Erlotinib-Related Spontaneous Pneumothorax in Patient with Primary Lung Cancer

  • Spontaneous pneumothorax (SPTx) associated with primary lung cancer is quite rare, but has been reported as the initial presentation or a complication of disease progression. Moreover, chemotherapy-related SPTx in primary lung cancer occurs at a very low frequency, accounting for less than 0.05% of all cases. Here, we report the first case of erlotinib-related SPTx in a patient with advanced lung adenocarcinoma in Korea. After 3 cycles of cisplatin-based chemotherapy as first-line therapy, erlotinib was administered as second-line treatment. Asymptomatic SPTx accompanied by a significant decrease in tumor size was observed in the left lung 7 weeks later. The patient received continuous administration of erlotinib, without additional treatment. This case showed that SPTx can occur in patients with primary lung cancer receiving erlotinib, and asymptomatic chemotherapy-related SPTx in primary lung cancer may not require therapeutic intervention.

  • Objectives: This study presents a case of a 53-year-old Korean male with rumination after meals. The aim was to assess the effectiveness of Korean medical treatment. Methods: The outpatient was treated with acupuncture, electroacupuncture, and herbal medicine (Hwangryun-tang, Hyangsapyungwi-san-gami, Anjung-san granules, and Hyangsapyungwi-san granules) for 3 months. We evaluated the patient's symptoms whenever he visited the clinic. Results: The patient showed significant improvements after Korean medical treatment. In particular, regurgitation of food disappeared after herbal medicine treatment. Conclusions: This case report suggests that Korean medical treatment could be effective in treating rumination syndrome. Further studies are needed.

  • This study presents the case of a 73-year-old Korean male with dysphagia caused by achalasia and aimed to assess the effectiveness of Korean medical treatment. The outpatient was treated with acupuncture, electroacupuncture, and herbal medicine (Bunsimgieum-gami, Gyejigayonggolmolyeo-tang, Bangpungtongsung-san) for about 6 months. We evaluated the patient's symptoms whenever he visited the clinic. The patient improved significantly after Korean medical treatment. In particular, dysphagia disappeared after herbal medicine treatment. This case report suggests that Korean medical treatment could be effective in treating achalasia. Further studies are needed.

  • Objectives: The purpose of this study was to report the effectiveness of Korean medicine in the treatment of persistent postural-perceptual dizziness. Methods: A patient with dizziness was treated with Banhabackchulchunma-tang and Gongjin-dan herbal medicines. The severity of symptoms was assessed with a daily visual analog scale (VAS) for dizziness and the frequency of subjective trembling feelings (over 30 minutes). Results: After the treatment, the severity of dizziness was reduced from VAS 5 to VAS 2, and fatigue was improved from VAS 5 to VAS 1. The frequency of subjective trembling feelings was reduced from once every 3-4 days to once every 10 days. Other symptoms, such as dyspepsia, sleep disturbance, and hot flashes, also showed improvement. Conclusion: Treatment with Korean medicine may be an effective option for the treatment of persistent postural-perceptual dizziness.

A Case Report of Chronic Migraine Lasting 30 Years and Treated with Acupuncture (침 치료로 호전된 30년간 지속된 만성 편두통 증례 1례)

  • Objectives: The purpose of this study was to report the effectiveness of Korean medicine in the treatment of chronic migraine persisting for 30 years. This patient had been taking various painkillers (acetaminophen, nonsteroidal anti-inflammatory drugs, caffeine hydrate, etc.) every day for 30 years. Methods: The patient with chronic migraine was treated with acupuncture. Acupuncture treatment was performed on the gall bladder meridian (足少陽膽經) and bladder meridian (足太陽膀胱經) line passing through 24 of the 31 injection sites used for chronic migraine prevention treatment with botulinum toxin type A. The severity of the symptom was assessed by the daily Numerical Rating Scale (NRS) score of pain. Results: The patient received outpatient treatment 42 times and received only acupuncture treatment each time. The patient said that after 4 sessions of acupuncture treatment, the severity of pain was reduced from NRS 7 to NRS 5 and that the frequency and duration of the symptoms were lower. After 8 sessions of acupuncture treatment, the symptoms were reduced to NRS 3 and, after 14 days of treatment, to 0~1. Now she barely has any severe headaches. Conclusion: The findings of this study suggest that treatment with Korean medicine can be an effective option in treating chronic migraine. In addition, acupuncture can be a good treatment method for chronic migraine that cannot be fundamentally controlled by Western pain relievers.

