ($5{\times}5cm$) and we stopped the Plavix medication. We also used herbal medicine and external therapy. Results: After cessation of the anti-platelet agents (Astrix, Plavix), no recurrence of hematoma was observed in lower extremity and there was no recurrence of cerebral infarction. Conclusions: When anti-platelet agents are administered, attention should be paid to the patient's age and adverse events of the drugs. Bleeding symptoms and adverse effects should be continuously monitored.

  • Objectives : This study investigated whether taking herbal medicine over the long-term had any side effects of liver damage. Methods : We checked LFT levels of the 58 admitted patients. Results : When we compared admission LFT levels with discharge LFT levels, we found the levels of AST, ALT and LDH had decreased. This test showed statistically significant decrease. When we compared admission LFT levels with discharge LFT levels, we found the increases of discharge LFT levels fell within the standard deviation. When we compared admission LFT levels with discharge LFT levels, we found the levels of LFT did not increase as much as two standard deviations. Conclusions : According to the above results, taking herbal medicine over a long-term did not have any side effects of drug-induced liver damage.

  • The purpose of this study is to report the clinical effect of Korean medicine on a patient who experienced headaches and nausea after post-concussion syndrome. The patient was treated with herbal medicine (Oryung-san, Hwangryunhaedok-tang-tablet) and acupuncture for four weeks. As a result, the pain degree compared to the day of hospitalization of headache and nausea and pain rating score of headache decreased after treatment. Therefore, this case is useful for describing Korean medicine treatment for headache and nausea after post-concussion syndrome, and further case reports and studies will be needed in the future.

  • Anterior cervical discectomy and fusion (ACDF) is commonly used to treat cervical radiculopathy by decompressing and stabilizing the spine, but complications such as postoperative pain and adjacent-level disc degeneration remain. This report presents a case of a patient with persistent right arm pain after ACDF for a herniated disc at C5/6 and congenital ankylosis at C6/7. The patient experienced severe pain managed with painkillers, which initially worsened but improved significantly with integrative Korean Medicine treatment, including pharmacopuncture, acupotomy, acupuncture, cupping, and herbal medicine. Pain levels decreased from a Numerical Rating Scale of 8 to 3, and the Neck Disability Index improved from 19 to 8, enhancing fine motor skills like handwriting. This case suggests the potential of integrative Korean Medicine in improving recovery post-ACDF and highlights the need for further research despite limitations in generalizability due to the single-case study format.

  • The first edition of 'A Standardized Pathology Report for Gastric Cancer' was initiated by the Gastrointestinal Pathology Study Group of the Korean Society of Pathologists and published 17 years ago. Since then, significant advances have been made in the pathologic diagnosis, molecular genetics, and management of gastric cancer (GC). To reflect those changes, a committee for publishing a second edition of the report was formed within the Gastrointestinal Pathology Study Group of the Korean Society of Pathologists. This second edition consists of two parts: standard data elements and conditional data elements. The standard data elements contain the basic pathologic findings and items necessary to predict the prognosis of GC patients, and they are adequate for routine surgical pathology service. Other diagnostic and prognostic factors relevant to adjuvant therapy, including molecular biomarkers, are classified as conditional data elements to allow each pathologist to selectively choose items appropriate to the environment in their institution. We trust that the standardized pathology report will be helpful for GC diagnosis and facilitate large-scale multidisciplinary collaborative studies.