A Case Report on Sojaganggi-tang Treatment for a Patient with Asthma (소자강기탕을 투여한 천식환자의 치험 1례)

  • Objectives: This study aimed to describe the effects of traditional Korean medical treatment on a patient with asthma. Methods: The patient (female, 88) was treated with herbal medicine (Sojaganggi-tang), acupuncture, and moxibustion. The effects of these treatments were evaluated using the Modified Borg Scale (MBS) and Quality of Life Questionnaire for Adult Korean Asthmatics (QLQAKA). Results: Following treatment, the MBS score decreased from 7 to 4, and the QLQAKA score increased from 45 to 57. Conclusion: The results suggest that traditional Korean medicine can effectively treat patients with asthma.

  • The Korea-Uzbekistan Friendship Hospital of Korean Medicine has been providing care and education of Korean medicine in Uzbekistan since 1997, while overseas expansion of Korean medicine is expanding each year. The status of these activities is investigated to suggest methods of development of Korean medicine in Uzbekistan. The hospital treats about 15000 people in Uzbekistan for free every year. Among patients visiting the hospital, the distribution of female, age 50s to 70s is most common, Uzbek, Russian, and Korean are common ethnicities, musculoskeletal disease, neurologic disease and gastrointestinal disease are common ailments. In addition, the hospital has educated students of Tashkent Medical Academy and doctors belonging to the Department of Acupuncturists of Korean Oriental Medicine of the Uzbekistan Medical Association. Korean medical care, education and research in the field of systematic support on a long-term roadmap should be continued for developing Korean medicine on Uzbekistan.

Successful Removal of Endobronchial Lipoma by Flexible Bronchoscopy Using Electrosurgical Snare

  • A 62-year-old man with a chronic cough presented with atelectasis of the left upper lobe on chest X-ray. Chest computed tomography showed an atelectasis in the left upper lobe with bronchial wall thickening, stenosis, dilatation, and mucoid impaction. We performed bronchoscopy and found a well-circumscribed mass on the left upper lobe bronchus. The mass was removed by flexible bronchoscopy using an electrosurgical snare and diagnosed with lipoma. An endobronchial lipoma is a rare benign tumor that can be treated by a surgical or endoscopic approach. We report the successful removal of endobronchial lipoma via flexible bronchoscopic electrosurgical snare.

One Case of Dizzness and Diplopia from Pontine Infarction Treated with Joganiknoe-tang (뇌교경색으로 인한 현훈 및 복시증상에 조간익뇌탕(助肝益腦湯)을 투여한 치험 1례)

  • Pontine infarction presents variable neurologic deficits because the pons is a very complicated organ with cranial nerve nuclei and several fiber tracts. A 65 year-old women with dizzness, ataxia and diplopia because of Pontine infarction was admitted at Wonkwang University Jeonju Oriental Medicine Hospital. She was treated with the herbal medicine Joganiknoe-tang(助肝益腦湯) and with acupunture. Improvement in these symptoms was observed, so the specifics of the process in which the patient was treated are here described. Results suggest that Joganiknoe-tang(助肝益腦湯) is an effective treatment for symptoms of pontine infarction.

Tracheal Stenosis after Tracheostomy Treated Successfully with Papillotome Electrocautery

  • Tracheal Stenosis after Tracheostomy Treated Successfully with Papillotome Electrocautery A 39-year-old woman presented with symptoms of dyspnea. Ten years previously, she had received a tracheostomy because of the decision to not continue taking an anticonvulsant drug. Presently, chest computed tomography showed diffuse stenosis and focal web at the cervical trachea. We performed bronchoscopy and found a two-thirds reduction of the upper trachea due to the web-like fibrotic stenosis. Papillotome electrocautery removed the stenotic lesion. Endobronchial electrocautery is a valuable tool with potential for therapy of an endobronchial obstructing airway lesion. We report this case to introduce the successful treatment with papillotome electrocautery.
